Follow-up strategy for early detection of delayed pseudoaneurysms in patients with blunt traumatic spleen injury: A single-center retrospective study

目的通过对4~6岁电声双模式助听儿童的高兴、中性及难过3种情感韵律进行研究,探讨不同听觉年龄电声双模式助听儿童的韵律识别能力。方法选取2022年9月至2023年12月某幼儿园就读的生理年龄4~6岁,听觉年龄1~4岁的电声双模式助听儿童32例,按照听觉年龄分为1.5岁组4例、2岁组6例、2.5岁组6例、3岁组6例、3.5岁组4例及4岁组6例。采用华东师范大学康复科学系自制的15个日常用句,分别用高兴、中性和难过3种不同的情感语气读出,让电声双模式助听儿童识别,共45句。分析不同听觉年龄对高兴、难过和中性3种情感韵律的识别能力。结果 1.5岁组、2岁组、2.5岁组、3岁组、3.5岁组及4岁组的情感韵律识别能力得分比较,差异无统计学意义(F=1.246,P>0.05)。6组3种不同类型情感韵律识别能力比较,差异无统计学意义(F=1.174,P>0.05),情感韵律类型和听觉年龄组之间无交互作用,差异无统计学意义(F=0.980,P>0.05)。6组不同听觉年龄情感韵律识别的正确率比较,差异有统计学意义(F=4.106,P<0.01)。结论听觉年龄1~4岁的大龄电声双模式助听儿童,情感韵律识别能力随听觉年龄的增长而升高;但不同听觉年龄对高兴、难过和中性3种情感韵律的识别不存在差异。随着听觉年龄的增长,要重视大龄电声双模式助听儿童情感韵律识别能力的训练。

Stereotactic body radiotherapy for isolated paraaortic lymph node recurrence from colorectal cancer

AIM： To evaluate the efficacy and complications of stereotactic body radiotherapy in localized paraaortic lymph node recurrence from colorectal cancer. METHODS： From 2003 to 2009, 7 patients with paraaortic lymph node recurrence （1-3 lesions） from colorectal cancer were treated with stereotactic body radiotherapy. Total gross tumor volumes ranged from 4 to 40 mL. The doses were escalated from 36 Gy/patient to 51 Gy/patient and were delivered in 3 fractions. RESULTS： One and 3 year overall survival rates were 100% and 71.4%, respectively, and median survival was 37 mo. Grade IV intestinal obstruction was reported in 1 of 7 patients. This patient received 48 Gy in 3 fractions with a maximum point dose to the intestine of 53 Gy and V45Gy = 3.6 mL. However, 6 patients received an intestinal maximum point dose of 〈 51 Gy and V45Gy of 〈 1 mL, and did not develop any severe complications. COnCLUSIOn： This pilot study suggests selected paraaortic lymph node recurrence （1-3 closed lesions） that failed to respond to chemotherapy can be potentially salvaged by stereotactic body radiotherapy.

CO_2 laser-micro plasma arc hybrid welding for galvanized steel sheets

A laser lap welding process for zinc-coated steel has a well-known unsolved problem-porosity formation. The boiling temperature of coated zinc is lower than the melting temperature of the base metal, which is steel. In the autogenous laser welding, the zinc vapor generates from the lapped surfaces expels the molten pool and the expulsion causes numerous weld defects, such as spatters and blow holes on the weld surface and porosity inside the welds. The laser-arc hybrid welding was suggested as an alternative method for the laser lap welding because the arc can preheat or post-beat the weldment according to the arrangement of the laser beam and the arc. CO2 laser-micro plasma hybrid welding was applied to the lap welding of zinc-coated steel with zero-gap. The relationships among the weld quality and process parameters of the laser-arc arrangement, and the laser-arc interspacing distance and arc current were investigated using a full-factorial experimental design. The effect of laser-arc arrangement is dominant because the leading plasma arc partially melts the upper steel sheets and vaporizes or oxidizes the coated zinc on the lapped surfaces. Compared with the result from the laser-TIG hybrid welding, the heat input from arc can be reduced by 40%.

Objective:To demonstrate the effect of dieckol from Eisenia bicyclis on osteoclastogenesis using RAW 264.7 ***:Murine macrophage RAW 264.7 cells were subjected to dieckol treatment,followed by treatment with receptor activator of nuclear factor kappa-B ligand(RAnKL)to induce ***-resistant acid phosphatase(TRAP)activity was examined using a TRAP activity *** blotting analysis was conducted to examine the level of osteoclast-related factors,including TRAP and calcitonin receptor(CTR),transcriptional factors,including c-Fos,c-Jun,and nuclear factor of activated T cells cytoplasmic 1(nFATc1),nuclear factor kappa-B(nF-κB),extracellular signal-regulated kinase(ERK),and c-Jun n-terminal kinase(JnK).Immunofluorescence staining was conducted to examine the expression of c-Fos,c-Jun,and ***:Among the four phlorotannin compounds present in Eisenia bicyclis,dieckol significantly hindered osteoclast differentiation and expression of RAnKL-induced TRAP and *** addition,dieckol downregulated the expression levels of c-Fos,c-Jun,nFATc1,ERK,and JnK,and suppressed nF-κB ***:Dieckol can suppress RAnKL-induced ***,it has therapeutic potential in treating osteoclastogenesis-associated diseases.

Mixing features in an electromagnetic rectangular micromixer for electrolyte solutions

In this study, a new magnetohydrodynamic（MHD） mixer for electrolyte solutions with pumping function is reported, and the mixing performance of the device for two different electrolyte solutions is numerically examined in a uniform magnetic field. Application of different potentials to different electrodes allows the current to be induced. The combination of the induced current and magnetic field yields Lorentz force, resulting in the fluid motion. The numerical simulation for the flows in the device is carried out with commercial software CFX. The validity of CFX code for the present numerical model is presented. The mixing performance of the fluids is investigated in many different cases with different combinations of input voltage of the electrode. This study shows that the mixing performance can be enhanced by applying spatially alternating positive and negative voltages to the electrodes. The present simulation results show that with a magnetic field intensity lower than 0.5 T, a voltage difference smaller than 2.0 V, and an electric conductivity of electrolyte solution of 1.5 S/m the pumping capabilities ranging 1.6×10^（-7）-3.6×10^（-6） kg/s and the mixing indexes higher than 0.90 can be obtained with sophisticated designs of the micromixer.

Flow imbalance in MHD flows in splitting ducts with locally different electric conductivities of the walls

In this study, three-dimensional liquid-metal magnetohydrodynamic splitting flows in a duct with one inflow channel and two outflow channels with locally different wall electric conductivities（different conductance ratios） are computationally examined. The numerical calculation is achieved with the use of CFX code, with a structured grid system chosen after a series of mesh independence tests. The detailed features of the MHD flows are depicted in a case where the electric conductivities of the two outflow ducts are quite different. Also, the interdependency among the current, fluid velocity, pressure and electric potential is elucidated in order to describe the electromagnetic characteristics of the liquid-metal flows. The result shows that the imbalance of the mass flow rate in the two outflow channels strongly depends on the wall conductivity（i.e., conductance ratio） of each outflow channel.

Clinical implications of fatty pancreas:Correlations between fatty pancreas and metabolic syndrome

AIM:To investigate the clinical implications of lipid deposition in the pancreas(fatty pancreas). METHODS:The subjects of this study were 293 patients who had undergone abdominal computed tomography(CT)and *** pancreas was diagnosed by sonographic findings and subdivided into mild,moderate,and severe fatty pancreas groups comparing to the retroperitoneal fat echogenicity. RESULTS:Fatty pancreas was associated with higher levels for visceral fat,waist circumference,aspartate aminotransferase(AST),alanine aminotransferase (ALT),total cholesterol,triglyceride,high density lipoprotein,free fatty acid,γ-GTP,insulin,and the homeostasis model assessment of insulin resistance (HOMA-IR)than the control group(Pnd ALT also tended to increase with the degree of fat deposition in the pancreas on *** a multivariate logistic regression analysis,HOMA-IR,visceral fat,and ALT level were independently related to fatty pancreas after adjustment for age,body mass index,and lipid *** incidence of metabolic syndrome in the fatty pancreas group was significantly higher than in the control group,and the numbers of metabolic syndrome parameters were significantly higher in the fatty pancreas group(PnCLUSIOn:Sonographic fatty pancrease showed higher insulin resistance,visceral fat area,triglyceride, and ALT levels than normal *** pancreas also showed a strong correlation with metabolic syndrome.
