The objectives of this study were to assess the safety and efficacy of intralesionally administered diclofenac in the treatment of cutaneous neurofibromas in patients with NF1. This was a proof-of-concept, prospective, safety and efficacy study of the effect of intralesionally administered diclofenac 25 mg/ml given once a week to 3 target cutaneous neurofibromas for 4 consecutive weeks. Overall, there was no significant change in neurofibroma size. During the study, some treated lesions developed signs of necrosis and fell off after a few weeks, but none of the control neurofibromas fell off. There were no significant changes in patient’s vital signs. A few adverse events occurred, mostly at the injection sites. During the study, some neurofibromas developed necrosis after the diclofenac injections and eventually detached from the patient. Overall, diclofenac was well tolerated, suggesting minimal systemic exposure, which required confirmation and further studies, including bioavailability analysis.

Effect of Ultra-Turrax on Nanocellulose Produced by Acid Hydrolysis and Modified by Nano ZnO by Sol-Gel Method

Cellulose nanocrystals (NCC) and cellulose nanofibrils (CNF) were obtained by a single step process, with synergy between 64% sulfuric acid hydrolysis and high shear from ultra-turrax stirring, which is an advantageous process for disintegrating cellulose microcrystalline and also may improve the hydrolysis process. The surface modification on the cellulose was performed by the sol-gel process, in which the sulfate groups from hydrolysis were replaced by nanoparticles of zinc oxide, which led to the increase of up to 54°C Tonset, according to thermogravimetric analysis (TGA) results. The morphology and crystallinity degree were characterized by Helium Ion Microscopy (HIM), atomic force microscopy (AFM) and X-ray diffraction. In addition, the ZnO band was observed in Fourier transform infrared spectroscopy, furthermore, the change in the zeta potential confirmed the cellulose modification. The changes in the values of proton spin-spin relaxation time for the systems showing the confined hydrogen in the rigid domains, confirmed the results observed with the aforementioned techniques, for both cellulose after hydrolysis and ZnO modified cellulose, suggesting that ZnO disrupted crystal formation in cellulose.

Exercise training in hypertension:Role of microRNAs

Hypertension is a complex disease that constitutes an important public health problem and demands many studies in order to understand the molecular mechanisms involving his pathophysiology. Therefore, an increasing number of studies have been conducted and new therapies are continually being discovered. In this context, exercise training has emerged as an important non-pharmacological therapy to treat hypertensive patients, minimizing the side effects of pharmacological therapies and frequently contributing to allow pharmacotherapy to be suspended. Several mechanisms have been associated with the pathogenesis of hypertension, such as hyperactivity of the sympathetic nervous system and renin-angiotensin aldosterone system,impaired endothelial nitric oxide production, increased oxygen-reactive species, vascular thickening and stiffening, cardiac hypertrophy, impaired angiogenesis, and sometimes genetic predisposition. With the advent of microRNAs(miRNAs), new insights have been added to the perspectives for the treatment of this disease, and exercise training has been shown to be able to modulate the miRNAs associated with it. Elucidation of the relationship between exercise training and miRNAs in the pathogenesis of hypertension is fundamental in order to understand how exercise modulates the cardiovascular system at genetic level. This can be promising even for the development of new drugs. This article is a review of how exercise training acts on hypertension by means of specific miRNAs in the heart, vascular system, and skeletal muscle.

Background: Oxidative stress and inflammation are related to the pathophysiology of diabetes mellitus (DM), being involved in the development of micro-and macrovascular complications. Physical activity is beneficial for DM patients, but little is known about the relationship between redox and inflammation biomarkers and the level of physical activity in these patients. Based on this, this research aims to evaluate the effects of physical activity level on redox stress parameters and inflammatory markers in T2DM patients. Methods: Eighty-four patients with T2DM were divided according to their physical activity level: group A (n = 48), sedentary;group B (n = 11) active (3 times a week, 150 min) and group C (n = 25), highly active (5 times a week, 150 - 300 mins, at least). Anthropometric and biochemical parameters, superoxide dismutase (SOD) and glutathione peroxidase (GPx) activities, as well as GSH, sRAGE and ICAM-1 levels were assessed. Results: Glycated haemoglobin, total cholesterol, and LDL-cholesterol levels were lower in the highly active group in comparison to other groups. Plasma SOD activity was higher in group C compared to Group A, while ICAM-1 levels were significantly higher in group B when compared to other groups. Conclusion: Our results suggest that the practice of physical activity is beneficial to T2DM patients, especially at high volume and frequency.

Production of Dietetic Jam of Umbu-Caja (<i>Spondias</i>sp.): Physical, Physicochemical and Sensorial Evaluations

The umbucajazeira is specie native to the Brazilian Northeast semiarid areas, which is economically explored based on the fruit. The fruit has a high agroindustrial potential, mainly for jam production. The aim of this study was to evaluate through sensorial analysis, two formulations of umbu-caja jams: a dietetic one with xylitol, used as an option for substitution of the sugars, and a conventional one. The proportion of 50:50:0.5 (pulp/sugar/pectin or pulp/xylitol/pectin) was used for jam processing. The mixture was concentrated up to reach 65°Brix. The jams were evaluated by 50 non-trained tasters, through a hedonic scale of nine points, whereas the following attributes were used: appearance, color, flavor, aroma and texture. The results of the physical and physicochemical analysis showed that the characteristics of jams prepared with sucrose are close to the ones made with xylitol, except for the total sugars and the energetic value. The jams had pH values ranging between 2.37 and 2.92;titratable acidity varying between 0.56 and 0.78 g citric acid/100 g pulp;soluble solids ranging between 60.92°and 65.08°Brix, respectively. A tendency for yellow color was noticed by the color analysis. Jam diet had the highest scores for appearance (7.4 - 7.7). No significant differences (p > 0.05) were obtained to flavor and texture. The fruit of the umbucajazeira showed to be adequate for jam manufature since they had good sensorial acceptance. The use of xylitol was satisfactory for obtaining a product with jam characteristics similar to the conventional jam, but with lower energetic value.

Better Management of Adverse Events Favors Sorafenib Treatment of HCC Patients and Impact on Survival

Introduction: Sorafenib is an orally active multikinase inhibitor approved for the treatment of advanced hepatocellular carcinoma (HCC) and is the only systemic treatment associated with a survival benefit in advanced stage. The aims of this study were to evaluate the tolerance and survival of sorafenib-treated patients, and to identify potential prognostic factors of survival. Methods: A total of 88 HCC patients treated with sorafenib from June 2010 to January 2014 were retrospectively reviewed. Tumour stage, liver function and adverse events to sorafenib were analyzed. Univariate and multivariate analysis were carried out to identify predictors of survival in patients with advanced HCC treated with sorafenib. Results: There were 64 (73%) males included in this study, with a median age of 61.16 years. Eight (91%) patients had Child-Pugh class A cirrhosis. Most patients were classified as BCLC C (82%). Hepatitis C virus was the predominant cause of HCC (68%). Sorafenib was the initial treatment modality in 43%. Median time of sorafenib therapy was 8.23 months. Overall survival in 1 year was 57.3% and 36.7% in 2 years. The median survival was 16.3 months. In the univariate analysis of the OS based on Child-Pugh score did not demonstrate a significant difference in our study (p = 0.62). The presence of dermatologic adverse event predicted a better overall survival in the multivariate analysis. Better survival was also observed in patients with AFP level <100 ng/ml in the start of sorafenib therapy (p = 0.001). Patients that used Sorafenib for ≥6 months had shown better outcome. None of the patients discontinued sorafenib because of adverse effects. Conclusions: Advanced HCC patients treated with sorafenib who experienced dermatologic adverse event and low AFP level <100 ng/ml showed better overall survival. Initial Development of Cowpea Bean Fertilized with Natural Phosphate in the Brazilian Cerrado Soil

Cowpea, string Bean, or Macassar bean [Vigna unguiculata (L.) Walp] presents a short life cycle, low water requirement, develops in soils of low fertility, and has the ability to fix nitrogen from the air. In the Cerrado, the oxisols occupy practically all the flat to soft-wavy areas with little reserve of nutrients for the plants, in general, they are soils with great limitations of fertility. A source of alternative phosphate fertilization is the use of reactive natural phosphates. The reactive natural phosphate of Bayóvar presents intermediate solubility between the sources of soluble phosphates and the natural phosphates. In this context, the objective was to evaluate the initial development of Cowpea beans fertilized with the natural Bayóvar phosphate used for cultivation of the Brazilian Cerrado Oxisol. The experimental design was completely randomized, with six doses of phosphorus (0, 100, 200, 300, 400, 500 mg·dm-3) using natural Bayovar phosphate as a source in four replications. The species used in this study was the legume cowpea bean [Vigna unguiculata (L.) Walp] cultivar Tumucumaque. The treatments were applied using the natural phosphate Bayóvar as a source of phosphorus (29% of P2O5). At 33 das (days after sowing), the variable number of leaves of the Cowpea bean was analyzed and at 40 das, the variables plant height, SPAD chlorophyll index (Soil Plant Analysis Development) and stem diameter were analyzed. For all variables analyzed there was a significant effect. The initial development of the cowpea bean cultivated in the Brazilian Cerrado Oxisol was significantly influenced by the Bayóvar natural phosphate fertilization with the best phosphorus (P2O5) doses in the range of 200 to 350 mg·

Rapid Differentiation of Closely Related Citrus Genotypes by Fluorescence Spectroscopy

The differentiation of closely related Citrus genotypes is a meticulous, laborious, and time-consuming task that involves the assessment of complex traits such as growth, tolerance to stress, photosynthetic efficiency, yield, and many others. Such a task is generally accomplished either by analyzing specific features of adult plants or by applying molecular markers to young trees. On one hand, only after plants start yielding can distinct genotypes be differentiated by comparing their fruit sizes, shapes, taste, and the number of seeds. On the other hand, molecular markers are expensive, and demand expertise and time for the analysis of a larger number of plants. For these reasons, the development of techniques that could assist in an early, quick and accurate differen-tiation of closely related Citrus varieties is of utmost importance. In this context, laser-induced fluorescence spectroscopy (LIFS) is a promising technique, since it is rapid, highly sensitive, and inexpensive. Previous studies showed that LIFS can differentiate a variety of sweet orange. However, this new study aimed to determine LIFS accuracy in the differentiation and grouping of very closely varieties of four Sunki mandarin selections: Comum, Florida, Tropical, and Maravilha. Furthermore we compared the results with ISSR and SSR molecular markers for the same varieties. LIFS technique distinguished the four selections with accuracy greater than 70%. Only with molecular markers was possible distinguishing clearly Tropical from Maravilha, but not Comum from Florida selections. In this way the results suggest that LIFS may be a sound tool for helping the identification of closely Citrus varieties.

Use of Initial Modified RECIST Tumor Response Evaluation Criteria for Predicting Survival in Patients with Hepatocellular Carcinoma Undergoing Transarterial Chemoembolization with Drug-Eluting Beads

Introduction: Transarterial chemoembolization (TACE) reduces tumor growth and increases survival in patients with hepatocellular carcinoma (HCC). Drug-eluting beads (DEB) deliver slow-release chemotherapy and reduce systemic toxicity during TACE. This study correlated initial tumor response according to modified RECIST (mRECIST) criteria and 1-year survival in patients with HCC treated with TACE-DEB, and identified predictors of tumor response. Methods: Fifty-two patients with HCC received TACE-DEB loaded with doxorubicin 75 mg during a 6-month period. Tumor response was evaluated 1 month after the procedure according to mRECIST criteria. Results: Most patients were cirrhotic and etiology of liver disease was hepatitis C in 26/52 (50%). Similar numbers of patients had Barcelona Clinic Liver Cancer (BCLC) A and BCLC B disease. Most patients had one nodule (66%). Complete response (CR) was achieved in 12/52 (23%), partial response in 19/52 (37%), stable disease in 4/52 (8%) and progressive disease in 17/52 (32%). Largest HCC ≤58 mm and BCLC stage A were associated with CR. The 1-year survival was 74%, with survival rates of 95% and 56% in the BCLC A and B groups, respectively. Variables reflecting tumor extension were associated with better survival. CR according to mRECIST criteria was a predictor of better 1-year survival (100% vs. 64%, P < 0.05). Conclusion: BCLC A and CR according to mRECIST criteria predict improved 1-year survival in patients with HCC treated with TACE-DEB. Further studies are needed to evaluate other predictors of survival and to determine if tumor response predicts long-term survival.
