Endotracheal Tube Displacement during Cardiac Surgery in Infants: A Retrospective Cohort Study of Its Incidence and Predictors

Introduction: With the traction on the trachea and intrathoracic manipulation during cardiac surgery, the position of the endotracheal tube (ETT) might be changed as compared to before surgery. Migration of the ETT during pediatric cardiac surgery is particularly problematic in infants. Methods: In this retrospective cohort study, chest X-rays were taken in the operating room just before and after surgery. The position of the ETT among all infants under 1 year of age who underwent cardiac surgery between December 2017 to December 2019 was evaluated. The displacement of the ETT position was examined by measuring the position of the tube tip from the tracheal bifurcation on a chest X-ray, and the relationship between surgery-related factors (age, height, weight, sex, surgery time, cardiopulmonary bypass, tube size, use of tube cuff) was analyzed. Results: Eighty-eight of the 141 patients were enrolled. There was a significant proximal displacement of the ETT tip during cardiac surgery. The distance from the carina to the tube tip after surgery was long, on average 2.5 mm, compared to that before surgery (P = 0.013). Cephalad displacement of the ETT either ≥5 mm or ≥2.5 mm was found in 28 and in 38 out of 88 infants after surgery, respectively. After performing multivariate analysis, the use of un-cuffed ETT was the sole exploratory variable predictive of tube tip displacement (OR 0.34, 95% CI 0.10 - 0.93 if ≥5 mm;and OR 0.24, 95% CI 0.08 - 0.75 if ≥2.5 mm displacement;P = 0.04 and 0.01, respectively). Conclusion: Proximal displacement of the ETT during cardiac surgery occurs more frequently in infants with un-cuffed ETT.

Offensive language detection has received important attention and plays a crucial role in promoting healthy communication on social platforms,as well as promoting the safe deployment of large language *** data is the basis for developing detectors;however,the available offense-related dataset in Chinese is severely limited in terms of data scale and coverage when compared to English *** significantly affects the accuracy of Chinese offensive language detectors in practical applications,especially when dealing with hard cases or out-of-domain *** alleviate the limitations posed by available datasets,we introduce AugCOLD(Augmented Chinese Offensive Language Dataset),a large-scale unsupervised dataset containing 1 million samples gathered by data crawling and model ***,we employ a multiteacher distillation framework to enhance detection performance with unsupervised *** is,we build multiple teachers with publicly accessible datasets and use them to assign soft labels to *** soft labels serve as a bridge for knowledge to be distilled from both AugCOLD and multiteacher to the student network,i.e.,the final offensive *** conduct experiments on multiple public test sets and our well-designed hard tests,demonstrating that our proposal can effectively improve the generalization and robustness of the offensive language detector.

Following the recent report by Dasenbrock-Gammon et al.[Nature 615,244–250(2023)]of near-ambient superconductivity in nitrogendoped lutetium trihydride(LuH_(3-δ)N_(ε)),significant debate has emerged surrounding the composition and interpretation of the observed sharp resistance ***,we meticulously revisit these claims through comprehensive characterization and *** definitively identify the reported material as lutetium dihydride(LuH_(2)),resolving the ambiguity surrounding its *** similar conditions(270–295 K and 1–2 GPa),we replicate the reported sharp decrease in electrical resistance with a 30%success rate,aligning with the observations by Dasenbrock-Gammon et ***,our extensive investigations reveal this phenomenon to be a novel pressure-induced metal-to-metal transition intrinsic to LuH_(2),distinct from ***,nitrogen doping exerts minimal impact on this *** work not only elucidates the fundamental properties of LuH_(2)andLuH_(3),but also critically challenges the notion of superconductivity in these lutetium hydride *** findings pave the way for future research on lutetium hydride systems,while emphasizing the crucial importance of rigorous verification in claims of ambient-temperature superconductivity.

High-pressure ultrafast dynamics,as a new crossed research direction,are sensitive to subtle non-equilibrium state changes that might be unresolved by equilibrium states measurements,providing crucial information for studying delicate phase transitions caused by complex interactions in Mott *** time-resolved transient reflectivity measurements,we identified the new phases in the spin–orbit Mott insulator Sr_(3)Ir_(2)O_7 at 300 K that was previously unidentified using conventional approaches such as x-ray *** pressure-dependent variation of the amplitude and lifetime obtained by fitting the reflectivity?R/R reveal the changes of electronic structure caused by lattice distortions,and reflect the critical phenomena of phase *** findings demonstrate the importance of ultrafast nonequilibrium dynamics under extreme conditions for understanding the phase transition of Mott insulators.

This study aimed to identify the reasons for transferring athletes to local medical facilities during the Olympic and Paralympic *** on 567 injuries and other illnesses of athletes treated at the on-site clinics were collected from the Tokyo 2020 Organizing *** these,84 athletes who required outpatient care during the Games were registered for this *** the Olympic and Paralympic Games,66(8.3/1000)and 18(7.2/1000)athletes,respectively,consulted external medical *** the Olympic Games,the reasons for these visits included 48 cases(72.7%)of injuries,13(19.7%)cases of illnesses,and 5(7.6%)cases of heat stroke illness(HSI).Of these patients,56(84.9%)were treated as outpatients and 10(15.1%)were hospitalized,while three of these patients required hospitalization for>7 *** the other hand,in the Paralympics Games,there were 7(38.8%)cases of injuries,9(50.0%)other illnesses,1(5.6%)case of HSI,and 1(5.6%)other cases,of which 11(61.1%)were treated as outpatients and 7(38.9%)were hospitalized,but none was hospitalized for>7 *** accounted for 70%of the total cases at the 2021 Olympic Games,but only three(0.05%)were severe cases that required hospitalization for more than 1 *** contrast,in the Paralympic Games,other illnesses accounted for approximately half of the total *** study provides details on the extent of injuries and other illnesses that were transferred to outside facilities,which has not been documented in previous games.

A weeding-duration model for Abies sachalinensis plantations in Hok-kaido, northern Japan

I developed a weeding-duration model for Sakhalin fir （Abies sachalinensis （Fr. Schmidt） Masters） plantations that employs a generalized linear model. The number of years following planting that weeding is necessary is the response variable, and elevation, slope steepness, maximum snow depth, annual precipitation, geology, soil, site index, slope aspect, and vegetation type are explanatory variables. Among the explanatory variables, geology, soil, slope aspect, and vegetation type are categorical data. A Poisson distribution is assumed for the response variable, with a log-link function. Elevation, slope steepness, maximum snow depth, annual precipitation, site index, and vegetation type had a significant effect on weeding duration. Among the eight models with the smallest Akaike information criterion （AIC）, I chose the model with no multicollinearity among the explanatory variables. The weeding-duration model includes site index, maximum snow depth, slope steepness （angle） and vegetation type as explanatory variables; elevation and annual precipitation were not included in the selected model because of multicollinearity with maximum snow depth. This model is useful for cost-benefit analyses of afforestation or reforestation with Abies sachalinensis.

Water is the most important component in Earth system evolution. Here, I review the current understanding of the fate of water in the mantle dynamics system based on high-pressure and temperature experiments, geochemical analyses, seismological and geomagnetic observations, and nu- merical modeling of both regional- and giobal-scale mantle dynamics. In addition, as a numerical ex- ample, I show that the water solubility of the deep mantle is strongly sensitive to global-scale water cir- culation in the mantle. In a numerical example shown here, water solubility maps as functions of tem- perature and pressure are extremely important for revealing the hydrous structures in both the mantle transition zone and the deep mantle. Particularly, the water solubility limit of lower mantle minerals should be not so large as ~100 ppm for the mantle transition zone to get the largest hydrous reservoir in the giobal-scale mantle dynamics system. This result is consistent with the current view of mantle water circulation provided by mineral physics, which is also found as a hydrous basaltic crust in the deep mantle and the water enhancement of the mantle transition zone simultaneously. In this paper, I also discuss some unresolved issues associated with mantle water circulation, its influence on the onset and stability of plate motion, and the requirements for developing Earth system evolution in mantle dy- namics simulations.

It has been established that cancer can be promoted and exacerbated by *** carcinoma(HCC) is the fifth most common cancer worldwide,and its long-term prognosis remains *** HCC is a complex and heterogeneous tumor with several genomic mutations,it usually develops in the context of chronic liver damage and inflammation,suggesting that understanding the mechanism(s) of inflammation-mediated hepatocarcinogenesis is essential for the treatment and prevention of *** liver damage induces a persistent cycle of necroinflammation and hepatocyte regeneration,resulting in genetic mutations in hepatocytes and expansion of initiated cells,eventually leading to HCC ***,several inflammation-and stress-related signaling pathways have been identified as key players in these processes,which include the nuclear factor B,signal transducer and activator of transcription,and stress-activated mitogen-activated protein kinase *** these pathways may suggest potential therapeutic targets,they have a wide range of functions and complex crosstalk occurs among *** review focuses on recent advances in our understanding of the roles of these signaling pathways in hepatocarcinogenesis.
