The “No-touch” technique improves the survival of patients with advanced hepatocellular carcinomas treated by liver transplantation: A single-center prospective randomized controlled trial

Background:Liver transplantation(LT)is the best treatment for patients with hepatocellular carcinoma(HCC).However,the surgical technique needs to be *** present study aimed to evaluate the“no-touch”technique in ***:From January 2018 to December 2019,we performed a prospective randomized controlled trial on HCC patients who underwent *** patients were randomized into two groups:a no-touch technique LT group(NT group,n=38)and a conventional LT technique group(CT group,n=46).Operative outcomes and survival in the two groups were ***:The perioperative parameters were comparable between the two groups(P>0.05).There was no significant difference between the two groups in disease-free survival(DFS)(P=0.732)or overall survival(OS)(P=0.891).Of 36 patients who were beyond the Hangzhou criteria for LT,the DFS of the patients in the NT group was significantly longer than that in the CT group(median 402 vs.126 days,P=0.025).In 31 patients who had portal vein tumor thrombosis(PVTT),DFS and OS in the NT group were significantly better than those in the CT group(median DFS 420 vs.167 days,P=0.022;2-year OS rate 93.8%vs.66.7%,P=0.043).In 14 patients who had diffuse-type HCCs,DFS and OS were significantly better in the NT group than those in the CT group(median DFS 141 vs.56 days,P=0.008;2-year OS rate 75.0%vs.33.3%,P=0.034).Multivariate analysis showed that for patients with PVTT and diffusetype HCCs,the no-touch technique was an independent favorable factor for OS(PVTT:HR=0.018,95%CI:0.001-0.408,P=0.012;diffuse-type HCCs:HR=0.034,95%CI:0.002-0.634,P=0.024).Conclusions:The no-touch technique improved the survival of patients with advanced HCC compared with the conventional *** no-touch technique may provide a new and effective LT technique for advanced HCCs.

BACKGROUND： Four tumor markers for hepatocellular carcinoma（HCC）, alpha-fetoprotein（AFP）, glypican-3（GPC3）, vascular endothelial growth factor（VEGF） and des-gammacarboxy prothrombin（DCP）, are closely associated with tumor invasion and patient＇s survival. This study estimated the predictability of preoperative tumor marker levels along with pathological parameters on HCC recurrence after ***： A total of 140 patients with HCC who underwent hepatectomy between January 2012 and August 2012 were enrolled. The demographics, clinical and follow-up data were collected and analyzed. The patients were divided into two groups： patients with macroscopic vascular invasion（Ma VI ＋） and those without Ma VI（Ma VI-）. The predictive value of tumor markers and clinical parameters were evaluated by univariate and multivariate ***： In all patients, tumor size（〉8 cm） and Ma VI were closely related to HCC recurrence after hepatectomy. For Ma VI＋ patients, VEGF（〉900 pg/m L） was a significant predictor for recurrence（RR=2.421; 95% CI： 1.272-4.606; P=0.007）. The 1- and 2-year tumor-free survival rates for Ma VI＋ patients with VEGF ≤900 pg/m L versus for those with VEGF 〉900 pg/m L were 51.5% and 17.6% versus 19.0% and 4.8%（P〈0.001）. For Ma VI- patients, DCP 〉445 m Au/m L and tumor size 〉8 cm were two independent risk factors for tumor recurrence（RR=2.307, 95% CI： 1.132-4.703, P=0.021; RR=3.150, 95% CI： 1.392-7.127, P=0.006; respectively）. The 1- and 2-year tumor-free survival rates for the patients with DCP ≤445 m Au/m L and those with DCP 〉445 m Au/m L were 90.4% and 70.7% versus 73.2% and 50.5% respectively（P=0.048）. The 1-and 2-year tumor-free survival rates for the patients with tumor size ≤8 cm and 〉8 cm were 83.2% and 62.1% versus 50.0% and 30.0%, respectively（P=0.003）.CONCLUSIONS： The Ma VI＋ patients with VEGF ≤900 pg/m L had a relatively high tumor-free survival than those with VEGF 〉900 pg/m L. To the Editor:Liver cancer is the fifth most common cancer and the second most frequent cause of cancer-related death *** carcinoma(HCC)accounts for 90%of primary liver cancers with the highest incidence in China(more than 50%of all cases worldwide)[1].Liver transplantation(LT)is regarded as an optimal therapy for selected HCC *** Milan criteria are the benchmark for candidate selection that ensure excellent prognosis for patients with HCC[2].The Hangzhou criteria expand 51.5%more of Milan criteria for LT candidates with comparable posttransplant survivals[3].However,LT recipients fulfilling Milan criteria or Hangzhou criteria are at the risk of up to 13%−18%HCC recurrence rate within five years[4].Only 25%−50%of recurrent HCC patients post-LT are eligible for surgical treatment which have consistently presented favored survival benefit than systemic therapy[5].

The Author Reply:We thank *** et *** the interest toward our arti-cle and the thoughtful *** are willing to further dis-cuss the topic of endovascular treatment(EVT)after liver trans-plantation(LT).Congratulation to their successful experience of *** *** emphasized,most of the transplant centers,even important centers,are hesitant to perform EVT early after LT[1-3].Their team demonstrated successful experience of EVT treating hepatic artery thrombosis and stenosis during very early period with high patency rates and absence of major complications[4].We strongly agree with ***’s attitude and admire the novel approach.

Spatial confinement:An effective strategy to improve H_(2)O and SO_(2) resistance of the expandable graphite-modified TiO_(2)-supported Pt nanocatalysts for CO oxidation

The expandable graphite(EG)modified TiO_(2) nanocomposites were prepared by the high shearmethod using the TiO_(2) nanoparticles(NPs)and EG as precursors,in which the amount of EG doped in TiO_(2) was 10 wt.%.Followed by the impregnation method,adjusting the pH of the solution to 10,and using the electrostatic adsorption to achieve spatial confinement,the Pt elementswere mainly distributed on the exposed TiO_(2),thus generating the Pt/10EG-TiO_(2)-10 *** best CO oxidation activity with the excellent resistance to H_(2)O and SO_(2) was obtained over the Pt/10EG-TiO_(2)-10 catalyst:CO conversion after 36 hr of the reaction was ca.85%under the harsh condition of 10 vol.%H_(2)O and 100 ppm SO_(2) at a high gaseous hourly space velocity(GHSV)of 400,000 hr−*** properties of the catalystswere characterized by various *** results showed that the electrostatic adsorption,which riveted the Pt elements mainly on the exposed TiO_(2) of the support surface,reduced the dispersion of Pt NPs on EG and achieved the effective dispersion of Pt NPs,hence significantly improving CO oxidation activity over the Pt/10EG-TiO_(2)-10 *** 10 wt.%EG doped in TiO_(2) caused the TiO_(2) support to form a more hydrophobic surface,which reduced the adsorption of H_(2)O and SO_(2) on the catalyst,greatly inhibited deposition of the TiOSO4 and formation of the PtSO4 species as well as suppressed the oxidation of SO_(2),thus resulting in an improvement in the resistance to H_(2)O and SO_(2) of the Pt/10EG-TiO_(2)-10 catalyst.

Secure beamforming and deployment design for rate-splitting multiple access-based UAV communications

Rate-splitting multiple access(RSMA) has recently gained attention as a potential robust multiple access(MA)scheme for upcoming wireless networks. Given its ability to efficiently utilize wireless resources and design interference management strategies, it can be applied to unmanned aerial vehicle(UAV) networks to provide convenient services for large-scale access ground users. However, due to the line-of-sight(LoS) broadcast nature of UAV transmission, information is susceptible to eavesdropping in RSMA-based UAV networks. Moreover, the superposition of signals at the receiver in such networks becomes complicated. To cope with the challenge, we propose a two-user multi-input single-output(MISO) RSMA-based UAV secure transmission framework in downlink communication networks. In a passive eavesdropping scenario, our goal is to maximize the sum secrecy rate by optimizing the transmit beamforming and deployment location of the UAV-base station(UAV-BS),while considering quality-of-service(QoS) constraints, maximum transmit power, and flight space limitations. To address the non-convexity of the proposed problem, the optimization problem is first decoupled into two subproblems. Then, the successive convex approximation(SCA) method is employed to solve each subproblem using different propositions. In addition, an alternating optimization(AO)-based location RSMA(L-RSMA) beamforming algorithm is developed to implement joint optimization to obtain the suboptimal solution. Numerical results demonstrate that(1) the proposed L-RSMA scheme yields a28.97% higher sum secrecy rate than the baseline L-space division multiple access(SDMA) scheme;(2) the proposed L-RSMA scheme improves the security performance by 42.61% compared to the L-non-orthogonal multiple access(NOMA) scheme.

Traffic emission and its impact on air quality in Guangzhou area

A comprehensive field measurement was set up in Guangzhou City of China and the key was placed on NO\-x and O\-3 pollution. The results indicated that the average driving speed of vehicle was only 14 km/h in downtown with high frequency of idle and acceleration. Upward fluxes of CO and NO were observed in Dongfeng Street. NO\-x annual mean concentration in urban area increased year by year, and NO\-x was identified as the most important pollutant since 1995. Photochemical smog pollution was serious in general, spatial and seasonal distribution of O\-3 was observed. O\-3 concentration was kept in a high level in autumn, and its formation was restrained in summer due to frequent thunderstorm and high humidity. The numerical simulation showed the average concentration and maximum concentration of O\-3 would increase 60%—100% if vehicular emission increased 100% in Guangzhou.

Reversible Li plating regulation on graphite anode through a barium sulfate nanofibers-based dielectric separator for fast charging and high-safety lithium-ion battery

Poor Li plating reversibility and high thermal runaway risks are key challenges for fast charging lithiumion batteries with graphite ***,a dielectric and fire-resistant separator based on hybrid nanofibers of barium sulfate（BS） and bacterial cellulose（BC） is developed to synchronously enhance the battery's fast charging and thermal-safety *** regulation mechanism of the dielectric BS/BC separator in enhancing the Li+ion transport and Li plating reversibility is revealed.（1） The Max-Wagner polarization electric field of the dielectric BS/BC separator can accelerate the desolvation of solvated Li+ions,enhancing their transport kinetics.（2） Moreover,due to the charge balancing effect,the dielectric BS/BC separator homogenizes the electric field/Li+ion flux at the graphite anode-separator interface,facilitating uniform Li plating and suppressing Li dendrite ***,the fast-charge graphite anode with the BS/BC separator shows higher Coulombic efficiency（99.0% vs.96.9%） and longer cycling lifespan（100 cycles vs.59 cycles） than that with the polypropylene（PP） separator in the constantlithiation cycling test at 2 mA *** high-loading LiFePO4(15.5 mg cm-2)//graphite(7.5 mg cm-2) full cell with the BS/BC separator exhibits excellent fast charging performance,retaining 70% of its capacity after 500 cycles at a high rate of 2C,which is significantly better than that of the cell with the PP separator（retaining only 27% of its capacity after 500 cycles）.More importantly,the thermally stable BS/BC separator effectively elevates the critical temperature and reduces the heat release rate during thermal runaway,thereby significantly enhancing the battery's safety.
