Feasibility of gastric endoscopic submucosal dissection with continuous low-dose aspirin for patients receiving dual antiplatelet therapy

BACKGROUND Endoscopic submucosal dissection(ESD) for gastric neoplasms during continuous low-dose aspirin(LDA) administration is generally acceptable according to recent guidelines. This retrospective study aimed to investigate the effect of continuous LDA on the postoperative bleeding after gastric ESD in patients receiving dual antiplatelet therapy(DAPT).AIM To investigate the feasibility of gastric ESD with continuous LDA in patients with *** A total of 597 patients with gastric neoplasms treated with ESD between January2010 and June 2017 were enrolled. The patients were categorized according to type of antiplatelet therapy(APT).RESULTS The postoperative bleeding rate was 6.9%(41/597) in all patients. Patients were divided into the following two groups: no APT(n = 443) and APT(n = 154). APT included single-LDA(n = 95) and DAPT(LDA plus clopidogrel, n = 59)subgroups. In the single-LDA and DAPT subgroups, 56 and 39 patients were received continuous LDA, respectively. The bleeding rate with continuous singleLDA(10.7%) was similar to that with discontinuous single-LDA(10.3%)(P >0.99). Although the bleeding rate with continuous LDA in patients receiving DAPT(23.1%) was higher than that with discontinuous LDA in patients receiving DAPT(5.0%), no significant difference was observed(P = 0.141).CONCLUSION The bleeding rate with continuous LDA in patients receiving DAPT was not statistically different from that with discontinuous LDA in patients receiving DAPT. Therefore, continuous LDA administration may be acceptable for ESD in patients receiving DAPT, although patients should be carefully monitored for possible bleeding.

Primary intraosseous squamous cell carcinoma of the mandible

Purpose: Intraosseous carcinoma was defined as a squamous cell carcinoma arising within the mandible without connection to the oral mucosa。We analyzed clinicopathological findings of intraosseous squamous cell carcinoma of mandible。Patients: There were 13 patients who visited our department from April 2001 to November 2007。They were all males who ranged in age from 34 to 75 years。Eight patients had no lymph node metastasis, and 5 patients had lymph node metastasis。Distant metastasis was not found in all patients。Results: The chief complaint was swelling of mandible in 8 patients, and incomplete healing of extraction socket in 5 patients。Surgery underwent in 5 patients, surgery with radiation therapy in 3 patients, and surgery with radiation and chemotherapy in 5 patients。As surgical treatment for the primary lesion, segmental mandibulectomy was performed in 8 patients and hemimandibulectomy in 5 patients。In 2 patients, excision of skin was carried out in combination with mandibulectomy。The surgical defect was reconstructed with scapular osteocutaneous flap in 7 patients, fibular osteocutaneous flap in one patient and rectus abdominis myocutaneous flap in 5 patients。Ten patients were alive with no evidence of disease, one patient were alive with the disease and two patients died of the disease。Histopathologically, the carcinoma involved the mandibular canal in l0 cases。The carcinoma invaded the muscles around the mandible in 7 cases。Cervical lymph node metastasis was confirmed in 5 of 7 cases in which the carcinoma invaded the muscles。Conclusions: These results suggest that invasion of intraosseous carcinoma to the muscles around mandible is a possible factor of the cervical lymph node metastasis。

Endoscopic submucosal dissection for small submucosal tumors of the rectum compared with endoscopic submucosal resection with a ligation device

AIMTo evaluate the efficacy and safety of endoscopic submucosal dissection (ESD) for small rectal submucosal tumors (SMTs). METHODSBetween August 2008 and March 2016, 39 patients were treated with endoscopic submucosal resection with a ligation device (ESMR-L) (n = 21) or ESD (n = 18) for small rectal SMTs in this study. Twenty-five lesions were confirmed by histological evaluation of endoscopic biopsy prior to the procedure, and 14 lesions were not evaluated by endoscopic biopsy. The results for the ESMR-L group and the ESD group were retrospectively compared, including baseline characteristics and therapeutic outcomes. RESULTSThe rate of en bloc resection was 100% in both groups. Although the rate of complete endoscopic resection was higher in the ESD group than in the ESMR-L group (100% vs 95.2%), there were no significant differences between the two groups (P = 0.462). In one patient in the ESMR-L group with a previously biopsied tumor, histological complete resection with a vertical margin involvement of carcinoid tumor could not be achieved, whereas there was no incomplete resection in the ESD group. The mean length of the procedure was significantly greater in the ESD group than in the ESMR-L group (14.7 ± 6.4 min vs 5.4 ± 1.7 min, P vs 2.8 ± 1.5 d, P CONCLUSIONBoth ESMR-L and ESD were effective for treatment of small rectal SMTs. ESMR-L was simpler to perform than ESD and took less time.

Low skeletal muscle mass and high visceral adiposity are associated with recurrence of acute cholecystitis after conservative management:A propensity score-matched cohort study

Nitrogen-based fertilizers differentially affect protist community composition in paddy field soils

Nitrogen fertilizers' effects on protists in three paddy field soils were *** nitrogen fertilizers had distinctive effects on the protist *** effect of nitrogen fertilizers on protist communities slightly depended on the soil *** protists were the main groups that were affected by nitrogen *** are one of the most diverse and dominant microbial groups and they play critical roles in the soil ecosystem. Although nitrogen fertilizers have a profound impact on protist communities, still less is known about how different nitrogen fertilizer types affect protist community composition in different soil types. Here we investigated the effects of six inorganic nitrogen fertilizers (urea, ammonium nitrate, ammonium sulfate, potassium nitrate, ammonium chloride, and diammonium hydrogen phosphate) and an organic fertilizer (a mixture of rice husk and cow manure) on protist community composition in three paddy field soils using a high-throughput sequencing method. The effect of the fertilizers on the functional groups of protists, namely consumers (predators and decomposers), photoautotrophs, and parasites (plant pathogens and animal parasites) was also analyzed. The results showed that nitrogen fertilizers had distinctive effects on the beta diversity of the protists, while we also observed that the same fertilizer had slightly different effects depending on the soil type. Amoebozoa and Rhizaria were the most affected protist taxonomical groups, while predatory protists were the main functional groups that were affected by nitrogen fertilizers. Random forest analysis showed that most of the fertilizer-affected protists were predators, among which Cercozoa was the most affected taxa. In conclusion, our results provide important insights into the impact of nitrogen fertilizers on soil protist communities.

Correlation between dose-volume parameters and rectal bleeding after 12 fractions of carbon ion radiotherapy for prostate cancer

BACKGROUND Carbon ion radiotherapy(CIRT)is currently used to treat prostate *** bleeding is a major cause of toxicity even with ***,to date,a correlation between the dose and volume parameters of the 12 fractions of CIRT for prostate cancer and rectal bleeding has not been ***,the clinical risk factors for rectal bleeding were absent after 12 fractions of *** To identify the risk factors for rectal bleeding in 12 fractions of CIRT for prostate *** Among 259 patients who received 51.6 Gy[relative biological effectiveness(RBE)],in 12 fractions of CIRT,15 had grade 1(5.8%)and nine had grade 2 rectal bleeding(3.5%).The dose-volume parameters included the volume(cc)of the rectum irradiated with at least x Gy(RBE)(Vx)and the minimum dose in the most irradiated x cc normal rectal volume(Dx).RESULTS The mean values of D6cc,D2cc,V10 Gy(RBE),V20 Gy(RBE),V30 Gy(RBE),and V40 Gy(RBE)were significantly higher in the patients with rectal bleeding than in those *** cutoff values were D6cc=34.34 Gy(RBE),D2cc=46.46 Gy(RBE),V10 Gy(RBE)=9.85 cc,V20 Gy(RBE)=7.00 cc,V30 Gy(RBE)=6.91 cc,and V40 Gy(RBE)=4.26 *** D2cc,V10 Gy(RBE),and V20 Gy(RBE)cutoff values were significant predictors of grade 2 rectal *** The above dose-volume parameters may serve as guidelines for preventing rectal bleeding after 12 fractions of CIRT for prostate cancer.
