Complete withdrawal of glucocorticoids after dupilumab therapy in allergic bronchopulmonary aspergillosis:A case report

BACKGROUND Allergic bronchopulmonary aspergillosis(ABPA)is an allergic reaction to Aspergillus species that aggravates bronchial *** studies demonstrated the glucocorticoid-sparing effect of dupilumab in patients with *** is no report of complete withdrawal of glucocorticoids after *** SUMMARY The patient was a 54-year-old woman with bronchial asthma treated with inhaled corticosteroids and a long-acting beta-2 *** consulted our institution for productive cough and fever in March *** computed tomography scan revealed mucoid impaction,and the bronchial lavage fluid culture was positive for Aspergillus *** diagnosis was *** patient was treated with oral glucocorticoids from April 2017 to November *** January 2019,she had bronchial asthma exacerbation,and a chest computed tomography scan showed recurrent mucoid *** was treated with oral glucocorticoids and *** February 2020,during tapering of oral glucocorticoid,she had the third episode of bronchial asthma exacerbation and a mucoid *** patient was treated with dupilumab in addition to oral glucocorticoid and *** clinical response improved,and oral glucocorticoid was discontinued in June *** This is the first case of ABPA in which complete withdrawal of glucocorticoid was possible after treatment with dupilumab.

The cerebral cortex is comprised of properly localized cell types that exert their specific *** the developing brain,cells migrate from the germinal region to their functional locations(Silva et al.,2019;Cossart and Garel,2022).For example,neocortical excitatory neurons are generated in the cerebral ventricular and subventricular zones,move to the developing cortical plate via radial migration,and reside in a radial array of six neuronal layers(Oishi and Nakajima,2018).On the other hand,cortical interneurons are mainly generated in ganglionic eminences,migrate tangentially across the cerebral cortex,and reach their final destinations in the cortex(Lim et al.,2018).The failure of neuronal migration leads to defects in cortical layer *** the mechanisms of neuronal distribution have been well examined,how astrocytes are diffusely distributed in the cortex is still *** are glial cells in the cerebral cortex with several functions,including metabolic support and synapse formation(Abbott et al.,2006;Bosworth and Allen,2017;Allen and Lyons,2018).For example,astrocytes establish synaptic connectivity in the developing brain while they contact numerous synapses and maintain optimal neuronal activity in the adult *** the developing brain,astrocytes are primarily generated from radial glia after the neurogenic *** a certain type of astrocyte called fibrous astrocytes populates the white matter,protoplasmic astrocytes migrate to the cortical plate during neural network formation.

Therapeutic potential of stem cells in subarachnoid hemorrhage

Euler Product Expressions of Absolute Tensor Products of Dirichlet L-Functions

In this paper, we calculate the absolute tensor square of the Dirichlet L-functions and show that it is expressed as an Euler product over pairs of primes. The method is to construct an equation to link primes to a series which has the factors of the absolute tensor product of the Dirichlet L-functions. This study is a generalization of Akatsuka’s theorem on the Riemann zeta function, and gives a proof of Kurokawa’s prediction proposed in 1992.

Can early precut reduce post-endoscopic retrograde cholangiopancreatography pancreatitis in patients with difficult bile duct cannulation?

BACKGROUND Endoscopic retrograde cholangiopancreatography(ERCP)is associated with a variety of adverse events(AEs).One of the most important AEs is post-ERCP pancreatitis(PEP),which is most common in cases of difficult biliary *** the precut technique has been reported as a PEP risk factor,recent studies indicate that early precut could reduce PEP,and that precut itself is not a risk *** To evaluate the safety of the precut technique,especially in terms of *** We conducted a retrospective study,spanning the period from November 2011 through December *** included 1556 patients,aged≥20 years,who underwent their initial ERCP attempt for biliary disease with a naïve papilla at the Kawasaki University General Medical *** compared the PEP risk between the early precut and the delayed precut *** The PEP incidence rate did not significantly differ between the precut and nonprecut ***,the PEP incidence was significantly lower in the early precut group than the delayed precut group(3.5%vs 10.5%;P=0.02).The PEP incidence in the delayed precut group without pancreatic stent insertion(17.3%)was significantly higher compared to other cases(P<0.01).CONCLUSION Our findings indicate that early precut may reduce PEP *** the precut decision is delayed,a pancreatic stent should be inserted to prevent PEP.

Periostin in cerebrovascular disease

Cerebrovascular diseases,which include ischemic and hemorrhagic strokes,remain serious conditions with high mortality and disability worldwide(He et al.,2018).Ischemic stroke accounts for around 80%of all strokes(He et al.,2018).Hemorrhagic stroke consisting of intracerebral hemorrhage and subarachnoid hemorrhage(SAH)occurs less frequently but may cause poorer outcomes than ischemic stroke(Ji et al.,2017;Luo et al.,2018).It is also well known that aneurysmal SAH may have delayed cerebral ischemia(DCI):thus aneurysmal SAH has both characteristics of hemorrhagic and ischemic strokes(Kanamaru et al.,2019).In spite of enormous efforts to improve outcomes,no conclusive treatment exists thus far,once stroke is completed.

Lessons from tenascin-C knockout mice and potential clinical application to subarachnoid hemorrhage

Subarachnoid hemorrhage by a ruptured cerebral aneurysmremains the most devastating cerebrovascular disorderswith high morbidity and mortality (Suzuki et al.,2017), which are mainly caused by early brain injury anddelayed cerebral ischemia. Despite many experimentaland clinical research efforts, the mechanisms of earlybrain injury as well as delayed cerebral ischemia remainunclarified. However, recent works have suggested thattenascin-C, which is a special type of secreted extracellularmatrix proteins, is promising as a novel therapeutictarget against post-subarachnoid hemorrhage early braininjury and delayed cerebral ischemia.
