A prospective cohort of men with localized prostate cancer on active surveillance protocol in Hong Kong,China:what did we learn?

This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH Ho Urology Centre,Prince of Wales Hospital,Hong Kong,*** investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease ***,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 *** study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated *** combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

Carvedilol Versus Other nonselective Beta Blockers for Variceal Bleeding Prophylaxis and Death: A network Meta-analysis

Background and Aims:We aimed to perform a network meta-analysis(nWM)to examine comparative effectiveness of non-selective beta blockers(nSBBs)on prophylaxis of gastroesophageal variceal bleeding(GVB)and mortality ***:MEDLInE(OVID)and EMBASE databases were searched for eligible randomized clinical trials(RCTs)from inception to July 3,*** of interest included primary/secondary prophylaxis of GVB,failure to achieve hepatic venous pressure gradient(HVPG)decremental response,liver-related and all-cause mortality.A Bayesian nWM was performed to derive relative risk(RR)with 95%credible intervals(CrIs).The ranking probability of each nSBB was assessed by surface under cumulative ranking curve(SUCRA).Results:Thirty-three RCTs including 3,188 cirrhosis patients with gastroesophageal varices were *** with placebo,nadolol ranked first for reducing variceal bleeding[RR:0.25,(95%CrI:0.11–0.51);SUCRA:0.898],followed by carvedilol[RR:0.33,(95%CrI:0.11–0.88);SUCRA:0.692]and propranolol[RR:0.52,(95%CrI:0.37–0.75);SUCRA:0.405].Carvedilol was more effective than propranolol in achieving HVPG decremental response[RR:0.43,(95%CrI:0.26–0.69)].Carvedilol ranked first for reducing all-cause mortality[RR:0.32,(95%CrI:0.17–0.57);SUCRA:0.963],followed by nadolol[RR:0.48,(95%CI:0.29–0.77);SUCRA:0.688],and propranolol[RR:0.77,(95%CI:0.58–1.02);SUCRA:0.337].Similar findings were observed for liver-related *** ranked the *** RR of adverse events was 4.38,(95%CrI:0.33–161.4);SUCRA:0.530,followed by propranolol[RR:7.54,(95%CrI:1.90–47.89);SUCRA:0.360],and nadolol[RR:18.24,(95%CrI:91.51–390.90);SUCRA:0.158].Conclusions:Carvedilol is the preferred nSBB with better survival benefit and lower occurrence of adverse events among patients with gastroesophageal varices.

Target-oriented robust optimization of a microgrid system investment model

An emerging alternative solution to address energy shortage is the construction of a microgrid system. This paper develops a mixed-integer linear programming microgrid investment model considering multi-period and multi-objective investment setups. It further investigates the effects of uncertain demand by using a target-oriented robust optimization （TORO） approach. The model was validated and analyzed by subjecting it in different scenarios. As a result, it is seen that there are four factors that affect the decision of the model： cost, budget, carbon emissions, and useful life. Since the objective of the model is to maximize the net present value （nPV） of the system, the model would choose to prioritize the least cost among the different distribution energy resources （DER）. The effects of load uncertainty was observed through the use of Monte Carlo simulation. As a result, the deterministic model shows a solution that might be too optimistic and might not be achievable in real life situations. Through the application of TORO, a profile of solutions is generated to serve as a guide to the investors in their decisions considering uncertain demand. The results show that pessimistic investors would have lower nPV targets since they would invest more in storage facilities, incurring more electricity stock out costs. On the contrary, an optimistic investor would tend to be aggressive in buying electricity generating equipment to meet most of the demand, however risking more storage stock out costs.

Outcomes of radical prostatectomy in a 20-year localized prostate cancer single institution series in China

HepG2 cells recovered from apoptosis show altered drug responses and invasiveness

BACKGROUnD: Cancer relapse, associated with increased drug resistance and rate of metastasis, often follows completion of chemotherapy but the cancer escape mechanisms are still incompletely understood. Percutaneous ethanol injection(PEI)has been used for treating hepatocellular carcinoma(HCC)for decades, while the recurrence after PEI treatment remains a major limitation. Recent evidence mounted that cancer cells could survive from chemical induced apoptosis, suggesting a potential route through which cancer relapse may occur. This study focuses on the consequence of HepG2 recovery from ethanol-induced apoptotic ***: The model of HepG2 recovery from ethanolinduced apoptotic event was established by live cell imaging,BrdU assay and Western blotting. MTT assay, wound healing assay and invasion assay were used to investigate the behavior of HepG2 after ***: HepG2 cells could recover from ethanol-induced apoptosis. These cells changed their behaviors such as drug resistance, mobility and invasiveness. On average, the recovered HepG2 cell clones were found to be 46% more resistant to ethanol and 84% higher in mobility. The recovered clones became 58.2% more sensitive to ***: HepG2 cells can recover from ethanolinduced apoptotic event. These cells became more resistant to ethanol and more invasive. Although the recovered cell clones were more resistant to ethanol, they became more sensitive to5-fluorouracil treatment.
