Favorable Reproducibility of Ki-67-Labeling Index between Core Needle Biopsy and Surgical Materials in Mammary Carcinoma: Reproducibility Influenced by Hot Spots, a High Ki-67 Labeling Index, and the Total Length of Biopsy Material

Aims: The reproducibility of Ki-67 between core-needle biopsies and surgical materials has not been well documented in the literature, although the concordance affects the utility of the Ki-67 labeling index based on the core-needle biopsy materials, which indicates the need for preoperative chemotherapy. The aim of this study was to reveal the reproducibility of Ki-67 between both materials and the cause of discrepancies. Methods and Results: We analyzed 137 cases of invasive carcinoma of the breast and the compared Ki-67-labeling index between core-needle biopsy and surgical materials. The Ki-67-labeling index of biopsy and surgical specimens ranged from 1% to 85% (median: 13%) and 1% to 80% (median: 12%), respectively. The discrepancy of Ki-67-labeling ranged from 0% to 55% (median: 4%) and could be calculated by the tumor size, hot spots of surgical materials, a high Ki-67-labeling index based on the core-needle biopsy materials, and the total length of core needles, respectively. Conclusions: The concordance rate of the Ki-67-labeling index between core-needle biopsies and surgical materials was favorable, so we can use each Ki-67-labeling index of core-needle biopsies as a marker for preoperative chemotherapy. Factors affecting the index discrepancy were hot spots, a high Ki-67-labeling index, and the total length of biopsy material. Judgements on the subtypes and clinical procedures of invasive breast carcinoma could be made comprehensively based on not only the Ki-67-labeling index but also the existence of hot spots and histological grade.

Usefulness of noninvasive transient elastography for assessment of liver fibrosis stage in chronic hepatitis C

AIM：To evaluate the method of noninvasive transient elastography for assessment of histological stage of liver fibrosis in patients with chronic hepatitis C （CHC）.METHODS： Two hundred and thirty-seven patients with CHC were included in this study. Liver biopsy was performed under ultrasonography on 217 of the patients, excluding twenty with clear clinical evidence of liver cirrhosis. Fifty subjects without liver disease were enrolled as a control group （stage 0）. Twentyfive patients with sustained virological response （SVR） to interferon （IFN） therapy were also enrolled. These patients underwent liver biopsy before IFN therapy. Examination of liver stiffness （LS） was performed by elastography.RESISTS： Medians （50% levels） of LS were 4.1 （3.5-4.9）, 6.3 （4.8-8.5）, 8.8 （6.8-12.0）, 14.6 （10.5-18.6）, and 22.2 （15.4-28.0）, respectively, in the fibrosis stages 0-4 （P 〈 0.001）. LS was significantly correlated with four serum fibrosis markers. LS values in patients with SVR were 3.8 （3.5-5.6）, 5.2 （4.4-6.8）, 6.8 （6.1-7.6）, and 6.1 （3.6-7.9）, respectively, in the fibrosis stages 1-4. In all stages, LS for patients with SVR was significantly lower than that for patients who did not undergo IFN therapy. LS was significantly correlated with serum concentrations of hyaluronic acid, type Ⅳ collagen, type Ⅳ collagen 7S, and type Ⅲ procollagen N peptide.CONCLUSION： LS correlated well with the histological stage of fibrosis. Changes in liver fibrosis stage may thus be estimated noninvasively using transient elastography.

引言最近已研制出具有清晰、高分辨率、高速记录的优点的各种激光记录系统。为了实现高速和宽带记录,重要的是研制许多具有良好性能的光学元件。在系统许多指标中,记录速度被记录介质的灵敏度、激光源的功率以及扫描、成像元件的光学效率所限制。记录的分辨率被光束斑点和调制器带宽所限制。电光调制器和声光调制器已被用来作激光记录系统的调制器。电光调制器具有高达100～200兆赫的宽带宽。但是,电光调制器既要对温度变化稳定,又需要高的驱动电压。而效率高的电光调制器由于它们自然双折射的改变而对温度变化是极其敏感的,它们不是那么合用的。

A 58-year-old man presented with the chief complaint of abdominal bloating and was incidentally found to have a liver *** diagnostic imaging studies could not rule out malignancy,the patient underwent partial resection of segment 3 of the *** lesion pathologically showed eosinophilic proliferation,in addition to immunohistochemical positivity for human melanoma black 45 and Melan-A,thereby leading to the diagnosis of a hepatic perivascular epithelioid cell tumor(PEComa).A PEComa arising from the liver is relatively ***,the name ‘PEComa' has not yet been widely recognized,and the same disease entity has been called epithelioid angiomyolipoma(EAML),further diminishing the recognition of *** addition,PEComa imaging findings mimic those of malignant liver tumors,and clinically,this tumor tends to ***,a PEComa is difficult to *** conducted a systematic review of PEComa and EAML cases and discuss the results,including findings useful for differentiating perivascular epithelioid cell tumors from malignant liver tumors.

探讨载脂蛋白Ｅ（ａｐｏＥ）－基因敲除鼠极低密度脂蛋白（ＶＬＤＬ）和中间密度脂蛋白（ＩＤＬ）组分（ａｐｏＥｋｏ－ＶＬＤＬ／ＩＤＬ）致动脉硬化作用．采用超离法从ａｐｏＥ－基因敲除鼠血浆中分离ＶＬＤＬ和ＩＤＬ组分，与鼠腹腔巨噬细胞共育，观察ａｐｏＥｋｏ－ＶＬＤＬ／ＩＤＬ与巨噬细胞的相互作用．ＡｐｏＥｋｏ－ＶＬＤＬ／ＩＤＬ导致细胞胆固醇酯含量显著增加．其诱导的细胞［３Ｈ］胆固醇油酸脂量高达１５．１ｎｍｏｌ／ｍｇ细胞蛋白，是天然低密度脂蛋白的８．４倍．形态学观察显示，经ａｐｏＥｋｏ－ＶＬＤＬ／ＩＤＬ处理的巨噬细胞与苏丹黑Ｂ呈阳性染色．细胞结合实验表明，１２５Ｉ－ａｐｏＥｋｏ－ＶＬＤＬ／ＩＤＬ与巨噬细胞的总结合可被非标记配基取代８０％以上，特异结合呈一饱和图形．同时细胞缔合和细胞内降解实验也显示相似结果．

PLASMID DNA SURVEY OF CLINICALLY ISOLATED SHIGELLA STRAINS IN SHANGHAI AREA

Plasmids and biological characteristics of Shigela-strains isolated in Shanghai were studied for comparison with those of strains isolated in Tokyo from travellers back from various areas of South East Asia. Invasive ability was confirmed to be related to 140 or 120 mega dalton (M) plasmids. Thirty to 70 M plasmids were found to be conjugative R plasmids. Incidences of ampicillin, trimethoprim and mercury resistant strains (R plasmids) were higher in Shanghai strains than in South East Asian strains. A variety of small plasmids were detected in all strains. In S flexneri strains, 2.0 and 2.6 M plasmids were found to be common in the strains from both areas, and in Sonnei strains, 3.4 M col El plasmids were common. The plasmid patterns formed by the small plasmid combinations were not always specific for species or serotype. But at least these small plasmid DNA patterns could be used to trace epidemic strains.

AIM: To investigate factors that accurately predict hepatocellular carcinoma(HCC) development after antiviral therapy in chronic hepatitis C(CHC) patients. METHODS: CHC patients who received pegylated interferon and ribavirin were enrolled in this cohort study that investigated the ability of alpha-fetoprotein(AFP) to predict HCC development after interferon(IFN) therapy. RESULTS: Of 1255 patients enrolled, 665 developed sustained virological response(SVR) during mean follow-up period of 5.4 years. HCC was occurred in 89 patients, and 20 SVR patients were included. Proportional hazard models showed that HCC occurred in SVR patients showing AFP ≥ 5 ng/m L before therapy and in non-SVR patients showing AFP ≥ 5 ng/m L before and 1 year after therapy besides older age, and low platelet counts. SVR patients showing AFP ≥ 5 ng/m L before therapy and no decrease in AFP to < 5 ng/m L 1 year after therapy had significantly higher HCC incidence than non-SVR patients showing AFP ≥ 5 ng/m L before therapy and decreased AFP(P = 0.043). AFP ≥ 5 ng/m L before therapy was significantly associated with low platelet counts and high values of alanine aminotransferase(ALT) in stepwise logistic regression analysis. After age, gender, platelet count, and ALT was matched by propensity score, significantly lower HCC incidence was shown in SVR patients showing AFP < 5 ng/m L before therapy than in those showing AFP ≥ 5 ng/m ***: The criteria of AFP < 5 ng/m L before and 1 year after IFN therapy is a benefical predictor for HCC development in CHC patients.
