I examine stigmatization of mental illness in the Japanese workplace. The participants were 25 Japanese adults with mental disorders who were interviewed regarding their experience of stigmatization in places they had worked. Narrative data from semi-structured interviews were analyzed qualitatively. Responses were classified as stigma toward treatment, stigmatizing attitudes, and stigmatization due to disclosure of illness, and the characteristics of stigmatization experienced by people with mental disorders were identified. The findings, in conjunction with analysis of anti-stigma programs worldwide, are expected to contribute to the development of effective anti-stigma measures in Japan.

The employment of people with mental illness is increasing each year in Japan. When they choose regular employment, some choose to disclose their illness, and others do not. In this qualitative study in Japan, I conducted a questionnaire survey among people with mental illness with an employment history about their experiences with disclosing their mental illness. After the data were coded, categorized, and analyzed, I identified three common factors for non-disclosure to an employer or supervisor: anxiety about dismissal and not being hired, anxiety about unfavorable treatment, and anxiety about stigma. Education and consideration for the illness were identified as requests for the employer. Based on these findings, I concluded that people with mental illness anticipate social alienation at the time of hiring or while they are on the job, that they must take measures to improve their working environment and decrease stigma in the workplace to continue working, and that education and increased awareness of mental illness in the workplace are needed.

Mental disorders are a significant cause of disability and loss of workplace productivity. Many people with mental illness want to work. Occupational health in the world has to face up to psychosocial risks to protect the health of employees. My study aims are to improve negative feelings in work place and work functioning and to stimulate better coping mechanisms. Participants were collected from “Mental health daycare facility”, “Regional living assistance center” and “B-type employment assistance office” in Japan. I took interviews with 25 members. The content of the interviews was categorized about negative feelings in work place. A total of 33 small category negative feelings of mental disorder were extracted and were classified into 11 subcategories. These subcategories were then classified into two higher-level categories: inward-directed feelings and outward-directed feelings. Inward-directed feelings include “reserve or timidity,” “hesitation about being open about the illness,” “anxiety about being dismissed,” “regrets about resigning,” “sense of isolation,” “feelings of remorse,” and “suffering.” Outward-directed feelings include “failure in interpersonal relationships,” “distrust,” “frustrations,” “negative feelings toward those with mental disorder.” Employment has been linked to having better social networks, to enhanced quality of life and to overall wellbeing. We will have to defend them legally. The legal and moral obligations placed on the employer require that it provides a healthy, harassment-free work environment.

The prodromal symptoms of schizophrenia are nonspecific and include social withdrawal, sleeplessness, decreased desire, and loss of concentration. Schizophrenia outcomes might be improved if greater therapeutic effort was focused on people with prodromal symptoms. To hasten help-seeking for schizophrenia, we developed a schizophrenia education program for families of patients. Help-seeking and schizophrenia attitudes and knowledge were analyzed among Japanese parents (666 fathers and 613 mothers) of high school students by using the Social Distance Scale-Japanese version, the Link Devaluation-Discrimination Measure and our own questions on help-seeking and schizophrenia knowledge. Statistical analysis included cross-tabulation, the χ2 test, and multiple regression analysis. Most (97.5%) fathers were aged 40 - 59 years;most (78.3%) mothers were aged 40 - 49 years. There was a significant difference between fathers and mothers in basic knowledge of schizophrenia (P 0.05). When asked what treatment they would seek for a child with sleeplessness or social withdrawal, fathers and mothers significantly differed in seeking help medical care at a department of psychosomatic medicine (P < 0.05) and in seeking help from the nurse at their child’s school (P < 0.001). Fathers and mothers also differed in relation to the decision not to seek help (P < 0.001). Multiple regression analysis revealed that basic knowledge significantly differed between fathers and mothers (P = 0.001). In an analysis of parental response to a child with sleepiness and social withdrawal, multiple regression analysis revealed that fathers and mothers significantly differed in the likelihood of seeking help at a department of psychosomatic medicine and from a school nurse and in the likelihood of not seeking help (P < 0.05, P = 0.001 and P = 0.001, respectively). The Great East Japan Earthquake, a magnitude 9.0 quake that occurred on March 11, 2011, left more than 20,000 killed or missing and resulted in more than 400,000 people being displaced. The Fukushima Nuclear Power Plant accident released large amounts of radioactive material into the air. Among the victims of this combined disaster were many pregnant and parturient women, and this study aimed to determine post-disaster anxiety among this specific population and measures for the future. Participants were 259 women (mean age 33.02 ± 4.79 years) who gave birth around the time of the earthquake in Miyagi Prefecture, one of the disaster areas. Sixteen months after the earthquake, we administered survey questionnaires on anxiety. We transcribed questionnaire responses, coded raw data by context, and categorized these codes by commonality. After extracting subcategories of anxiety-related factors, we categorized them into more abstract concepts. Among the participants, 126 (48.6%) reported having no available professionals with whom they could consult about childrearing. Participants reported anxiety in the following 12 categories: “radiation,” “child’s physical and mental growth/development,” “recurrence of earthquake and tsunami,” “financial issues,” “childrearing environment,” “living environment,” “maternal employment,” “stigma,” “familial issues,” “maternal health,” “childrearing,” and “the future”. A beneficial way to reduce maternal anxiety in the 12 areas identified would be to develop support systems that provide continuous support for children’s mental health care needs, psychological guidance, community support for maternal empowerment, outreach for individual support, and professional consultation for mothers who have high anxiety about radioactivity.

Social distance toward schizophrenia is a reason for delays in receiving early treatment for the disorder. This study attempted to identify the factors underlying social distance. The participants were 2690 parents of adolescents. Factor analysis using a social distance scale identified two factors: private relationship and social relationship. Regression analysis revealed that gender, age, and participation in welfare activities for people with mental illnesses had independent effects on the factor private relationship (p < 0.05). These results suggest that women aged 40 - 49 years should be targeted for activities that attempt to counteract social distance and that they would benefit from taking part in public welfare activities for individuals with mental illnesses.

Association of social distance toward schizophrenia with help-seeking among mothers of adolescents in Japan

Negative maternal attitudes toward schizophrenia may be linked with delayed treatment of their children. We investigated the relation between negative attitudes toward schizophrenia and help-seeking among mothers of junior and senior high school students in Japan. The participants were 1309 Japanese mothers of junior and senior high school students. Social distance was evaluated by using the Social Distance Scale-Japanese version (SDS-J). In addition, mothers were asked about help-seeking for a child with sleeplessness, social withdrawal, and strange behavior. One-way analysis of variance and Student’s t-test were used to evaluate associations between social distance toward schizophrenia and help-seeking. Most (76.4%) participants were aged 40 - 49 years. Maternal demographic characteristics significantly associated with social distance were employment and participation in welfare activities for people with mental illness. In responding to a child with sleeplessness, social withdrawal, and strange behavior, the level of maternal social distance was not significantly associated with the likelyhood of seeking psychiatric help. However, mothers with greater social distance were less likely to seek help at a psychiatric clinic. Maternal social distance toward schizophrenia was not significantly associated with seeking psychiatric help;however, it did affect the type of facility selected among those would seek such help.

The purpose of this study was to understand the impact of childbirth and the experience of fatherhood on Japanese men in Japan and a foreign country. Descriptive phenomenology was used to study a total of 14 Japanese men who attended childbirth and experienced parenting in the United States and Japan. The Colaizzi method of data analysis was used to analyze responses to open-ended questions. Responses to these questions showed several similarities between the two groups of men. First, men in both countries felt closer to with their spouses having gone through the experience of childbirth together. Second, both groups nevertheless recognized a strong bond between mother and the baby, leading them to feel at times isolated. Third, both groups were concerned about their wives’ emotional swings during pregnancy and child rearing. Finally, both groups were more focused on their wives and babies than themselves. There were also several interesting differences. Japanese men who were living in Hawaii were more involved in taking care of their children and in helping with household chores than those living in Japan. This was due to living in a more family-oriented society, as well as a result of limited support from their extended families back in Japan. A result of spending more time with their wives and babies was that Japanese men in the United States understood more fully the stress of childcare. On the other hand, due to Japan’s work-oriented society, men in Japan relied more on support from their extended families, leaving them less time with their wives and children. This study clearly shows that social support systems alter gender roles and behavior, leading to significant differences in the experience of parenthood in Japan and a foreign country.

Purpose: This study aims to establish criteria to determine the need for admission of people with dementia to dementia nursing wards of psychiatric hospitals—based on the experience of nurses working in dementia nursing wards. Methods: Semi-structured interviews were conducted with nurses who had worked in dementia nursing wards of psychiatric hospitals for more than 3 years, to collect data related to the “condition at the time of admission and the process of hospitalization of dementia patients”. Data were analyzed using the content analysis approach. Focusing on “What is the condition of patients with dementia admitted to the dementia nursing ward?”, we created codes according to similarities in the meaning, and classified these into categories where they were evaluated to fully fit in. Results and Discussion: The analysis yielded 4 categories, and 44 codes. The four categories are as follows: [Appearance of a state where self-control is difficult] which expresses a state where behavioral and psychological symptoms of dementia (BPSD) have appeared, and the remaining three categories [Insufficient support provided], [Appearance of symptoms that make living difficult], and [Appearance of physical symptoms that require treatment] express the states that may trigger the appearance of BPSD. These show that the appearance of BPSD is a criterion for determining hospitalization. By providing support to prevent the states described in the 44 codes, nurses may help people with dementia avoid being hospitalized in dementia nursing wards in psychiatric hospitals.

Determination of Apparent Formation Constants of Eu(III) with Humic Substances by Ion Selective Liquid Membrane Electrode

This is the novel measurement of the apparent formation constants of trivalent lanthanide with humic substances by potentiometric titration using an ion selective electrode (ISE) consisting of bis(diphenylphosphoryl)methane as an ionophore. The ISE prepared exhibited a Nernstian response to the Eu3+ concentration in the test solutions in the presence of humic acid and fulvic acid. The effect of the solution conditions, including the pH, initial metal and humic substance concentrations, and ionic strength, on the formation constants was examined. The present formation constants were compared with the reported data using solvent extraction method.
