Quality of life (QOL) is an important topic in social and medical sciences, it has a multidimensional nature and is influenced by many factors. Aim of the Work: In this study we want to assess the impact of cognitive impairment on the health related quality of life (HR-QOL) of community dwelling non demented elderly. Subject and method: 115 non demented elderly, 60 years and older recruited from outpatient geriatric clinic at Al mansoura General Hospital, Dakahlia, Egypt. Each participant underwent, comprehensive geriatric assessment, assessing cognitive function using the mini-mental state examination (mmSE) and montreal cognitive function test, assessing the health related quality of life (HR-QOL) by the RAND-36 health survey. Results: We found that the elderly with impaired cognition by both mmSE and montreal test were significantly older;the ones with lower education, with more depressive symptoms, had more functional impairment and had lower HR-QOL scores than the elderly with normal cognitive function, after controlling for confounders still cognition was a determinant of HR-QOL. Also by linear correlation coefficient a significant correlation between HR-QOL and age, function, cognition and depression was found. Conclusion: Cognition affects significantly HR-QOL of the elderly, so we can say that interventions targeting cognition in the elderly can significantly improve their QOL.

Background: In the developing world, pneumonia is not only more common than it is in Europe or North America;it is also more severe and considered the leading cause of under-five mortality. Effective management of pneumonia in children under 3-year-old-age is still challenging due to various causes. Some authors believed in non-calcemic role of vitamin D as a potential factor in the pathogenesis, prevention, or therapy of pneumonia in this specific age. Purpose: Investigate the Vitamin D status in pneumonia among children between 6 months to 3 years of age. Subjects and Methods: This study was conducted in a Pediatric point of care at Al-Zahraa University Hospital on 90 children aged 6 months to 3 years old;divided into two groups: group I (study group) including 47 patients presented with pneumonia and group II (control group) including 43 apparently healthy children of matched age and sex with no evidence of pneumonia. This study examined if the anti-tumorigenesis effect of Exendin-4 in HT29 and HCT116 colorectal cancer(CRC)involves modulation of SIRT1 and Akt/GSR3K/β-catenin/NF-κB ***29 and HCT116 cells were treated either with increasing levels of Exendin-4(0.0-200μm)or with Exendin-4(at its IC50)in the presence or absence of EX-527(10μm/a selective SIRT1 inhibitor)or Exendin-4(9-39)amide(E(9-39)A)(1μm/an Exendin-4 antagonist).In a dose-dependent manner,Exendin-4 inhibited cell survival,but enhanced levels of lactate dehydrogenase(LDH)and single-stranded DNA(ssDNA)in both HT29 and *** both cell lines and at it has an IC50(45μm for HT29 and 35μm for HCT1165),Exendin-4 also significantly reduced cell survival,migration,and invasion of both cell types,with no effect on the expression GLP-1 receptors(GLPRs)nor of the activity of *** these doses,Exendin-4 also increased the expression of SIRT1 but reduced the acetylation of NF-κB and the expression of Bax and cleaved caspase-3 and in both cell ***,protein levels of p-GS3Kβ(Ser9),total and acetylatedβ-catenin,and Anix2 were significantly decreased,but levels of p-GS3Kβ(Ser9)and p-β-catenin(Ser33/37/Thr41)were significantly increased in both HT29 and HCT116-exendin-4 treated *** the effects exerted by Exendin-4 were completely prevented by Ex527 or E(9-39)*** conclusion,Exendin-4 suppresses the tumorigenesis of HT29 and HCT116 CRC cell activation of GS3Kβ-induced inhibition ofβ-catenin and NF-κβin a SIRT1-dependent mechanism.

Increasing land reclamation in El-Fayoum governorate has led to increase the amount of drainage water that discharged without prior treatments to two main drains, El-Wadi and El-Bats drainages loaded by salts and heavy metals. So, the present investigation aims to evaluate quality of water samples collected from a branch of the river Nile (El-Lahon canal) and the main drains (El-Bats and El-Wadi) at Fayoum governorate on some biomarkers of the Nile catfish;Clarias gariepinus inhabiting the studied degradable aquatic habitats. Water and nighty fish samples colleted from the different studied sites of collection by the help of fishermen for physicochemical analyes of water and fish biochemical analyses and histopathological alteration. Results revealed highly significant differences at P ≤ 0.01 with the highest water ammonia, nitrite, heavy metals (Cu, Zn, Pb and Cd) and lowest dissolved oxygen content accompanied by metals bioaccumulation in vital organs of fish collected from the main drains (El-Bats and El-Wadi) in comparision with that collected from the studied branch of the river Nile. Higher bioaccumulation of the studied heavy metals in gills, liver, kidney and muscles of Clarias gariepinus collected from the main drains significant increase in serum glucose, liver and kidney functions, disturbance in protein profile of fish collected from the studied main drains than that of fish collected from the river Nile branch, El-Lahon that showed more or less normal values. Improving ICU Nurses’ Practices of Physical Restraints in Jordan: Effect of an Educational Program

Background: Physical Restraint (PR) has been widely used in the intensive care units, which aims to protect patients and maintain their safety. With all benefits of PR, there are many complications and risks from improper utilization. Further, no Jordanian studies have found to educate nurses regarding the proper PR utilization. Purpose: To evaluate the impact of an educational program on nurses’ knowledge, attitude and practice regarding PR use in ICUs. methods: A pre-experiment with one-group pretest-posttest design was used among 40 ICU nurses. A self-administered questionnaire was used to evaluate nurses before the education program and two weeks after its completion. Results: After implementation of the program, nurses demonstrated a significant difference in knowledge scores between pretest (m = 10.35 ± 1.73) and posttest (m = 13.30 ± 1.47) (t = 9.92;df = 39;P = 0.000), attitudes scores (m = 27.90 ± 2.79 vs. m = 30.40 ± 2.32) (t = 4.61;df = 39;P = 0.000), and practice scores (m = 36.85 ± 1.98 vs. m = 40.52 ± 1.60) (t = 11.59;df = 39;P = 0.000). Conclusion: Service-education program can improve ICU nurses’ knowledge, and enhance their attitudes and practices towards physical restraint. Such improvement is expected to be reflected in improving the quality of care delivered to the patients, and decreasing complications of malpractice regarding physical restraint.

Risk Stratification Treatment of Pediatric Rhabdomyosarcoma: South Egypt Cancer Institute Experience

Risk stratification allows tailoring of treatment protocol using, for selected patients, reduced total chemotherapy exposure, including decreases in alkylator therapy and avoidance of agents with recognized risk of late complications (anthracyclines), elimination of irradiation and reduction of radiotherapy dose. Patients and methods: Twenty-nine newly diagnosed pediatric rhabdomyosarcoma patients attended the pediatric oncology department between January 2008 and may 2011. Patients were divided into 3 groups according to age, stage, group, pathology and site of the tumor. Treatment protocol tailored according to risk group. Results: Twenty-nine newly diagnosed pediatric rhabdomyosarcoma patients were evaluated. Seven patients had low risk, Intermediate risk included 12 patients, and 10 patients had high risk. After three years median follow up, event free survival was 51.7% for all patients however it was 86%, 67% and 10% for low, intermediate and high risk respectively (P = 0.0002). There was statistical difference for survival among different sites, histology, clinical group and stage as risk factors within each risk group, no statistically survival significance of any of these factors within the same risk group. Conclusion: Risk stratification is the best single predictor factor for pediatric rhabdomyosarcoma and allows tailoring of the treatment protocol. For selected patients, reductions in total chemotherapy exposure, elimination of irradiation in selected low risk patients and reduction of radiotherapy dose according to postoperative margin and nodal status is safe.

Diffuse large B cell lymphoma (DLBCL) is a heterogeneous disease that displays a highly variable clinical outcome. It is a neoplasm of large transformed B cells with a diffuse growth pattern. DLBCL is the most common type of non-Hodgkin’s lymphoma (NHL) (31% of all cases). Approximately half of patients with DLBCL are cured with current chemotherapy regimens. The purpose of this study was to evaluate BCl2 expression in 45 patients diagnosed with DLBCL of head and neck region and correlate the level of its immunohistochemical expression with different clinicopathological variables with emphasis upon patients’ age, gender, nodal or extra-nodal location of lymphoma, patients’ response to chemotherapy, progression-free survival (PFS) and overall survival (OS). A retrospective analysis of 45 patients diagnosed to have DLBCL. A cut off value of ≥ 50% protein expression denoted BCL2 positivity. Out of 45 cases, 36 cases (80%) revealed BCL2 positive expression and 9 cases (20%) were BCL2 negative. We found statistically significant differences in BCL2 expression regarding different patients’ responses to chemotherapy, patients’ OS and PFS (p ≤ 0.05). No statistically significant differences in BCL2 expression regarding the patients’ Ann Arbor clinical stage, age group and tumor site (nodal or extra-nodal, p > 0.05) using the Chi-square test. BCL2 expression was analyzed in relation to 5 years OS and PFS using Kaplan meier curves and Log Rank test for survival analysis. Cases that demonstrated BCL2 positivity revealed shortened OS and PFS with highly statistically significant differences among the studied variables (p = 0.000). We also found that patients who respond well to the chemotherapeutic regimen had negative BCL2 expression, the differences were statistically significant (p = 0.015). In conclusion, BCL2 expression could be considered a predictor for patients’ chemotherapeutic response, OS and PFS.

Study Question: The question: is there any difference in pregnancy rate between embryo transfer day 5 and day 6 in IVF? What Is Known Already? Blastocyst transfer is increasingly popular in assisted reproductive technology (ART) centers today. Very few articles concentrate on comparing Day 5 and Day 6 embryo transfer with conflict results. Objective, Study Design: Systematic review and meta-analysis of published controlled studies. Searches conducted from 2001-2020 on Pubmed. medline, EmBASE, and ISI Web of Science Electronic database is used to collect data, using the following search terms: blastocyst, Day 5, Day 6, embryo transfer (E.T.) and pregnancy rate. materials, Setting, methods: A total of 6 full-text articles preselected from 211 references, based on title and abstract. Two independent reviewers performed data selection and extraction according to the Preferred Reporting Items for Systematic Reviews and meta-Analysis statement (PRISmA). This systematic review was conducted according to (PICO) standard. Random-effect meta-analysis performed on all data (overall analysis). Results and the Role of Chance: Data from 6 relevant articles were extracted and integrated into the meta-analysis that reported clinical pregnancy rate (CPR) as an outcome in 5640 cycles, 2274 cycle had embryo transfer at day 6 and the control was 3366 cycle had embryo transfer day 5 showed a significantly higher clinical pregnancy rate following Day 5 embryo transfer compared with Day 6 embryo transfer with odd ratio and 95% confidence limit 0.73 (0.66 - 0.82)m> pm> value y analysis led to similar results and conclusions. Acute upper gastrointestinal bleeding (UGIB) remains an important emergency situation. In the last two decades, major developments took place influencing incidence, etiology and outcome of patients with acute UGIB. Peptic ulcer bleeding is the most significant complication of ulcer disease being responsible for 50% of all cases mortality. Aim of the study: To compare between endoscopic clip application versus argon plasma coagulation in management of bleeding peptic ulcer (BPU). Patients and methods: Sixty patients suffering from acute UGIB were randomly divided into two groups: group I included 30 patients treated with endoscopic clip application and Group II included 30 patients subjected to endoscopic APC. All patients were classified according to Forrest classification and the clinical Rockall score. Results: There were significant differences between the two groups as regard Forrest classification (P 0.05). Re bleeding was significant with higher Rockall score in group I (P 0.05). Conclusion: Endoscopic application of hemoclips have a less re bleeding rate than Argon plasma coagulation for treatment of bleeding peptic ulcer, although this was statistically insignificant, meanwhile APC is still less cost and easy. Clinical and endoscopic assessment (through Rokcall score and Forrest classification) could help in making best choice for endoscopic management.
