Cement Kiln Dust Chemical Stabilization of Expansive Soil Exposed at el-Kawther Quarter, Sohag Region, Egypt

This work dealt with a chemical stabilization of an expansive high plastic soil of Pliocene deposits exposed at el-Kawther quarter using cement kiln dust (CKD) and cement kiln dust with lime (L) to reduce their swelling and improve their geotechnical properties. Several specimens of the studied expansive soil were collected from el-Kawther quarter. Chemical analysis of the used cement kiln dust and the lime was conducted. Microstructural changes were examined using scanning electron microscope (SEM) before and after chemical treatment of the studied soil. Geotechnical properties including plasticity, compaction parameters, unconfined compressive strength (qu), ultrasonic velocities and free swelling of the studied soil were measured before and after the treatment. An optimum content of the cement kiln dust was 16% (CKD). The optimum content of the cement kiln dust with the lime was 14% (CKD) with 3% (L) according to pH-test. The results showed that the addition of cement kiln dust and cement kiln dust with lime led to a decrease in maximum dry density and an increase in optimum water content. Unconfined compressive strength values were increased using cement kiln dust and cement kiln dust with lime at 7 days curing time. Ultrasonic longitudinal (Vp) and shear (Vs) velocities values were also increased by addition of the cement kiln dust and the cement kiln dust with lime at 7 days curing time. Increment of the curing time from 7 to 28 days led to an increase in both unconfined compressive strength and ultrasonic velocities values. Free swelling percent of the studied soil was reduced from 80.0% to 0.0% after the treatment.

Acute and chronic hyperkalemia is linked to higher hospitalization rates and mortality rates. It has a high prevalence among dialysis and non-dialysis patients in Egypt. The current therapy options are not that ideal. Moreover, we have a critical management paradox in cardiorenal diseases: Should we use the optimum dose of RAASi with its higher incidence of HK, or should we decrease the dose or even stop it with all its harmful hazards? Therefore, in light of the recent updates in different clinical practice guidelines, we, a country-specific (Egypt) task force, gathered to develop a clear, evidence-based, and multi-disciplinary consensus for managing HK. This manuscript illustrates the recommendations of this expert committee. The panel recommends new evidenced K binders like Sodium Zirconium Cyclosilicate (SZC) and patiromer to help manage HK in cardiorenal patients as stated by different international guidelines. In emergency settings, SZC can have a role in managing acute HK;however, it should be used in addition to other drugs like insulin and glucose. Local research studies on the utilization of these novel K binders are highly recommended. The utilization of these novel K binders as prophylaxis should be tested first in a well-designed randomized controlled trial.

Non-invasive prediction of endometriosis revisited;3 biomarkers as Angiopoietin-2, Interleukin-1β and Vascular Endothelial Growth Factor

Introduction: Endometriosis affects up to 1 every 5 women at their reproductive age, with variable and complex symptomatology. Patients may be asymptomatic but may have pain episodes or subfertility. Its negative impact is on patients’ health and quality of life. Objective: it was to investigate the serum and peritoneal fluid (PF) concentrations of Angiopoietin- 2, Interleukin-1β, and Vascular Endothelial Growth Factor, aiming to evaluate their diagnostic performance in endometriosis. Methods: Serum and peritoneal fluid samples were taken from 112 women undergoing laparoscopy for infertility, pelvic pain or adnexal masses. 61 diagnosed with endometriosis and 51 controlled. Primary outcome was to estimate serum and PF concentrations of Angio-2, IL-1β and VEGF and secondarily correlate these concentrations to disease stages thus assuming their diagnostic potential. Results: Significant differences were found between patients and control as regards serum and PF concentration of all studied markers except serum IL-1β. Serum Angio-2 and PF VEGF showed a significantly higher level in more advanced stages of endometriosis. PF VEGF showed a positively significant correlation with the stage of the disease, spearman coefficient t = 0.442 p = 0.014. PF concentrations of Angio-2 and Serum VEGF did not show significant pattern changes with stage-related levels. Diagnostic potential of serum and PF concentrations of the 3 markers were assessed by the ROC curve. Angio-2 proved an excellent diagnostic ability for endometriosis. PF and serum VEGF proved an equal diagnostic performance, whereas, PF IL-1β was the least efficient. Based on the results, we suggested preliminary serum threshold values for these markers to be used as diagnostic or follow-up landmarks with relatively acceptable sensitivity, specificity, positive and negative predictive values. Involvement of Male Youth into Accidents in Upper Egypt: Pattern and Risk Analysis

Background: Accidents pose a serious threat to health and economy in Egypt. Objectives: To identify and analyze the epidemiological factors associated with different types of accidents among adult males in Upper Egypt. Methods: A sample of 500 Egyptian males 20 - 28 years of age attending for health care at Quena University Hospital (QUH) or affiliated health institutions in South Egyptian governorates of Luxor and Red Sea was studied. The demographic, socioeconomic, and habitual data, as well as health status, medical history, and history of engagement into accidents for the recruits all were evaluated. Results: Accidents occurrences accounted up to 23/500 (4.6%) of the studied group. Road traffic accidents (RTAs) represented the highest percentage (30.4%), followed by street fights (21.7%) and animal bites (13.1%). Occupational accidents, electrical injuries, and near-drowning, all were equally encountered (8.7%), whereas falls and burns were least prevalent (4.3% each). Drug abuse, manual work, obesity, and having epilepsy were significant risk factors for accident injuries (OR = 6.35, 95% CI: 1.64 - 24.58, OR = 3.36, 95% CI: 1.22 - 9.73, OR = 4.81, 95% CI: 1.50 - 15.46, OR = 45.33, 95% CI: 3.94 - 518.01, respectively). Conclusions: Engagement of young Upper Egyptian males into accidents is alarming. Most of the accident mechanisms encountered are preventable. Enforcement of standard occupational, traffic and public safety laws is needed to minimize the burden of accidents upon the public health and economy in Upper Egypt.

In recent times,Internet of Things(IoT)and Cloud Computing(CC)paradigms are commonly employed in different healthcare *** gadgets generate huge volumes of patient data in healthcare domain,which can be examined on cloud over the available storage and computation resources in mobile *** Kidney Disease(CKD)is one of the deadliest diseases that has high mortality rate across the *** current research work presents a novel IoT and cloud-based CKD diagnosis model called Flower Pollination Algorithm(FPA)-based Deep Neural Network(DNN)model abbreviated as *** steps involved in the presented FPA-DNN model are data collection,preprocessing,Feature Selection(FS),and ***,the IoT gadgets are utilized in the collection of a patient’s health *** proposed FPA-DNN model deploys Oppositional Crow Search(OCS)algorithm for FS,which selects the optimal subset of features from the preprocessed *** application of FPA helps in tuning the DNN parameters for better classification *** simulation analysis of the proposed FPA-DNN model was performed against the benchmark CKD *** results were examined under different *** simulation outcomes established the superior performance of FPA-DNN technique by achieving the highest sensitivity of 98.80%,specificity of 98.66%,accuracy of 98.75%,F-score of 99%,and kappa of 97.33%.

Diagnostic Delay and Associated Clinical Features of Tuberculosis among Adult Patients with Acute Myeloid Leukemia in Doha, Qatar

Background: Tuberculosis is currently the world’s leading cause of death arising from a single infectious condition. While T cell mediated immunity is recognized to have a major contribution to tuberculosis activation, the present investigation confirmed that TB was more prevalent among patients with acute myeloid rather than lymphoid leukemia and such association was frequently overlooked. The primary objective of this study is to estimate the diagnostic delay of tuberculosis among patients with acute myeloid leukemia (AML) and compare it to the general population in Qatar. Secondary objective is to study the clinical and epidemiologic characteristics of tuberculosis in patients with AML. Methods: This is a retrospective study of tuberculosis cases diagnosed in subjects with AML during the period from January 2008 till December 2016. Results: Among 215 subjects with AML identified during the study period, 12 (5.58%) received the diagnosis of tuberculosis. The estimated incidence of tuberculosis among AML cases was 7.14 cases per 1000 per year. The mean delay in diagnosis of tuberculosis was 64.2 days (95% CI: 26.8 - 101.5) and the median was 45 days (interquartile range;Q1 - Q3, 29.5 - 97.5). Prolonged fever was the most common presentation (100% of cases). Parenchymal lung involvement was the most common radiologic abnormality (83.3% of cases). Three patients (25%) died and 8 patients completed 9 to 12 months of anti-tuberculous treatment with clinical and radiological remission. Conclusion: Infections caused by Mycobacterium tuberculosis are not uncommon in patients with AML especially in patients from tuberculosis endemic regions. It constitutes a diagnostic challenge so high index of suspicion is of paramount importance.

Geotechnical Evaluation of Sedimentary Rocks and Karstification Phenomena of Carbonates Exposed along New Upper Egypt-Red Sea Road, Eastern Desert, Egypt

Geotechnical properties of sedimentary rocks exposed along upper Egypt-Red Sea road were investigated to evaluate their geotechnical behavior. One of the most important aims of this work was to determine the engineering problems affected on the road due to the geotechnical behavior of the studied sedimentary rocks and to suggest the suitable solution. Specific weight, unconfined compressive strength, slaking durability and free swelling were measured. The results showed that the studied crystalline limestone, marly limestone and Issawia limestone had higher unconfined compressive strength values ranging from 34,184.00 kN/m2 to 42,602.00 kN/m2 and the studied shale, siltstone and coral reef had lower values ranging from 3,689.00 kN/m2 to 6,933.00 kN/m2. The results showed also that the studied shale, siltstone and mudstone had fast to very fast slaking rate and their slaking amount described as low to medium after Gamble＇s slake durability classification. Replacement and/or chemical stabilization （using lime and/or cement kiln dust） of the expansive sedimentary rocks （shale and mudstone） were recommended to reduce the swelling. Geophysical investigations including shallow seismic, microgravity and GPR （ground penetrating radar） were recommended to scan the sub-grade of the road at el Maaza plateau to determine possible other caves and large voids under the road to avoid its collapse.

Anti-inflammatory potential of black cumin seed oil and its nanoemulsion formulation against lipopolysaccharide-induced liver injury in mice

Objective:To evaluate the anti-inflammatory effect of black cumin seed oil(BCSO)and its nanoemulsion on lipopolysaccharide(LPS)-induced liver injury in ***:LPS-induced acute liver injury mouse model was used to evaluate the effects of BCSO and its nanoemulsion formulation on liver *** inflammatory markers including Toll-like receptor 4(TLR4),interleukin(IL)-1β,heme-oxygenase 1,BAX,and BCL-2 were assessed using real-time ***,protein levels of reduced glutathione,tumor necrosis factor-α,and IL-6 were measured using elISA,and histological analysis was *** was used as a standard positive control for ***:BCSO reduced LPS-induced liver injury and exhibited strong anti-inflammatory effects by downregulating the expression of TLR4,IL-1β,IL-6,tumor necrosis factor-α,and heme-oxygenase ***,BCSO demonstrated antioxidant properties by increasing reduced glutathione protein levels and decreasing key apoptotic markers BAX and BCL-2 in *** nanoemulsion formulation further enhanced these anti-inflammatory,antioxidant,and anti-apoptotic effects,and histological examination confirmed this *** BCSO with indomethacin at a lower dose improved efficacy,thereby reducing its potential side ***:The investigation reveals the anti-inflammatory impact of BCSO and its nanoemulsion formulation on LPS-induced liver oxidative stress,inflammation,and apoptosis.

目的评估再生疗法（MEBT／MEBO）治疗各种急慢性开放性创面的安全性和有效性。方法将2007年1月至2012年1月期间因各种开放性创伤就诊于3家不同医院的276例患者（376处创面）随机分组，接受MEBT／MEBO治疗（1组：患者141例，创面190处）或生理盐水湿敷治疗（2组：患者135例，创面186处）。前瞻性收集人口统计学、诱发疾病、合并症和创面特征等数据；每隔2周计算和对比刨面面积和愈合指数，持续12周；记录治疗后24周的二次截肢情况。结果开放性创伤包括慢性褥疮（202处）、糖尿病足溃疡（119处）和各种其它创面（55处）。受试患者中171例（63％）为男性，105例（37％）为女性；年龄介于5～102岁，平均年龄63岁±14．5岁。两组患者在人口统计学、临床特点、生化检查和创面特征方面接近。MEBT／MEBO治疗组在治疗2周时，其愈合指数显著升高、创面面积显著缩小；在12周时，MEBT／MEBO组中66．3％（126／190）的创面实现了完全愈合（HI=1），而生理盐水湿敷组仅22．6％（42／186）的创面完全愈合（，=71．4，P=0．000）。治疗12周时，MEBT／MEBO组无l例患者的愈合指数〈0．5；而生理盐水湿敷组38．7％（72／186）的患者愈合指数〈0．5X=165，P=0．000）。两组患者均无不良反应或过敏反应发生。治疗后24周，与生理盐水湿敷组相比，MEBT／MEBO治疗组的二次截肢率明显降低（3．3％：19．0％）（X。=7．52，P=0．006）。结论MEBT／MEBO不仅操作简便、使用安全，还能显著促进开放性创面的愈合，治疗2周就可显著提高创面愈合指数，12周时几乎2／3的创面可完全愈合，24周时糖尿病足溃疡患者中发生二次截肢的患者明显减少。
