Intensive care unit outcomes in patients with hematological malignancy

Hematological malignancies are usually life-limiting *** of care need to be decided early,based on acceptability to the patient,family,physician,and *** intensive care unit(ICU)admission is likely to result in significant physical,psychological,and economic *** is little published on the impact of non-acute preadmission disease factors on ICU outcomes in hematological ***:To identify baseline performance and disease-associated factors before admission to ICU in patients with hematological malignancy that contribute to subsequent ICU ***:A retrospective analysis of electronic medical records,laboratory results,and Intensive Care data for all patients(n=184)with hematological malignancy admitted to the Calvary Mater Hospital ICU between January 1,2013 and June 30,2017 was *** age,gender,condition,Eastern Cooperative Oncology,and Charlson Comorbidity scores were compared to ICU outcome and overall ***-specific prognostic risk scores were compared to ICU ***:Overall,73.9%survived the ICU admission,with 31.6%surviving at 12 *** ejection fractions(>55%)and prognosis>12 months(based on disease-specific risk scores)were significantly associated with overall survival(P=0.024 and P=0.001).Induction and posttransplantation therapy were predictive of poor ICU survival outcome(P<0.0001 and P=0.041).APACHE scores were significant predictors of ICU mortality(P=0.002 for APACHE II and P<0.0001 for APACHE III).Conclusion:Survival outcomes for patients with hematological malignancy admitted to the ICU correlate with functional and comorbidity ***-specific prognostic scores can assist in recognizing patients likely to benefit from ICU admission.

【目的】在任何地方进行游戏是儿童的天性。以北京中心城区高密度城市环境为例,深入探究儿童如何创造性地使用城市户外公共空间。【方法】从景观—社会学视角,通过半构建访谈、照片访谈和行为观察等方法收集儿童户外活动空间数据。【结果】将儿童使用的多样户外空间分为专属空间和发现空间,以及介于两者之间的灵活空间3种类别。深入探究不同类别的活动空间的特点发现,配置有固定游戏设备的儿童专属游乐场所提供的游戏机会单一,无法满足全龄儿童进行多样户外游戏的需求,但专属空间为儿童提供了最为正式且相对安全的游戏环境;儿童广泛使用的灵活空间,如广场和可进入的绿地,其开放性为更为广泛的年龄群体提供了可供奔跑、跳跃和骑车等高强度活动的机会,但营造公平且友好的共享公共环境是灵活空间面临的挑战;发现空间的设计初衷并非为儿童使用,因此儿童在使用这类空间的过程中普遍存在潜在安全隐患。【结论】针对高密度城市环境中有限的公共空间提出了更加包容且儿童友好的营造方式以及优化设计建议,期望为提升高密度城市公共空间服务效能、实现城市可持续发展提供一定科学参考。

Accuracy between clinical and radiological diagnoses compared to surgical orbital biopsies

AIM:To assess the concordance between diagnosing orbital lesions by clinical examination,orbital imaging,and histological evaluation,in order to help guide future research and clinical ***:A retrospective analysis was undertaken at a large regional tertiary referral centre of all surgical orbital biopsies performed over a 5-year period,from 1st January 2015 until 31st December *** and concordance between clinical,radiological and histological diagnoses are reported as percentage sensitivity and positive predictive ***:A total of 128 operations involving 111 patients were ***,sensitivities of 47.7%for clinical and 37.3%for radiological diagnoses were found when compared to the histological gold *** lesions that have characteristic clinical and radiological features had the highest sensitivity at 71.4%and 57.1%,*** conditions showed the lowest sensitivity in both clinical(30.3%)and radiological(18.2%)*** PPV for inflammatory conditions were 47.6%for clinical and 30.0%for radiological ***:Accurate diagnoses are difficult to reach by relying on clinical examination and imaging *** orbital biopsy with histological diagnosis should remain the gold standard approach for definitively identifying orbital *** larger scale prospective studies would help further refine concordance and guide future research avenues.

Pure and noble metal(Pt,Pd,and Au)doped TiO_(2)nanoceramics have been prepared from TiO_(2)nanoparticles through traditional pressing and *** those samples sintered at 550℃,a typical premature sintering occurred,which led to the formation of a highly porous microstructure with a Brunauer-Emmett-Teller(BET)specific surface area of 23 m^(2)/*** room temperature,only Pt-doped samples showed obvious response to hydrogen,with sensitivities as high as~500 for 1000 ppm H_(2)in N_(2);at 300℃,all samples showed obvious responses to CO,while the responses of noble metal doped samples were much higher than that of the undoped *** mechanism for the observed sensing capabilities has been discussed,in which the catalytic effect of Pt for hydrogen is believed responsible for the room-temperature hydrogen sensing capabilities,and the absence of glass frit as commonly used in commercial thick-film metal oxide gas sensors is related to the high *** is proposed that much attention should be paid to metal oxide porous nanoceramics in developing gas sensors with high sensitivities and low working temperatures.

Lead strontium titanate(Pb_(0.5)Sr_(0.5))TiO_(3)(PST)nanotube arrays are prepared by a sol-gel and spin-coating process using an anodic aluminium oxide *** structure and morphology of the as-prepared sample are characterized by x-ray diffraction,scanning electron microscopy and transmission electron *** results show that the as-prepared PST sample possesses a perovskite structure with a relatively uniform diameter of about 200 ***(PL)spectrum of the sample shows emission bands centered at about 2.87 eV at room temperature,which consists of three intense emission sub-bands at 3.02,2.87 and 2.76 eV,*** energy corresponding to the central PL peak is lower than the band-gap energy(3.2 eV)of PST bulk materials,which may originate from the size effect of the PST nanotube *** multi-peaks in the PL spectrum may be attributed to the radiative recombination of trapped electrons and holes in tail and gap states induced by local defect and oxygen *** Raman scattering spectrum of the PST nanotube arrays,which are obviously broadened in comparison with the bulk materials,show that their phonon modes are in agreement with those allowed in the tetragonal phase.

The development of microenvironment-responsive nanoprobes has shown great promise for use in magnetic resonance imaging(MRI),with the advantage of significantly improved specificity and good ***,the clinical application of responsive probes is hampered by a lack of biological sensitivity for early molecular diagnostics and visualizing microenvionment of metabolism reprogramming in tumor ***,we report on a dual-ratiometric magnetic resonance tunable(DMRT)nanoprobe designed by crosslinking different ratios of transferrin chelating gadolinium and superparamagnetic nanoparticles,complexed to a pH responsive biocompatible *** dually activatable nanoprobe enables pH-dependent tumor microenvironment visualization,providing exceptional quantitative pathophysiological information in vitro and in *** used in combination with dual-contrast enhancement triple subtraction imaging technique(DETSI),this smart nanoprobe guarantees the diagnosis of early-stage *** envisage that this novel integrated nanoplatform will provide a new paradigm for the clinical translation of robust DMRT nanoprobes for early disease detection and staging,as well as microenvironment visualization and disease progression monitoring.

Progranulin （PGRN） is a growth factor implicated in various pathophysiological processes, including wound healing, inflammation, tumorigenesis, and neurodegeneration. It was previously reported that PGRN binds to tumor necrosis factor receptors （TNFR） and has thera- peutic effects in inflammatory arthritis （tang et. al, in Science 332：478-484, 2011）; however, Chen et al. reported their inability to demonstrate the PGRN-TNFR interactions under their own conditions （Chert et. al, in J Neurosci 33：9202-9213, 2013）. A letter-to-editor was then published by the original group in response to the Chen et al. paper that discussed the reasons for the latter＇s inability to recapitulate the interactions. In addition, the group published follow-up studies that further reinforced and dissected the interactions of PGRN- TNFR. Recently, the dispute about the legitimacy of PGRN-TNFR interactions appears to be finally settled with independent confirmations of these interactions in various conditions by numerous laboratories. This review presents a chronological update on the story of PGRN-TNFR interactions, highlighting the independent confirmations of these interactions in various diseases and conditions.

Two complementary classes of approaches exist to protect high security mobile ad hoc networks （MANETs）,prevention-based approaches,such as authentication,and detection-based approaches,such as intrusion *** previous work studies these two classes of issues *** this paper,we propose a framework of combining intrusion detection and continuous authentication in *** this framework,multimodal biometrics are used for continuous authentication,and intrusion detection is modeled as sensors to detect system security *** formulate the system and use dynamic programmingbased algorithms to derive the optimal schemes for both intrusion detection and continuous *** examples show the effectiveness of the proposed scheme.

The Sino-Japanese exchange of culture and painting reached its prime during ancient China’s tang and Song dynasties. However, in modern times, due to the sharp decrease in the exchange of painting between the two countries, we know little about each other’s *** Beijing International Art Biennale was held in this year’s golden autumn, which included the Painting Exhibition of Takayama Tatsuo - one of the "Four Masters of Modern Japanese Painting". It is undeniable that the biennial event offers the general public a golden chance to have a close look at Japanese painting and appreciate its unique style and great charm from a closer distance.

Uptake of prostate cancer screening and associated factors among Chinese men aged 50 or more: a population-based survey

Objective: To investigate the uptake rate of prostate specific antigen(PSA) testing among Hong Kong Chinese males aged 50 or above, and identify factors associated with the likelihood of undergoing a PSA ***: A population-based telephone survey was conducted in Hong Kong in 2007. The survey covered demographic information, perceived health status, use of complementary therapy, cancer screening behavior, perceived susceptibility to cancer and family history of cancer. Descriptive statistics, percentages and logistic regression analysis were used for data ***: A total of 1,002 men aged 50 or above took part in the study(response rate =67%), and the uptake rate of PSA testing was found to be 10%. Employment status, use of complementary therapy, perceiving regular visits to a doctor as good for health and the recommendations of health professionals were significant factors associated with PSA ***: The uptake rate of PSA testing in the study population was very low. Among all the factors identified, recommendations from health professionals had the strongest association with the uptake of PSA testing, and they should therefore take an active role in educating this population about cancer prevention and detection.
