Recently,nano-systems based on molecular communications via diffusion(mCvD)have been implemented in a variety of nanomedical applications,most notably in targeted drug delivery system(TDDS)***,because the mCvD is unreliable and there exists molecular noise and inter symbol interference(ISI),cooperative nano-relays can acquire the reliability for drug delivery to targeted diseased cells,especially if the separation distance between the nano transmitter and nano receiver is *** this work,we propose an approach for optimizing the performance of the nano system using cooperative molecular communications with a nano relay scheme,while accounting for blood flow effects in terms of drift *** fractions of the molecular drug that should be allocated to the nano transmitter and nano relay positioning are computed using a collaborative optimization problem solved by themodified Central Force Optimization(mCFO)*** the previous work,the probability of bit error is expressed in a closed-form *** is used as an objective function to determine the optimal velocity of the drug molecules and the detection threshold at the nano *** simulation results show that the probability of bit error can be dramatically reduced by optimizing the drift velocity,detection threshold,location of the nano-relay in the proposed nano system,and molecular drug budget.

Due to exponential increase in smart resource limited devices and high speed communication technologies,Internet of Things(IoT)have received significant attention in different application ***,IoT environment is highly susceptible to cyber-attacks because of memory,processing,and communication *** traditional models are not adequate for accomplishing security in the IoT environment,the recent developments of deep learning(DL)models find *** study introduces novel hybrid metaheuristics feature selection with stacked deep learning enabled cyber-attack detection(HmFS-SDLCAD)*** major intention of the HmFS-SDLCAD model is to recognize the occurrence of cyberattacks in the IoT *** the preliminary stage,data pre-processing is carried out to transform the input data into useful *** addition,salp swarm optimization based on particle swarm optimization(SSOPSO)algorithm is used for feature selection ***,stacked bidirectional gated recurrent unit(SBiGRU)model is utilized for the identification and classification of ***,whale optimization algorithm(WOA)is employed for optimal hyperparameter optimization *** experimental analysis of the HmFS-SDLCAD model is validated using benchmark dataset and the results are assessed under several *** simulation outcomes pointed out the improvements of the HmFS-SDLCAD model over recent approaches.

Improving the Nutritional Values of Wheat and Rice Crackers by Using Cauliflowers

The present study described the enhancement of the nutritional values of wheat and rice crackers by adding cauliflowers. The results showed that 25% - 75% cauliflower replaced in crackers increased the consumer acceptability, nutritional value, antioxidant activity, minerals, and anti-carcinogenic effect. moreover, it also improved the functional properties and sensory quality. The results revealed that the raw samples of cauliflower recorded the highest value of moisture, ash, protein and fiber 89.29%, 7.68%, 19.04% and 10.18% on dry weight, respectively. The crackers formula of cauliflower: wheat 75%:25% and cauliflower: rice 75%:25% recorded the highest values in ash, protein, fat, and fiber. The data showed that increasing cauliflower percentage in crackers formula causing a parallel increase in all determined chemical composition. Cauliflower samples exhibited the highest antioxidant activity (92.94%). Also, crackers samples of cauliflower: wheat 75%:25% crackers, recorded 57.6% for antioxidant activity. The highest values of phenols and flavonoids for cauliflower cracker samples (cauliflower: wheat 75%:25% and cauliflower: rice 50%:50%) have increased up to (5.75 and 5.49) respectively. The results showed that the higher mineral content is the sulfur (635.96 mg/100g) in raw cauliflower, while (1.00 mg/100g, and 0.99 mg/100g sulfur content) in wheat and rice respectively. moreover, the result found that as the cauliflower percentage increases, the minerals contents especially sulfur increase. Furthermore, the cauliflower displayed moderate activity for human hepatocellular carcinoma, colon cancer and intestinal epithelial barrier treatment, (IC50 mily:Verdana;">= 183.4 ± 6.8, 175.8 ± 7.1 and 207.2 ± 8.5 μg/mL, respectively). Barrett's esophagus:Prevalence and risk factors in patients with chronic GERD in Upper Egypt

AIm： To determine the prevalence and possible risk factors of Barrett＇s esophagus （BE） in patients with chronic gastroesophageal reflux disease （GERD） in EI minya and Assuit, Upper Egypt. mETHODS： One thousand consecutive patients with chronic GERD symptoms were included in the study over 2 years. They were subjected to history taking including a questionnaire for GERD symptoms, clinical examination and upper digestive tract endoscopy. Endoscopic signs suggestive of columnar-lined esophagus （CLE） were defined as mucosal tongues or an upward shift of the squamocolumnar junction. BF was diagnosed by pathological examination when specialized intestinal metaplasia was detected histologically in suspected CLE. pH was monitored in 40 patients. RESULTS： BE was present in 7.3% of patients with chronic GERD symptoms, with a mean age of 48.3 ± 8.2 years, which was significantly higher than patients with GERD without BE （37.4 ± 13.6 years）. Adenocarcinoma was detected in eight cases （0.8%）, six of them in BE patients. There was no significant difference between patients with BE and GERD regarding sex, smoking, alcohol consumption or symptoms of GERD. Patients with BE had significantly longer esophageal acid exposure time in the supine position, measured by pH monitoring. CONCLUSION： The prevalence of BE in patients with GERD who were referred for endoscopy was 7.3%. BE seems to be associated with older age and more in patients with nocturnal gastroesophageal reflux.

Evaluation of Venous Ammonia Level, Splenic Longitudinal Diameter, Portal Vein and Splenic Vein Diameters as Non-Invasive Indicators for the Presence of Portosystemic Collaterals in Egyptian Cirrhotic Patients

Introduction and Aim of the Work: The identification of cirrhotic patients with esophageal varices or other portosystemic collateral by non-invasive means is appealing in that it could decrease the necessity of endoscopic screening. This study was to evaluate the diagnostic utility of venous ammonia level with other ultrasonographic parameters as non-invasive markers for the presence of portosystemic shunts. Patients and methods: The study included 3 groups of Child Pugh class A and early B patients. Group (A): 25 patients with evidence of both esophageal varices and portosystemic collaterals;group (B) 25 patients with neither evidence of varices nor portosystemic collaterals and group (C): 25 patients with evidence of varices but no collaterals. measurement of venous ammonia level was done for all patients. Results: serum ammonia level was significantly higher in group A (222.8 ± 54 μg/dL) than that in group B (85 ± 21.1 μg/dL) and group C (148.2 ± 19.6 μg/dL). The cut-off value of serum ammonia level 113 μg/dL was a good predictor for the presence of esophageal varices, while the cut-off value of serum ammonia level at 133 μg/dL was a good predictor for the presence of both esophageal varices and abdominal collaterals. Combination of portal vein diameter > 13mm + splenic vein diameter > 8.9mm + ammonia level > 133 μg/dL gives 100% of sensitivity and 96% of specificity for the prediction of the presence of portosystemic shunts. Conclusion: Determination of serum ammonia level, splenic, portal vein and splenic vein diameters are considered as good predictors for the presence of portosystemic shunts in patients with liver cirrhosis.

A 9 - 10.6 GHz microstrip Antenna—UWB Low Noise Amplifier with Differential Noise Canceling Technique for IoT Applications

An ultra-wide band (UWB) receiver front-end that operates at the UWB frequency range, starting from 9 GHz - 10.6 GHz is proposed in this paper. The proposed system consists of an off-chip microstrip antenna and CmOS differential low noise amplifier with a differential noise canceling (DNC) technique. The proposed antenna is trapezoidal dipole shaped with balun and printed on a low-cost FR4 substrate with dimensions 10 × 10 × 0.8 mm3. The balun circuit integrated with the ground antenna to improve the antenna impedance matching. Noise canceling is obtained by using a differential block with each stage having 2 amplifiers that generate differential signals, subtracted to improve total noise performance. The proposed DNC block improves NF by 50% while increasing total power consumption with only 0.1 mw. The differential CmOS cascode LNA with DNC block is implemented using UmC 0.13 μm CmOS process, exhibits a flat gain of 19 dB, maximum noise figure of 2.75 dB, 1 dB compression point −16 dBm and 3rd order intercept point (IIP3) −10 dBm. The proposed system has total DC power consumption of 2.8 mW at 1.2 V power supply.

The online education market is expanding both globally and locally;Arab countries are paying special attention to the growth of this sector. Reports showed that the e-learning market arrived at $27.1 billion in the year 2009 and is expected to surpass $49.6 billion by the year 2014. This paper presents and surveys the perception of students and instructors regarding mobile learning and mobile examination system in some Arab countries. many universities in the Arab world are under progress in the implementation of this new technology and many have already implemented it. Strong tools are required to improve e-learning system of education. This research supports the transition of education from conventional methods to m-learning and m-exam systems. The purpose of this research is to study the perception of both instructors and students regarding mobile learning and mobile examination systems. The introduction of such systems to the educational process requires people involved to have basic technical skills and to be aware of the benefits of such systems. Results showed different perspectives from three countries;also showed that online examination systems could be very helpful, but many factors should be considered and they should be implemented carefully to guarantee the successful adoption, fairness and reliability.

Is Cognition a Determinant of Health Related Quality of Life in Community Dwelling Non Demented Elderly?

Quality of life (QOL) is an important topic in social and medical sciences, it has a multidimensional nature and is influenced by many factors. Aim of the Work: In this study we want to assess the impact of cognitive impairment on the health related quality of life (HR-QOL) of community dwelling non demented elderly. Subject and method: 115 non demented elderly, 60 years and older recruited from outpatient geriatric clinic at Al mansoura General Hospital, Dakahlia, Egypt. Each participant underwent, comprehensive geriatric assessment, assessing cognitive function using the mini-mental state examination (mmSE) and montreal cognitive function test, assessing the health related quality of life (HR-QOL) by the RAND-36 health survey. Results: We found that the elderly with impaired cognition by both mmSE and montreal test were significantly older;the ones with lower education, with more depressive symptoms, had more functional impairment and had lower HR-QOL scores than the elderly with normal cognitive function, after controlling for confounders still cognition was a determinant of HR-QOL. Also by linear correlation coefficient a significant correlation between HR-QOL and age, function, cognition and depression was found. Conclusion: Cognition affects significantly HR-QOL of the elderly, so we can say that interventions targeting cognition in the elderly can significantly improve their QOL.

Study of Vitamin D Status in Pneumonia among Children between 6 months to 3 Years of Age

Background: In the developing world, pneumonia is not only more common than it is in Europe or North America;it is also more severe and considered the leading cause of under-five mortality. Effective management of pneumonia in children under 3-year-old-age is still challenging due to various causes. Some authors believed in non-calcemic role of vitamin D as a potential factor in the pathogenesis, prevention, or therapy of pneumonia in this specific age. Purpose: Investigate the Vitamin D status in pneumonia among children between 6 months to 3 years of age. Subjects and Methods: This study was conducted in a Pediatric point of care at Al-Zahraa University Hospital on 90 children aged 6 months to 3 years old;divided into two groups: group I (study group) including 47 patients presented with pneumonia and group II (control group) including 43 apparently healthy children of matched age and sex with no evidence of pneumonia. This study examined if the anti-tumorigenesis effect of Exendin-4 in HT29 and HCT116 colorectal cancer(CRC)involves modulation of SIRT1 and Akt/GSR3K/β-catenin/NF-κB ***29 and HCT116 cells were treated either with increasing levels of Exendin-4(0.0-200μm)or with Exendin-4(at its IC50)in the presence or absence of EX-527(10μm/a selective SIRT1 inhibitor)or Exendin-4(9-39)amide(E(9-39)A)(1μm/an Exendin-4 antagonist).In a dose-dependent manner,Exendin-4 inhibited cell survival,but enhanced levels of lactate dehydrogenase(LDH)and single-stranded DNA(ssDNA)in both HT29 and *** both cell lines and at it has an IC50(45μm for HT29 and 35μm for HCT1165),Exendin-4 also significantly reduced cell survival,migration,and invasion of both cell types,with no effect on the expression GLP-1 receptors(GLPRs)nor of the activity of *** these doses,Exendin-4 also increased the expression of SIRT1 but reduced the acetylation of NF-κB and the expression of Bax and cleaved caspase-3 and in both cell ***,protein levels of p-GS3Kβ(Ser9),total and acetylatedβ-catenin,and Anix2 were significantly decreased,but levels of p-GS3Kβ(Ser9)and p-β-catenin(Ser33/37/Thr41)were significantly increased in both HT29 and HCT116-exendin-4 treated *** the effects exerted by Exendin-4 were completely prevented by Ex527 or E(9-39)*** conclusion,Exendin-4 suppresses the tumorigenesis of HT29 and HCT116 CRC cell activation of GS3Kβ-induced inhibition ofβ-catenin and NF-κβin a SIRT1-dependent mechanism.
