Endoscopic Assisted Eyebrow Craniotomy for Anterior Cranial Fossa Lesions: Clinical and Cosmetic Outcomes

Background: The eyebrow supraorbital keyhole approach could be considered a modified minimally invasive model for the classic pterional subfrontal approach in which an eyebrow incision and supraorbital mini craniotomy are performed for exposure of the anterior cranial fossa corridor. Methods: This study was retrospectively conducted on twenty four patients, age ranging from 20 to 65 years old, with anterior cranial fossa lesions who were meeting the eligibility criteria for eyebrow craniotomy in the period from August 2019 to January 2023. These patients were operated through eyebrow supraorbital approach in which microscopic endoscopic assisted technique were used. Extent of resection, clinical and cosmetic outcomes and complication incidence were assessed. Results: We included the twenty four patients who met inclusion criteria (17 females and 7 males) their ages ranged from 20 to 65 years. The most common pathology was meningioma in 19 patients. Two patients experienced supraorbital loss of sensation and only one patient experienced palsy of frontalis branch of facial nerve. Frontal sinus was breached in 3 patients with no patient experienced postoperative CSF leak. Total excision was accomplished for 23 patients. Four patients who had preoperative visual compromise, improvement of visual acuity and field defects was observed in 3 patients. No major intraoperative complications occurred. All patients filled cosmetic satisfaction questionnaire during their outpatient visits. For the eyebrow supraorbital approach, no incision related intolerable pain, no craniotomy defects or irregularities, no cosmetic complaints nor limitation of jaw opening were reported, and only minor symptoms in the form of limited eyebrow elevation, swelling and numbness in the forehead. Conclusions: The eyebrow craniotomy could be used safely as a more cosmetic and minimally invasive approach for a variety of anterior cranial fossa lesions. Transabdominal Pre-Peritoneal Mesh for Inguinal Hernia Repair with External Fixation versus Mesh Stapling

Background: It is unknown whether stapling the mesh affects recurrence rate, incidence of neuralgia, and port-site hernia. We chose to fix it to the exterior reducing port size, cost and pain, at the same comparing this with traditional mesh stapling. Methods: We conducted a prospective trial for laparoscopic TAPP inguinal hernia repair on 120 patients in which we fixed the mesh to the anterior abdominal wall using either two prolene threads that passed to the exterior and tied in place or traditional mesh stapling. Results: The operative time is ranged from 35 to 70 minutes for external fixation, 30 to 60 minutes for mesh stapling, and 4 to 51 months for follow-up, and no recurrence occurred in both groups during the procedure. Two cases with post TAPP pain in mesh stapling patients are discussed with reduction of the cost and port size in external fixation patients. Conclusion: It is not necessary to secure the mesh during laparoscopic TAPP inguinal hernia repair from the interior and it is fixed only to the exterior allowing a reduction in size of the ports and considerable reduction in cost with elimination of TAPP associated post operative pain.

Concordance of HER-2/Neu over Expression with Steroid Receptor Status in Female Breast Cancers

HER-2/neu over expression is associated with increased tumor aggressiveness, increased rates of recurrence, and increased mortality in node positive patients. It is amplified and/or over expressed in approximately 30% of female breast cancers. This study was to detect the relation between HER-2/neu over expression and steroid hormone receptor status. It was conducted at Cairo and Bani Suif university hospitals between February 2012 to February 2014. HER-2/neu over expression was found to be positive in thirteen patients (41%) out of the thirty two patients included in the study, more positive (52%) HER-2/neu was found among estrogen receptor (ER) positive patients, on the other hand, only 18% positive HER-2/neu was detected among ER negative patients. This difference was statistically significant (P < 0.03). The same outcome was with progesterone receptors (PR) and HER-2/neu with statistically significant difference also (P = 0.02). Conclusion: HER-2/neu over expression is strongly related to ER and PR, this was more evident in negative steroid receptors status. The failure of treatment in positive steroid receptors could be explained by HER-2/neu positivity.

Muscle fatigue is common in many populations,particularly *** increases the incidence of muscle fatigue and delays its *** is a huge debate about the current treatments for muscle fatigue,particularly in ***,it has been discovered that mechanoreceptors have an important role as a sensory system in sensing muscle fatigue which could enhance the body's response to muscle *** function of mechanoreceptors could be enhanced by applying either suprathreshold or subthreshold *** suprathreshold vibration improves muscle fatigue,it can cause desensitization of cutaneous receptors,discomfort,and paresthesia,which are barriers to clinical *** vibration has been approved as a safe and effective method of training for mechanoreceptors;however,its use and effectiveness in muscle fatigue have never been tested or *** physiological effects of subthreshold vibration in the treatment of muscle fatigue include:(1)Enhancing the function of mechanoreceptors themselves;(2)Increasing the firing rate and function of alpha motor neurons;(3)Increasing blood flow to fatigued muscles;(4)Decreasing the rate of muscle cell death in elderlies(sarcopenia);and(5)Driving motor commands and allow better performance of muscles to decrease fatigue *** conclusion,the use of subthreshold vibration could be a safe and effective treatment for muscle fatigue in *** could enhance recovery from muscle ***,Subthreshold Vibration is safe and effective in treating muscle fatigue in comparison to suprathreshold vibration.

Ozonation of ibuprofen in presence of SrWO4/ZnO photo-catalyst

The study was performed for removing ibuprofen from water using ozonation with visible light SrWO4/ZnO *** the removal of Ibuprofen,the overall reaction was carried out in a glass reactor in the presence of UV light and constant supply of *** maximum removal efficiency of Ibuprofen was 93%at 0.1 mg/L initial Ibuprofen concentration,and 0.35 g/L catalyst concentration keeping the contact time 30 *** a certain value,an increase in a concentration of doping agent,catalyst as well as initial Ibuprofen concentration results in a decrease of efficiency of the *** prepared photo catalysts were characterized by X-ray diffraction,FTIR spectra and SEM *** peaks and FTIR spectra has clearly shown that the doping of SrWO4 with ZnO showed better result as compared to *** analysis showed that they spherical nano-composite crystals were dominant in *** character was found to reduce after ozonation of *** by products were formed and were identified through GC-MS *** results,it was found that combination of SrWO4/ZnO was able to remove 93%of ibuprofen and 0.4%dopping of ZnO on SrWO4 was found to be *** pattern of by products were identitified in GC-MS with various mass ratios as 177,92,57,*** 55%removal of BOD was observed in this *** study was found to be useful for removal of pharmaceuticals and other similar pollutants from water effectively.

Evaluation of suturless scleral fixation with posterior chamber foldable intraocular lens implantation

AIM: To assess the sutureless scleral fixation technique for posterior chamber foldable intraocular lens(PCIOL) implantation in aphakic eyes with insufficient or no capsular support. METHODS: A technique for sutureless intrascleral fixation of the haptics of a standard 3-piece PCIOL was used which ensures sutureless fixation by permanent incarceration of the haptics in a scleral tunnel parallel to the limbus. All patients were evaluated for preoperative status [visual acuity, refractive error, K readings, intraocular pressure(IOP) measurement, slit lamp examination, fundus examination and optical biometry], postoperative status and complications. Ultrasound biomicroscopy(UBM) was done for 10 cases to evaluate optic tilt. RESULTS: The study evaluated 42 eyes of 42 patients. The follow-up period was 6 mo. Improvement of best corrected visual acuity(BCVA) one line occurred in 10 cases(23.8%) and loss of one line in 3 cases(7.1%). Intraoperative complications included: haptic kink in 4 cases(9.5%), haptic breakage in 1 case(2.4%), haptic dislocation in 1 case(2.4%), haptic slippage in 3 cases(7.1%), IOL dislocation in 1 case(2.4%) and sclerotomy related bleeding in 1 case(2.4%). Postoperative complications included: transient mild vitreous hemorrhage in 3 cases(7.1%), choroidal detachment in 1 case(2.4%), cystoid macular edema(CME) in 1 case(2.4%), optic capture in 1 case(2.4%), subconjunctival haptic in 2 cases(4.8%), ocular hypotony in 4 cases(9.5%) and ocular hypertension in 1 case(2.4%). There were no cases of retinal detachment or endophthalmitis. UBM showed optic tilt in 3 cases(30%). CONCLUSION: Fixation of three-piece foldable IOL haptics in scleral tunnel parallel to the limbus-providedaxial stability and proper centration of the IOL with minimal or no tilt in most cases and a low complication rate during the follow up period which lasted 6 mo.

Minimally Invasive 360 Degrees Decompression for Ligamentous, Bony and Discogenic Lumbar Canal Stenosis

Background: For decades, traditional open surgical techniques were used to treat lumbar disc herniation and lumbar canal stenosis (LCS). However, seeking for better outcomes for patients and avoiding extensive bony loss with its sequences had raised minimally invasive technique for treating these disorders as an alternative surgery. Methods: This is a retrospective study in which 54 patients of LCS were operated upon via unilateral minimally invasive technique to decompress the canal in a 360 degrees fashion through laminotomy, deroofing of opposite laminar side, sublaminar ligamintectomy, bilateral foraminotomies and discectomy. We used VAS scores and ODI to assess clinical outcomes with a period of one year follow-up. Results: Our results demonstrated that minimally invasive techniques for treating these disorders are effective procedures. Minimally invasive 360 degrees decompression for treating LCS had better outcomes regarding postoperative back pain, smaller incisions, less bony loss and early ambulation. Conclusion: Minimally invasive techniques for treating lumbar canal stenosis of different causes could be considered a better option instead of traditional full laminectomy with better outcomes as regards respecting the anatomical layers such as posterior spinal integrity and musculature, postoperative pain, accompanied with less blood loss, shorter hospital stays, and shorter recovery periods.

Novel colored flames via chromaticity of essential colors

Colored flame compositions have distinctive variety of applications ranging from military signaling,rocket tracking, and illuminating devices. Certain elements and compounds when heated to high temperature are able to emit unique wavelengths in the visible region. This study, reports on the development of novel colored flames that cannot be generated by emitting atomic/molecular species. This was achieved by using chromaticity of basic colored flames. Mixing of high quality primary colored flames including Blue, Yellow, and Red in proper ratio was conducted; any interfering incandescent emission resulted from MgO was eliminated using Al metal fuel. The spectral characteristics in terms of luminous intensity, and color quality were evaluated using digital luxmeter and UV-Vis. spectrometer *** quality mixed colored flames include violet, sweet pink, and marigold were developed. This study shaded the light on the state of the art for the real development of novel colored flame compositions and chromaticity of basic colored flames.

Effective damping of local low frequency oscillations in power systems integrated with bulk PV generation

High penetration of renewable sources into conventional power systems results in reduction of system inertia and noticeable low-frequency oscillations (LFOs) in the rotor speed of synchronous generators. In this paper, we propose effective damping of LFOs by incorporating a supplementary damping controller with a photovoltaic (PV) generating station, where the parameters of this controller are coordinated optimally with those of a power system stabilizer (PSS). The proposed method is applied to damp local electromechanical modes by studying a system comprising a synchronous generator and a PV station connected to an infinite bus. The PV station is modeled following the instructions of the Western Electricity Coordinating Council. The problem is modeled as an optimization problem, where the damping ratio of the electromechanical modes is designed as the objective function. Constraints including upper and lower limits of decision parameters and damping ratio of other modes are considered by imposing penalties on the objective function. Different optimization algorithms are used to pursue the optimal design, such as political, improved gray wolves and equilibrium optimizers. The results validate the effectiveness of the proposed controller with PSS in damping local modes of oscillations.
