Hepatitis B viral load affects prognosis of hepatocellular carcinoma

Hepatocellular carcinoma(HCC)is a complex disease that is dually challenging to treat due to underlying chronic liver disease in addition to the cancer *** prognosis of patients with HCC is determined by intrahepatic tumor status and reserved hepatic *** B virus(HBV)is an established major risk factor of HCC development,and HBV viral load is being increasingly recognized as a prognostic factor in the presence of established *** HBV viral load may affect the prognosis of HBV-related HCC patients in several ***,it is associated with more frequent recurrence of HBV-related HCC after ***,it is associated with more occurrence and severity of potentially life-threatening HBV ***,it is associated with more worsened liver function,which limits the therapeutic options for HBV-related ***,directly or indirectly,can induce *** patients with a high HBV DNA level and subsequent active hepatitis,adhesion molecules expressed on the sinusoidal cells are up-regulated and may increase intrahepatic *** progression after treatment can lead to a poor prognosis by reducing number of normal functioning ***,high HBV viral load can affect the prognosis of patientswith HCC by frequent recurrence after treatment for HCC and deterioration of hepatic function associated with HCC *** meta-analysis showed that antiviral treatment reduces HCC recurrence and liver-related mortality after curative therapy of *** the strong relationship between high HBV DNA load and poor survival outcome of HCC patients due to cancer progression,it is expected that long-term antiviral therapy results in the sustained HBV suppression,control of inflammation,reduction in HCC progression,and eventually in improved overall survival.

Recently,virtual reality technology that can interact with various data is used for urban design and ***,one of the most important elements in virtual reality technology,means visual expression so that a person can experience three-dimensional space like *** obtain this realism,real-world data are used in the various *** example,in order to increase the realism of 3D modeled building textures real aerial images are utilized in 3D ***,the aerial image captured during the day can be shadowed by the sun and it can cause the distortion or deterioration of *** resolve this problem,researches on detecting and removing shadows have been conducted,but the detecting and removing shadow is still considered as a challenging *** this paper,we propose a novel method for detecting and removing shadows using deep *** this work,we first a build a new dataset of photo-realistic synthetic urban data based on the virtual environment using 3D spatial information provided by *** detecting and removing shadow from the dataset,firstly,the 1-channel shadow mask image is inferred from the 3-channel shadow image through the ***,to generate a shadow-free image,a 3-channel shadow image and a detected 1-channel shadow mask into the GAN is *** the experiments,we can prove that the proposed method outperforms the existing methods in detecting and removing shadow.

Current Usage of Terminologies Related to Acupotomy: A Literature Research and Standardization Suggestion

Objective: To investigate the current usage of terminologies related to acupotomy through systematic search and analyze the pros and cons of each for proposing a standard terminology. Methods: Seven medical journal databases were searched including PubMed, EMBASE, Cochrane Central Register of Controlled Trials, China National Knowledge Infrastructure, Oriental Medicine Advanced Searching Integrated System, KoreaMed, and Korean studies Information Service System using 10 candidate terminologies as searching terms. All studies published from their inception to July 26, 2017 were collected. Articles were included if the title stated one of the 10 candidate terminologies consistent with the definition of acupotomy. Priorly established frequency and consistency of each candidate terminology from medical databases were calculated and evaluated. Moreover, the pros and cons of each were analyzed to propose a standard terminology. Results: A total of 112 studies in English databases, 1,129 studies in Chinese database, and 44 studies in Korean databases were included. The most frequently used terminologies were needle knife(35.71%), acupotomy(48.54%) and acupotomy(90.90%) in English, Chinese and Korean database, respectively. Overall, acupotomy and needle knife were the most frequently used. Others like acupotomology, needle scalpel, miniscalpel acupuncture and miniscalpel needle were used within 10% of the total searched literature. Acupotome, stiletto needle, sword like needle, and Xiaozhendao were rarely used. Acupotomy had the advantages of high frequency and consistency but lacked representativeness. Needle knife also showed a high frequency, but the consistency was poor. Though miniscalpel acupuncture and miniscule needle were used less frequently, they had advantages of inclusiveness and clarity. Conclusions: A debate for standardization of the terminology is necessary. This preliminary research can provide a basic outline for the standardization consensus process, and

Prevalence of extracardiac findings in the evaluation of ischemic heart disease by multidetector computed tomography

Objective Multidector computed tomography (MDCT) is now commonly used for the evaluation of coronary artery disease. Because MDCT images include many non-cardiac organs and the patient population evaluated is highly susceptible to extracardiac diseases, this study was designed to evaluate the prevalence of extracardiac findings in the MDCT evaluation of ischemic heart disease. Methods From March 2007 to March 2008, a total of six-hundred twenty patients, who underwent 64-slice MDCT evaluations for chest pain, or dyspnea, were enrolled in this study. Cardiac and non-cardiac findings were comprehensively evaluated by a radiologist. Results Enrolled patients included 306 men (49.4%), with a mean age of 66 years. Significant coronary artery stenosis was found in 41.6%of the patients. A total of 158 extracardiac findings were observed in 110 (17.7%) patients. Commonly involved extracardiac organs were lung (36.7%), hepatobiliary system (21.5%), thyroid (19.6%), kidney (10.8%), spine (9.7%) and breast (0.6%). Of those 110 patients, 50 (45.5%) patients underwent further diagnostic investigations. Malignant disease was detected in three (2.7%) patients (lung cancer, pancreatic cancer, and thyroid cancer). Conclusions Extracardiac findings are frequently present and should be a concern in the MDCT evaluation of chest pain syndrome.

Design of a novel 50kW fast charger for electric vehicles

A novel 50 kW fast charger was proposed for electric vehicles. The proposed fast charger is divided into two main sections an AC-DC converter performing a PFC function and a DC-DC converter performing a charging function. A transformer including leakage inductances was used in the AC-DC converter in order to obtain isolation and inductance. A series-connection topology was used in the DC-DC converter between the DC-bus and outlet. This topology enables high power conversion efficiency up to 95% for the DC-DC converter. In order to reduce the impact of the 50 kW charging on the AC grid, the proposed fast charger system includes a buffering battery unit between the two main power conversion units. This leads to reductions in the power installation costs of power companies and to improvements in the power quality were verified through simulations and experimental results. on the AC grid. Characterization of Wollastonite Glass-ceramics Made from Waste Glass and Coal Fly Ash

The crystallization behavior of wollastonite glass-ceramics was investigated by means of X-ray diffraction （XRD） analysis and surface morphological observations, and the chemical compositions were evaluated by field emission-scanning electron microscopy （FE-SEM） and energy dispersive X-ray spectroscopy （EDS）. Various heat treatment temperatures （850, 900, 950, 1000 and 1050 ℃） were used to obtain glass-ceramics of the ideal wollastonite crystal phase as well as optimum mechanical properties and chemical durability. From XRD, FE-SEM and EDS, the crystallization of acicular crystal phase in the matrix was achieved at heat treatment temperature of 1000 and 1050 ℃, and wollastonite （CaSiO3） was found in the acicular type main crystal phase in the glass-ceramics. Various properties, such as density, compressive strength, bending strength and chemical durability were also examined. The mechanical properties of glass-ceramics obtained at the heat treatment temperature of 1000 and 1050 ℃ were superior to those obtained at the heat treatment temperature of 850 ℃.

Regional anesthesia in a patient with primary ciliary dyskinesia:A case report

BACKGROUND Primary ciliary dyskinesia(PCD)is an inherited autosomal-recessive disorder of impaired mucociliary clearance characterized by chronic respiratory diseases,otolaryngological diseases,central nervous system abnormalities,reproductive system abnormalities,and cardiac function *** anesthesia in these patients is associated with a higher incidence of respiratory complications than in patients without the *** SUMMARY A 16-year-old male patient was referred to the emergency room complaining of right ankle pain due to distal tibiofibular *** years prior,he had been diagnosed with *** that time,he had experienced several episodes of pneumonia,sinusitis,and chronic middle ear infections,for which he underwent surgical *** the current admission,he presented with cough and sputum but no other respiratory symptoms.A chest computed tomography scan revealed centrilobular ground-glass opacities in both lower lobes and a calcified nodule in the left lower *** the surgical procedure and postoperative pain management,combined spinal-epidural anesthesia was *** patient’s postoperative pain score was measured by the numerical rating scale(NRS).On the day of surgery,his NRS was 5 *** the second postoperative day,the NRS score had decreased to 2–3 *** epidural catheter was removed on the fourth day following the *** patient was subsequently discharged no respiratory *** We performed combined spinal-epidural anesthesia in a patient with *** patient experienced no additional respiratory complications and was discharged with a low NRS score for pain.

Development of twin wheel creep-feed grinding machine using continuous dressing for machining of aircraft rotary wing

Wintertime suspended sediment simulation in the Huanghai and East China Seas

A simulation of suspended sediment movement relating to tidal and wave forcing during a winter monsoon in November 1983 in the Huanghai and East China Seas continental shelf is attempted by using the model describing the cohesive/non-cohesive sediment resus-pension generated by interactions between currents and *** simulation showed that sediment concentration was increased by resuspension at shallow depths during the strong storm conditions due to high bottom stress interacted between currents and waves. This result is in general agreement with observations in horizontal distribution of suspended sediment distribution. At three current meter mooring positions off the southern Shandong Peninsula resuspension occurred only at a depth of 22 m,nearest coastal position and at deeper parts at depths of 51 and 80 m wave-current interaction effects were not significant. It has shown that the present model simulation demonstrated the capability of reproduction of suspended sediment movement under wintertime extreme event reasonably well.

CT Thresholds for Spinal Bone Mineral Attenuation to Prioritize Eligible Older Women for Osteoporosis Screening

Introduction: Computed tomography (CT) measurements of bone mineral attenuation may be a useful means to identify older women who should be prioritized for bone mineral density screening. Methods: We compared bone mineral attenuation as measured in the L1 vertebra of CT studies to the results of dual-energy x-ray absorptiometry (DEXA) studies to determine what CT attenuation thresholds might yield a reasonable combination of sensitivity and specificity for the detection of osteoporosis. The study was limited to women between the ages of 65 and 75 years who had a DEXA study and a CT that included the L1 or adjacent vertebra performed within 3 years of the DEXA study. Results: There were 1226 women in this study, of whom 452 (38%) had osteoporosis based on a T-score ≤ −2.5 by DEXA. There were 830 CT studies performed with contrast and 396 studies which were performed without contrast. There was a statistically significant difference in the mean HU of those studies performed without contrast compared to those with contrast (unenhanced mean 103 HU versus 125 HU, p < 0.001). Different CT attenuation thresholds provided the most appropriate combination of sensitivity and specificity for the detection of osteoporosis when comparing CT studies performed without or with IV contrast and when all the CT data were used in aggregate. Conclusion: Different thresholds appear necessary when using the mean CT vertebral attenuation to identify older women for preferential referral for DEXA studies.
