Background:?Disabling internal rotation contractures are frequently experienced in children with unresolved birth brachial plexus palsies. Multiple surgical options like muscle release, tendon transfer, or humeral osteotomy are available to treat such cases. Purpose:?Evaluation of the outcome of subscapularis release and latissimus dorsi and teres major tendon transfer in the management of obstetric brachial palsies in Mansoura University neurosurgical department. Study type: Retrospective observational study. Patients and Methods: Twenty-five cases who underwent subscapularis release and latissimus dorsi and teres major transfer were included in the study. All patients were subjected to complete history taking, through clinical examination. The degree of shoulder movement and disability was assessed via Modified Gilbert shoulder evaluation scale. Results: The least follow up period for our patients was 9 months. There was a clear improvement of shoulder function evaluated using Modified Gilbert shoulder evaluation scale as there were 73% of postoperative group between GIV and GV while about 84% of preoperative group were between GII and GIII. Conclusion: Tendon transfer is a valid easy procedure for correction of shoulder deformities in patients with obstetrical brachial plexus palsy. It is considered a very good option for patients who missed the chance of microsurgical repair or patients with poor shoulder recovery after surgery. Although some authors reported deterioration of shoulder function with log time follow up after tendon transfer, it is still better than those who were not operated.

Locking-steel-pipe(LSP)piles connect with adjacent joints to form a pile row enclosure *** to the advantages of quick construction,efficiency in installation,and recycle utilization,the connected LSP piles are frequently used as retaining structure in deep ***,systematic studies of the deformation mechanism of the LSP pile retaining structure are rarely reported,and it still lack of experimental evidence to optimize the *** this study,a braced supported excavation experimental model test in sand was designed and conducted to investigate the deformation characteristics of LSP pile retaining *** dimensional(3D)printing technique was creatively applied to manufacture LSP model *** experimental results show that,a“S”shaped distribution of bending moments is observed along pile shaft when excavation is executed;the deflection of pile shaft develops deep-seated movements toward the excavation side as excavation went deeper,resulting in a“bowl”ground *** the deflection of LSP piles,a rotating trend was occurred between pairs of locking joint,and the severe open deformation of locking joint arose on excavation *** was a gradual reduction in earth pressure behind the LSP pile retaining wall with excavation *** earth pressure between two struts level had no obvious changing,owing to the supported effect of inner struts.

25 November 2009 is an unforgettable day for the people in Jeddah, the second largest city in the Kingdom of Saudi Arabia (KSA). On that day, Jeddah turned into a disaster zone following a short heavy rainfall event that triggered flash floods leaving 122 fatalities and considerable losses. Numerical experiments using the Pennsylvania State University-National Center for Atmospheric research mesoscale meteorological model (MM5) have been performed to investigate the event. It was caused by a short quasi-stationary mesoscale convective system that developed over Jeddah and lasted for about 8 hours. Rainfall totals computed by the model exceeded 400 mmin some localities in the southern part of Jeddah city and to the north of Jeddah in Thuwal city. The limited available observed rainfall totals, atKingAbdulAzizInternationalAirportand wadiQaws rain gauges, and Jeddah’s weather radar observations corroborates the ability of the model to reproduce the spatial and temporal characteristics of the rainfall event. A synoptic environment characterized by warmRed Seasurface temperatures and high humidity in the low levels of the troposphere. A stationary anticyclone centered over the southeast of theArabian Peninsulaconcentrated the water vapour flow to a narrow passage over Jeddah. Simulation results suggested that the development of a mesolow by latent heat release, as well as cyclogenesis induced by Al Hejaz escarpments, could have played an important role in enhancing the event by providing low-level convergence and enhanced upslope winds, and upper level atmospheric instability.

A new spherical triangular finite element based on shallow shell formulation is developed in this paper. The element has six degrees of freedom at each comer node, five of which are the essential external degrees of freedom and the additional sixth is associated with the in-plane shell rotation. The displacement fields of the element satisfy the exact requirement of rigid body modes of motion. The element is based on independent strain assumption insofar as it is allowed by the compatibility equations. The element developed herein is first validated by applying it to the analysis of a benchmark problem involving a standard spherical shell with simply supported edges. The results of the analysis showed that reasonably accurate results were obtained even when modeling the shells using fewer elements compared to other shell element types. The element is then used in a finite element model to analyze polygon shaped spherical roof structures. The distribution of the various components of deflection and stress is obtained. Furthermore, the effect of introducing circular arched beams as stiffeners spanning the two diagonally opposite end comers is investigated. It is found that the stiffeners reduced the deflections and the stresses in the roof structure by considerable value.

Objective： Preservation of the pelvic autonomic nerves in order to lower bladder and sexual dysfunction after radical rectal cancer surgery ＆ to evaluate functional outcome, local recurrence. Methods： A prospective study was under- taken on Egyptian patients. Forty one patients participated in the study in the period from December 2002 till June 2004 where they underwent radical surgery but with preservation of the pelvic autonomic nerves this was followed by adjuvant pelvic radiotherapy. Results： Six months, 1-year and 2-year follow-up of urinary function was complete in 32 out of 41 （78%）, 30 out of 41 （73%） and 27 out of 41 patients （65%） respectively There was no statistically significant correlation between the extent of nerve preservation and the reported minor voiding dysfunction. None of the patients reported major incontinence. Six months, 1-year and 2-year follow-up of sexual function revealed that 22 out of 41 patients （53%） were sexually active. Three out of 41 patients （7.3%） developed local recurrence. 38 （92.7%） patients were free of local recurrence, regarding pa- tients who received adjuvant radiotherapy 3 out of the 34 （8.8%） patients developed local pelvic recurrence while 9 patients （26.5%） developed distant metastases （3 of them did not receive adjuvant chemotherapy）, while patients who received adju- vant chemotherapy, 2 out of 20 patients （10%） developed local recurrence while distant metastases developed in 6 patients （30%）. Conclusion： Preservation of the pelvic autonomic nerves minimizes bladder and sexual dysfunction especially in male patients after rectal cancer surgery.

The effect of lead anode, rotating cylinder electrode (RCE), amount of 1,2-dihydroxypropane (12-DHP), temperature and rotation on the electrowinning of copper from low concentration acidified copper sulphate solution has been investigated. Copper powder was electrodeposited onto RCE that made of pure copper. From cyclic voltammetry experiments, an empirical parameter called the departure percent, S, was obtained which may represent the stability of the organic additive in the given medium and under the experimental conditions. The inhibition percentage, P, was 0.00 - 89.91% depending on the experimental variables. P was affected by temperature and mole fraction of 12-DHP, while rotation did not show any influence on it. Values of activation energy of electrodeposition process, Ea, were found to be less than 28 kJ mol-1 indicating diffusion controlled process. The overall mass transfer correlations under the present conditions have been computed using the dimensional analysis method. The data were valid for 90 electrodeposited copper powder as well as deposits composition and crystallite size have been studied. Various crystallite sizes ranged 7.1 nm - 250.6 nm were obtained and characterized by EDS and XRD. Different topographs proved that the rate of copper electrodeposition increased by increasing deposition time, temperature and the speed of rotation. Also, they proved that the deposition rate decreased by adding 12-DHP to the solution. Therefore, the results obtained by SEM supported those achieved by measuring the limiting current density and follow the normal manner when organic solvents were added to the electrodeposition bath.

Intrathoracic latissimus dorsi muscle transposition: a reliable technique for prevention of bronchopleural fistula developing after extrapleural pneumonectomy and external beam radiotherapy in malignant pleural mesothelioma

Objective: Bronchopleural fistula (BPF) is a life threatening complication after pneumonectomy. Extra thoracic skeletal muscle transposition especially latissimus dorsi muscle flap (LDMF) had been used to prevent this complication. The aim of this study was to assess the effectiveness of LDMF in preventing BPF developing after extrapleural pneumonectomy (EPP) and external radiation therapy in malignant pleural mesothelioma (MPM). Methods: Between May 1999 and Dec. 2008, 37 patients with MPM were operated upon by EPP using LDMF prophylactically to reinforce the bronchial stump, and then received external radiation therapy with or without postoperative chemotherapy. Results: The mean age of all patients was 46.7 (range 26-57) years. Twenty five patients were males and 12 patients were females. Twenty three patients had MPM of the right side and 14 patients had MPM of the left side. The peri-operative mortality was 2.7% and only few flap related postoperative morbidity were reported in the form of minor seroma and subcutaneous surgical emphysema. The median follow up was 17 (range 9-43) months. All cases completed their postoperative external radiation therapy with no reported cases of early or late BPF. Conclusion: Intrathoracic pedicled LDMF transposition is proved to be effective in prevention of BPF developing after EPP and external radiation therapy in MPM and it is advised to be a routine step in EPP in these cases and to use more sophisticated technique of postoperative external beam radiotherapy (3D conformal or IMRT) to minimize this complication.
