A compressible lattice Boltzmann-finite difference method is extended by the phase-field approach into a monolithic scheme to study fluid flow and heat transfer through regular arrangements of solid bodies of circular,elliptical and irregular *** advantage of using the phase-field method is demon-strated both in its simplicity of accounting for flow and thermal boundary conditions at solid surfaces with irregular shapes and in the capability of generating such complex-shaped *** an array of discs,numerical results for the overall solid-to-gas heat transfer rate are validated via experiments on flow through arrays of hot *** thus validated compressible LB-FD-PF hybrid scheme is used to study the dependence of heat transfer on flow and thermal boundary conditions(Reynolds number,temperature difference between the hot solid bodies and the inlet gas),porosity as well as on the shape of solid *** are rationalized in terms of the residence time of the gas close to the solid body and downstream variations of gas velocity and *** for further applications of the proposed methodology are also discussed.

Effectiveness of aromatherapy with lavender compared to progressive muscle relaxation on anxiety and vital signs in patients under spinal anesthesia:A randomized clinical trial

Objective:This study aimed to determine the effectiveness of aromatherapy with lavender essential oil compared to progressive muscle relaxation(PMR)on anxiety and vital signs of patients under spinal *** and Methods:This clinical trial was conducted on 120 spinal anesthesia candidates who were randomly assigned into three groups of 40 including control,PMR(Jacobsen group),and *** state-trait anxiety inventory was completed on surgery day and 15 min after the end of the intervention by the samples of all three groups,and at the same time as completing the questionnaire,vital signs were also measured and ***:The mean score of anxiety after intervention was lower than that before the intervention in the aromatherapy group(P<0.001).The mean score of anxiety in the aromatherapy group was significantly lower than that in the Jacobsen group(P<0.001).Moreover,data analysis showed a significant decrease in the mean arterial blood pressure scores of the PMR(P=008)and aromatherapy(P<0.001)groups and a statistically significant increase in the mean heart rate scores in the control group(P=0.002).Conclusion:The use of aromatherapy with lavender is more effective than PMR therapy in reducing the anxiety level of patients undergoing spinal *** to the high level of anxiety and its serious effects on the patient’s hemodynamics,aromatherapy with lavender can be used as an easy and cheap method to reduce anxiety in operation rooms.

Effects of L-carnitine on treatment outcomes of COVID-19 patients hospitalized in intensive care units:A double-blind randomized clinical trial

Objective:To assess the effects of L-carnitine on the outcomes of patients with moderate to severe coronavirus disease 2019(COVID-19)in intensive care unit(ICU).Methods:This double-blind clinical trial was carried out in 2022-2023.64 Patients with COVID-19 were selected from Amiralmomenin and Khansari hospitals in Arak,*** were randomly assigned to the control and the L-carnitine treatment group via block *** blood gases,disease severity,and levels of D-dimer,lactate dehydrogenase,ferritin,and C-reactive protein were daily assessed during the seven days of the intervention,and the length of ICU stay,the need for endotracheal intubation,and mortality rate were ***:There were significant differences in length of ICU stay,the need for endotracheal intubation,and levels of D-dimer,lactate dehydrogenase,ferritin,APACHE栻score,and C-reactive protein between the two groups(P0.05).Conclusions:L-carnitine can improve outcomes of patients with ***,it can be used as an adjuvant therapy for these patients.

Dynamic transformation above the A_(e3) in a 0.06%C low carbon steel

A 0.06%C low carbon steel was deformed in torsion over the temperature range 877-917℃in a 2% H_2 - Ar gas *** of 0.25 -5.0 were applied at strain rates ofε= 0.04 s^(-1) andε= 0.4 s^(-1) to study the formation of ferrite by dynamic transformation(DT) at temperatures above the A_(e3).The critical strain for ferrite formation by DT was aboutε= 0.2 and its volume fraction increased with strain and decreased with temperature above the A_(e3).Average ferrite grain sizes of 1.5μm to 5μm were produced,which decreased with strain *** the lower strain rate(ε= 0.04 s^(-1)) reverse transformation(RT) took place during deformation once an incubation time of about 40 s,was *** increase in strain rate fromε= 0.04 s^(-1) toε= 0.4 s^(-1) arrested RT during testing at all temperatures as the total test times did not exceed 13 *** present work shows that DT is favored at higher strain rates by increasing the driving force(*** energy ) and by suppressing RT.

Comparative effect of intrathecal meperidine,tramadol,magnesium sulfate,and dexmedetomidine on preventing post-spinal anesthesia shivering and adverse events in hip fracture repair patients:A randomized clinical trial

Objective:To compare effect of intrathecal meperidine,tramadol,magnesium sulfate,and dexmedetomidine on the prevention of post-spinal anesthesia shivering and adverse events in hip fracture repair ***:In a randomized,double-blind trial,132 patients with American Society of Anesthesiology(ASA)ⅠandⅡspinal anesthesia who needed hip fracture surgery were *** were stratified into 4 intervention groups based on a randomized block pattern:meperidine,tramadol,magnesium sulfate,and *** parameters including blood pressure,heart rate,and oxygen saturation,as well as the severity of shivering,core body temperature,Ramsay sedation score,adverse events,meperidine consumption were recorded and ***:There was no statistically significant difference in the normal hemodynamic parameters,temperature,duration of surgery,meperidine consumption,and adverse events such as dizziness,hypotension,nausea,and bradycardia among groups(P>0.05).Compared to other groups,severity of shivering was the lower in the dexmedetomidine group 6 and 8 h after *** Ramsay sedation scores were higher in the dexmedetomidine and meperidine groups 4 h after surgery(P=0.020).Conclusion:Dexmedetomidine acts better than the other three adjuvants in reducing complications such as ***,these four adjuvants are helpful to prevent postoperative shivering and could be put forward as promising local anesthetics in spinal anesthesia,based on anesthesiologists’discretion and patients’general *** registration:The study was approved by the Research and Ethics Committee at the Valiasr Hospital(Arak,Iran)with the clinical trial code of IRCT20141209020258N153.

Spatio-temporal history of H9N2 viruses in Iran and neighbor countries by Bayesian analysis and molecular characterization

Objective: To delineate the H9N2 influenza virus circulation within Iran and its neighboring countries, the potential source of the epidemic in these countries, and its date of ***: We obtained all hemagglutinin(HA) and neuraminidase(NA) nucleotide sequences of influenza H9N2 available up to December 25, 2020 from Iran and its neighboring countries(i.e., Pakistan, Afghanistan, Turkmenistan, Armenia, Azerbaijan, Turkey, and Iraq). We also performed a Bayesian Markov chain Monte Carlo method to infer the evolutionary dynamic and the most recent common ancestor for the HA and NA ***: H9N2 epidemic may have started in Iran and Pakistan much earlier than the other investigated countries in the region, and an ongoing bidirectional dispersion of the virus between the investigated countries was also observed. The mean time of the most recent common ancestor of H9N2 viruses was 1988 for HA, and 1992 for ***: Strains from investigated countries rooted in Pakistan and Iran. Regular surveillance of H9N2 viruses, especially in the live bird markets, enhancing the biosecurity of poultry industry and screening newly arriving immigrants and tourists from neighboring countries at border should be considered to control spread of the virus. Furthermore, surveillance of viral molecular evolution should be initiated for effective prevention of epidemic and pandemic spreads.

Comparison of impact of adjuvant treatment of midazolam, fentanyl, and magnesium sulfate with intrathecal bupivacaine on block characteristics and postoperative analgesia in knee arthroplasty: A randomized clinical trial

Objective:To compare the efficacy of midazolam,fentanyl,and magnesium sulfate as adjuvants to intrathecal bupivacaine on both block characteristics and postoperative analgesia in knee ***:This randomized double-blind clinical trial recruited spinal anesthesia patients of the American Society of Anesthesiologists classⅠorⅡ,who needed knee *** were stratified into three intervention groups,including the midazolam group,the fentanyl group,and the magnesium sulfate group,and the patients were administered with midazolam,fentanyl,and magnesium sulfate,*** parameters,sensory and motor block,and pain score(Visual Analogue Scale)were measured and compared among the three ***:A total of 105 patients were included in this study with 35 patients in each *** was no statistically significant difference in terms of oxygen saturation,mean blood pressure,duration of surgery,and postoperative complications,including nausea,vomiting,bradycardia,dizziness,and hypotension,as well as the time of opioid administration among the three groups(P>0.05).Statistically significant differences were found in terms of heart rate at 15,30,45,60,75,and 105 min after beginning of operation among the three groups,which was lower in the midazolam group(P0.05).Conclusion:Midazolam resulted in a shorter time to achieve sensory and motor block to T8 or higher,the onset of motor block and sensory block after spinal anesthesia,and time to achieve sensory block to T12 and L1,and the pain scores were not sign

Effect of oral premedication of midazolam, ketamine, and dexmedetomidine on pediatric sedation and ease of parental separation in anesthesia induction for elective surgery: A randomized clinical trial

Objective:To compare effect of midazolam,dexmedetomidine,and ketamine as oral premedication on pediatric sedation and ease of parental separation anxiety in anesthesia ***:This multicenter,prospective,randomized,double-blind,clinical trial focused on a pediatric population aged 2-7 years(n=153)with the American Society of AnesthesiologistsⅠ-Ⅱwho required elective *** patients were stratified into three intervention groups:midazolam,ketamine,and *** parameters(blood pressure,heart rate,and oxygen saturation)every 5 min until induction of anesthesia along with non-hemodynamic factors,comprised of sedation score before the administration and at the time of being separated from the parents,as well as parental separation anxiety scale,acceptance of anesthesia induction,and side effects were recorded and ***:No statistically significant difference in oxygen saturation,heart rate,blood pressure,duration of surgery,time to achieve an Aldrete score of 9-10,or sedation score was noted in the study *** patients in the dexmedetomidine and midazolam groups could better ease parental separation anxiety than the ketamine group(P=0.001).Moreover,fewer patients accept anesthesia induction(P=0.001)and more had side effects in the ketamine group(P=0.047).Conclusions:Our findings indicate that compared to the ketamine group,dexmedetomidine and midazolam are better in easing parental separation anxiety and accepting induction of anesthesia with fewer side *** and midazolam may be considered better ***,the final choice hinges on the patient's specific physical condition and the anesthesiologist's *** registarion:This study is registered in the Iranian Registry Clinical Trial center with the clinical trial code of IRCT20211007052693N1.

Objective:To compare the effect of ketamine,lidocaine,acetaminophen,and dexmedetomidine combined with morphine patient-controlled analgesia for opium addicts after tibial fracture ***:This double-blind clinical trial included opium-addicted patients undergoing tibia fracture *** were recruited and randomized to four different groups including the ketamine group,the lidocaine group,the acetaminophen group,and the dexmedetomidine *** hemodynamic parameters such as heart rate(HR),mean arterial pressure,and arterial SaO2,alongside visual analog scale pain scores,sedation assessed by Ramsay score,nausea and vomiting,and opioid use were recorded and compared among the four ***:This study included 140 patients,aged 37(32,41)years,with 92 males and 48 females,and each group had 35 ***-sedated subjects had the lowest blood pressure from 1 to 24 h after surgery,decreased HR at 12 and 24 h after surgery,and more satisfactory sedation(P0.05).Conclusions:Dexmedetomidine has a better sedation effect compared to ketamine,lidocaine,and acetaminophen for pain control,but the final choice hinges on the patients’physical condition and the anesthesiologist's *** registration:It is registered in Iranian Registry Clinical Trial by code IRCT20141209020258N146.

A numerical simulation case study of the coastal currents and upwelling in the western Persian Gulf

The present study employed COHERENS model as a three-dimensional hydrodynamic model to address the coastal currents in the western Persian Gulf with a horizontal resolution of 2-minutes of latitude and *** obtained results suggested that Iranian northwestward coastal currents developed from January to April and experienced their maximum intensity from June to August when the rate of the surface inflow current increases through the Strait of Hormuz and the stability of the seasonal thermocline becomes gradually *** simulation results reflected that expansions of the thermocline area began by the arrival of the warm *** such circumstances,some stronger coastal currents were generated in *** strong coastal surface current in the vicinity of the western part of Iranian coasts flew southeastward,while the reverse current flew with a northwestern direction in the *** obtained results suggested that a consequence of the influence of salinity flux,especially on the Iranian coasts,the speed of currents *** findings of this study showed that more kinetic energy was accumulated along Iranian ***,the current vectors on Iranian coasts were greater than those on Arabian coasts,which in turn resulted in the creation of the upwelling currents along western Iranian coasts.
