We shall give a simple （basically） the Igusa tower over Shimura varieties of PEL purely in characteristic p proof of the irreducibility of type. Our result covers Shimura variety of type A and type C classical groups, in particular, the Siegel modular varieties, the Hilbert-Siegel modular varieties, Picard surfaces and Shimura varieties of inner forms of unitary and symplectic groups over totally real fields.

Niemann-Pick disease (NPD) refers to a group of patients who have varying degrees of lipid storage and foam cell infiltration in tissues, as well as overlapping clinical features, including hepatosplenomegaly, insufficiency pulmonary and/or central nervous system (CNS). Thanks to the pioneering work of Roscoe Brady and colleagues, we now know that there are two distinct metabolic abnormalities that explain NPD. The first is due to the deficient activity of the acid sphingomyelinase enzyme (ASM;NPD “types A and B”), and the second is due to defective functioning in the transport of cholesterol (NPD “type C”). We report the case of a 13-year-old adolescent diagnosed with Niemann-Pick A/B disease.

Lemierre’s syndrome is a rare and severe sepsis, capable of rapidly compromising the vital prognosis in the absence of early intervention, characterized by an oropharyngeal infection complicated by septic thrombosis of the internal jugular vein and the formation of septic emboli primarily localized at the pulmonary level. We report the case of a 14-year-old boy, with no notable pathological history, who had presented for 15 days with unilateral orbital swelling which then became bilateral, all evolving in a context of fever.

The objective of our work is to study the multi-systemic inflammatory syndrome (PIMS) in children, to determine its frequency, by analyzing the epidemiological, clinical, paraclinical, therapeutic, and evolutionary profile of these patients. A retrospective study spanning a period of 2 years from April 2020 to March 2022. It concerns all children under the age of 16 admitted and cared for in the pediatric emergency department of the university hospital Hassan II of Fez for multi-system inflammatory syndrome (PIMS). Twenty cases of PIMS were collected over this period. Multi-system inflammatory syndrome in children has been described in temporal association with COVID-19, usually within 2 to 6 weeks of illness or exposure. The age of the patients varies between 8 months and 15 years. All patients presented with fever and cutaneous signs, followed by digestive signs and neurological signs. The inflammatory syndrome is frankly positive in all patients who had a COVID-19 PCR and/or positive serology. The treatment is based on the administration of immunoglobulins in association with corticosteroid therapy and non-specific antibiotic therapy in the majority of cases (80%). The evolution was favorable. PIMS should be considered in all children presenting with a clinical and/or biological inflammatory syndrome associated with COVID-19.

Active Tuberculosis in Children Receiving Chemotherapy

Tuberculosis infections is a serious illness and can be life threatening for patients with hematologic malignancies. We are reporting the observations of two children aged 7 and 5 years undergoing anticancer treatment for hematological malignancies. The symptoms were, in the first case, cough, purulent sputum and signs of tuberculosis impregnation, for the second case, cervical adenopathies (PDA) with inflammatory signs. The evolution under antituberculosis treatment was favorable for both patients, with a 5-year follow-up for the first patient and one year for the second case. The tuberculosis did not affect chemotherapy. The goal of our work is to specify the etio-pathogenic mechanisms, the clinical, therapeutic and evolutionary particularities of this rare association.

Erysipelas in Children: Risk Factors and Management in the Pediatric Emergency Department

The aim of this study is to describe the epidemiological, clinical, biological profile, etiology and management of erysipelas in pediatric emergency departments. In this retrospective study carried out over a period of 12 months, we identified 20 cases of erysipelas, with an incidence of 2%, and a female predominance of 65% of cases. Age average patient age was 6 years, and the average consultation time was 5 days. The Diagnosis of erysipelas is mainly based on clinical features, including the presence of erythema associated with inflammatory signs, which were present in all cases. Satellite lymphadenopathy was observed in five cases, and fever was present in all patients. The therapeutic approach is mainly based on the administration of injectable antibiotics in children, using amoxicillin-clavulanate dose of 80 mg/kg/day in three doses.

47XYY syndrome is a rare sex chromosome variant with an extra Y chromosome. Most patients with a 47XYY karyotype have a normal phenotype. This disorder seems to be associated with a higher risk of developing behavior and cognitive problems, tall stature and infertility in adulthood. We report here a rare case of 47, XYY syndrome associated with an 11-beta-hydroxylase deficiency revealed by a stature advance along with precocious puberty after obtaining informed consent from parents. To our knowledge, this is the first case reported in the literature.

Comparison Study between Ultra-Chopper and Divide &Conquer Techniques

Background: Hard brunescent nuclei are difficult to chop with traditional techniques, because leathery tough strands may connect the posterior surface and span across the fragments, which poses a challenge to completely divide the nuclei. The ultra-chopping technique was designed to mitigate this issue. Purpose: To compare the intraoperative parameters between Ultra-Chopper and Divide & Conquer Techniques. Setting: This study was performed at Hospital Oftalmológico de Brasília, Brasília, DF, Brazil. Patients and Methods: A prospective, randomized and comparative study. Patients with the diagnosis of dense cataract and surgical extraction programmed were divided into two groups: Ultra-Chopper and Divide & Conquer. Intraoperative data were collected and submitted for the statistical analysis. Results: 36 eyes were included, 19 eyes with Ultra-Chopper and 17 eyes with Divide & Conquer. Groups were statistically equivalent in age and nucleus density. There were no surgical complications. Torsional time and cumulative dissipated energy were significantly reduced in the ultra-chopper group. The ultra-chopper group had less total case time, fluid usage and aspiration (ASP) time. Conclusion: The ultra-chopper tech- nique can reduce ultrasound energy dissipation during cataract surgery, and decrease case time, fluid usage and ASP time.

AIM: To assess the efficacy of topical Semaphorin 3A(SEMA3A) in the treatment of allergic ***: Experimental allergic conjunctivitis(EAC)mice model induced by short ragweed pollen(SRW) in 4-week-old of BALB/c mice, mice were evaluated using haematoxylin and eosin(H&E) staining, immunofluor-escence and light microscope photographs. Early phase took the samples in 24 h after instillation and late phase took the samples between 4 to 14 d after the start of treatment. The study use of topical SEMA3A(10 U, 100 U,1000 U) eye drops and subconjunctival injection of SEMA3 A with same concentration. For comparison, five types of allergy eyedrops were quantified using clinical characteristics.· RESULTS: Clinical score of composite ocular symptoms of the mice treated with SEMA3 A were significantly decreased both in the immediate phase and the late phase compared to those treated with commercial ophthalmic formulations and non-treatment mice. SEMA3 A treatment attenuates infiltration of eosinophils entering into conjunctiva in EAC mice. The score of eosinophil infiltration in the conjunctiva of SEMA3 A 1000 U-treated group were significantly lower than low-concentration of SEMA3 A treated groups and non-treated group. SEMA3 A treatment also suppressed T-cell proliferation in vitro and decreased serum total Ig E levels in EAC mice. Moreover, treatment of SEMA3 A suppressed Th2-related cytokines(IL-5, IL-13 and IL-4)and pro-inflammatory cytokines(IFN-γ, IL-17 and TNF-α)release, but increased regulatory cytokine IL-10 concentration in the conjunctiva of EAC ***: SEMA3 A as a biological agent, showed the beneficial activity in ocular allergic processes with the less damage to the intraocular tissue. It is expected that SEMA3 A may be contributed in patients with a more severe spectrum of refractory ocular allergic diseases including allergic conjunctivitis in the near future.
