Prognostic Value of Lymphocyte Vascular Density and e-Cadherin in Inflammatory Breast Cancer

Background: We recently evaluated four laboratory assays, vascular endothelial growth factor D (VeGF-D), e-cadherin, lymphatic vessel density (LVD) measured by podoplanin, and intra-lymphatic tumor emboli (ILTe), which showed notable differences between inflammatory breast cancer (IBC) and non-inflammatory locally advanced breast cancer (LABC). In this study we investigated the potential of the three most quantitatively measured markers, e-cadherin, LVD and VeGF-D, to predict survival in the IBC patients. Materials and Methods: This study involved the 100 cases identified in the Inflammatory Breast Cancer Registry (IBCR) whose tumors were previously evaluated for the four assays noted above. Living patients were recontacted and survival data were available for up to 17 years. Overall survival (OS) was analyzed through the Kaplan-Meier method stratified by e-cadherin, LVD, VeGF-D, and response to chemotherapy. The differences in OS curves were compared using the log-rank test. Results: The median OS for patients with high LVD was 6.63 years (95% CI: 4.06 to 10.14), compared to median at 10 years not reached in those with low LVD (p = 0.03). There was a trend towards a longer median OS in patients with high e-cadherin (10.14, 95% CI: 6.63 to 11.67), compared with those with low e-cadherin (6.26, 95% CI: 3.42 to undeterminable). VeGF-D levels showed no correlation with survival. Conclusion: Low LVD significantly predicts better survival. High e-cadherin expression, as with non-IBC breast cancer and several other malignancies, tends to be associated with a better prognosis.

Peritraumatic Behavior Questionnaire- Observer Rated: Validation of the objective version of a measure for combatrelated peritraumatic stress

AIM: To validate the first third-person-rated measure assessing combat-related peritraumatic stress symptoms and evaluate its psychometric properties and war-zone ***: The valid assessment of peritraumatic symptoms in the theater of military operations represents a significant challenge in combat-related, mental health research, which mainly relies on retrospective, subjective self-report ratings. This longitudinal observational study used data from actively deployed troops to correlate third-person observer ratings of deployment peritraumatic behaviors [Peritraumatic Behavior Questionnaire- Observer Rated(PBQ-OR)] collected on a bimonthly basis with post-deployment(1-wk follow-up) ratings of the previously validated PBQ self-rate version(PBQ-SR), and(3-mo follow-up) clinician assessed and self-report posttraumatic stress disorder(PTSD) symptoms(Clinician Administered PTSD Scale, PTSD Checklist). Cronbach's alpha(α) and correlation coefficients were calculated to assess internal reliability and concurrent validity respectively. ReSULTS: eight hundred and sixty male Marines were included in this study after signing informed consents at pre-deployment(mean age 23.2 ± 2.6 years). Although our findings were limited by an overall sparse return rate of PBQ-OR ratings, the main results indicate satisfactory psychometric properties with good internal consistency for the PBQ-OR(α = 0.88) and high convergent and concurrent validity with 1-wk post-deployment PBQSR ratings and 3-mo posttraumatic stress symptoms. Overall, later PBQ-OR report date was associated with higher correlation between PBQ-OR and postdeployment measures. Kappa analysis between PBQOR and PBQ-SR single items, showed best agreement in questions relating of mortal peril, desire for revenge, and experience of intense physical reactions. Logistic regression demonstrated satisfactory predictive validity of PBQ-OR total score with respect to PTSD caseness(OR = 1.0513; 95%CI: 1.011-1.093; P = 0.0

Remote Control of Fed-Batch Fermentation Systems

Bioreactor operation requires continuous monitoring of fermentation parameters and real-time control over bioreactor devices. Remote monitoring and control of the bioreactor’s computer via the Internet avoids the necessity of personnel being continually onsite during operation. A two liter Sartorius-stedim Biostat? A Plus fermentation system was networked and interfaced with the commercial software from GoToMyPC? to allow remote control of the fermentation system utilizing the internet. The fermentation vessel was equipped with hardware calibrated for monitoring and controlling culture parameters during experimentations. The uniform resource locator controlled night-vision web camera allowed continuous monitoring of the glass fermentation vessel during the day and at night. The main window screen of the laboratory computer can be securely accessed from any portable device (i. e. laptop) capable of establishing an Internet connection and executing the commercial software from GoToMyPC?. The secured internet protocol address, provided by GoToMyPC?, assures that the system can only be controlled by authorized users who have been given access to the account. This interface permits the remote control of the Biostat? A Plus fermentation system and possibly other automated or potentially automated culturing systems at the convenience of the user(s).

This qualitative literature review aimed to describe the totality of peer-reviewed scientific evidence from 1990 to 2017 concerning validity of self-reported *** review included articles about mammography containing the words accuracy,validity,specificity,sensitivity,reliability or reproducibility;titles containing self-report,recall or patient reports,and breast or‘mammo’;and references of identified citations focusing on evaluation of 2-year *** 45 publications meeting the eligibility criteria,2 conducted in 1993 and 1995 at health maintenance organisations in Western USA which primarily served highly educated whites provided support for self-reports of mammography over 2 *** concerns about validity of self-reports included(1)telescoping,(2)biased overestimates particularly among black women,(3)failure to distinguish screening and diagnostic mammography,and(4)failure to address episodic versus consistent mammography *** current totality of evidence supports the need for research to reconsider the validity of self-reported mammography data as well as the feasibility of alternative surveillance data sources to achieve the goals of the Healthy People Initiative.

目的：在马拉维儿童恶性营养不良发生的高危人群中评价抗氧化剂的补充对预防恶性营养不良的效果。研究设计：前瞻性、双盲、安慰剂对照试验，以家庭为单位随机分组。地点：马拉维南部农村的8个村庄。研究对象：来自2156个家庭，年龄1～4周岁的2372名儿童参加研究，其中2332人完成试验。干预措施：营养干预组每天补充含有核黄素、维生素e、硒和N-乙酰半胱氨酸的抗氧化剂药粉，补充剂量为推荐饮食摄入量的3倍。营养干预组和安慰剂组分别服药20周。主要结果测定指标：主要结果指标为水肿的发生率，其次为体重、身高及感染症状天数的改变比率。结果：62名儿童发生了恶性营养不良(根据水肿的出现判断)；患病情况为抗氧化剂组39/1184(3.3%)，而安慰剂组23/1188(1.9%)(相对危险度1.70，95%可信区间为0.98～2.42)。这两个组的体重或身高增加比率没有差别。补充抗氧化剂的儿童发烧、咳嗽或腹泻的发生与对照组相比并未减少。结论：按本研究剂量补充抗氧化剂不能预防恶性营养不良的发生。本研究结果不支持维生素e、硒、半胱氨酸或核黄素缺乏会引起恶性营养不良发生的假设。
