Sister Mary Joseph’s nodule(SMJN)is a rare umbilical nodule that develops secondary to metastatic *** malignancies are located in the abdomen or *** with SMJN have a poor *** 83-year-old woman presented to our hospital with a1-month history of a rapidly enlarging umbilical *** findings revealed advanced transverse colon *** tomography and fluorodeoxyglucose-positron emission tomography revealed tumors of the transverse colon,umbilicus,right inguinal lymph nodes,and left *** feeding arteries and drainage veins for the SMJN were the inferior epigastric *** findings of the left lung tumor allowed for identification of the primary lung cancer,and a diagnosis of advanced transverse colon cancer with SMJN and primary lung cancer was *** patient underwent local resection of the SMNJ and subsequent single-site laparoscopic surgery involving right hemicolectomy and paracolic lymph node ***-abdominal dissemination to the mesocolon was confirmed during ***,the transverse colon cancer was confirmed to be moderately differentiated tubular *** suspect that SMJN may occur via a hematogenous *** chemotherapy for colon cancer and thoracoscopic surgery for the primary lung cancer were scheduled,the patient and her family desired home *** months after surgery,she died of rapidly growing lung cancer.

Role of STAT3 in inflammatory bowel disease

Signal transducers and activators of transcription 3 (STAT3) play an important role in various autoimmune disorders including inflammatory bowel disease (IBD). Recent studies have revealed that STAT3 activation plays distinctly different roles between innate immune responses and acquired immune responses in colitis. STAT3-mediated activation of acquired immune re-sponses plays a pathogenic role in colitis by enhancing the survival of pathogenic T cells. In contrast, STAT3-mediated activation of innate responses contributes to the suppression of colitis. This review will summarize the current understanding of the roles of STAT3 in IBD and the potential of targeting STAT3 for the treatment of IBD, emphasizing recent observations.

Evaluation of Important Factors Affecting Quench Distortion of Carburized Hypoid Gear with Shaft by Using Computer Simulation Methods

To minimize quenching distortion and dispersion, carburizing and quenching process conditions must be optimized; this includes the parts racking design used for quenching. We investigated some factors affecting carburized quenching distortion with an experiment using a hypoid gear having a shaft and with numerical simulation methods. The experimental results and those obtained from simulation were generally in agreement. Focusing on the surface temperature distribution in the gear, we studied quenching distortion characteristics in terms of changes in tooth profile and helix deviation. In our experiments, distortions occur during quenching in 373 K oil after austenitized temperature treatments conducted with various attitudes. We calculated the distortions by simulating the carburized oil-quenching process for the hypoid gear. Our results show large differences between the cooling rates of the tooth toe, middle section, and heel edges, and these greatly influence the change in tooth profile and helix deviation. We found that reducing the differences in temperatures on the gear surfaces during quenching is most important for minimizing the quench distortion of the hypoid gear.

A Simple Method for Synthesizing Ultra-high-molecular-weight Polystyrene through Emulsion Polymerization Using Alkyl-9-BBN as an Initiator

Styrene emulsion polymerization using an alkyl-9-BBN, synthesized by reacting 9-BBN（9-borabicyclo-[3.3.1]-nonane） and styrene, in an aqueous sodium dodecyl sulfate（SDS） solution was studied. Ultra-high-molecular-weight（〉 1.0 × 10~7） polystyrene was synthesized using a radical initiator formed through the aerobic oxidation of this alkyl-9-BBN in a high yield（〉 80%）. The kinetics of this emulsion polymerization of styrene with the alkyl-9-BBN was investigated. We confirmed that in the initial stage of the polymerization, the initial reaction rate followed first-order kinetics. The activation energy for this emulsion polymerization of styrene was approximately 56.2 kJ/mol.

On the other hand, the issue of overtreatment is emerging as a serious problem. Active surveillance （AS） should be more prevalent than now to avoid overtreatment for patients with favourablerisk prostate cancer （PCa） detected by PSA test.

Efficacy of tailored Helicobacter pylori eradication therapy based on antibiotic susceptibility and CYP2C19 genotype

The cure rates of Helicobacter pylori (H. pylori) eradication therapy using a proton pump inhibitor (PPI) and antimicrobial agents such as amoxicillin, clarithromycin, and metronidazole are mainly influenced by bacterial susceptibility to antimicrobial agents and the magnitude of the inhibition of acid secretion. Annual cure rates have gradually decreased because of the increased prevalence of H. pylori strains resistant to antimicrobial agents, especially to clarithromycin. Alternative regimens have therefore been developed incorporating different antimicrobial agents. Further, standard PPI therapy (twice-daily dosing) often fails to induce a long-term increase in intragastric pH > 4.0. Increasing the eradication rate requires more frequent and higher doses of PPIs. Therapeutic efficacy related to acid secretion is influenced by genetic factors such as variants of the genes encoding drug-metabolizing enzymes (e.g., cytochrome P450 2C19, CYP2C19), drug transporters (e.g., multidrug resistance protein-1; ABCB1), and inflammatory cytokines (e.g., interleukin-1β). For example, quadruple daily administration of PPI therapy potently inhibits acid secretion within 24 h, irrespective of CYP2C19 genotype. Therefore, tailored H. pylori eradication regimens that address acid secretion and employ optimal antimicrobial agents based on results of antimicrobial agent-susceptibility testing may prove effective in attaining higher eradication rates.

Colonoscopy techniques combining or replacing air insufflation with water infusion are becoming *** were originally designed to reduce colonic spasms,facilitate cecal intubation,and lower patient discomfort and the need for *** maneuvers straighten the rectosigmoid colon and enable the colonoscope to be inserted deeply without causing looping of the ***-immersion colonoscopy minimizes colonic distension and improves visibility by introducing a small amount of *** addition,since pain during colonoscopy indicates risk of bowel perforation and sedation masks this important warning,this method has the potential to be the favored insertion technique because it promotes patient safety without ***,this water-immersion method has not only been used for colonoscope insertion,but has also been applied to therapy for sigmoid volvulus,removal of lesions,lower gastrointestinal bleeding,and therapeutic diagnosis of abnormal bowel morphology and irritable bowel *** a larger sample size and prospective head-to-head-designed studies will be needed,this review focuses on the usefulness of waterimmersion colonoscopy for diagnostic and therapeutic applications.

A method for estimating the fraction of mineral dust in particulate matter using PM2.5-to-PM10 ratios

A simple method for estimating the contributions of mineral dust to PM2.5, PM10, or TSP is presented. The method is based on the assumption of external mixing of two types of particles with different PM2.5/PM10 ratios, The method was applied to local and transported dust events observed in Tsukuba, Japan, and was compared with collocated polarization lidar measurements. The method was then applied to three dust events that occurred in Oki, Rishiri, and Ochiishi,Japan, in 2012. The results showed that the method was useful for detecting mineral dust and for qualitatively describing the mixing of dust with anthropogenic aerosols.

Influence of interleukin polymorphisms on development of gastric cancer and peptic ulcer

Pro-inflammatory cytokines are produced in the gastric mucosa by inflammatory cells activated by chronic Helicobacter pylori (H. pylori) infection. Polymorphisms of these cytokine genes are associated with individual differences in gastric mucosal cytokine mRNA level, which result in differences in gastric mucosal inflammation, acid inhibition and gastroduodenal disease risk in response to H. pylori infection. Although polymorphisms of interleukin (IL)-1B, IL-1RN and TNF-A have been reported to relate well with gastric cancer and peptic ulcer risk, those of IL-2, IL-4, IL-6 and IL-8 genes are unclear. In combined analyses using data from previous studies, we found that the risk of gastric non-cardia cancer development was significantly associated with IL-4-168 C allele (OR: 0.81, 95% CI: 0.69-1.00) and IL-4-590 T allele carrier status (0.61, 0.53-0.73), and IL-6-174 G/G genotype (2Pro-inflammatory cytokines are produced in the gastric mucosa by inflammatory cells activated by chronic Helicobacter pylori (H. pylori) infection. Provenance Description of Metadata Vocabularies for the Long-term Maintenance of Metadata

Purpose： The purpose of this paper is to discuss provenance description of metadata terms and metadata vocabularies as a set of metadata terms. Provenance is crucial information to keep track of changes of metadata terms and metadata vocabularies for their consistent maintenance. Design/methodology/approach： The W3C PROV standard for general provenance description and Resource Description Framework（RDF） are adopted as the base models to formally define provenance description for metadata ***： This paper defines a few primitive change types of metadata terms, and a provenance description model of the metadata terms based on the primitive change types. We also provide examples of provenance description in RDF graphs to show the proposed *** limitations： The model proposed in this paper is defined based on a few primitive relationships（e.g. addition, deletion, and replacement） between pre-version and post-version of a metadata term. The model is simplified and the practical changes of metadata terms can be more complicated than the primitive relationships discussed in the model. Practical implications： Formal provenance description of metadata vocabularies can improve maintainability of metadata vocabularies over time. Conventional maintenance of metadata terms is the maintenance of documents of terms. The proposed model enables effective and automated tracking of change history of metadata vocabularies using simple formal description scheme defined based on widely-used ***/value： Changes in metadata vocabularies may cause inconsistencies in the longterm use of metadata. This paper proposes a simple and formal scheme of provenance description of metadata vocabularies. The proposed model works as the basis of automated maintenance of metadata terms and their vocabularies and is applicable to various types of changes.
