Bleeding from gastro-esophageal varices can often present as the first decompensating event in patients with cirrhosis. This can be a potentially life threatening event associated with a 15%-20% early mortality. We present a rare case of new onset ascites due to intra-abdominal hemorrhage from ruptured mesenteric varices; in a 37 years old male with newly diagnosed nonalcoholic steatohepatitis induced cirrhosis as the first decompensating event. The patient was successfully resuscitated with emergent evacuation of ascites for diagnosis, identification and control of bleeding mesenteric varices and eventually orthotopic liver transplantation with successful outcome. Various clinical presentations, available treatment options and outcomes of ectopic variceal bleeding are discussed in this report.

Acute cytomegalovirus(CMV) infection is a commonly encountered complication in the post liver transplant setting. We present a case of a 71-year-old male with acute CMV infection, initially presenting with a gastro- intestinal bleed due to acute CMV gastritis and later on complicated by acute venous thromboembolism occur- ring as an unprovoked event in the post liver transplant period. Traditional risk factors for venous thromboem- bolism have been well described in the medical litera- ture. Sporadic cases of thromboembolism due to CMV infection in the immune compromised patients have been described, especially in the post kidney transplant patients. Liver transplant recipients are equally prone to CMV infection particularly in the first year after suc- cessful transplantation. Venous thromboembolism in this special population is particularly challenging due to the fact that these patients may have persistent throm- bocytopenia and anticoagulation may be a challenge for the treating physician. Since liver transplantation is severely and universally limited by the availability of donor organs, we feel that this case report will provide valuable knowledge in the day to day management of these patients, whose clinical needs are complex and require a multidisciplinary approach in their care and management. Evidence and pathophysiology link- ing both the conditions is presented along with a brief discussion on the management, common scenarios en- countered and potential impact in this special group of patients.

Orthodontic treatment offers great advantages in improving facial and smile aesthetics, self-confidence and the function of the stomatognathic apparatus. The pursuit of these advantages makes use of orthodontic appliances that could be fixed or removable. However, it’s worth stating that these appliances interfere with tooth brushing, making it more difficult to brush teeth effectively. Orthodontics appliances therefore promote the accumulation of dental plaque, which results in both quantitative and qualitative changes in the oral microbiota, hence, exposing patients to several adverse effects such as White spot lesions, dental caries, periodontal pathologies and halitosis. For this reason, oral assessment of patients before, during and after treatment is necessary as well as oral hygiene instructions and motivation. Orthodontists therefore, should educate patients on oral and periodontal hygiene in order to control dental and periodontal complications. Prescriptions of plaque control materials adapted to each patient are done in order to optimize the final result and minimize unwanted complications.

Velamentous insertion of the umbilical cord corresponds to the insertion of the cord directly on amniotic membranes. It is a rare situation whose frequency varies from 0.5% to 1.69% of single pregnancies. It must be diagnosed during the morphological ultrasound of the 2nd trimester, actively looking for the association with a vasa previa, due to the risk of fetal haemorrhagic threat. We report an antenatal diagnosis of velamentous cord insertion and its management with literature review.

Extra Renal Rhabdoid Tumor: A Rare Cause of Congenital Soft Tissue Tumor

Rhabdoid tumors (RTs) are a well-defined entity in the kidney or central nervous system of infants or children. However, soft-tissue involvement is uncommon. It’s an exceptional neonatal tumor of soft tissue. The imaging characteristics of this tumor are not specific. Biopsy allows diagnosis;the histomorphological characteristics of rhabdoid tumors, their immunoreactivity to epithelial markers and vimentin, and the INI-1 loss are important tools for diagnosis. RT tumors are aggressive and have a rapidly fatal clinical course in most cases. Despite multidisciplinary therapy, the survival rate is very low. We report a rare case occurring in a male neonate who presents at birth with a voluminous right axillary mass. A CT scan showed a well-limited tumor mass with lobulated contours. An ultrasound-guided biopsy was performed on day 8, showing the morphology and immunoprofile of RT. The mass showed rapid growth. The child was admitted for respiratory distress at 3 weeks. A thoraco-abdominal CT showed an increase in the size of the mass with the appearance of multiple lymph nodes and pleural, hepatic, and renal metastases. The child died two days later.

In recent years,Deep Learning models have become indispensable in several fields such as computer vision,automatic object recognition,and automatic natural language processing.The implementation of a robust and efficient handwritten text recognition system remains a challenge for the research community in this field,especially for the Arabic language,which,compared to other languages,has a dearth of published works.In this work,we presented an efficient and new system for offline Arabic handwritten text recognition.Our new approach is based on the combination of a Convolutional Neural Network(CNN)and a Bidirectional Long-Term Memory(BLSTM)followed by a Connectionist Temporal Classification layer(CTC).Moreover,during the training phase of the model,we introduce an algorithm of data augmentation to increase the quality of data.Our proposed approach can recognize Arabic handwritten texts without the need to segment the characters,thus overcoming several problems related to this point.To train and test(evaluate)our approach,we used two Arabic handwritten text recognition databases,which are IFN/ENIT and KHATT.The Experimental results show that our new approach,compared to other methods in the literature,gives better results.

This paper presents a fuzzy adaptive sliding mode controller(FASMC)for electrically driven wheeled mobile robot for trajectory tracking task in the presence of uncertainties and disturbances.First,a finite-time kinematic controller is developed to compute the auxiliary velocity vector.Second,the FASMC,based on the nonlinear dynamic model of the robot and its actuators,is used to guarantee the stability and the convergence of the closed-loop system.Moreover,by employing the advantages of the fuzzy logic systems,the developed controller ensures the robustness of the system against dynamic disturbances and uncertainties,the smoothness of the computing voltage against the chattering phenomenon,and the optimal convergence of the velocity and posture errors.The Lyapunov theory is used to analyse the stability of this algorithm.In order to evaluate the effectiveness of the developed method,numerical simulations are done in the Mahlab/Simulink environment.

In this paper,an Improved Extended State Observer-based Finite-Time adaptive sliding mode control,is investigated for trajectory tracking control of a wheeled mobile robot.First,a novel finite-time adaptive sliding mode control,based on the fractional power of the sliding surface,is developed to deal with the chattering problem.Moreover,this strategy improves the conver-gence rate by adjusting online the switching part in sliding mode control.Second,an improved Non-linear ESO is employed to reconstruct and compensate for the unknown disturbances.To complete the trajectory tracking control,the kinematic algorithm is in troduced.Theoretically,the proposed control scheme converges within finite-time thanks to the Lyapunov method.Finally,numerical simulations show the efficiency of the designed controller.

In order to mitigate the risks stemming from the utilization of mineral and synthetic organic substances,consensus exists in the literature that additional efforts should be devoted to the identification of adequate equivalent natural(ecological)materials.This work presents the outcomes of a preliminary study where the physical,mechanical,chemical and thermal properties of natural fibers have been considered.Initially,areca,and materials such as Moroccan doum and jute are considered.As a case study,a brake pad based on natural fibers is critically assessed.

This paper proposes an advanced method for estimating numerous parameters in a wind-energy-conversion system with high precision,especially in a transient state,including the rotation speed and mechanical torque of the turbine as well as wind velocity.The suggested approach is designed into two parts.First,a fourth-order Luenberger observer is proposed to take into account the significant fluctuations of the mechanical torque that can be caused by wind gusts.This observer provides an accurate estimate of speed and mechanical torque in all weather conditions and especially when the wind is gusting.At the same time,the wind velocity is calculated using the Luenberger observer outputs and a model of the mechanical power generated by the turbine.Second,these estimated parameters are exploited as input in a maximum-power-point tracking(MPPT)algorithm using the tip-speed ratio(TSR)to improve the sensorless strategy control.Simulation results were performed using MATLAB®/Simulink®for both wind gust and real wind profiles.We have verified that for wind gusts with jumps ranging from 3 to 7 m/s,the new observer manages to better follow the rotation speed and the torque of the turbine compared to a usual observer.In addition,we demonstrated that by applying the proposed estimator in the improved TSR-MPPT strategy,it is possible to extract 3.3%more energy compared to traditional approaches.
