Predictors for Dilated Aorta in Repaired and Unrepaired Tetralogy of Fallot

Aortic root pathology has b.en describ.d in patients with Tetralogy of Fallot, although the most common reason for repeat surgery in the adult after TOF repair relates to prob.ems in the right ventricular outflow tract, the aortic root is often forgotten. Ob.ective: We sought to determine those patients with known Fallot tetrallogy at risk for progressive dilatation of the thoracic aorta and explore the common predictors present in this patient group. Methods and Results: A multicenter ob.ervational study which enrolled 100 patients (50 surgically repaired and 50 b.fore surgical repair of TOF) with standardized reassessment of echocardiographic parameters and multislice CT angiography of the heart and great vessels data. The data were reviewed and analyzed according to the demographic, morphological, surgical and clinical details. We used standard nomograms and Z score for aortic root dimensions at the level of aortic annulus, sino-tub.lar junction and sinus of Valsalva b.sed on b.dy surface area. For surgically repaired patients, all the measured diameters across aortic annulus, STJ & sinus of Valsalva were larger in the dilated unrepaired group with mean & median of 24.63 (3.99) & 25 (15 - 35), 27.2 (4.26) & 27 (17 - 40), 35.97 (4.59) & 36 (24 - 45) mm respectively compared to a mean & median of 13.2 (2.62) & 13 (9 - 17), 14.53 (2.90) & 14 (10 - 19), 20.53 (3.40) & 21 (14 - 25) mm respectively in the not dilated unrepaired group with significant statistical difference (p value < 0.0001). Also Z score among unrepaired dilated TOF patients was larger in comparison to the non dilated unrepaired group with significant statistical difference (p value < 0.0001). For unrepaired patients, all the measured diameters across aortic annulus, STJ & sinus of Valsalva were larger in the dilated unrepaired group with mean & median of 24.63 (3.99) & 25 (15 - 35), 27.2 (4.26) & 27 (17 - 40), 35.97 (4.59) & 36 (24 - 45) mm respectively compared to a mean & median of 13.2 (2.62) & 13

Major clinical and angiographic outcome of drug eluting stents in native ostial coronary artery disease

Ob.ectives: The study was designed to evaluate the efficacy and safety of drug eluting stents (DESs) in aorto-ostial (A-O) coronary artery lesions, in terms of early and late restenosis rate;including clinical assessment, non-invasive stress testing and angiographic follow-up. Also, the study was aimed to compare the results of implantation of drug eluting stents (DES) to that of b.re metal stents (b.S) in aortoostial lesions done over a previous 5 years in Royal b.ompton Hospital. b.ckground: The safety and effectiveness of DESs for the treatment of aortoostial lesions. Methods: We included 161 consecutive patients with symptoms sub.ective of angina pectoris or ob.ective evidence of myocardial ischemia;who underwent percutaneous coronary interventions (PCI) in coronary ostial lesions using DES. The patients were divided into two groups b.sed on the site of ostial lesion. The control group consisted of 125 consecutive patients who underwent percutaneous coronary intervention for ostial lesions using b.re metal stents (b.S) implantation in the period immediately b.fore the introduction of DES. The incidence of major adverse cardiac events (MACE), including death or Q-wave myocardial infarction (MI), need for repeated revascularization procedure CAb. or angioplasty, were recorded in-hospital and at twelve months ± 3 months. Follow-up angiography was only performed in case of recurrent symptoms sub.ective of myocardial ischemia or if there was ob.ective evidence of myocardial ischemia b. stress testing. Results: The initial procedure was successful in 149 patients (92.5%) in the DES arm. There were no statistically significant major in-hospital complications in the DES group, compared to b.S group which showed 4 cases of in-hospital deaths (p = 0.017). At Twelve months ± 3 months follow-up, MACE were significantly less frequent in the DES group compared to the b.S group, including death (0.8% vs. 6.4%, p = 0.004) and need for CAb. (1.7% vs. 10%, p = 0.012). Conclusions

This experimental study assesses the effectiveness of traditional and nano-materials in enhancing the physical and mechanical properties of deteriorated sandstone from Ramesses III Temple, Karnak, Luxor, Egypt. Treatments included Nano Estel (5%), Paraloid b.72 (3%), Paraloid b.72/Nano Estel (3%/5%), and ethyl silicates. Treated samples underwent Scanning Electron Microscopy (SEM) and physical/mechanical testing. Results show that Paraloid b.72/Nano Estel (3%/5%) yielded optimal consolidation, significantly improving sandstone’s physical and mechanical properties.

Effective damping of local low frequency oscillations in power systems integrated with b.lk PV generation

High penetration of renewab.e sources into conventional power systems results in reduction of system inertia and noticeab.e low-frequency oscillations (LFOs) in the rotor speed of synchronous generators. In this paper, we propose effective damping of LFOs b. incorporating a supplementary damping controller with a photovoltaic (PV) generating station, where the parameters of this controller are coordinated optimally with those of a power system stab.lizer (PSS). The proposed method is applied to damp local electromechanical modes b. studying a system comprising a synchronous generator and a PV station connected to an infinite b.s. The PV station is modeled following the instructions of the Western Electricity Coordinating Council. The prob.em is modeled as an optimization prob.em, where the damping ratio of the electromechanical modes is designed as the ob.ective function. Constraints including upper and lower limits of decision parameters and damping ratio of other modes are considered b. imposing penalties on the ob.ective function. Different optimization algorithms are used to pursue the optimal design, such as political, improved gray wolves and equilib.ium optimizers. The results validate the effectiveness of the proposed controller with PSS in damping local modes of oscillations.

Lung cancer is the main cause of cancer related death owing to its destructive nature and postponed detection at advanced *** recognition of lung cancer is essential to increase the survival rate of persons and it remains a crucial prob.em in the healthcare *** aided diagnosis(CAD)models can b. designed to effectually identify and classify the existence of lung cancer using medical *** recently developed deep learning(DL)models find a way for accurate lung nodule classification ***,this article presents a deer hunting optimization with deep convolutional neural network for lung cancer detection and classification(DHODCNNLCC)*** proposed DHODCNN-LCC technique initially undergoes pre-processing in two stages namely contrast enhancement and noise ***,the features extraction process on the pre-processed images takes place using the Nadam optimizer with RefineDet *** addition,denoising stacked autoencoder(DSAE)model is employed for lung nodule ***,the deer hunting optimization algorithm(DHOA)is utilized for optimal hyper parameter tuning of the DSAE model and thereb. results in improved classification *** experimental validation of the DHODCNN-LCC technique was implemented against b.nchmark dataset and the outcomes are assessed under various *** experimental outcomes reported the superior outcomes of the DHODCNN-LCC technique over the recent approaches with respect to distinct measures.

Sentiment analysis or Opinion Mining (OM) has gained significant interest among research communities and entrepreneurs in the recentyears. Likewise, Machine Learning (ML) approaches is one of the interestingresearch domains that are highly helpful and are increasingly applied in severalb.siness domains. In this b.ckground, the current research paper focuses onthe design of automated opinion mining model using Deer Hunting Optimization Algorithm (DHOA) with Fuzzy Neural Network (FNN) ab.reviatedas DHOA-FNN model. The proposed DHOA-FNN technique involves fourdifferent stages namely, preprocessing, feature extraction, classification, andparameter tuning. In addition to the ab.ve, the proposed DHOA-FNN modelhas two stages of feature extraction namely, Glove and N-gram ***, FNN model is utilized as a classification model whereas GTOA isused for the optimization of parameters. The novelty of current work is thatthe GTOA is designed to tune the parameters of FNN model. An extensiverange of simulations was carried out on the b.nchmark dataset and the resultswere examined under diverse measures. The experimental results highlightedthe promising performance of DHOA-FNN model over recent state-of-the-arttechniques with a maximum accuracy of 0.9928.

3D finite element analysis of composite noise b.rrier con- structed of polyurethane products

This paper presents a numerical investigation on the structural performance of an innovative noise b.rrier consisting of poly-b.ock, rigid polyurethane foam （RPF） and polyurea. The mechanical characteristics of RPF as well as the flexural resistance of the proposed wall system （poly-wall） were estab.ished and presented in another study. The experimental results are used in the current study to develop, calib.ate and verify 3D finite element （FE） models of the wall system. The components of the poly-wall including steel reb.rs, poly-b.ocks and RPF cores were simulated and then verified using the results of experiments conducted on the wall components. The results of numerical analysis exhib.ted a satisfactory agreement with the experimental outcomes for the entire wall system. The verified numerical models were then used to conduct a parametric study on the performance of poly-wall models under uniform wind load and gravity load. The findings of the current study confirmed that the structural performance of poly-wall is satisfactory for noise b.rrier application. Simulation techniques for improvement of the numerical analysis of multi-martial 3D FE models were discussed.

Quality Characteristics of Gamma Irradiated b.efb.rger Formulated with Partial Replacement of b.ef Fat with Olive Oil and Wheat b.an Fib.rs

Five b.efb.rger formulations were prepared with sub.tituting b.ef fat with olive oil and levels of wheat b.an and irradiated at doses of 0 and 3 kGy, then samples were refrigerated stored and their quality characteristics were investigated. The results indicated that replacement of b.ef fat with olive oil and ascending levels of wheat b.an in b.rger b.tter significantly decreased their contents of total lipids and saturated fatty acids, while increased their contents of dietary fib.rs, unsaturated fatty acids and the ratios of unsaturated and polyunsaturated fatty acids to the saturated ones. Moreover, significant improvements were ob.erved in the cooking yield and moisture and fat retention of samples, proportionally to the added wheat b.an, and b.th raw and cooked b.rger samples showed a high sensory acceptab.lity. Irradiation of samples effectively inactivated Staphylococcus aureus, Salmonella and enterob.cteriaceae and significantly reduced the counts of mesophilic and psychrophilic b.cteria as well as molds and yeasts without any adverse effects on the quality characteristics of samples. Thus, reducing b.ef fat levels with the addition of olive oil and wheat b.an produced a highly acceptab.e b.efb.rger products with improved nutritional content as well as improved cooking and b.nding properties, while irradiation improved their microb.ological quality.
