Survival predictors in patients treated with a molecular adsorbent recirculating system

AIM:To identify prognostic factors for survival in patients with liver failure treated with a molecular adsorbent recirculating system(MARS). METHODS:MARS is a liver-assisting device that has been used in the treatment of liver failure to enable native liver recovery,and as a bridge to liver transplantation(LTX).We analyzed the 1-year outcomes of 188 patients treated with MARS,from 2001 to 2007, in an intensive care unit specializing in liver disease. Demographic,clinical and laboratory parameters were recorded before and after each ***-year survival and the number of LTXs were *** regression analysis was performed to determine factors predicting survival. RESULTS:The study included 113 patients with acute liver failure(ALF),62 with acute-on-chronic liver failure(AOCLF),11 with graft failure(GF),and six with miscellaneous liver *** was performed for 29% of patients with ALF,18% with AOCLF and 55%with *** overall 1-year survival rate was 74% for ALF,27% for AOCLF,and 73% for *** poorest survival rate,6%,was noted in non-transplanted patients with alcohol-related AOCLF and cirrhosis,whereas,patients with enlarged and steatotic liver had 55% *** etiology of liver failure was the most important predictor of survival(P<0.0001).Other prognostic factors were encephalopathy(P=0.001)in paracetamol-related ALF, coagulation factors(P=0.049)and encephalopathy(P=0.064)in non-paracetamol-related toxic ALF,and alanine aminotransferase(P=0.013)and factor V levels(P =0.022)in ALF of unknown etiology. CONCLUSION:The etiology of liver disease was the most important prognostic *** treatment appears to be ineffective in AOCLF with end-stage cirrhosis without an LTX option.

Cocrystallization-like strategy for the codelivery of hydrophobic and hydrophilic drugs in a single carrier material formulation

Adoption and Impact of Modern Rice Varieties on Poverty in Eastern India

The factors affecting the adoption of modern varieties(MVs) of rice and impact on poverty in Odisha, India were discussed. A total of 363 households from Cuttack and Sambalpur districts of Odisha via multistage sampling technique participated in the survey. The Cragg's Double hurdle model was used to model the determinants of adoption and intensity of adoption of MVs of rice, and the propensity score matching was used to analyze the impact of adoption on poverty. The results showed that age, education, risk aversion, land size, yield, perception of MVs as high yielding, resistant to diseases and availability of MVs positively influenced the decision to adopt. However, variables such as household size, experience of a farmer, off-farm job participation, amount of credit received, cost of seeds, insecticides and fertilizers negatively influenced the adoption of MVs. Intensity of adoption of MVs was negatively influenced by experience of a farmer, cost of fertilizer and marketability of MVs, and positively affected by household size, risk aversion, land size, cost of insecticides, perception of MVs as high yielding and availability of MV seeds. Poverty incidence, gap and severity were high among non-adopters to adopters of MVs. After matching adopters and non-adopters of MV groups using four different algorithms of nearest neighbour matching, stratification matching, radius matching and kernel matching, the impact of MV adoption resulted in higher per capita monthly household expenditure by about US$ 52.82 to US$ 63.17.

The use of valley excitonic states of transition metal dichalcogenides to store and manipulate information is hampered by fast carrier recombination and short valley *** propose theoretically a scheme to overcome such an obstacle,by applying a tilted exchange field through the magnetic proximity effect on monolayer MoS_(2).While the in-plane component of the exchange field brightens the dark exciton by spin mixing,the out-of-plane field can effectively gate the emission with an ON/OFF ratio of ***,the brightening is valley selective,leading to nearly 100% valley and spin polarization at room *** resulting strongly gateable dark-exciton emission with long lifetime and near unity valley polarization makes it convenient to manipulate the valley degree of freedom,which may offer new paradigm for information processing and transmission.

Uterine cervical carcinoma treated with chemoradiotherapy: impact of three-month MRI follow-up on clinical management and outcome

Aim:The aim of this study was to evaluate the impact of MRI performed three months after treatment on further follow-up interventions and outcome in patients with uterine cervical carcinoma treated with definitive ***:Sixty consecutive women diagnosed with uterine cervical cancer FIGO 2009 stage IB1-IVA during 2011-2012 treated with definitive chemoradiotherapy/radiotherapy with curative intent at the Department of Gynaecological Oncology at Karolinska University Hospital were retrospectively included.A review of MRI reports and medical records with focus on follow-up interventions associated to imaging was ***:On follow-up MRI three months post treatment,29/60 women had complete remission(mrCR),24/60 women had partial remission(mrPR)and 7/60 had progressive disease(mrPD).In patients with mrCR,no additional procedures were *** group with mrPR had 27 additional MRIs,3 PET/CT examinations and 9 biopsy procedures,none leading to diagnosis of residual *** control rate was 96%after 6.5 months(median).No patient had cervical relapse only;2/53 had cervical relapse in combination with non-regional lymph nodes and distant *** was no statistically significant difference in overall survival between patients with mrCR and mrPR(HR=2.2,P=0.21).Conclusion:Patients with residual changes on MRI at three months post treatment have a low risk for locoregional *** this is not recognised,follow-up MRI results in unnecessary additional procedures with low impact on treatment *** studies are needed regarding the most appropriate imaging modality and timing of post-treatment evaluation.

Functional Status Information(FSI)describes physical and mental wellness at the whole-person *** includes information on activity performance,social role participation,and environmental and personal factors that affect the well-being and quality of *** and analyzing this information is critical to address the needs for caring for an aging global population,and to provide effective care for individuals with chronic conditions,multi-morbidity,and *** knowledge graphs(PKGs)represent a suitable way for meaning in a complete and structured way all information related to people's FSI and reasoning over them to build tailored coaching solutions supporting them in daily life for conducting a healthy *** this paper,we present the development process related to the creation of a PKG by starting from the HeLis ontology in order to enable the design of an Al-enabled system with the aim of increasing,within people,the self-awareness of their own functional *** particular,we focus on the three modules extending the HeLis ontology aiming to represent(i)enablers and(ii)barriers playing potential roles in improving(or deteriorating)own functional status and(iii)arguments driving the FSI collection ***,we show how these modules have been instantiated into real-world scenarios.
