The details of the shock propagated through the right angled multiple elbows (mainly double elbow and fourfold elbow) are investigated by the numerical study. Computations were carried out by solving the two-dimensional compressible Navier-Stokes equations by using the total variation diminishing (TVD) scheme. Computations were performed for six types of elbow (two types of double elbow and four types of fourfold elbow) and two incident shock Mach numbers (Ms=1.5, 2.3). The effects of the direction and number of the bends in the stabilizing, merging and attenuating processes of the shock are numerically explored by various diagrams such as pressure contours, pressure distributions on the walls and time histories of the pressure contours, and the strength of the shock front at the location of about 20 times the length for each type of elbows was displayed.

Background: Ovarian clear cell carcinoma (CCC) is often diagnosed at stage I. However, because of the poor prognosis of recurrent cases, even for stage Ia CCC, treatment strategies such as expansion of fertility-sparing treatment and omission of adjuvant chemotherapy have been carefully discussed in recent years. We previously reported the possibility of the maximum standardized uptake value (SUVmax) as a biomarker of CCC prognosis prediction at all stages. In this study, we confirmed differences in SUVmax within stage I CCC and considered treatment strategies. Methods: We selected all 31 patients with ovarian CCC stage I who underwent fluorodeoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) before treatment between 2006 and 2013 at our institution. This retrospective study was based on their medical records. Results: Clinical tumor stage was Ia in 13 patients, and Ic in 18 (Ic (b) in 11, and Ic (1) + Ic (2) in seven). There were no differences in serum CA125 level, maximum tumor diameter or mural nodules. Median SUVmax was significantly higher in stage Ic (5.87) than stage Ia (3.08) cases (P = 0.02). Progression-free survival was longer in the low SUVmax group than the high SUVmax group (P = 0.08). Conclusions: SUVmax for primary lesions in CCC was significantly higher in stage Ic than stage Ia. As SUVmax represents a prognostic factor in stage I CCC, these findings may suggest SUVmax as an indicator for the application of fertility-sparing surgery and omission of adjuvant chemotherapy for stage Ia CCC.

For patients with epidermal growth factor receptor (EGFR) mutation-positive lung cancer, the relationship between the dose or duration of treatment with tyrosine kinase inhibitor (TKI) and overall survival remains unclear. Here, we analyzed clinical data of 39 patients who were diagnosed with EGFR mutation-positive non-small cell lung cancer and treated with TKI, but subsequently died. Several parameters were measured in this study: overall survival; first, second, and overall TKI therapy durations; first TKI intensity (actual dose/normal dose); and TKI rate (overall TKI therapy duration/overall survival). The response rate to TKI therapy was 50% , and the median survival was 553 days. After TKI therapy failed, 38.5% patients were re-challenged with TKI. We observed a moderate relationship [r = 0.534, 95% confidential interval (CI) = 0.263 to 0.727, P < 0.001] between overall TKI therapy duration and overall survival. However, we found no relationship between overall survival and first TKI intensity (r = 0.073, 95% CI = -0.380 to 0.247, P = 0.657) or TKI rate (r = 0.0345, 95% CI = -0.284 to 0.346, P = 0.835). Nonsmall cell lung cancer patients with mutation-positive tumors remained on TKI therapy for, on average, 33% of the overall survival time. These findings suggest that patients with EGFR mutation-positive tumors should not stick to using TKIs.

α-Synuclein is the major component of the filamentous Lewy bodies and Lewy neurites that define neuropathological features and dementia with Lewy bodies. To investigate the role of dopamine (DA) in α-synuclein fibrillation, the structural propensities to form oligomers at the initial stage fibrillation were studied using size exclusion chromatography and various biophysical techniques. Interactions with DA were observed for wild-type α-synuclein and its mutants, A30P, E46K and A53T, using electrospray ionization mass spectrometry (ESI-MS). The results of ESI-MS indicate that an intact α-synuclein, which was not oxidized, had an ability to bind with three molecules of DA at the initial stage. Furthermore, upon binding to DA, α-synuclein oligomerizes to higher molecular weight species. These oligomers are structurally different from amyloid fibrils, as confirmed by thioflavin T and CD analysis.

Background: As the prognosis of lung cancer (LC) patients improves, subcutaneously implanted central venous access device ports (CV-ports) have frequently been used for continuing chemotherapy (CC) or palliative care (PC). In this study, we examined the clinical course of LC patients with subcutaneously implanted CV-ports from the time of receiving chemotherapy to the endpoint of cancer. Materials and Methods: We retrospectively reviewed the clinical data and treatment history of LC patients with subcutaneously implanted CV-ports between June 2008 and November 2013 using clinical records and a pharmacy database. Results: Of the 132 LC patients with subcutaneously implanted CV-ports, 79 (59.8%) had CV-ports for CC (the CC group) and 53 (40.2%) had CV-ports for PC (the PC group). After CV-port implantation, LC patients in the CC group received a median of two regimens with a median of 6 cycles. The median survival time of patients in the CC and PC groups was 457 and 44 days, respectively. In the CC group, the median survival time of small cell and non-small cell LC patients was 342 (95% confidence interval, 235 - 627) and 563 (95% confidence interval, 368 - 728) days, respectively. Nine patients (6.8%) had their CV-ports removed due to complications. Forty (30.3%) of the 132 enrolled patients were referred for at-home PC. The at-home death rate observed among these 40 patients was 30.0% (N = 12). Conclusion: CV-ports may contribute to seamless oncological care.
