Laboratory characteristics of recent hepatitis A in Korea:Ongoing epidemiological shift

AIM:To evaluate seroprevalence of hepatitis A virus (HAV) antibody and investigate demographic,clinical,and laboratory features of recent cases in ***:For the evaluation of hepatitis A seroprevalence,we analyzed the data from 3127 subjects including,healthcare workers and patients who visited Konkuk University Hospital,a secondary referral center,from January to October *** sera with positive IgM were excluded from seroprevalence data for total HAV *** retrospectively reviewed the electronic medical records of 419 patients with HAV,who were diagnosed by the presence of serum IgM antibodies against *** patients presented at Konkuk University Hospital between August 2005 and September ***:Among 3127 sera tested,1428 (45.7%)were positive for anti-HAV *** seroprevalence was very low in teenagers or those in their twenties,increased in those in their thirties,and was > 90% in older *** children younger than 10 years,seroprevalence was increased *** patients with HAV hepatitis were in their twenties and *** γ-glutamyl transpeptidase increased with age and was significantly higher in patients older than 30 *** of severity,such as decreased albumin and increased bilirubin,were also more prominent in the older age group;however,the leukocyte count was higher and the frequency of leukopenia was lower in younger patients than in older ***:There has been an apparent epidemiological shift in HAV seroprevalence and a change in the peak age of HAV *** study could provide baseline data of recent hepatitis A in Asia.

Gene expression profiling reveals sequential changes in gastric tubular adenoma and carcinoma in situ

AIM: To analyze the expression profiles of premalignant and/or preclinical lesions of gastric ***: We analyzed the expression profiles of normal gastric pit, tubular adenoma and carcinoma in situ using microdissected cells from routine gastric biopsies. For the DNA microarray analysis of formalin-fixed samples,we developed a simple and reproducible RNA extraction and linear amplification procedure applying two polymerasebinding sites. The amplification procedure took only 8 h and yielded comparable DNA microarray data between formalin-fixed tissues and unfixed ***: In comparison with normal pit, adenoma/carcinoma showed 504 up-regulated and 29 down-regulated genes at the expected false significance rate 0.15%. The differential expression between adenoma and carcinoma in situ was subtle: 50 and 22 genes were up-, and down-regulated in carcinomas at the expected false significance rate of 0.61%, respectively. Differentially expressed genes were grouped according to patterns of the sequential changes for the 'tendency analysis' in the gastric mucosaadenoma-carcinoma ***: Groups of genes are shown to reflect the sequential expression changes in the early carcinogenic steps of stomach cancer. It is suggested that molecular carcinogenic pathways could be analyzed using routinely processed biopsies.

Selective inhibition of cell growth by activin in SNU-16 cells

AIM： To investigate whether activin regulates the cell proliferation of human gastric cancer cell line SNU-16 through the mRNA changes in activin receptors, Smads and p21^CIP1/WAF1. METHODS： The human gastric cancer cell lines were cultured, RNAs were purified, and RT-PCRs were carried out with specifically designed primer for each gene. Among them, the two cell lines SNU-5 and SNU-16 were cultured with activin A for 24, 48 and 72 h. The cell proliferation was measured by MTT assay. For SNU-16, changes in ActRIA, ActRIB, ActRIIA, ActRIIB, Smad2, Smad4, Smad7, and p21^CIP1/WAF1 mRNAs were detected with RT-PCR after the cells were cultured with activin A for 24, 48 and 72 h. RESULTS： The proliferation of SNU-16 cells was down regulated by activin A whereas other cells showed no change. Basal level of inhibin/activin subunits, activin receptors, Smads, and p21^CIP1/WAF1 except for activin βB mRNAs was observed to have differential expression patterns in the human gastric cancer cell lines, AGS, KATO III, SNU-1, SNU-5, SNU-16, SNU-484, SNU-601, SNU-638, SNU-668, and SNU-719. Interestingly, significantly higher expressions of ActR IIA and IIB mRNAs were observed in SNU-16 cells when compared to other cells. After activin treatment, ActR IA, IB, and IIA mRNA levels were decreased whereas ActR IIB mRNA level increased in SNU-16 cells. Smad4 mRNA increased for up to 48 h whereas Smad7 mRNA increased sharply at 24 h and returned to the initial level at 48 h in SNU-16 cells. In addition, expression of the p21^CIP1/WAF1 the mitotic inhibitor, peaked at 72 h after activin treatment in SNU-16 cells. CONCLUSION： Our results suggest that inhibition of cell growth by activin is regulated by the negative feedback effect of Smad7 on the activin signaling pathway, and is mediated through p21^CIP1/WAF1 activation in SNU-16 cells.

The temporal and spatial variability of the Yellow Sea Cold Water Mass in the southeastern Yellow Sea, 2009–2011

The Yellow Sea Cold Water Mass （YSCWM） is one of the important water mass in the Yellow Sea （YS）. It is distributed in the lower layer in the Yellow Sea central trough with the temperature less than 10℃ and the salinity lower than 33.0. To understand the variability of the YSCWM, the hydrographic data obtained in April and August during 2009-2011 are analyzed in the southeastern Yellow Sea. In August 2011, relatively warm and saline water compared with that in 2009 and 2010 was detected in the lower layer in the Yellow Sea central area. Although the typhoon passed before the cruise, the salinity in the Yellow Sea central trough is much higher than the previous season. It means that the saline event cannot be explained by the typhoon but only by the intrusion of saline water during the previous winter. In April 2011, actually, warm and saline water （T 〉 10~C, S 〉34） was observed in the deepest water depth of the southeastern area of the Yellow Sea. The wind data show that the northerly wind in 2011 winter is stronger than in 2009 and 2010 winter season. The strong northerly wind can trigger the intrusion of warm and saline Yellow Sea Warm Current. Therefore, it is proposed that the strong northerly wind in winter season leads to the intrusion of the Yellow Sea Warm Current into the Yellow Sea central trough and influenced a variability of the YSCWM in summer.

Spectral domain optical coherence tomography crosssectional image of optic nerve head during intraocular pressure elevation

AIM: To analyze changes of the optic nerve head(ONH) and peripapillary region during intraocular pressure(IOP) elevation in patients using spectral domain optical coherence tomography(SD-OCT).METHODS: Both an optic disc 200×200 cube scan and a high-definition 5-line raster scan were obtained from open angle glaucoma patients presented with monocular elevation of IOP(≥30 mm Hg) using SD-OCT. Additional baseline characteristics included age, gender, diagnosis,best-corrected visual acuity, refractive error, findings of slit lamp biomicroscopy, findings of dilated stereoscopic examination of the ONH and fundus, IOP, pachymetry findings, and the results of visual ***: The 24 patients were selected and divided into two groups: group 1 patients had no history of IOP elevation or glaucoma(n =14), and group 2 patients did have history of IOP elevation or glaucoma(n =10). In each patient, the study eye with elevated IOP was classified into group H(high), and the fellow eye was classified into group L(low). The mean deviation(MD)differed significantly between groups H and L when all eyes were considered(P =0.047) and in group 2(P =0.042), not in group 1(P =0.893). Retinal nerve fiber layer(RNFL) average thickness(P =0.050), rim area(P =0.015),vertical cup/disc ratio(P =0.011), cup volume(P =0.028),inferior quadrant RNFL thickness(P =0.017), and clockhour(1, 5, and 6) RNFL thicknesses(P =0.050, 0.012, and0.018, respectively), cup depth(P =0.008), central prelaminar layer thickness(P =0.023), mid-inferior prelaminar layer thickness(P =0.023), and nasal retinal slope(P =0.034)were significantly different between the eyes with groups H and ***:RNFLaveragethickness,rim area,vertical cup/disc ratio, cup volume, inferior quadrant RNFL thickness, and clock-hour(1, 5, and 6) RNFL thicknesses significantly changed during acute IOP elevation.

Chikungunya fever is a vector-borne viral disease transmitted to humans by chikungunya virus(CHIKV)-infected *** have been many outbreaks of CHIKV infection worldwide,and the virus poses ongoing risks to global *** prevent and control CHIKV infection,it is important to improve the current CHIKV diagnostic approaches to allow for the detection of low CHIKV concentrations and to correctly distinguish CHIKV infections from those due to other mosquito-transmitted viruses,including dengue virus(DENV),Japanese encephalitis virus(JEV),and Zika virus(ZIKV).Here,we produced monoclonal antibodies(mAbs)against the CHIKV envelope 2 protein(CHIKV-E2)and compared their sensitivity and specificity with commercially available mAbs using enzyme-linked immunosorbent assays(ELISA).Two anti-CHIKV-E2 mAbs,19-1 and 21-1,showed higher binding affinities to CHIKV-E2 protein than the commercial mAbs *** particular,the 19-1 mAb had the strongest binding affinity to inactivated ***,the 19-1 mAb had very little cross-reactivity with other mosquito-borne viruses,such as ZIKV,JEV,and *** results suggest that the newly produced anti-CHIKV-E2 mAb,19-1,could he used for CHIKV diagnostic approaches.

Case mistaken for leukemia after mRNA COVID-19 vaccine administration:A case report

BACKGROUND Following the global outbreak of coronavirus disease 2019(COVID-19),unlike other vaccines,COVID-19 vaccines were developed and commercialized in a relatively short period of *** large-scale administration of this vaccine in a short time-period led to various unexpected side effects,including severe cytopenia and thrombosis with thrombocytopenia *** many reports on adverse reactions,vaccination was necessary to prevent the spread of COVID-19;thus,it is essential to understand and discuss various cases of adverse reactions after *** SUMMARY A 77-year-old woman was administered the second dose of Pfizer mRNA COVID-19 *** vaccination she experienced fever,myalgia,and *** were subsequently administered for several days,but there was no improvement in the *** patient showed severe thrombocytopenia and *** and abdominopelvic computed tomography showed no infection related findings,but splenomegaly and cirrhotic liver features were observed.A large number of immature cells were observed in the peripheral blood smear;thus,bone marrow examination was performed for acute ***,there were no *** patient recovered after administration of hepatotoxins and transfusion treatment for cytopenia and was diagnosed with an adverse reaction to COVID-19 *** Adverse reactions of vaccination could be mistaken for hematologic malignancies including *** report a patient with leukocytosis following COVID-19 vaccination.

Dear Editor,I am Dr. Jungyul Park from the Division of Oculoplasty, Department of Ophthalmology, Pusan National University Hospital, Busan, Korea. I am writing to present a case of direct approach to thrombosed superior ophthalmic vein of recalcitrant indirect carotid cavernous fistula which was failed to treat with multiple conventional embolization treatment. To our knowledge this is very challenging procedure to the ophthalmologist, and not well described in literatures, especially in thrombocythemia patient with thrombosed superior ophthalmic vein.
