The minimal invasiveness of endoscopic submucosal dissection(ESD) prompted us to apply this technique to large-size early esophageal squamous cell carcinoma and Barrett's adenocarcinoma, despite the limitationsin the study population and surveillance duration. A post-ESD ulceration of greater than three-fourths of esophageal circumference was advocated as an important risk factor for refractory strictures that require several sessions of dilation therapy. Most of the preoperative conditions are asymptomatic, but dilatation treatment for dysphagia associated with the stricture has potential risks of severe complications and a worsening of quality of life. Possible mechanisms of dysphasia were demonstrated based on dysmotility and pathological abnormalities at the site:(1) delayed mucosal healing;(2) severe inflammation and disorganized fibrosis with abundant extracellular matrices in the submucosa; and(3) atrophy in the muscularis proper. However, reports on the administration of anti-scarring agents, preventive dilation therapies, and regenerative medicine demonstrated limited success in stricture prevention, and there were discrepancies in the study designs and protocols of these reports. The development and consequent long-term assessments of new prophylactic technologies on the promotion of wound healing and control of the inflammatory/tumor microenvironment will require collaboration among various research fields because of the limited accuracy of preoperative staging and high-risk of local recurrence.

The Models-3 Community Multi-scale Air Quality modeling system (CMAQ) coupled with the Regional Atmospheric Modeling System (RAMS) is applied to East Asia to study the transport and photochemical transformation of tropospheric ozone in March 1998. The calculated mixing ratios of ozone and carbon monoxide are compared with ground level observations at three remote sites in Japan and it is found that the model reproduces the observed features very well. Examination of several high episodes of ozone and carbon monoxide indicates that these elevated levels are found in association with continental outflow, demonstrating the critical role of the rapid transport of carbon monoxide and other ozone precursors from the continental boundary layer. In comparison with available ozonesonde data, it is found that the model-calculated ozone concentrations are generally in good agreement with the measurements, and the stratospheric contribution to surface ozone mixing ratios is quite limited.

Obesity is becoming an inevitable pandemic all over the *** World Obesity Federation predicts in the 2022 World Obesity Atlas that one billion people worldwide,including 1 in 5 women and 1 in 7 men,will be living with obesity by ***,the prevalence of diabetes is increasing worldwide,and diabetes is becoming more of a public health *** insulin resistance due to obesity and deficiency in insulin secretion are the two main causes of type 2 diabetes mellitus(T2DM).An exogenous chemical or mixture of chemicals that interferes with any aspect of hormone action was defined as endocrine-disrupting chemicals(EDCs).Bisphenol A(BPA),the first known EDC,was synthesized and was considered to be *** production of BPA has increased progressively from 5 to 8 million tons(MT)between 2010 and ***,researchers estimated that the production should reach 10.2 MT by *** human population is exposed to EDCs in daily life in such forms as pesticides/herbicides,industrial and household products,plastics,detergents,and personal care *** term obesogen was used for chemicals that promote weight gain and obesity by increasing the number of adipocytes and fat storage in existing adipocytes,changing the energy balance,and finally regulating appetite and *** the obesogenic effect,EDCs can cause T2DM through alteration inßcell function and morphology and insulin *** this review,we provide clinical and mechanistic evidence regarding EDCs as obesogen and ***,those studies are not enough methodologically to indicate *** this respect,randomized clinical trials are needed to investigate the association between obesogen,diabetogen and the related metabolic clinical picture.

The small Rab GTPases are key regulators of membrane vesicle trafficking. Ovaries of Periplaneta americana （Linnaeus）（Blattodea： Blattidae） have small molecular weight GTP/ATP-binding proteins during early and late vitellogenic periods of oogenesis. However, the identification and characterization of the detected proteins have not been yet reported. Herein, we cloned a cDNA encoding Rab5 from the American cockroach, P. americana, ovaries （PamRab5）. It comprises 796 bp, encoding a protein of 213 amino acid residues with a predicted molecular weight of 23.5 kDa. PamRab5 exists as a single-copy gene in the P. americana genome, as revealed by Southern blot analysis. An approximate 2.6 kb ovarian mRNA was transcribed especially at high levels in the previtellogenic ovaries, detected by Northern blot analysis. The muscle and head tissues also showed high levels of PamRab5 transcript. PamRab5 protein was localized, via immunofluorescence labeling, to germline-derived cells of the oocytes, very early during oocyte differentiation. Immunoblotting detected a ~25 kDa signal as a membrane-associated form revealed after application of detergent in the extraction buffer, and 23 kDa as a cytosolic form consistent with the predicted molecular weight from amino acid sequence in different tissues including ovary, muscles and head. The PamRab5 during late vitellogenic periods is required to regulate the endocytotic machinery during oogenesis in this cockroach. This is the first report on Rab5 from a hemimetabolan, and presents an inaugural step in probing the molecular premises of insect oocyte endocytotic trafficking important for oogenesis and embryonic development.

Impairment of IFN-a production capacity in patients with hepatitis C virus and the risk of the development of hepatocellular carcinoma

AIM: To determine the utility of interferon (IFN) -αproduction capacity in patients with hepatitis C virus (HCV) infection for the measurement of immunosurveillance potential and for the early detection of hepatocellular carcinoma (HCC) by investigating the Sendai virus (HVJ) stimulated IFN-α production capacity of patients with HCV ***: HVJ stimulated IFN-α production was determined in a large number of patients with HCV infection and the development of HCC was monitored for 3 years in patients with liver cirrhosis (LC).RESULTS: IFN-α production capacity decreases gradually with the progression of liver disease from chronic hepatitis (CH) to HCC. A significant correlation between the duration of HCV infection and impaired IFN-α production capacity was observed. IFN-α production in patients who developed HCC within 3 years was significantly lower than that of patients who remained in LC without developing ***: Measurement of IFN-α production in LC patients may be useful for the early detection of HCC.

Pilot Study on Gas Patterns of Irritable Bowel Syndrome and Small Intestinal Bacterial Overgrowth Following Ingestion of Lactulose

Background & Aims: Fermentation site and increasing time of symptomatic gas would be different between irritable bowel syndrome (IBS) and small intestine bacterial overgrowth (SIBO). The aim of this study was to determine the time for increase in abdominal gas following ingestion of lactulose and the possibility of differential diagnosis of SIBO and IBS. Methods: A prospective study was conducted on a series of IBS patients (n = 14) who were referred to the Oroku-Hospital (Okinawa, Japan) from June 6, 2014 to December 30, 2014. Imaging was first performed in early morning after fasting. After ingestion of lactulose, 1 - 4 plain abdominal radiographs were taken for investigation of increased gas during the indicated timeframe. Regions of interest of the gas areas were highlighted on the images obtained. Gas images were divided into three areas, the stomach, small intestine, and large intestine, and each total area was calculated. Results: At a dose of 10 g lactulose and an observation time of two hours, patients displayed no symptoms, and the gas volume was only slightly increased. However, when the dose of lactulose was increased (13 g/50 kg), and the observation time for the lactulose challenge was extended to 240 - 300 minutes, the results clearly demonstrated an increase in the gas produced in IBS patients. Conclusions: An increased dose of lactulose coupled with an extended observation time for the lactulose challenge clearly demonstrated an increase in the gas produced in IBS patients. Alterations in diet rather than antibiotics might reduce IBS symptoms.

To improve several agronomic traits in crisphead lettuce(Lactuca sativa L.)under high-temperature growth conditions,we investigated the correlation among those traits in multiple cultivars and performed genetic mapping of their causal *** a field cultivation test of Empire type(serrated leaf)and Salinas type(wavy leaf)cultivars,Empire type cultivars showed increased tipburn susceptibility and late bolting compared with Salinas type *** attempted genetic mapping of leaf shape and bolting time by ddRAD-seq using the F2 population derived from a cross between‘VI185’(Empire type)and‘ShinanoGreen’(Salinas type).These analyses suggested that both traits are controlled by a single locus in *** of 51 commercial lettuce cultivars with a tightly linked marker(LG5_v8_252.743Mbp)at this locus showed an association between its genotype and the serrated leaf *** further fine mapping and transcriptome analysis,a gene encoding putative CIN-like TCP transcription factor was determined to be a candidate gene at this locus and was designated as *** insertion of retrotransposable element was found in the allele of‘VI185’,and its transcript level in the leaves was lower than that in‘ShinanoGreen’.Because shapes of leaf epidermal cells in‘VI185’were similar to those in the TCP family mutant of Arabidopsis thaliana,the leaf shape phenotype was likely caused by reduced expression of ***,because it is known that the TCP family protein also controls flowering time via interaction with FT in ***,it was highly possible that LsTCP4 gave pleiotropic effects on both leaf shape and bolting time in lettuce.

This study provided foundational data for use in nursing interventional methods for improving nurse-patient relationships. This was a descriptive study on the effectiveness of a Japanese tea ceremony (in Japanese: chado) intervention for improving nurse-patient relationships. I conducted a Japanese tea ceremony and examined changes in nurses’ awareness regarding interactions with patients after this intervention. The tea ceremonies were conducted with the cooperation of an Urasenke tea ceremony lecturer. A quiet environment with chairs and tables was provided for all participants while they provided written answers to a descriptive survey, which was administered before and after the intervention;they required approximately 20 min to complete the survey. The mean length of each nurse’s description was 800 characters. The tea ceremony was effective in bringing about definite changes in nurses’ awareness concerning interactions with patients. This study is useful in that it suggests how nurses can maintain good interpersonal relationships with patients.

Role of Anthropogenic Volatile Organic Compounds and Nitrogen Oxides as Ozone Precursors in the Wintertime over East Asia

As an exercise in model sensitivity, the Models-3 Community Multiscale Air Quality (CMAQ) modeling system with meteorological fields from the Regional Atmospheric Modeling System (RAMS) was applied to study the response of photochemical oxidants to systematic increases in anthropogenic volatile organic compounds (VOC) and nitrogen oxides (NO x) emissions in January 1997 over East Asia. Three simulations-one base case and two sensitivity simulations were carried out. Two sensitivity simulations were performed by assuming a 35% separate increase in anthropogenic VOC and NO x emissions comparing with the base case. Biogenic VOC emissions were held constant across the sensitivity simulations. To evaluate the model performance, ozone mixing ratios (O 3) from the base case simulation were compared with surface observations at five remote sites in Japan, and it was found that the model reproduces most of the important features in the observations. Monthly average O 3 concentrations in the daytime were examined to gain an understanding of how the increase in anthropogenic emissions affected the overall chemical system for each sensitivity simulation.

Replacement of Unesthetic Posterior Metal Crowns with Monolithic Zirconia Crowns: A Case Report

Advances in metal-free materials and the popularization of Computer-Aided Design and Manufacturing (CAD/CAM) have led to the wide clinical use of all-ceramic crowns for esthetic restorations. A 72-year-old woman presented to our hospital with unesthetic restorations on the right upper and lower posterior teeth. Intraoral examination revealed poorly fitting metal crown margins. Defective prostheses were removed, and provisional restorations were provided to stabilize the mandibular position. Optical impressions and the maxillomandibular relationship were recorded using an intraoral scanner, and monolithic zirconia crowns were fabricated using CAD/CAM technology for complete veneer crown restorative treatment. Occlusal examination revealed an improvement in occlusal force distribution at initial examination (right side: 33.5%, left side: 66.5%) after placement of the zirconia crowns (right side: 54.9%, left side: 45.1%). Occlusal force and occlusal force distribution area also showed an increasing trend. The Oral Health Impact Profile short form (OHIP-14) score decreased from 7 points at initial examination to 0 points after prosthodontic treatment. Appropriate diagnosis and treatment planning contributed to the increased occlusal force and balanced occlusal force distribution. Therefore, the present case indicates the potential of monolithic zirconia crowns to achieve both esthetic and stable functional outcomes.
