SARS-CoV-2 variants are associated with different clinical courses in children with MIS-C

Background Recent infection with SARS‑CoV-2 in children has been associated with multisystem inflammatory syndrome in children(MIS-C).SARS‑CoV-2 has undergone different mutations.Few publications exist about specific variants and their correlation with the severity of MIS-C.Methods This was a single-center,retrospective study including all patients admitted with MIS-C at Rady Children’s Hospital-San Diego between May 2020 and March 2022.Local epidemiologic data,including viral genomic information,were obtained from public records.Demographics,clinical presentation,laboratory values,and outcomes were obtained from electronic medical records.Results The analysis included 104 pediatric patients.Four MIS-C waves were identified.Circulating variants in San Diego during the first wave included clades 20A to C.During the second wave,there were variants from clades 20A to C,20G,21C(Epsilon),20I(Alpha),and 20j(Gamma).The third wave had Delta strains(clades 21A,21I,and 21j),and the fourth had Omicron variants(clades 21K,21L,and 22C).MIS-C presented with similar symptoms and laboratory findings across all waves.More patients were admitted to the pediatric intensive care unit(PICU)(74%)and required inotropic support(63%)during the second wave.None of the patients required mechanical circulatory support,and only two required invasive ventilatory support.There was no mortality.Conclusions The various strains of SARS-CoV-2 triggered MIS-C with differing severities,with the second wave having a more severe clinical course.Whether the differences in disease severity across variants were due to changes in the virus or other factors remains unknown.

BACKGROUND Sleep disturbance is a core feature of post-traumatic stress disorder(PTSD).Given the relationship between sleep disturbance and PTSD,there has been a relative paucity of studies examining the potential therapeutic impact of using pharmacotherapy to target sleep disturbance in patients with PTSD.Eszopiclone(ESZ)is a non-benzodiazepine y-aminobutyric acid-A receptor agonist indicated for the treatment of sleep and may affect sleep in patients with PTSD.AIM To evaluate the efficacy of ESZ vs placebo(PBO)for patients with PTSD and insomnia.METHODS The study was a 12-wk,double blind,randomized controlled trial with 3 mg of ESZ(n=13)or PBO(n=12).RESULTS Patients in both arms experienced significant improvement in PTSD symptoms as assessed by the Clinician-Administered PTSD Scale for DSM-IV(CAPS):ESZ(t11=-3.12,P=0.005)and PBO(t11=-3.5,P=0.002)and by self-report with the Short PTSD Rating Interview(ESZ t11=-3.38,P=0.003 and PBO t11=-4.48,P=0.0005).There were no significant differences between treatments on the CAPS(t22=-0.13,P=0.70)or the Short PTSD Rating Interview(t22=-0.58,P=0.56).Similarly,both treated groups improved on sleep measures as assessed by the Pittsburgh Sleep Quality Index with PTSD Addendum(PSQI)and on total sleep time(TST)and sleep latency assessed by actigraphy with no significant differences between groups(PSQI t22=-0.24,P=0.81;total sleep time t10=0.13,P=0.90 and sleep latency t10=0.68,P=0.50).There was a significant correlation between improvement in sleep and overall improvement in PTSD as measured by change scores on the PSQI and CAPS,r(8)=0.79,P=0.01 for ESZ treated subjects,but not for those treated with PBO r(9)=0.16,P=0.69.Adverse events of ESZ were consistent with the known profile of the medication including dysgeusia(30%,mild),sedation(20%,mild)and headache(20%,moderate to severe).CONCLUSION Results do not support the hypothesis of a specific positive effect of ESZ compared to PBO for measures of PTSD and associated sleep disturbance.

Likelihood-based estimation and prediction for a measles outbreak in Samoa

Prediction of the progression of an infectious disease outbreak is important for planning and coordinating a response.Differential equations are often used to model an epidemic outbreak's behaviour but are challenging to parameterise.Furthermore,these models can suffer from misspecification,which biases predictions and parameter estimates.Stochastic models can help with misspecification but are even more expensive to simulate and perform inference with.Here,we develop an explicitly likelihood-based variation of the generalised profiling method as a tool for prediction and inference under model mis-specification.Our approach allows us to carry out identifiability analysis and uncertainty quantification using profile likelihood-based methods without the need for marginalisation.We provide justification for this approach by introducing a new interpretation of the model approximation component as a stochastic constraint.This preserves the rationale for using profiling rather than integration to remove nuisance parameters while also providing a link back to stochastic models.We applied an initial version of this method during an outbreak of measles in Samoa in 2019e2020 and found that it achieved relatively fast,accurate predictions.Here we present the most recent version of our method and its application to this measles outbreak,along with additional validation.

Background Children with rare diseases experience challenges at home and school and frequently require multi-disciplinary healthcare.We aimed to determine health service utilization by Australian children with rare diseases and barriers to access-ing healthcare.Methods Parents completed an online survey on health professional and emergency department(ED)presentations,hospi-talization,and barriers to accessing services.Potential barriers to service access included residential location(city,regional,remote)and child health-related functioning,determined using a validated,parent-completed measure-of-function tool.Results Parents of 462 children with over 240 rare diseases completed the survey.Compared with the general population,these children were more likely to be hospitalized[odds ratio(OR)=17.25,95%confidence interval(CI)=15.50-19.20]and present to the ED(OR=4.15,95%CI=3.68-4.68)or a family physician(OR=4.14,95%CI=3.72-4.60).Child functional impairment was nil/mild(31%),moderate(48%)or severe(22%).Compared to children with nil/mild impair-ment,those with severe impairment were more likely to be hospitalized(OR=13.39,95%CI=7.65-23.44)and present to the ED(OR=11.16,95%CI=6.46-19.27).Most children(75%)lived in major cities,but children from regional(OR=2.78,95%CI=1.72-4.55)and remote areas(OR=9.09,95%CI=3.03-25.00)experienced significantly more barriers to healthcare access than children from major cities.Barriers included distance to travel,out-of-pocket costs,and lack of specialist medical and other health services.Conclusions Children with rare diseases,especially those with severe functional impairment have an enormous impact on health services,and better integrated multidisciplinary services with patient-centered care are needed.Access must be improved for children living in rural and remote settings.

The Internet of Things(IoT)technologies has gained significant interest in the design of smart grids(SGs).The increasing amount of distributed generations,maturity of existing grid infrastructures,and demand network transformation have received maximum attention.An essential energy storing model mostly the electrical energy stored methods are developing as the diagnoses for its procedure was becoming further compelling.The dynamic electrical energy stored model using Electric Vehicles(EVs)is comparatively standard because of its excellent electrical property and flexibility however the chance of damage to its battery was there in event of overcharging or deep discharging and its mass penetration deeply influences the grids.This paper offers a new Hybridization of Bacterial foraging optimization with Sparse Autoencoder(HBFOA-SAE)model for IoT Enabled energy systems.The proposed HBFOA-SAE model majorly intends to effectually estimate the state of charge(SOC)values in the IoT based energy system.To accomplish this,the SAE technique was executed to proper determination of the SOC values in the energy systems.Next,for improving the performance of the SOC estimation process,the HBFOA is employed.In addition,the HBFOA technique is derived by the integration of the hill climbing(HC)concepts with the BFOA to improve the overall efficiency.For ensuring better outcomes for the HBFOA-SAE model,a comprehensive set of simulations were performed and the outcomes are inspected under several aspects.The experimental results reported the supremacy of the HBFOA-SAE model over the recent state of art approaches.

The role of crystallinity on differential attachment/ proliferation of osteoblasts and fibroblasts on poly （caprolactone-co-glycolide） polymeric surfaces

The objective of the present study is to systematically evaluate the role of polymer crystallinity on fibroblast and osteoblast adhesion and proliferation using a series of poly（caprolactone-co-glycolide）（PCL/PGA） polymers. PCL/PGA polymers were selected since they reflect both highly crystalline and amorphous materials. PCL/PGA polymeric materials were fabricated by compression molding into thin films. Five compositions, from PCL or PGA to intermediate copolymeric compositions of PCL/PGA in ratios of 25：75, 35：65 and 45：55, were studied. Pure PCL and PGA represented the crystalline materials while the copolymers were amorphous. The polymers/copolymers were characterized using DSC to assess crystallinity, contact angle measurement for hydrophobicity, and AFM for nanotopography. The PCL/PGA films demonstrated similar hydrophobicity and nanotopography whereas they differed significantly in crystallinity. Cell adhesion to and proliferation on PCL/PGA films and proliferation studies were performed using osteoblasts and NIH-3T3 fibroblasts. It was observed that highly crystalline and rigid PCL and PGA surfaces were significantly more efficient in supporting fibroblast growth, whereas amorphous/flexible PCL/PGA 35：65 was significantly more efficient in supporting growth of osteoblasts. This study demonstrated that while chemical composition, hydrophobicity and surface roughness of PCL/PGA polymers were held constant, crystallinity and rigidity of PCL/PGA played major roles in determining cell responses.

Adult celiac disease with acetylcholine receptor antibody positive myasthenia gravis

Celiac disease has been associated with some autoimmune disorders. A 40-year-old competitive strongman with celiac disease responded to a gluten-free diet, but developed profound and generalized motor weakness with acetylcholine receptor antibody positive myasthenia gravis, a disorder reported to occur in about 1 in 5000. This possible relationship between myasthenia gravis and celiac disease was further explored in serological studies. Frozen stored serum samples from 23 acetylcholine receptor antibody positive myasthenia gravis patients with no intestinal symptoms were used to screen for celiac disease. Both endomysial and tissue transglutaminase antibodies were examined. One of 23 (or, about 4.3%) was positive for both IgA-endomysial and IgA tissue transglutaminase antibodies. Endoscopic studies subsequently showed duodenal mucosal scalloping and biopsies confirmed the histopathological changes of celiac disease. Celiac disease and myasthenia gravis may occur together more often than is currently appreciated. The presence of motor weakness in celiac disease may be a clue to occult myasthenia gravis, even in the absence of intestinal symptoms.

Helicobacter pylori(H pylori) infection is a leading world-wide infectious disease as it affects more than half of the world population and causes chronic gastritis,peptic ulcer disease and gastric malignancies.The infection elicits a chronic cellular inflammatory response in the gastric mucosa.However,the effects of this local inflammation may not be confi ned solely to the digestive tract but may spread to involve extraintestinal tissues and/or organs.Indeed,H pylori infection has been epidemiologically linked to extra-digestive conditions and diseases.In this context,it has been speculated that H pylori infection may be responsible for various endocrine disorders,such as autoimmune thyroid diseases,diabetes mellitus,dyslipidemia,obesity,osteoporosis and primary hyperparathyroidism.This is a review of the relationship between H pylori infection and these endocrine disorders.

Adiponectin signalling in bone homeostasis,with age and in disease

Adiponectin is the most abundant circulating adipokine and is primarily involved in glucose metabolism and insulin resistance.Within the bone,osteoblasts and osteoclasts express the adiponectin receptors,however,there are conflicting reports on the effects of adiponectin on bone formation and turnover.Many studies have shown a pro-osteogenic role for adiponectin in in vivo murine models and in vitro:with increased osteoblast differentiation and activity,alongside lower levels of osteoclastogenesis.However,human studies often demonstrate an inverse relationship between adiponectin concentration and bone activity.Moreover,the presence of multiple isoforms of adiponectin and multiple receptor subtypes has the potential to lead to more complex signalling and functional consequences.As such,we still do not fully understand the importance of the adiponectin signalling pathway in regulating bone homeostasis and repair in health,with age and in disease.In this review,we explore our current understanding of adiponectin bioactivity in the bone;the significance of its different isoforms;and how adiponectin biology is altered in disease.Ultimately,furthering our understanding of adiponectin regulation of bone biology is key to developing pharmacological and non-pharmacological(lifestyle)interventions that target adiponectin signalling to boost bone growth and repair in healthy ageing,following injury or in disease.
