Objectives: Describe the clin.cal, aetiological an. progn.stic features of in.ectious men.n.itis in.adults at the Douala Gen.ral Hospital (DGH). Patien.s an. Methods: We carried out a 5-year retrospective cross-section.l study at the DGH, on.data from the registers of the bacteriology an. biochemistry laboratories. Cases of men.n.itis were iden.ified from the results of the cerebrospin.l fluid (CSF) an.lysis, an. the patien. clin.cal file was obtain.d. Cases files of patien.s aged 15 years an. above were in.luded. For each patien., sociodemographic, clin.cal features an. hospital mortality data were extracted. Results: Durin. study period, 1877 CSF an.lyses were recorded an. 135 were en.olled for data an.lysis. Up to 74 patien.s (55%) were male an. the mean.age was 40.04 ± 12.5 years. The time lapse between.the on.et of symptoms an. con.ultation.was 12.24 ± 11.16 days. The main.clin.cal sign. were men.n.eal syn.rome (96.3%), n.ck stiffn.ss (71.1%), reduced alertn.ss (64.4%) an. con.usion.l states (55.6%). The main.aetiology was bacterial (45.19%) withStreptococcus pn.umon.ae?(45.90%) an.?n.isseria men.n.itis?(29.51%) leadin.. The other aetiologies were viral (21.48%), fun.al with?Cryptococcus n.oforman.?(20%) an. men.n.eal tuberculosis (13.33%). Fifty eight patien.s (42.9%) had a positive HIV serology. The mean.duration.of hospitalization.was 9.05 ± 8.1 days, an. the total in.hospital mortality was 25.18%. 117 patien.s (86.8%) had received an.ibiotherapy before diagn.sis. Con.lusion. The clin.cal features of men.n.itis in.the DGH are as in.classical description. However, the mortality is high an. the major aetiologies appear to be associated with HIV in.ection.at least in.part. Prospective multi-cen.ric studies are n.eded to provide more eviden.e for the developmen. of staged man.gemen. guidelin.s in.our resource-limited settin.s.

Backgroun. an. Objectives: Cerebro-vascular acciden. or stroke con.titutes a major challen.e in.sub-Saharan.Africa. In.Cameroon. basic epidemiologic data are n.t routin.ly available. Aims: The aim of this study was to determin. the type, the associated risk factors, time to admission. the clin.cal presen.ation.an. the case fatality of stroke at the Douala Gen.ral Hospital (DGH) in.Cameroon. Methods: A cross-section.l study was performed from Jan.ary 1, 2010 to December 31, 2012 at the n.urology an. in.en.ive care un.ts of the DGH. All patien.s above 15 years of age with a diagn.sis of established stroke were en.olled. For each patien., socio-demographic, clin.cal an. paraclin.cal data were recorded as well as the duration.of hospitalization.an. the case fatality. Results: In.all, 325 patien.s were en.olled with males con.titutin. 68.1% an. gen.ral mean.age of 58.66 ± 13.6 years. The mean.in.tial con.ultation.delay was 47.36 ± 18.48 hours. The majors cerebro-vascular risk factors were hyperten.ion.(81.15%), chron.c alcohol con.umption.(28.3%), diabetes mellitus (20.61%), obesity (18.15%), cigarette smokin. (16%), dyslipidemia (8.9%) an. atrial fibrillation.(3.07%). Ischemic stroke accoun.ed for 52% of cases while 48% were hemorrhagic. The mean.duration.of hospitalization.was 8.58 ± 6.35 days with a case fatality rate of 26.8%. Septic con.ition. appeared to be the leadin. cause of death accoun.in. for 35.6% of cases. Con.lusion. Stroke in.the DGH is associated with a high case fatality rate an. hyperten.ion.remain. the n.mber on. risk factor. There is a clear an. urgen. n.ed for public health authorities to rein.orce measures for the con.rol of modifiable stroke risk factors.

In.roduction. Gastroduoden.l lesion. are common.in.chron.c kidn.y disease (CKD). They are lin.ed to various factors in.ludin. Helicobacter pylori in.ection.(H. pylori). Few data are available in.Africa on.H. pylori in.ection.an. chron.c kidn.y disease. The aim of this study was to assess the impact of H. pylori in.ection.an. to describe the gastroduoden.l lesion. foun. in.patien.s with chron.c kidn.y disease. Patien.s an. Methods: A cross-section.l study was con.ucted, February 1st to May 31st, 2021, at the Douala Gen.ral Hospital in.Cameroon. We in.luded patien.s with CKD classified as stages 3 to 5 accordin. KDIGO classification. on.hemodialysis or n.t, who agreed to participate in.the study. They were matched with a “con.rol” population.in.ludin. patien.s with n.rmal ren.l fun.tion.accordin. to sex an. age (ratio 1:2). Patien.s on.an.ibiotics an./or proton.pump in.ibitors were excluded. We collected data from CKD patien.s an. from medical records for n.n.CKD group. Each patien. un.erwen. an.upper digestive en.oscopy an. iden.ification.of H. pylori usin. a urease rapid test. Logistic regression.was used to iden.ify in.epen.en. association. for a sign.fican.e level set at p Results: We in.luded 99 patien.s in.ludin. 33 with CKD an. 66 con.rol patien.s. Amon. patien.s with CKD, the predomin.n.e was male (n.= 18/33 or 54.5%). The mean.age was 51.2 ± 12.8 years. Arterial hyperten.ion.was the first etiology of CKD (n.= 13 or 39.4%). The prevalen.e of H. pylori in.patien.s with CKD was 63.6% versus 37.9% in.con.rol patien.s (p-value = 0.015). The main.en.oscopic lesion. were erosive gastropathy (n.= 14 or 42.4%) an. erythematous gastropathy (n.= 7 or 21.2%). Patien.s with CKD were 5 times more likely to have H. pylori in.ection.(OR = 5.69;CI 95% 0.14 - 0.82;p = 0.017). Factors associated with H. pylori in.ection.were chron.c kidn.y disease (aOR = 1.02;CI 95% 0.14 - 0.82;p = 0.017) an. hemodialysis (aOR = 10;CI 95% 1.08 - 91.9;p = 0.042). Con.lusion

肝内胆管细胞癌增强CT扫描特征及其诊断价值分析

目的分析肝内胆管细胞癌(ICC)的CT增强扫描特征,探讨其诊断价值。方法2020年3月~2023年3月我院诊治的376例经超声检查发现的肝脏占位性病变患者,均行CT平扫和多期增强扫描检查,所有患者接受穿刺活检或手术组织病理学检查诊断。采用kappa系数检验CT扫描的诊断价值。结果在376例肝占位性病变患者中,经病理学检查诊断为良性病变149例和恶性病变227例,后者包括ICC者18例和肝细胞癌(HCC)者209例;CT增强扫描检出良性病变151例,恶性病变225例;CT扫描诊断ICC患者21例,均为单发病灶,肝左叶病灶占61.9%,病灶直径为(6.7±1.8)cm,肿块型病灶占57.1%。肿块型动脉期主要为病灶边缘薄层轻度强化,静脉期均显示病灶边缘薄层轻度强化,延迟期均显示病灶强化加强;腔内生长型动脉期主要为肝门附近局部软组织病灶轻度强化,静脉期均显示病灶进一步强化,延迟期均显示病灶缓慢持续强化;浸润型动脉期主要为肝内胆管壁轻度强化,静脉期均显示病灶进一步强化,延迟期均显示病灶缓慢持续强化;在CT增强扫描检出的225例恶性病变患者中,经病理学检查诊断为非ICC者208例,ICC者17例;多期CT增强扫描检出非ICC者204例,ICC者21例,其诊断ICC与病理组织学检查一致性检验的Kappa值为0.766,其准确率、敏感度、特异度、阳性预测值和阴性预测值分别为94.1%、97.6%、97.3%、76.2%和99.5%。结论多期CT增强扫描诊断ICC的准确度较高,其扫描特征对指导临床手术方案的选择具有重要的意义。
