Jejunal.diverticul. are rare and subsequent compl.cations even more so. The usual.smal. bowel.diverticul.m encountered by general.surgeons is a Meckel.s. These are embryol.gical.remnants of the vitel.o-intestinal.duct and are on the anti-mesenteric surface of the terminal.il.um. They may contain heterotopic gastric or pancreatic mucosa. Herein we expl.re the case of a young girl.who presented with features of peritonitis secondary to a compl.cation from a jejunal.diverticul.m. The case, pathol.gy, compl.cations and treatment of jejunal.diverticul.sis and heterotopic gastric mucosa in the jejunum are expl.red.

Monocytes in systematic infl.mmatory response syndrome:Differences between sepsis and acute pancreatitis

The neural.differentiation of embryonic stem cel.s(ESCs) is a dynamic and compl.x process, which has been shown to be regul.ted by several.different signal.ng pathways. Ca2+is a highl. versatil. intra-and intercel.ul.r second messenger, has been reported to be invol.ed in several.of these regul.tory pathways. Thus, by inference we hypothesized that Ca2+ may pl.y an important rol. in regul.ting neural.differentiation of ESCs. The goal.of our research was, therefore, to investigate the rol. of canonical.transient receptor potential.channel.subtype 3(TRPC3), a cation channel.that is permeabl. to both Ca2+ and Na+, in the regul.tion of the neural.differentiation of mouse ESCs(m ESCs). We demonstrate that TRPC3 is expressed in m ESCs and that it contributes to store-operated Ca2+ entry(SOCE) in these cel.s fol.owing treatment with thapsigargin. We al.o show that the l.vel.of TRPC3 expression gradual.y increases as m ESCs differentiate into cel.s of the neural.l.neage. sh RNA knockdown of TRPC3 suppressed its expression by 0% and l.d to a reduction in the expression of the neural.differentiation markers: nestin, sox1 and tuj1. Furthermore, treatment of m ESCs with the TRPC3 inhibitor, pyr3, al.o inhibited the expression of these neural.differentiation markers and reduced the amount of SOCE. Interestingl., TRPC3 knockout with CRISPR/Cas9, or inhibition with pyr3 al.o resul.ed in dramatic cel. death during the earl. stages of m ESC-derived neural.differentiation. Taken together, TRPC3 appears to positivel. regul.te the neural.differentiation of m ESCs and it is al.o appears to be required for cel. survival.during this process. We are now studying how the suppression of TRPC3 affects the cel.ul.r and mol.cul.r mechanisms invol.ed in the inhibition of neural.differentiation and cel. survival.

AIM: To unravel.the differences between systematic in- fl.mmatory response syndrome (SIRS) of acute pancre- atitis compared to the same syndrome in sepsis. METHODS: Twenty-five patients were enrol.ed, 12 with sepsis and 13 acute pancreatitis. After diagnosis 20 ml.bl.od was sampl.d. Hal. were assayed for isol.tion of monocytes and 10 ml.was centrifuged for serum test of tumor necrosis factor al.ha (TNFα) and interl.ukin-6 (Il.6). Hal. of monocytes were incubated in the presence of patients’ serum and supernatants were col.ected. The other hal. was treated for estimation of optical.photom- etry under caspase-3 inhibition. TNFα and Il.6 were es- timated by an enzyme immunoassay. RESUl.S: median ± SE of serum Il.6 in septic patients and acute pancreatitis patients was 192.30 ± 35.40 ng/l.and 21.00 ± 16.05 ng/l. respectivel. (P l.es of caspase-3 were 0.94 ± 0.17 pmol.min 104 cel.s and 0.34 ± 0.09 pmol.min 104 cel.s (P l.6 of monocyte supernatants of patients with sepsis was significantl. increased after addition of patients’ serum, whil. that of patients with acute pancreatitis did not show significant difference. CONCl.SION: The data have shown that monocyte activity is different between acute pancreatitis and sepsis. This phenomenon might be expl.ined as a different pathway to the pro-infl.mmatory cytokines rel.ase or coul. be a novel.anti-infl.mmatory response in acute pancreatitis.

AIM To eval.ate incidence, risk factors and treatment outcome of BK pol.omavirus nephropathy(BKVN) in a cohort of renal.transpl.nt recipients in the Auckl.nd region without a formal.BK pol.omavirus(BKV) surveil.ance *** A cohort of 226 patients who received their renal.transpl.nts from 2006 to 2012 was retrospectivel. *** Seventy-six recipients(33.6%) had a BK viral.l.ad(BKVl. test and 9 patients(3.9%) devel.ped BKVN. Col. ischaemia time(HR = 1.18, 95%CI: 1.04-1.35) was found to be a risk factor for BKVN. Four recipients with BKVN had compl.te resol.tion of their BKV infection; 1 recipient had BKVl.l.ss than 625 copies/ml. 3 recipients had BKVl.more than 1000 copies/ml.and 1 had graft fail.re from BKVN. BKVN has a negative impact on graft function [median estimated gl.merul.r fil.ration rate(eG FR) 22.5(IQR 18.5-53.0) ml./min per 1.73 m^2, P = 0.015), but no statistical.y significant difference(P = 0.374) in renal.al.ograft function was found among negative BK viraemia group [median e GFR 60.0(IQR 48.5-74.2) ml./min per 1.73 m^2), positive BK viraemia without BKVN group [median eG FR 55.0(IQR 47.0-76.0) ml./min per 1.73 m^2] and unknown BKV status group [median eG FR 54.0(IQR 43.8-71.0) ml./min per 1.73 m^2]. The incidence and treatment outcomes of BKVN were simil.r to some centres with BKV surveil.ance *** Recipients with BVKN have poorer graft function. Al.hough active surveil.ance for BKV has been shown to be effective in reducing incidence of BKVN, it shoul. be tail.red specifical.y to that transpl.nt centre based on its epidemiol.gy and outcomes of BKVN, particul.rl. in centres with l.mited resources.

e-rel.ted macul.r degeneration （AMD） causes irreversibl. l.ss of central.vision for which there is no effective treatment. Incipient pathol.gy is thought to occur in the retina for many years before AMD manifests from midl.fe onwards to affect a l.rge proportion of the el.erl.. Al.hough genetic as wel. as non-genetic/environmental.risks are recognized, its compl.x aetiol.gy makes it difficul. to identify susceptibil.ty, or indeed what type of AMD devel.ps or how quickl. it progresses in different individual.. Here we summarize the l.terature describing how the Al.heimer＇s-l.nked amyl.id beta （Aβ） group of misfol.ing proteins accumul.te in the retina. The discovery of this key driver of Al.heimer＇s disease in the senescent retina was unexpected and surprising, enabl.ng an al.ogether different perspective of AMD. We argue that Aβ fundamental.y differs from other substances which accumul.te in the ageing retina, and discuss our l.test findings from a mouse model.in which physiol.gical.amounts of Aβ were subretinal.y-injected to recapitul.te sal.ent features of earl. AMD within a short period. Our discoveries as wel. as those of others suggest the pattern of Aβ accumul.tion and pathol.gy in donor aged/AMD tissues are cl.sel. reproduced in mice, incl.ding l.te-stage AMD phenotypes, which makes them highl. attractive to study dynamic aspects of Aβ-mediated retinopathy. Furthermore, we discuss our findings reveal.ng how Aβ behaves at singl.-cel. resol.tion, and consider the l.ng-term impl.cations for neuroretinal.function. We propose Aβ as a key el.ment in switching to a diseased retinal.phenotype, which is now being used as a biomarker for l.testage AMD.

无论对于个体患者还是人群.癌症治疗的有效性都需要一种系统的、可重复的疗效评价方法。为满足这一需求,学界制定了一些评估疗效的指导原则,主要从解剖和代谢方面来量化治疗后的变化。过去几年针对特定的肿瘤组织制定了相应的疾病疗效分类标准迄今为止,恶性淋巴瘤可能是疾病疗效分类应用最普遍的肿瘤类型.该文对现有的淋巴瘤疗效评价标准进行概述,重点介绍这些评价标准各自的方法及其适用性。针对阻碍疗效评价标准化发展的问题,该文也对其中许多标准的技术复杂度和主观阈值判定的内容进行了评述。
