Hypnotherapy for overweight and obese patients:A narrative review

Obesity and overweight problems are serious global health issues today and despite many efforts,the prevalence has continued to rise for ***,hypnotherapy has been gaining recognition as an effective treatment for obesity and overweight *** review compiles contemporary scientific research on the effectiveness of hypnotherapy for weight ***,Pub med and EBSCO Host databases were applied in the *** search identified 119 articles,of which seven met the inclusion criteria.A total of 539 respondents(82.7%women and 17.3%men)between the ages of 17 and 67 years were represented in the seven *** studies incorporated lifestyle changes,such as changes of dietary habit and behavioral recommendations in the hypnotic *** results suggested that the use of hypnotherapy not only promoted weight reduction during the treatment period but also after treatment cessation,and in some cases,one to ten kilograms were lost during follow-up *** addition,one study even showed increased physical activity among the hypnotised *** use of hypnotherapy also improved respondents'eating behavior and quality of ***,a definitive conclusion could not be drawn due to several methodological flaws and the limited number of published studies in this ***,further well-designed studies are needed to substantiate the effectiveness of hypnotherapy for this modern-day health problem.

Antibody to El peptide of hepatitis C virus genotype 4 inhibits virus binding and entry to HepG2 cells in vitro

AIm： To analyze the neutralizing activity of antibodies against E1 region of hepatitis C virus （HCV）. Specific polydonal antibody was raised via immunization of New Zealand rabbits with a synthetic peptide that had been derived from the E1 region of HCV and was shown to be highly conserved among HCV published genotypes. mETHODS： Hyper-immune HCV E1 antibodies were incubated over night at 4 ℃ with serum samples positive for HCV RNA, with viral loads ranging from 615 to 3.2 million IU/mL. Treated sera were incubated with HepG2 cells for 90 min. Blocking of viral binding and entry into cells by anti E1 antibody were tested by means of RTPCR and flow cytometry. RESULTS： Direct immunostaining using FITC conjugated E1 antibody followed by Flow cytometric analysis showed reduced mean fluorescence intensity in samples pre-incubated with E1 antibody compared with untreated samples. Furthermore, 13 out of 18 positive sera （72%） showed complete inhibition of infectivity as detected by RT-PCR. CONCLUSION： In house produced E1 antibody, blocks binding and entry of HCV virion infection to target cells suggesting the involvement of this epitope in virus binding and entry. Isolation of these antibodies that block virus attachment to human cells are useful as therapeutic reagents.

Postpartum Intrauterine Device: Use and Follow-Up of Users in the maternity Ward of the Ratoma Communal medical Center in Conakry, Guinea

Objective: The aim of this study was to describe the use of the postpartum intrauterine device in the maternity ward of the Ratoma communal medical center in Conakry. methods: This was a descriptive cross-sectional study carried out between July 1st 2015 and June 30 2016, i.e. a duration of one year. Results: A total of 551 patients received advice on various contraceptive methods. most of this advice was given in the post-partum period (40.2%) and during antenatal care (39.1%). Of the patients advised, 87 (15.8%) used the intrauterine device. The majority of users (93%) were married and uneducated (63.2%), and 39.1% were poor. The majority (56.3%) of intra-uterine devices were inserted in the immediate post-partum period. The majority of women had no adverse events either during the first six weeks (n = 57;65.5%) or at 3rd months (n = 75;86.2%) or 6th months (n = 76;87.4%) after IUD insertion. most users remained complication-free throughout the follow-up period (n = 76;87.4% at 6th weeks and 3rd months, and n = 77;88.5% at 6th months). The continuation rate was 89.7% at 6 weeks and 3rd months, and 87.4% at 6th months after insertion. The majority of users (87.0%) were satisfied with the care they received. Conclusion: This study showed very few complications among intrauterine device users, and high continuation and satisfaction rates. The intrauterine device is a long-acting, effective, reversible and safe contraceptive that can be used by most women for birth spacing in Guinea, where women do not regularly visit health facilities.

Health consequences of early-onset compared with late-onset type 2 diabetes mellitus

Background:Although cumulating evidence has suggested that early-onset type 2 diabetes mellitus(T2Dm)conferred on patients a broader tendency for complications beyond vascular ones,a comprehensive analysis of patterns of complications across all relevant systems is currently ***:We prospectively studied 1777 early-onset(age at diagnosis≤45 years)and 35889 late-onset(>45 years)T2Dm patients with matched unexposed individuals from the UK ***-specific and-related complications were examined using phenomewide association analysis,with patterns identified by comorbidity network *** also evaluated the effect of lifestyle modifications and glycemic control on complication ***:The median follow-up times for early-onset and late-onset T2Dm patients were 17.83 and 9.39 years,*** to late-onset T2Dm patients,patients with early-onset T2Dm faced a significantly higher relative risk of developing subsequent complications that primarily affected sense organs[hazard ratio(HR)3.46 vs.1.72],the endocrine/metabolic system(HR 3.08 vs.2.01),and the neurological system(HR 2.70 vs.1.81).Despite large similarities in comorbidity patterns,a more complex and well-connected network was observed for early-onset ***,while patients with early-onset T2Dm got fewer benefits(12.67%reduction in pooled HR for all studied complications)through fair glycemic control(median HbA1c≤53 mmol/mol)compared to late-onset T2Dm patients(18.01%reduction),they seemed to benefit more from favorable lifestyles,including weight control,healthy diet,and adequate physical ***:Our analyses reveal that early-onset T2Dm is an aggressive disease resulting in more complex complication networks than late-onset *** glucose-lowering intervention,complemented by lifestyle modifications,are feasible strategies for controlling early-onset T2Dm-related complications.

A mechanism for Inhaled Anesthetic-Induced Solid Organ Injury: Inflammation

Background: Inhaled anesthetics, including halothane, iso- and sevoflurane induce proinflammatory cytokine release. Halothane is an inhaled anesthetic agent that is metabolized by the liver into a highly reactive product, trifluoroacetyl chloride, which can react endogenously to form a trifluoroacetyl-adduct (TFA-adduct). The mAA-adduct is formed by acetaldehyde and malondialdehyde reacting with endogenousproteins and is found in both patients and animals post-consumption of alcohol. These TFA and mAA-adducts have been shown to cause the release of proinflammatorycytokines by endogenous inflammatory cells. If both adducts share a similar mechanism of cell activation, receiving general anesthesia following alcohol ingestion could exacerbate the inflammatory response caused by the inhaled general anesthetic halothane and lead to solid organ (including liver and brain) injury. methods: Control diet and alcohol-fed rats were randomized to receive halothane pretreatments by intraperitoneal injection mixed in sesame oil. Following the intraperitoneal injections, the intact heart was removed, HECs were isolated and stimulated with unmodified bovine serum albumin (Alb), mAA-modified Alb (mAA-Alb), Hexyl-mAA, or lipopolysaccharide (LPS), and supernatant concentrations of TNF-α were determined. Results: Halothane pre-treated rat HECs demonstrated significantly greater TNF-α concentration following mAA-adduct and LPS stimulation than the non-halothane pre-treated in both pair and alcohol-fed rats, but was significantly greater in the alcohol-fed groups. Conclusion: These results demonstrate that halothane and mAA-adduct pre-treatment will increase the inflammatory response (TNF-α release) in rat HECs following LPS and mAA stimulation in vitro. Also, these results suggest that halothane exposure may increase the risk of alcohol-induced solid organ injury secondary to TNF-induced inflammation. Altered Oncogene Activity Contributes to Compensation for Antisense Suppression of Bcl-2 and Tumor Resistance

Antisense oligonucleotides (oligos) have targeted growth regulatory proteins in prostate cancer models. To identify compensatory alterations in the expression of non-targeted genes we evaluate mono- and bispecific oligos targeting and equally suppressing the expression of the apoptosis inhibitory protein bcl-2. Bcl-2 is chosen because oligos directed towards it have entered clinical trials to restore apoptosis in cancer patients. Treated LNCaP cells compensate for the diminished bcl-2 by suppressing caspase-3 (an apoptosis promoter) while enhancing expression of AKT-1 (another apoptosis inhibitor), androgen receptor (AR) and its (p300 and IL-6) coactivators. Additional proteins are enhanced including PD-1, its ligand PD-L1 (immune checkpoint blockade markers) and fas-ligand, which activate apoptosis through the signal transduction, along with suppressor protein p53, polymerase transcription mediator med-12 and signal transducer STAT-3. These alterations in expression may contribute to a greatly enhanced expression of the proliferation marker KI-67. This suggests that therapeutic approaches to restore apoptosis through suppression of bcl-2 lead to an altered expression in non-targeted genes involving apoptosis, androgen sensitivity, transcriptional activity and immune responsiveness, leads to an increase in proliferation (and a more androgen driven aggressive phenotype). In this study we evaluate the expression of two oncogenes (v-myc and K-ras) and find a large and significant enhancement of v-myc activity, which is produced by oligos targeting bcl-2 at the 5’ position. For K-ras, although significant suppression is produced by the bispecific targeting bcl-2 at the 3’ position, the percent change is relatively small compared with other compensatory alterations we have measured, and much less than in v-myc. Electroencephalogram(EEG)is a method of capturing the electrophy-siological signal of the *** EEG headset is a wearable device that records electrophysiological data from the *** paper presents the design and fab-rication of a customized low-cost Electroencephalogram(EEG)headset based on the open-source OpenBCI Ultracortex mark IV *** electrode placement locations are modiﬁed under a 10–20 standard *** fabricated headset is then compared to commercially available headsets based on the following para-meters:affordability,accessibility,noise,signal quality,and ***,the data is recorded from 20 subjects who used the EEG Headset,and signals were ***,the participants marked the accuracy,set up time,participant comfort,and participant perceived ease of set-up on a scale of 1 to 7(7 being excellent).Thirdly,the self-designed EEG headband is used by 5 participants for slide *** raw EEG signal is decomposed into a series of band sig-nals using discrete wavelet transform(DWT).Lastly,theseﬁndings have been compared to previously reported *** concluded that when used for slide-changing control,our self-designed EEG headband had an accuracy of 82.0 *** also concluded from the results that our headset performed well on the cost-effectiveness scale,had a reduced setup time of 2±0.5 min(the short-est among all being compared),and demonstrated greater ease of use.

Sperm-specific phospholipase C zeta(PLCζ)initiates intracellular calcium(Ca2+)transients which drive a series of concurrent events collectively termed oocyte *** investigations have linked abrogation and absence/reduction of PLCζwith forms of male infertility in humans where oocyte activation ***,very few studies have examined potential relationships between PLCζand advancing male age,both of which are increasingly considered to be major effectors of male *** efforts in humans may be hindered by inherent PLCζvariability within the human population,alongside a lack of sufficient controllable ***,utilizing immunoblotting,immunofluorescence,and quantitative reverse transcription PCR(qRT-PCR)we examined for the first time PLCζprotein levels and localization patterns in sperm,and PLCζmRNA levels within testes,from mice at 8 weeks,12 weeks,24 weeks,and 36 weeks of age,from two separate strains of mice,C57BL/6(B6;inbred)and CD1(outbred).Collectively,advancing male age generally diminished levels and variability of PLCζprotein and mRNA in sperm and testes,respectively,when both strains were ***,advancing male age altered the predominant pattern of PLCζlocalization in mouse sperm,with younger mice exhibiting predominantly post-acrosomal,and older mice exhibiting both post-acrosomal and acrosomal populations of PLCζ.However,the specific pattern of such decline in levels of protein and mRNA was ***,our results demonstrate a negative relationship between advancing male age and PLCζlevels and localization patterns,indicating that aging male mice from different strains may serve as useful models to investigate PLCζin cases of male infertility and subfertility in humans.

Perioperative Adjunct magnesium Decreases Postoperative Opioid Requirements—A meta-Analysis

Objectives: magnesium (mg) is the fourth most common cation in the body and has numerous physiological activities and anti-nociceptive effects. The anti-nociceptive effects are primarily mediated by regulation of calcium influx into the cell and antagonism of the N-methyl-D-aspartate glutamate receptors. Opioids are widely used as analgesics to minimize postoperative pain, but their use is associated with various side effects as well as the potential for addiction and tolerance. Systemic mg has been proposed as an adjunct to minimize postoperative pain in numerous clinical studies. This meta-analysis aims to critically examine the ability of perioperative intravenous (IV) mg to reduce opioid use and its’ side effects. methods: A comprehensive literature search of Pub med, Cochrane Central Registry of Controlled Trials, and Google Scholar (1966-2016) was performed to identify all randomized control trials (RCTs) assessing the use of perioperative IV mg in the reduction of postoperative opioid consumption. Keywords searched included combinations of “magnesium”, “pain”, “postoperative”, “preoperative”, “analgesia” and “opioid”. Inclusion criteria included RCTs comparing the use of perioperative IV mg with a control group in adult patients (>18 yrs) undergoing elective surgery. Cumulative opioid consumption within the first 24 hours (hrs) postoperative period and the incidence of nausea and vomiting were analyzed. Results: 14 RCTs involving 910 patients were identified (455 patients received mg and 455 patients received placebo or no therapy). Opioid consumption was significantly decreased in the systemic mg group (standard mean difference [SmD]: 1.39, 95% CI 1.83 to -0.96;p p p = 0.234). Systemic mg adjunct had no significant effect on postoperative nausea and vomiting (RR = 0.63;95% CI 0.38 to 1.04;p = 0.07). Addition of hip exercises to treatment of patellofemoral pain syndrome:A meta-analysis

AIm:To determine if the addition of hip-strengthening exercises decreases pain and improves function in patients with patellofemoral pain ***:The authors completed a systematic reviewsearching eight databases(i.e.,Pub med,Cochrane,CINHAL,medLINE,Sports Discus,EmBASE,APTA Hooked on Evidence,and PEDro).Two independent reviewers screened and excluded studies if they did not meet the following inclusion criteria:subjects had a primary diagnosis of patellofemoral pain syndrome(PFPS),intervention group included hip-strengthening exercises,control group included a traditional physical therapy intervention,study included outcome measures of pain and/or function,study used a randomized controlled trial design,PEDro score was ≥ 7,and study was published in a peer-reviewed *** outcome measures were subjective scales of pain and *** measures were converted to standardized mean difference [effect size(ES)],and a random-effects model was used to calculate the overall ***:Two hundred eighty-three studies were screened for inclusion in our *** studies were deemed suitable for data extraction and analysis.A total of 426 subjects were used in the nine ***,there was a significant positive effect of hip-strengthening exercises on measures of pain and function in subjects with PFPS(ES = 0.94,P = 0.00004).None of the individual studies had a negative ES,with study ES ranging from 0.35 to *** of the high degree of between-study variance(I2 = 76%;Q = 34.0,P meta-analyses and meta-regressions were *** of the potential moderator variables that were investigated(e.g.,outcome type,hip region targeted,duration of treatment) could explain a significant amount of the between-study variance in ES(P ≥ 0.23).CONCLUSION:Overall,the addition of hip-strengthening exercises to traditional physical therapy produced greater improvements in measures of pain and function.
