Platelet biomarkers in Alzheimer's disease

The search for diagnostic and prognostic markers in Alzheimer's disease(AD) has been an area of active research in the last decades. Biochemical markers are correlates of intracerebral changes that can be identified in biological fluids, namely: peripheral blood(total blood, red and white blood cells, platelets, plasma and serum), saliva, urine and cerebrospinal fluid. An important feature of a biomarker is that it can be measured objectively and evaluated as(1) an indicator of disease mechanisms(markers of core pathogenic processes or the expression of downstream effects of these processes), or(2) biochemical responses to pharmacological or therapeutic intervention, which can be indicative of disease modification. Platelets have been used in neuropharmacological models since the mid-fifties, as they share several homeostatic functions with neurons, such as accumulation and release of neurotransmitters, responsiveness to variations in calcium concentration, and expression of membrane-bound compounds. Recent studies have shown that platelets also express several components related to the pathogenesis of AD,in particular to the amyloid cascade and the regulation of oxidative stress: thus they can be used in the search for biomarkers of the disease process. For instance, platelets are the most important source of circulating forms of the amyloid precursor protein and other important proteins such as Tau and glycogen synthase kinase-3B. Moreover, platelets express enzymes involved in membrane homeostasis(e.g., phospholipase A2), and markers of the inflammatory process and oxidative stress. In this review we summarize the available literature and discuss evidence concerning the potential use of platelet markers in AD.

为建立BMY牛的活体采卵(OPU)-体外胚胎生产(IVP)-胚胎移植(ET)快速扩繁技术体系,从人工草地全放牧牛群中选择健康的经产空怀BMY牛作为供体(n=14)集中OPU一次;卵母细胞经TCM-199体外成熟培养(IVM)、Fert-TALP液体外受精(IVF),受精卵用合成输卵管液(SOF)体外培养(IVC)生产胚胎;BMY牛受体(n=81)用CUE-MATETM孕酮阴道栓+EB+FSH+pg方法同期发情处理,对黄体合格的移植OPU-IVP来源鲜胚。研究结果表明:①OPU头均获卵母细胞23.50枚,获10枚以上卵母细胞的供体占85.7%;IVP的平均囊胚数和可用胚数分别为5.21和4.00枚;OPU前8～9d卵巢上有黄体(CL)的供体,头均获卵母细胞数及IVP的囊胚数、可移植胚数、囊胚率和可移植胚率分别比对照组高7.28枚(P0.05)。结果显示,BMY母牛具有良好的OPU潜力,但个体间差异较大;可通过严格供体选择及改善IVP技术等措施,实现OPU-IVP效率最大化。CUE-MATETM+EB+FSH+pg同期发情处理方法适于在牛规模化胚胎移植(ET)和人工授精(AI)中应用。综合应用OPU、IVP、同期发情和ET技术是加快优良BMY牛扩繁的重要途径之一。

Bone Dysmorphia-Induced Blindness Following a Secondary Hyperparathyroidism: A Case Report

Introduction: Conjunctival-corneal or choroidal calcifications are frequent in SHPT, blindness is however exceptional. We report a case of blindness secondary to compressive ischemic optic neuropathy. Case Report: Mr. B.E.K., 49 years old, has a chronic renal failure secondary to unlabeled glomerular nephropathy for 17 years. He has been on chronic hemodialysis for 12 years and has had SHPT for nine years. He secondarily developed disabling segmental osteoarticular deformities associated with kyphoscoliosis, “drumstick” fingers and facial dysmorphism. Five months before admission he developed eye pain and reduced visual acuity progressing within one month to blindness. Biology noted: serum creatinine at 726 umol/l (60 - 120 umol/L), azotemia at 14.3 mmol/l (2.5 - 7.5 mmol/L), serum calcium at 2.25 (2.25 - 2.55 mmol/L), phosphatemia at 1.13 (0.8 - 1.35 mmol/L), alkaline phosphatases at 2196 (5 - 270 IU/L) and parathyroid hormone level at 2257 (10 - 60 pg/mL). Retinal angiography revealed lesions suggestive of ischemic neuropathy. The orbit CT scan with 3D coronal reconstruction revealed narrowing of the caliber of the optical channels with dystrophic thickening of the skull base and cranial vault. Cranioencephalic and orbital MRI revealed diffuse brown tumors and pre-chiasmatic optic atrophy. Discussion: The most frequent ocular complications of SHPT are conjunctival-corneal or sclero-choroidal calcifications, asymptomatic, associated with hypercalcemia. Compressive manifestations are rarer, represented mainly by an amputation of the visual field, diplopia, ptosis or blindness, as described in our patient. The main cause is osteodystrophy and brown tumors of the skull base (1% - 2%). Conclusion: This case report underlines the importance of early detection of SHPT, in order to avoid its major complications, such as blindness, especially since current preventive and curative measures have proven their

Serum proinflammatory cytokines and nutritional status in pediatric chronic liver disease

AIM: To evaluate the nutritional status and its association with proinflammatory cytokines in children with chronic liver ***: We performed a cross-sectional study with 43 children and adolescents, aged 0 to 17 years, diagnosed with chronic liver disease. All patients regularly attended the Pediatric Hepatology Unit and were under nutritional follow up. The exclusion criteria were fever from any etiology at the time of enrollment, inborn errors of the metabolism and any chronic illness. The severity of liver disease was assessed by Child-Pugh, Model for End-stage Liver Disease(MELD) and Pediatric End Stage Liver Disease(PELD) scores. Anthropometric parameters were height/age, body mass index/age and triceps skinfold/age according to World Health Organization standards. The cutoff points for nutritional status were risk of malnutrition(Z-score pg/m L vs 1.63(0.53-3.43) pg/m L; P = 0.02], correlating inversely with triceps skinfold-for-age z-score(rs =-0.61; P < 0.001). IL-6 levels were associated with liver disease severity assessed by Child-Pugh score(P = 0.001).
