Brain organoids mimic closely the embryonic human brain:Over the last decade,the development of human organoid systems has evolved rapidly.Different tissues have been modeled with organoids,such as the gut,lung,liver,kidney retina and brain.These systems have a high cellular heterogeneity,with many cell types integrated into the same system.Organoids'cellular populations interact between and amongst each other in a cellular and molecular level,which represents an advantage with respects to monolayer 2D cell culture systems.

Emerging evidence and perspectives have pointed towards the heart playing an important role in hepatorenal syndrome(HRS),outside of conventional understanding that liver cirrhosis is traditionally considered the sole origin of a cascade of pathophysiological mechanisms directly affecting the kidneys in this context.In the absence of established heart disease,cirrhotic cardiomyopathy may occur more frequently in those with liver cirrhosis and kidney disease.It is a specific form of cardiac dysfunction characterized by blunted contractile responsiveness to stress stimuli and altered diastolic relaxation with electrophysiological abnormalities.Despite the clinical description of these potential cardiac-related complications of the liver,the role of the heart has traditionally been an overlooked aspect of circulatory dysfunction in HRS.Yet from a physiological sense,temporality(prior onset)of cardiorenal interactions in HRS and positive effects stemming from portosystemic shunting demonstrated an important role of the heart in the development and progression of kidney dysfunction in cirrhotic patients.In this review,we discuss current concepts surrounding how the heart may influence the development and progression of HRS,and the role of systemic inflammation and endothelial dysfunction causing circulatory dysfunction within this setting.The temporality of heart and kidney dysfunction in HRS will be discussed.For a subgroup of patients who receive portosystemic shunting,the dynamics of cardiorenal interactions following treatment is reviewed.Continued research to determine the unknowns in this topic is anticipated,hopefully to further clarify the intricacies surrounding the liver-heart-kidney connection and improve strategies for management.

Intelligent and connected vehicles(ICVs)are confronted with critically complex traffic scenarios and safety challenges that have attracted increasing attention from academia and industry.The intelligent safety of ICVs involves several technologies and requires the common efforts of researchers and engineers,including the safety of the intended functionality(SOTIF),the safety of artificial intelligence,the intricacies of cybersecurity,and ethical dilemmas.Ensuring the intelligent safety of ICVs presents substantial chal-lenges in both research and commercialization,particularly SOTIF,which refers to the absence of unreasonable risk owing to hazards resulting from functional insufficiencies of the intended functiona-lity or reasonably foreseeable misuse by person.Because of the limited scene coverage ability of training samples and the practical application conditions without boundary restrictions,it is very dif-ficult to find the functional insufficiency of the intended function-ality and overcome it under dynamic unknown scenarios,which is also the source of SOTIF.

Myocarditis is a disease process that every emergency physician fears missing.Its severity can be mild to life-threatening,and many cases are likely undetected because they are subclinical with nonspecifi c signs.[1]Subtle cardiac signs may be overshadowed by systemic symptoms of the underlying infectious process.Fever,myalgias,lethargy,symptoms commonly associated with viral syndrome,can mask the life-threatening myocarditis that may be present.In fact,in the United States Myocarditis Treatment Trial,almost 90%of patients reported symptoms consistent with a viral prodrome.[2]Ammirati et al[3]reported that 27%of patients with myocarditis had either reduced left ventricular ejection fraction,ventricular arrhythmias,or low cardiac output.Here,we present a case report,in which handheld point-of-care ultrasound was utilized at the bedside to aid in the critical diagnosis of myocarditis.With the additional information provided through this imaging modality,this patient was able to be transferred to the appropriate tertiary care facility in an expeditious manner and receive possible defi nitive treatment.

Background： The purpose of this study was to determine the association of three proteins involved in sperm function on the freezability of porcine semen： the heat shock protein 90 alpha（HSP90a）, the Niemann-Pick disease type C2 protein（NPC2）, and lipocalin-type prostaglandin D synthase（L-PGDS）.Six adult boars（each boar was ejaculated three times, 18 in total） were classified by freezability based on the percentage of functionally competent sperm. The male semen with highest freezability（MHF） and the male semen with lowest freezability（MLF） were centrifuged immediately after collection to separate seminal plasma and spermatozoa to make four possible combinations of these two components and to incubate them for 3 h,adjusting the temperature to 17 °C, to freeze them afterwards. The quantification of proteins was performed in two stages： at zero and at 3 h after incubation of the four combinations.Results： The spermatozoa × incubation time（IT） interaction only had effect（P 〈 0.01） on HSP90 a levels; this protein increased in seminal plasma, after 3 h of incubation, in larger quantity（P 〈 0.05） in combinations with MLF spermatozoa. In relation with the NPC2 protein, two isoforms of 16 and 19 kDa were identified. The 19 kDa isoform was affected（P 〈 0.01） only by the seminal plasma × IT interaction, with superior values（P 〈 0.01） both at zero and three hours of incubation, in the combinations with MHF seminal plasma; and 16 kDa isoform was affected（P 〈 0.01） only by the IT with reduction after 3 h of incubation. The levels of L-PGDS was affected（P 〈 0.01）only by the spermatozoa × IT interaction, which reduced（P 〈 0.01） in combinations with MLF spermatozoa after 3 h of incubation.Conclusions： It is possible to consider that the three proteins evaluated were associated with freezability of boar semen due, especially, to the fact that mixtures with MLF spermatozoa showed greater increase levels of the HSP90 a protein and reduction of L-PGDS in plasma. David Alexander Yuen was born on June 14,1948,in Shanghai during the Chinese Civil War.His family moved to Hong Kong the following year.Thanks to the help of Senator Mike Mansfield,a Democrat from Montana,who met Dave's father during World War II,the entire family of seven moved to New York City in 1956.Dave showed much enthusiasm as a student,graduating from Xavier High School in Lower Manhattan in 1965,along with other successful Americans such as Supreme Court Justice Antonin Scalia.Dave proceeded to Caltech to study chemical physics and then moved to the chemistry department at the University of California,Berkeley in 1969,where the beginning of the plate tectonics revolution inspired him to change his focus to geophysics.

The prevalence of diabetes mellitus and its associated complications,particularly diabetic foot pathologies,poses significant healthcare challenges and economic burdens globally.This review synthesises current evidence on the surgical management of the diabetic foot,focusing on the interplay between neuropathy,ischemia,and infection that commonly culminates in ulcers,infections,and,in severe cases,amputations.The escalating incidence of diabetes mellitus underscores the urgency for effective management strategies,as diabetic foot complications are a leading cause of hospital admissions among diabetic patients,significantly impacting morbidity and mortality rates.This review explores the pathophysiological mechanisms underlying diabetic foot complications and further examines diabetic foot ulcers,infections,and skeletal pathologies such as Charcot arthropathy,emphasising the critical role of early diagnosis,comprehensive management strategies,and interdisciplinary care in mitigating adverse outcomes.In addressing surgical interventions,this review evaluates conservative surgeries,amputations,and reconstructive procedures,highlighting the importance of tailored approaches based on individual patient profiles and the specific characteristics of foot pathologies.The integration of advanced diagnostic tools,novel surgical techniques,and postoperative care,including offloading and infection control,are discussed in the context of optimising healing and preserving limb function.

Moirésuperlattices,arising from the controlled twisting of van der Waals homostructures at specific angles,have emerged as a promising platform for quantum emission applications.Concurrently,the manipulation of strain provides a versatile strategy to finely adjust electronic band structures,enhance exciton luminescence efficiency,and establish a robust foundation for two-dimensional quantum light sources.However,the intricate interplay between strain and moirépotential remains partially unexplored.Here,we introduce a meticulously designed fusion of strain engineering and the twisted 2L-WSe_(2)/2L-WSe_(2) homobilayers,resulting in the precise localization of moiréexcitons.Employing low-temperature photoluminescence spectroscopy,we unveil the emergence of highly localized moiré-enhanced emission,characterized by the presence of multiple distinct emission lines.Furthermore,our investigation demonstrates the effective regulation of moirépotential depths through strain engineering,with the potential depths of strained and unstrained regions differing by 91%.By combining both experimental and theoretical approaches,our study elucidates the complex relationship between strain and moirépotential,thereby opening avenues for generating strain-induced moiréexciton single-photon sources.

Robotic-assisted retroperitoneal lymph node dissection for stage II testicular cancer

Objective:To evaluate the perioperative as well as early oncological outcomes of patients undergoing robotic retroperitoneal lymph node dissection for treatment of testicular cancer.Methods:We conducted a prospective consecutive case series of patients undergoing robotic assisted retroperitoneal lymph node dissection for metastatic testicular cancer between May 2018 and July 2021 at our institution.Data were collected on patient and tumour characteristics,intraoperative and postoperative parameters,and functional and oncological outcomes.Descriptive statistics are presented.Results:Nineteen patients were identified;18(94.7%)completed the procedure robotically and one was converted to open surgery;78.9%of patients had stage≥IIB and 12(63.2%)patients had undergone prior chemotherapy.The median operative time was 300(interquartile range[IQR]240-315)min.Median blood loss was 100(IQR 50-175)mL.Median length of stay was 2(range 1-11)days.All robotically completed patients commenced diet and passed flatus on Day 1 and were discharged by Day 3.The median lymph node yield was 40.5(IQR 38-51)nodes.All patients undergoing nerve-sparing procedures recovered antegrade ejaculatory function.One patient had a Clavien-Dindo III complication(chylous ascites requiring drainage).At a median follow-up of 22.3(IQR 16.3-24.9)months,one patient developed retroperitoneal recurrence,which was successfully treated with second-line chemotherapy;no other patients have had recurrences.Conclusion:Robotic retroperitoneal lymph node dissection is a safe and feasible alternative to open surgery in appropriately selected patients,offering low morbidity.Early oncological outcomes are promising.Larger cohorts and longer follow-ups are required to validate our institution's findings.
