Impact of field strength and RF excitation on abdominal diffusion-weighted magnetic resonance imaging

AIM:To retrospectively and prospectively compare diffusion-weighted(DW)images in the abdomen in a1.5T system and 3.0T systems with and without twochannel functionality for ***:DW images of the abdomen were obtained on 1.5T and 3.0T(with and without two-channel functionality for B1shimming)scanners on 150 patients(retrospective study population)and 10 volunteers(prospective study population).Eight regions were selected for clinical significance or artifact susceptibility(at higher field strengths).Objective grading quantified signal-to-noise ratio(SNR),and subjective evaluation qualified image quality,ghosting artifacts,anddiagnostic *** significance was calculated usingχ2tests(categorical variables)and independent two-sided t tests or Mann-Whitney U tests(continuous variables).RESULTS:The 3.0T using dual-source parallel transmit(dpTX 3.0T)provided the significantly highest SNRs in nearly all *** regions susceptible to artifacts at higher field strengths(left lobe of liver,head of pancreas),the SNR was better or similar to the 1.5T ***,both dpTX 3.0T and 1.5T systems provided higher image quality,diagnostic value,and less ghosting artifact(P<0.01,most values)compared to the 3.0T system without dual-source parallel transmit(non-dpTX 3.0T).CONCLUSION:The dpTX 3.0T scanner provided the highest *** image quality,lack of ghosting,and diagnostic value were equal to or outperformed most currently used systems.

Due to the coarse scale of soil moisture products retrieved from passive microwave observations(SMPMW),several downscaling methods have been developed to enable regional scale ***,it can be challenging for users to access final data products and algorithms,as well as managing different data sources and formats,various data processing methods,and the complexity of the workflows from raw data to information ***,the Google Earth Engine(GEE),which as of late offers SMPMW,is used to implement a workflow for retrieving 1 km SM at a depth of 0-5 cm using MODIS optical/thermal measurements,the SM_(PMW)coarse scale product,and a random forest *** proposed method was implemented on the African continent to estimate weekly SM *** results of this study were evaluated against in-situ measurements of three validation ***,in comparison to the original SM_(PMW)product,which was limited by a spatial resolution of only 9 km,this method is able to estimate SM at 1 km spatial resolution with acceptable accuracy(an average correlation coefficient of 0.64 and a ubRMSD of 0.069 m^(3)/m^(3)).The results show that the proposed method in GEE provides a precise estimation of SM with a higher spatial resolution across the entire continent.

Evaluation of a handheld creatinine measurement device for real-time determination of serum creatinine in radiology departments

AIM: To assess the feasibility/accuracy of a commercial handheld device in the context of increased demand for point-of-care serum creatinine (SCr) determination. METHODS: In this institutional review board-approved, prospective study, 401 patients referred for contrastenhanced computed tomography were included at two centres. Capillary (c)SCr was determined using two devices A+B and venous (v)SCr was determined in the centre's laboratory. Method comparison statistics for both centres and for vSCr1.2 mg/dL, receiver operating characteristic analysis, negative predictive values (NPV), sensitivity and specificity were calculated pre-/ post-curve offset correction with vSCr. RESULTS: Pearson's coefficients for cSCr vs vSCr were: centre 1-A:0.93/B:0.92; centre 2-A:0.85/B:0.82 (all P 1.2 mg/dL. The area under the receiver operating characteristic curves showed a high accuracy for cSCr, but the device underestimated SCr, which was confirmed by Bland-Altman plot. Addition of the offset correction factor to the original data from centre 1 resulted in an improvement in sensitivity for detecting patients at risk (> 1.2 mg/dL), whilst maintaining acceptable specificity and improving NPV. CONCLUSION: This study showed the feasibility of SCr determination using the evaluated handheld device in a routine clinical setting. The device showed high sensitivity and high NPV, but may significantly underestimate SCr without offset correction to local laboratories.

胆道纤维化(BF)是病理性胆道损伤修复的结果,主要表现为胆管壁的增厚硬化和管腔的进行性狭窄,最终可能导致胆道梗阻、胆汁性肝硬化、肝衰竭、肝胆恶性肿瘤等严重不良结局。目前的研究将BF描述为某些胆管病的病理特征,对其病因、病理生理、分子机制和治疗缺乏系统总结。BF是胆道系统普遍存在但容易被忽略的疾病状态,可能通过病理性胆道损伤后的异常修复机制促进肝胆疾病的发生和进展,笔者结合国内外最新研究进展,综述BF的概念、临床表现、病因、发病机制和治疗策略,为临床医师提供参考。

Effects of a novel bacterial phytase expressed in Aspergillus Oryzae on digestibility of calcium and phosphorus in diets fed to weanling or growing pigs

In 2 experiments, 48 weanling （initial BW： 13.5 ± 2.4 kg, Exp. 1） were used to determine effects of a novel bacterial 6-phytase and 24 growing pigs （initial BW： 36.2 ± 4.0 kg, Exp. 2） expressed in Aspergillus oryzae on the apparent total tract digestibility （ATTD） of phosphorus and calcium in corn-soybean meal diets fed to weanling and growing pigs. In Exp. 1 and 2, pigs were randomly allotted to 6 dietary treatments using a randomized complete block design and a balanced 2 period changeover design, respectively. In both experiments, 6 diets were formulated. The positive control diet was a corn-soybean meal diet with added inorganic phosphorus （Exp. 1：0.42 and 0.86% standardized total tract digestible phosphorus and total calcium, respectively; Exp. 2：0.32 and 0.79% standardized total tract digestible phosphorus and total calcium, respectively）. A negative control diet and 4 diets with the novel phytase （Ronozyme HiPhos, DSM Nutritional Products Inc., Parsippany, N j） added to the negative control diet at levels of 500, 1,000, 2,000, and 4,000 phytase units （FYT）/kg were also formulated. In Exp. 1, the ATTD of phosphorus was greater （P 〈 0.01） for the positive control diet （60.5%） than for the negative control diet （40.5%）, but increased （linear and quadratic, P 〈 0.01） as phytase was added to the negative control diet （40.5% vs. 61.6%, 65.1%, 68.7%, and 68.0%）. The breakpoint for the ATTD of phosphorus （68.4%） was reached at a phytase inclusion level of 1,016 FYT/kg. In Exp. 2, the ATTD of phosphorus was greater （P〈 0.01） for the positive control diet （59.4%） than for the negative control diet （39.8%） and increased （linear and quadratic, P〈 0.01） as phytase was added to the negative control diet （39.8% vs. 58.1%, 65.4%, 69.1%, and 72.8%）. The breakpoint for the ATTD of phosphorus （69.1%） was reached at a phytase inclusion level of 801 FYT/kg. in conclusion, the novel bacterial 6-phytase improved the ATTD of phosphorus and calcium in both weanling and growing pigs.

Zinc deficiency in patients with chronic pancreatitis

BACKGROUND Zinc is a key element in numerous proteins and plays an important role in essential cell functions such as defense against free radicals and DNA damage repair. Chronic pancreatitis(CP) is a chronic inflammation with progressive fibrosis of pancreas ultimately resulting in pancreatic exocrine insufficiency(PEI),which is associated with malnutrition. Studies analyzing zinc levels in patients with CP are sparse and lead to conflicting *** To investigate serum zinc levels in patients with CP of various *** Between October 2015 and March 2018, patients with a diagnosis of CP were identified and recruited from the Pancreatic Outpatient Clinic at the Karolinska University Hospital in Stockholm, Sweden. Demographic, clinical and laboratory data were analyzed. Etiology of CP was determined according to the MANNHEIM classification system into the following etiological subcategories:alcohol consumption, nicotine consumption, hereditary factors, efferent pancreatic duct factors and immunological factors. Pancreatic exocrine function was defined as normal(fecal elastase 1 > 200 μg/g), mildly reduced(100-200μg/g) and severely reduced(fecal elastase 1 0.002]. Smoking and number of packyears were additionally associated with low zinc levels. Ischemia/reperfusion injury in porcine intestine-Viability assessment

AIM To investigate viability assessment of segmental small bowel ischemia/reperfusion in a porcine *** In 15 pigs, five or six 30-cm segments of jejunum were simultaneously made ischemic by clamping the mesenteric arteries and veins for 1 to 16 h. Reperfusion was initiated after different intervals of ischemia(1-8 h) and subsequently monitored for 5-15 h. The intestinal segments were regularly photographed and assessed visually and by palpation. Intraluminal lactate and glycerol concentrations were measured by microdialysis, and samples were collected for light microscopy and transmission electron microscopy. The histological changes were described and *** Using light microscopy, the jejunum was considered as viable until 6 h of ischemia, while with transmission electron microscopy the ischemic muscularis propria was considered viable until 5 h of ischemia. However, following ≥ 1 h of reperfusion, only segments that had been ischemic for ≤ 3 h appeared viable, suggesting a possible upper limit for viability in the porcine mesenteric occlusion model. Although intraluminal microdialysis allowed us to closely monitor the onset and duration of ischemia and the onset of reperfusion, we were unable to find sufficient level of association between tissue viability and metabolic markers to conclude that microdialysis is clinically relevant for viability assessment. Evaluation of color and motility appears to be poor indicators of intestinal *** Three hours of total ischemia of the small bowel followed by reperfusion appears to be the upper limit for viability in this porcine mesenteric ischemia model.
