Proton pump inhibitor step-down therapy for GERD:A multi-center study in Japan

AIM:To investigate the predictors of success in stepdown of proton pump inhibitor and to assess the quality of life(QOL).METHODS:Patients who had heartburn twice a week or more were treated with 20 mg omeprazole(OPZ) once daily for 8 wk as an initial therapy(study 1).Patients whose heartburn decreased to once a week or less at the end of the initial therapy were enrolled in study 2 and treated with 10 mg OPZ as maintenance therapy for an additional 6 mo(study 2).QOL was in-vestigated using the gastrointestinal symptom rating scale(GSRS)before initial therapy,after both 4 and 8 wk of initial therapy,and at 1,2,3,and 6 mo after starting maintenance ***:In study 1,108 patients were *** characteristics were as follows;median age:63(range: 20-88)years,sex:46 women and 62 *** success rate of the initial therapy was 76%.In the patients with successful initial therapy,abdominal pain,indigestion and reflux GSRS scores were *** study 2,83 patients were *** of 83 patients completed the study 2 *** the per-protocol analysis,80%of 70 patients were successful for *** multivariate analysis of baseline demographic data and clinical information,no previous treatment for gastroesophageal reflux disease(GERD)[odds ratio (OR)0.255,95%CI:0.06-0.98]and a lower indigestion score in GSRS at the beginning of step-down therapy(OR 0.214,95%CI:0.06-0.73)were found to be the predictors of successful step-down *** improved GSRS scores by initial therapy were maintained through the step-down ***:OPZ was effective for most GERD ***,those who have had previous treatment for GERD and experience dyspepsia before stepdown require particular monitoring for relapse.

Effect of BNT162b2 mRNA COVID-19 vaccine on sperm morphokinetics and DNA integrity: A prospective observational study in Japan

Objective:To assess whether the coronavirus disease 2019(COVID-19)mRNA vaccine affects sperm morphokinetics using a computer-assisted semen analyzer and other semen parameters using a sperm chromatin structure ***:Healthy male volunteers in two Japanese clinics between May 2021 and December 2021 were prospectively *** donated sperm twice,two days apart,in the following phases:before vaccination,2 weeks after the first vaccine dose,and 2,4,and 12 weeks after the second *** sperm parameters,sperm motility characteristics,and the percentage of DNA-damaged sperm were compared among the different ***:Ninety-six semen samples from ten volunteers,who were vaccinated with the BNT162b2 mRNA vaccine,were *** were no significant differences between any phases in basic semen findings and parameters of the sperm chromatin structure *** sperm motion characteristics,the average linear velocity,beat-cross frequency,and sperm motility index significantly decreased after the second vaccine dose(P=0.018,P=0.003,and P=0.027,respectively),with no significant differences between any two phases by post-hoc pairwise ***:After COVID-19 mRNA vaccination,while sperm motion characteristics might fluctuate,no apparent deterioration of basic sperm parameters or sperm DNA integrity was *** the adverse effects of COVID-19 on sperm,our findings suggest that there might be no reason to refrain from vaccination for healthy individuals.

In this paper we propose a COncurrent Production Engineering System （COPES） for the flexible transfer line （FTL） layout design in a restricted area. COPES first determines the buffer size in front of the bay of each machine tool in the FTL and then initializes a computer aided design （CAD） system to draw the FTL in a restricted area. We develop a set of modules systems which have been integrated into a single framework, in accordance with the practice of concurrent engineering. Concurrent engineering involves the cooperation of these activities. It＇s expected that the developed COPES can improve the cooperation between production engineers＇ and the plant designer. This can be done by enabling the production engineers＇ to make better decision regarding FTL buffer size.

AIM: To investigate whether semi-vegetarian diet (SVD) has a preventive effect against relapse of Crohn’s disease (CD) in patients who have achieved remission,who are a high-risk group for ***: A prospective,single center,2-year clinical trial was ***-two adult CD patients who achieved clinical remission either medically (n = 17) or surgically (n = 5) and consumed an SVD during hospitalization were advised to continue with an SVD and avoid known high-risk foods for inflammatory bowel *** primary endpoint was clinical relapse defi ned as the appearance of active symptoms of ***-Meier survival analysis was used to calculate the cumulative proportion of patients who had a relapse.A 2-year analysis of relapse rates of patients who followed an SVD and those who did not (an omnivorous diet group) was ***: SVD was continued by 16 patients (compliance 73%).Remission was maintained in 15 of 16 patients (94%) in the SVD group vs two of six (33%)in the omnivorous *** rate with SVD was 100% at 1 year and 92% at 2 *** showed signif icant prevention in the time to relapse compared to that in the omnivorous group (P = 0.0003,log rank test).The concentration of C-reactive protein was normal at the f inal visit in more than half of the patients in remission who were taking an SVD,who maintained remission during the study (9/15;60%),who terminated follow-up (8/12;67%),and who completed 2 years follow-up (7/10;70%).There was no untoward effect of ***: SVD was highly effective in preventing relapse in CD.

Lansoprazole-associated collagenous colitis:Diffuse mucosal cloudiness mimicking ulcerative colitis

There have only been a few reports on lansoprazole-associated collagenous colitis. Colonic mucosa of collagenous colitis is known to be endoscopically normal. We present a case of collagenous colitis where the mucosa showed diffuse cloudiness mimicking ulcerative colitis. A 70-year-old woman developed watery diarrhea four to nine times a day. She had interstitial pneumonia at 67 and reflux esophagitis at 70 years. Lansoprazole 30 mg/d had been prescribed for reflux esophagitis for nearly 6 mo. Lansoprazole was withdrawn due to its possible side effect of diarrhea. Colonoscopy disclosed diffuse cloudiness of the mucosa which suggested ulcerative colitis. Consequently sulfasalazine 2 g/d was started. The patient's diarrhea dramatically disappeared on the following day. However, biopsy specimens showed subepithelial collagenous thickening and infi ltration of inflammatory cells in the lamina propria, confirming the diagnosis of collagenous colitis. One month after sulfasalazine therapy was initiated, colonoscopic and histological abnormalities resolved completely. Five months later the diarrhea recurred. The findings on colonoscopy and histology were the same as before, confirming a diagnosis of collagenous colitis relapse. We found that the patient had begun to take lansoprazole again 3 mo ahead of the recent diarrhea. Withdrawal of lansoprazole promptly resolved the diarrhea. Endoscopic and histological abnormalities were also completely resolved, similar to the first episode. Retrospectively, the date of commencement of sulfasalazine and discontinuation of lansoprazole in the first episode was found to be the same. We conclude that this patient had lansoprazole-associated collagenous colitis.

我们从南朝鲜七个有代表性的金矿床中挑选十块标本进行K-Ar年龄测定.从与金井矿床含金白岗岩脉有关的伟晶岩中分离出来的微斜长石测出的年龄为晚元古代.从天安地区浩美、天宝和南仓三矿床的矿石和有关的伟晶岩中采集的白云母的年龄为130～160百万年,进一步证实了该区的金矿化时代为中侏罗-晚侏罗世.无极、林川和居昌三矿床绢云母的测定结果为100百万年左右.这些成果加上从文献中收集到的数据表明南朝鲜在目前剥蚀面上出露的大部分贵金属、贱金属和重金属矿床形成于白垩纪时期,与Bulgugsa长英质岩浆活动有成因联系.

Granitic rocks related to the formation of Haobugao Zn-Pb-Cu-Sn skarn deposit,Inner Mongolia,China,show unusual low whole-rockδO values down to -8.8‰(V-SMOW),whereas separated quartz crystals from those rocks give positiveδO values of +4.1 to +9.9‰.Chemical analyses and microscopic observation of those granitic rocks confirm that they suffered hydrothermal *** skarn specimens and quartz from the Haobugao deposit also show negativeδO *** isotopic evidence indicates that intensive meteoric water circulation occurred at the time of granitic intrusion, and caused the pervasive hydrothermal alteration of granitic rocks and the precipitation of skarn deposit in this area.
