Surgical management of Perforated Peptic Ulcer;Closure with Omental Patch versus Definitive Repair with Vagotomy and Gastrojejunostomy: A Comparative Study

Background: The perforation of peptic ulcer is a common and serious life threatening surgical emergency. Up-till now no consensus was reached regarding the best practice in management of perforated peptic ulcer. The aim of this study is to evaluate and compare between both management strategies of perforated peptic ulcer;performing simple closure of the perforation with an omental patch then H. pylori eradication and inhibition of acid secretion using long time proton pump inhibitors versus performing definitive repair of perforated peptic ulcer (closure of the perforation with an omental patch, truncal vagotomy and gastrojejunostomy to discover a proper management strategy of perforated peptic ulcer. Patients and methods: In the current study we included 30 patients which were divided into 2 groups: group 1 included 15 patients where we managed them by simple closure of the perforation with an omental patch then H. pylori eradication and inhibition of acid secretion using long time proton pump inhibitors and group 2 included 15 patients where we performed closure of the perforation with an omental patch, truncal vagotomy and gastrojejunostomy. Results: We found that younger patient underwent vagotomy and gastro-jejunostomy technique (p Conclusions: Peptic ulcer perforation could be safely managed by primary closure and covering by omentum in addition to medical treatment of H. pylori infection and inhibition of acid secretion especially in old patients with comorbid condition who presented late or with shock.

Prevelance of Bovine Cysticercosis in Egypt and the Cysticidal Effect of Two Extracts Obtained from Balanites aegyptiaca and moringa oleifera on mice model Affected with T. saginata Cysticerci

The aim of the present study was to determine the prevalence of bovine cysticercosis in both cattle and buffloas, in Egypt and to assess the cysticidal efficacy of Balanites aegyptiaca fruits (B. aegyptiaca) and moringa oleifera seeds (m. oleifera) extracts in experimentally infected mice. The study detected the level of tumor necrosis factor (TNF-α) to monitor the immune and inflammatory responses of experimentally infected mice. Through meat inspection, a total number of 2125 male bovine, 2 to 5 years old, (1125 cattle and 1000 buffloes) were examined under the authority of Albsatine and Alwaraq official abattoirs in Cairo Governorate, Egypt covering the period extended from march 2022 to April 2023. The overall prevalence of the disease among bovine was 7.8% (6.31% of cattle and 9.5% of buffloes). Besides, B. aegyptiaca and m. oleifera extracts showed cysticidal activity in experimentally infected mice. A decrease in the numbers of cysticerci was found in all treated mice groups, and up to 88% reduction was achieved in the B. aegyptiaca-treated group;higher than that was recorded in both m. oleifera (72.23%) and albendazole-treated ones (80.56%). Postmortem findings proved that m. oleifera and B. aegyptiaca reduced cysticerci numbers comparable to a commercial anthelmintic. The study showed a significant decrease (P 0.001) in TNF-α levels after treatment with Balanites and moringa extracts, compared with the untreated control and the albendazole-treated groups.

Surrogate Role of CD85k on monocytic Lineage Involved Leukemogenesis Biology and Clinical Aspect

Background: Unique receptor involved in leukemogenesis is CD85k;an immuneglobulin receptor for immune tolerance, CD36 is glycoprotein mediates cellular adhesion and metastatic spread, CD14, CD15 considered common monocytic markers. Aims: to investigate CD85k with monocytic lineage involved leukemia (mLIL) markers in leukemia pathogenesis and clinical presentation. Patients and methods: 47 patients (32 diagnosed acute myeloid leukemia (AmL);15 non-malignant hematological disease as a control), were included, aged from 2 to 80 years, all subjected to peripheral blood (***) and bone marrow (B.m) examination, immunophenotyping (IPT) using FASC Canto four color flow cytometer (FCm) Becton Dickenson (BD) USA, for CD13, CD33, mPO, HLA-DR, CD34, CD38, CD117, CD14, CD15 and CD36 the mo Abs supplied by B.D Bioscience, and anti CD85k mo Abs by Aveda de Coimbra Flamenco, reference No. 1399990130. Results: Frequency of CD85k is 19/32 (59.37%) of AmL;14/14 (m4/m5) 100% positive CD85k, insignificant correlations of CD85k to sex, lymphadenopathy or organomegaly, platelets count and *** blast (P > 0.05), significant to age 50,000 × 109/l, Hb 0.05). Conclusion: Although CD85k is mLIL associated marker, it is not correlated with other mLIL markers with frequency 100% in mLIL and 59.37% in AmL, age predisposition is myeloid markers, it’s a crucial role in leukemogenesis biology, not in clinical presentations, considered good follow up predictor mLIL marker.

The glycolytic enzymes play a crucial role in the anaerobic glycolysis of apicomplexan parasites for energy generation,consequently it consider as potential targets for new drugs *** studies revealed that lactate dehydrogenase(LDH),a glycolytic enzyme,is a potential drug target in Plasmodium,Toxoplasma,Cryptosporidium and ***,Babesia orientalis(Bo) LDH was used as a model to reveal the structural basis of LDH in Babesia spp.,we solved the crystal structure of apo BoLDH at 2.67 ? resolution in the space group *** extra insertion(five amino acid residues) appears in the active pocket loop of BoLDH and creates a larger catalytic bonding pocket,like other protozoa(except for Babesia microit LDH) and unlike its mammalian *** ligands the apo BoLDH takes R-state(relaxed) with active-site loop open,the feature is obviously different from that of allosteric LDHs in T-state with active-site loop *** with allosteric LDHs the increased numbers of ionic bonds and hydrogen bonds make the subunit interfaces of BoLDH more stabilization,and a specific structure of N-terminal "arm" is considered as a dominating factor for the stabilization of BoLDH R-state ***,the enzymatic activity of BoLDH was inhibited non-competitively by polyphenolic gossypol with a K value of 10.3 μm,BoLDH is sensitive to inhibition of gossypol and possibly to new derivatives of the *** current work provides the structural basis of BoLDH for the first time,and suggests further investigation on the structure of Babesia *** which indeed would facilitate the screening and designing of new LDH inhibitors for controlling the intracellular proliferation of Babesia spp.

BACKGROUND Pancreatic cystic lesions(PCLs) are common in clinical practice. The accurate classification and diagnosis of these lesions are crucial to avoid unnecessary treatment of benign lesions and missed opportunities for early treatment of potentially malignant *** To evaluate the role of cyst fluid analysis of different tumor markers such as cancer antigens [e.g., cancer antigen(CA)19-9, CA72-4], carcinoembryonic antigen(CEA), serine protease inhibitor Kazal-type 1(SPINK1), interleukin 1 beta(IL1-β), vascular endothelial growth factor A(VEGF-A), and prostaglandin E2(PGE2)], amylase, and mucin stain in diagnosing pancreatic cysts and differentiating malignant from benign *** This study included 76 patients diagnosed with PCLs using different imaging modalities. All patients underwent endoscopic ultrasound(EUS) and EUS-fine needle aspiration(EUS-FNA) for characterization and sampling of different *** The mean age of studied patients was 47.4 ± 11.4 years, with a slight female predominance(59.2%). mucin stain showed high statistical significance in predicting malignancy with a sensitivity of 87.1% and specificity of 95.56%.
