Automated evaluation of parapapillary choroidal microvasculature in crowded optic discs:a controlled,optical coherence tomography angiography study

AIM:To compare superficial and deep vascular properties of optic discs between crowded discs and controls using optical coherence tomography angiography(OCT-A).METHODS:Thirty patients with crowded discs,and 47 control subjects were enrolled in the study.One eye of each individual was included and OCT-A scans of optic discs were obtained in a 4.5×4.5 mm^(2) rectangular area.Radial peripapillary capillary(RPC)density,peripapillary retinal nerve fiber layer(pRNFL)thickness,cup volume,rim area,disc area,cup-to-disc(c/d)area ratio,and vertical c/d ratio were obtained automatically using device software.Automated parapapillary choroidal microvasculature(PPCMv)density was calculated using MATLAB software.When the vertical c/d ratio of the optic disc was absent or small cup,it was considered as a crowded disc.RESULTS:The mean signal strength index of OCT-A images was similar between the crowded discs and control eyes(P=0.740).There was no difference in pRNFL between the two groups(P=0.102).There were no differences in RPC density in whole image(P=0.826)and peripapillary region(P=0.923),but inside disc RPC density was higher in crowded optic discs(P=0.003).The PPCMv density in the inner-hemisuperior region was also lower in crowded discs(P=0.026).The pRNFL thickness was positively correlated with peripapillary RPC density(r=0.498,P<0.001).The inside disc RPC density was negatively correlated with c/d area ratio(r=-0.341,P=0.002).CONCLUSION:The higher inside disc RPC density and lower inner-hemisuperior PPCMv density are found in eyes with crowded optic discs.

Pandemics obligate providers to transform their clinical practice.An extensive effort has been put to find out feasible approaches for gastrointestinal diseases and also to manage coronavirus disease 2019(COVID-19)related gastrointestinal conditions.Diarrhea,hepatitis,and pancreatitis can be seen in the COVID-19 course.Endoscopic procedures increase the risk of contamination for medical staff and patients despite precautions,therefore indications should be tailored to balance risks vs benefits.Furthermore,whether the immunosupression in inflammatory bowel diseases,liver transplantation,and autoimmune liver diseases increases COVID-19 related risks and how to modify immunosupression are topics of ongoing debate.This review aims to provide most up to date practical approaches that a gastrointestinal fellow should be aware on the problems and management of gastrointestinal and hepatobiliary diseases during the COVID-19 pandemic.

A novel terminologic “naming-meshing” system using anterior chamber sedimentation for early diagnosis and prompt treatment of ocular or systemic diseases: is it hypopyon or pseudohypopyon?

A novel,algorithmic“naming-meshing”system was introduced for the distinction of hypopyon from pseudohypopyon to make an early diagnosis and prompt treatment of anterior chamber collection standardized to encompass all sediment characteristics.For this reason,a literature review of“hypopyon”and“pseudohypopyon”was conducted in MEDLINE/PubMed,Scopus,and Web of Science from 1966 to May 15,2023.Two issues were clarified:1)which strategies should the ophthalmologist follow when asked to evaluate an eye with anterior chamber sedimentation to distinguish hypopyon from pseudohypopyon,and 2)in which systemic disorders should a non-ophthalmologist order a prompt ophthalmic consultation to distinguish pseudohypopyon from hypopyon.Pathognomonic characteristics of the sediment were examined;scleral show(warm/cold),location(corneal/anterior chamber/capsular/posterior),visibility(macro/micro/occult-angle),orientation(horizontal/vertical/oblique),number(single/double),shape(convex/triangular/pyramidal/ring/lumpy/inverse),and color(white/yellow/pink/brown/black).Associated findings were then assessed;acute/chronic,spontaneous/provoked,unilateral/bilateral,inflammatory/non-inflammatory,suppurative(nonsterile)/non-suppurative(sterile),granulomatous/nongranulomatous,recurrent/non-recurrent,shifting/nonshifting,and transient/persistent.The type of precipitation was named(naming)and matched(meshing)to a potential list of etiologies(inflammatory,infective,therapeutic,masquerades).Given that(pseudo)hypopyon predominantly afflicts younger patients in their most productive years,clinicians supervising such patients should be aware of all sediment characteristics.The ophthalmologist should never ask non-ophthalmologists to run the full battery of tests in a patient with(pseudo)hypopyon,and rather indicate which type of collection is present,what its pathognomonic feature is,and what the most likely diagnoses to be excluded are.

This research investigates a broad range of possible factors affecting the adoption of new technology in the banking industry using adaptive LASSO and a standard logit model.The research integrated the adoption of the innovation framework and the technology acceptance theory to develop a conceptual framework for the analysis.Primary data was collected from 400 bank customers in North Cyprus.Risk perception and other customerspecific factors such as perceived risk index and negative attitude toward new technologies index were formulated for the proposed conceptual model.The findings indicated that individuals with a negative attitude toward new technology are least likely to adopt internet banking.In addition,the logit model suggested that age,education level,and general(innate)innovativeness significantly impact the adoption of internet banking.However,gender,income,occupation,perceived risk,familiarity with the internet,and social inclusion have no significant impact on internet banking adoption in North Cyprus.

学术讲座作为高等院校学生获取知识的主要途径之一,对学生而言具有不可或缺的重要性。为了确保学生能够充分吸收讲座内容,做笔记成为了一项至关重要的技能。事实上,记笔记的能力以及笔记的质量与学生的学习成绩之间存在着密切的关联。因此,在EAP课程中,记笔记教学被视为一个不可或缺的组成部分,旨在培养学生良好的笔记习惯,从而助力他们取得更好的学术成绩。本研究旨在构建一个基于体裁的教学框架(Genre-Based Instruction,GBI),用于指导笔记教学的实践。为实现这一目标,本研究对八个跨学科讲座进行了深入分析。首先,我们依据Bouziri(2020)所建立的框架,对修辞功能进行了明确界定。随后,我们采纳了?del(2010)与Hyland(2005)共同提出的元话语框架,并据此确定了体现组织结构的元话语要素。经过深入研究,我们发现了修辞功能与元话语要素之间存在紧密的关联。也就是说,某些元话语项目可以作为使用某些修辞功能的信号。因此,记笔记的GBI框架是根据修辞功能与元话语要素之间的关联而构建的。

Evaluation of macular choroidal and microvascular network changes by activity scores and serum antibodies in thyroid eye patients and healthy subjects

AIM:To investigate the choroidal thickness and the microvascular network changes around the macula in thyroid eye disease(TED)patients at different stages and the relationship of those changes with risk factors,serum antibodies and the severity of TED.METHODS:A total of 85 participants were enrolled.All participants underwent ophthalmology and endocrinology examinations.Subfoveal choroidal thickness(SFCT),superficial(s)and deep(d)foveal avascular zone(FAZ)area,mean(m)and central(c)superficial vascular density(SVD),deep vascular density(DVD)measurements of the enrolled cases were performed with Topcon swept source optical coherence tomography(OCT)/OCT angiography(OCTA)DRI OCT Triton.Multiple linear regression analysis was used to explore the associations between SFCT,FAZ area,SVD,DVD and the relevant factors of TED.RESULTS:Those with active TED patients had higher c-DVD and m-DVD levels(P<0.05),however there is no statistically significant difference in SFCT between active and stable TED patients.Among the serum antibodies,it was observed that s-FAZ and d-FAZ increased,c-SVD and m-SVD decreased in patients with high thyroid stimulating hormone-receptor autoantibodies(TRAB)level,whereas SFCT thickened in patients with high levels of both TRAB and human thyroglobulin(hTG).There was no significant difference in SFCT,FAZ,SVD and DVD measurement at gender,between hyperthyroid and euthyroid patients and among those with or without thyroid papillary carcinoma.CONCLUSION:The results show that both disease activation and serum antibodies differentially affect both superficial and deep retinal vascular density.It has also been shown that high serum antibody levels affect choroidal thickness independent of clinical activity.

Efficacy of custom-made soft keratoconus lenses on corneal aberrations and photic phenomena in patients with keratoconus:a corneal topography imaging based study

AIM:To evaluate the efficacy of custom-made soft keratoconus(KC)lenses on corneal aberrations and photic phenomena in two different optical zones in patients with KC.METHODS:Sixty eyes of 42 patients with KC were examined at baseline and after fitting HydroCone(Toris K)soft silicone hydrogel lenses.Best spectacle-corrected visual acuity(BSCVA)and contact lens-corrected visual acuity(CLCVA)were recorded.Lower-and higher-order corneal aberrations(LOAs and HOAs)were measured with and without Toris K lenses and compared in central 4.5 mm and 7 mm zones.Mesopic pupil diameter and subjective photic phenomena were also assessed.RESULTS:Mean CLCVA was significantly improved compared to BSCVA(P6.00 mm(85.7%).CONCLUSION:Toris K lenses provide good visual results and a significant reduction in central corneal aberrations in patients with KC but could cause an increase in peripheral aberrations.This practical and nonsurgical approach appears to be an effective method for the visual management of KC.

COVID-19 vaccine hesitancy and related factors among primary healthcare workers in a district of Istanbul:a cross-sectional study from Turkey

Objectives This study aimed to determine the COVID-19 risk perceptions,vaccination intentions and predictive factors of family physicians and family healthcare staff working in primary care inÜsküdar.Design A cross-sectional study was performed using an online questionnaire to determine the demographic and general characteristics of the participants and their willingness to be vaccinated.Setting An online questionnaire was applied to family physicians and family health workers working in primary care family health centres inÜsküdar between 25 and 29 December 2020.Multivariate analysis was performed to identify independent predictors of the willingness of individuals to be vaccinated.Participants Out of 323 health workers working in 44 family health centres in the district,a total of 276 health workers were reached,including 126 physicians(n=158,79.7%)and 150 midwives/nurses(n=165,90.9%)(response rate 85.4%).Results 50.4%(n=139)of the healthcare workers were willing to have the COVID-19 vaccine,29%(n=80)were undecided and 20.7%(n=57)refused the vaccine.The rate of acceptance to be vaccinated was higher in physicians,in men and in those who had not received a seasonal influenza vaccination regularly each year.Conclusions Half of the primary healthcare workers,one of the high-risk groups in the pandemic,were hesitant or refused to be vaccinated for COVID-19.Knowing the factors affecting the vaccine acceptance rates of healthcare professionals can be considered one of the most strategic moves in reaching the target of high community vaccination rates.For evidence-based planning in vaccination studies,there is a need to investigate the reasons for COVID-19 vaccine acceptance by healthcare workers at all levels.

Many gluten-free foods are prepared from starch-rich ingredients with low protein and fiber content,resulting in poor nutritional and functional quality.In this study,five different composite legume flours which are prepared from different levels of chickpea,lentil,common bean,soybean and lupin flours were used at 75%level in gluten-free tarhana formulations to improve nutritional quality.Also,effect of baker’s yeast(Saccharomyces cerevisiae)at different levels(2.5%and 5.0%)on physicochemical and sensory attributes of the samples containing composite flours was investigated.The inclusion of composite flours(75%)resulted in lower L^(*) and hue values in tarhana than gluten-free sample without composite flour.The viscosity of tarhana soup containing“25%chickpea+50%lentil”composite flour(F2)was similar to traditional wheat tarhana soup(p>0.05).The utilization of composite flours containing“50%chickpea+25%lentil”(F1),F2,and“25%chickpea+25%lentil+25%common bean”(F3)in tarhana revealed comparable oil absorption capacity to wheat tarhana.Besides,pH and foaming capacity values of the samples containing composite flours were greater than wheat tarhana and gluten-free tarhana without composite flour.The addition of composite flours elicited markedly higher ash,protein,fat,and mineral content.While phytic acid concentration of tarhana enhanced with the incorporation of composite flour,the use of 5.0%yeast considerably decreased phytic acid content and enriched ash,protein,total phenolic content,Ca,Cu,K,Mg,and P concentrations of tarhana in comparison with tarhana containing 2.5%yeast.The inclusion of F1,F2,and F3 elicited acceptable sensory scores in tarhana.It was concluded that gluten-free tarhana prepared from legume composite flours(especially F1,F2,and F3)+5.0%yeast could offer a nutritious and acceptable alternative for gluten-free diet with greater protein and mineral contents.Based on the findings,further studies may evaluate the use of hydrocolloids to improve the technological and sensory properties of functional
