Splenectomy in living donor liver transplantation and risk factors of portal vein thrombosis

Background:Graft inflow modulation(GIM)during adult-to-adult living donor liver transplantation(LDLT)is a common strategy to avoid small-for-size syndrome,and some transplant surgeons attempt small size graft strategy with frequent GIM procedures,which are mostly performed by splenectomy,in ***,splenectomy can cause serious complications such as portal vein thrombosis and overwhelming postsplenectomy ***:Forty-eight adult-to-adult LDLT recipients were enrolled in this study and retrospectively *** applied the graft selection criteria,which routinely fulfill graft-to-recipient weight ratio≥0.8%,and consider GIM as a backup strategy for high portal venous pressure(PVP).Results:In our current strategy of LDLT,splenectomy was performed mostly due to hepatitis C and splenic arterial aneurysms,but splenectomy for GIM was intended to only one patient(2.1%).The final PVP values≤20 mmHg were achieved in all recipients,and no significant difference was observed in patient survival or postoperative clinical course based on whether splenectomy was performed or ***,6 of 18 patients with splenectomy(33.3%)developed postsplenectomy portal vein thrombosis(PVT),while none of the 30 patients without splenectomy developed PVT after *** was identified as a risk factor of PVT in this study(P<0.001).Our study revealed that a lower final PVP could be risk factor of postsplenectomy ***:Using sufficient size grafts was one of the direct solutions to control PVP,and allowed GIM to be reserved as a backup *** should be avoided as much as possible during LDLT because splenectomy was found to be a definite risk factor of *** splenectomy cases with a lower final PVP,a close follow-up is required for early detection and treatment of PVT.

This study proposed a partitioned method to analyze maneuvering of insects during flapping *** method decomposed the insect flapping flight into wing and body subsystems and then coupled them via boundary conditions imposed on the wing’s base using one-way *** the wing subsystem,the strong coupling of the flexible wings and surrounding fluid was accurately analyzed using the finite element method to obtain the thrust forces acting on the insect’s *** resulting thrust forces were passed from the wing subsystem to the body subsystem,and then rigid body motion was analyzed in the body *** rolling,yawing,and pitching motions were simulated using the proposed method as follows:In the rolling simulation,the difference of the stroke angle between the right and left wings caused a roll *** the yawing simulation,the initial feathering angle in the right wing only caused a yaw *** the pitching simulation,the difference between the front-and back-stroke angles in both the right and left wings caused a pitch *** three torques generated maneuvering motion comparable with that obtained in actual observations of insect *** results demonstrate that the proposed method can adequately simulate the fundamental maneuvers of insect flapping *** the present simulations,the maneuvering mechanisms were investigated at the governing equation level,which might be difficult using other ***,the proposed method will contribute to revealing the underlying insect flight mechanisms.

Polyvinylidene fluoride （PVDF） hollow fiber membranes prepared from spinning solutions with different polyvinyl pyrrolidone （PVP） contents （1% and 5%） at different extrusion rates were obtained by wet/dry phase process keeping all other spinning parameters constant. In spinning these PVDF hollow fibers, dimethylacetamide （DMAc） and PVP were used as a solvent and an additive, respectively. Water was used as the inner coagulant. Dimethylformamide （DMF） and water （30/70） were used as the external coagulant. The performances of membranes were characterized in terms of water flux, solute rejection for the wet membranes. The structure and morphology of PVDF hollow fiber were examined by BET adsorption, dry/wet weight method and scanning electron microscopy （SEM）. It is found that the increase in PVP content and extrusion rate of spinning solution can result in the increase of water flux and decrease of solute rejection. The improvements of interconnected porous structure and pore size are induced by shear-thinning behavior of spinning solution at high extrusion rates, which could result in the increase of water flux of hollow fiber membranes. The increase of extrusion rate also leads to the increase of membrane thickness due to the recovery effect of elastic property of polymer chains.

AIMTo investigate factors, including psychosocial factors, associated with alcoholic use relapse after liver transplantation (LT) for alcoholic liver disease (ALD).METHODSThe clinical records of 102 patients with ALD who were referred to Nagoya University Hospital for LT between May 2003 and March 2015 were retrospectively evaluated. History of alcohol intake was obtained from their clinical records and scored according to the High-Risk Alcoholism Relapse scale, which includes duration of heavy drinking, types and amount of alcohol usually consumed, and previous inpatient treatment history for alcoholism. All patients were assessed for eligibility for LT according to comprehensive criteria, including Child-Pugh score, Model for End-Stage Liver Disease score, and psychosocial *** the 102 patients with ALD referred for LT, seven (6.9%) underwent LT. One (14.3%) of these seven patients returned to heavy drinking, but that patient was able to successfully quit drinking following an immediate intervention, consisting of psychotherapeutic education and supportive psychotherapy, by a psychiatrist. A comparison between the transplantation/registration (T/R) group, consisting of the seven patients who underwent LT and 10 patients listed for deceased donor LT, and 50 patients who did not undergo LT and were not listed for deceased donor LT (non-T/R group), showed statistically significant differences in duration of abstinence period (P < 0.01), duration of heavy drinking (P < 0.05), adherence to medical treatment (P < 0.01), and declaration of abstinence (P < 0.05).CONCLUSIONPatients with ALD referred for LT require comprehensive evaluation, including evaluation of psychosocial criteria, to prevent alcoholic recidivism.

This research examines the micro-level correlation between traditional marketing actions(TV ads and public relations) and pre-release consumers’ social learning about videogame consoles(Wii and PS3, launched in 2006). We evaluate consumers’ learning processes via the perusal of information in online communities using "pageview" data for multiple websites from a clickstream panel as indicators. We propose a bivariate Bayesian learning model combined with complementary purchase *** proposed model enables simpler estimation of parameters and allows to accommodate detailed information about interactions between social and personal learning processes. From the results, we find empirical evidence that companies’ traditional marketing actions have a greater impact on social learning than on regular personal learning during the pre-launch period. When consumers make purchase decisions, their social beliefs about product quality are weighed at least three times more heavily than their personal beliefs. Counterfactual simulations suggest that by optimizing marketing actions,firms can stimulate consumers’ learning and promote increased product engagement.

This paper analyzed regional industrial energy efficiency in China with Total-Factor Energy Efficiency （TFEE）. The East region has the best energy efficiency and the Central and the West regions stand as the second and the third respectively. However, it is found that industrial energy efficiency of all regions increased from 1998 to 2006. This result is consistent with level of economic development of every region. The industries of all provinces in China are not yet at the frontier efficiency position, therefore, to the frontier as target, their technol- ogy levels and production processes should be adjusted accordingly. Compared with the conventional energy efficiency, the inverse of energy intensity, which is defined as the ratio of actual output to energy input, is regarded as Single-Factor Energy Efficiency （SFEE） index. Although TFEE ranks are not changed for each region, they are different for each province. The comparative result also shows that the substitution among inputs （labor, capital stock, and energy） to produce the output is significant. The SFEE scores could be over-estimated if energy is taken as the single input in the production. Finally, we identified determining factors affecting industrial energy efficiency using Tobit model. The results indicate that an increase of per capita Gross Domestic Product （GDP）, the percentage of output value of industry invested by Hong Kong, Macao, Taiwan and abroad, energy price and investment of scientific and technological activities for industry could be possible contributors and drivers to the industrial energy efficiency. However, increasing of heavy industry will lead to worse industrial energy efficiency.
