Nalbuphine versus Dexmedetomidine as an Analgesic Additive to Lidocaine in Intravenous Regional Anesthesia IVRA

Intravenous Regional Anesthesia (IVRA) is easy to administer and reliable. But delayed onset and lack of postoperative analgesia are the major limitations. Accordingly, many additives have been tried. Dexmedetomidine is a highly selective α-2 adrenoceptor agonist. Addition of dexmedetomidine to lignocaine is effective in decreasing the anesthetic requirements and prolonging the analgesic duration. On the other hand, many theories explain that opioids may exert their peripheral action through peripheral opioid receptors. The aim of the study was to compare the analgesic efficacy of nalbuphine and dexmedetomidine when used separately as adjuvants to lidocaine during IVRA with the effect of lidocaine alone. Sixty adult patients, who were scheduled for surgery of the hand or the forearm under intravenous regional anesthesia, were included in this study. The patients were randomly allocated into three equal groups. The syringes in all groups contained 3 mg/kg of lidocaine 0.5% diluted in 40 ml isotonic saline. Group C: Control group. Group D: Dexmedetomidine group, 1 mic/kg dexmedetomidine diluted was added. Group N: Nalbuphine group, 20 mg nalbuphine was added. Sensory onset time (min) as well as motor block onset time (min) were significantly shorter in Groups N (2.0 ± 1.7) (3.8 ± 2.1) respectively, and D (2.2 ± 1.8) (4.6 ± 2.2) respectively compared to Group C (3.6 ± 1.6) (7.1 ± 1.4) (P < 0.05), with no significant differences between nalbuphine and dexmedetomidine groups. Sensory and motor block recovery times (min) were significantly longer in Groups N (9.6 ± 0.7) (10.3 ± 1.2) and D (8.1 ± 1.1) (9.1 ± 2.1) when compared to Group C (3.4 ± 2.1) (3.7 ± 3.1) (P < 0.05), without significant differences between nalbuphine and dexmedetomidine. Ramsay sedation score was significantly higher (RSS = 2) in 14 patients (70%) in Group D compared to Groups C and N during the first 30 min after the release of tourniquet.

Thoracic Epidural Analgesia versus Dexmedetomidine Infusion in Traumatic Flail Chest

Background: Traumatic flail chest is a serious injury that can impair ventilation and affect patient outcome. Thoracic epidural analgesia is the gold standard to provide adequate analgesia in flail chest, however, it may be unavailable in some patients due to coagulopathy, failure or difficult insertion. We compared between parenteral dexmedetomidine and thoracic epidural block with plain local anesthetic in flail chest cases. Patients and methods: fifty eight trauma patients with flail chest randomly allocated into either Group E (n = 29): epidural group, patients received mid-thoracic epidural analgesia using 6 ml mixture of 0.125% bupivacaine and 2 μg/ml fentanyl, which followed by continuous infusion of 6 ml/hour;Group D (n = 29): dexmedetomidine group, patients received loading dose of dexmedetomidine 1 μg/kg over 30 min, after a continuous infusion at a rate of 0.5 μg/kg/hr. The primary outcomes were to assess the effect of analgesic type on ventilation (PaO2/FIO2 ratio, PaCO2). The secondary outcomes were to compare analgesic effect, hemodynamics, the need for ventilation and ICU stay. Result: PaO2/FIO2 ratio was significantly higher in epidural group and PaCO2 was significantly lower in epidural group (p value < 0.05). The incidence of mechanical ventilation was significantly lower in epidural group than in dexmedetomidine group (6 patients group versus 13 patients, p value < 0.04). Mean arterial blood pressure was significantly lower in dexmedetomidine group than in epidural group (94.3 ± 6.84 mmHg versus 102 ± 5.72 mmHg, p value < 0.001). Moreover, heart rate was significantly lower in dexmedetomidine group than epidural group (89.97 ± 6.22 bpm versus 96.07 ± 9.3 bpm, p value = 0.004). VAS was significantly lower in epidural group (p value < 0.001). Throughout different measuring points, RAMSAY score was significantly higher in dexmedetomidine group. Physical and orbital properties of the stellar system HIP 43766

In this paper,we present the analysis of the stellar binary system HIP 43766 to determine its *** rely on dynamical modeling and atmospheric modeling with recent data to determine the orbital solution and the physical properties of the *** is a consistency between observed and synthetic photometry obtained using atmospheric *** calculated dynamical mass sum of the system ranged between 1.691 and 2.609 solar masses,while it ranges between 2.0 and 2.1 as estimated utilizing atmospheric *** could be due to inaccuracy in estimating the orbit,which could be modified with future observations with more relative positional *** parameters of the system and the position of the components on the evolutionary tracks show that the system consists of F5 and G5 subgiant stars,mostly formed by fragmentation.A dynamical mass sum is predicted for the system.

Symbol Error Probability of Incremental Relaying with Distributed/Centralized Relay Selection

In this paper,we derive the Symbol Error Probability(SEP)of cooperative systems with incremental relaying and Distributed Relay Selection(DRS).The relays remain idle when the Signal to Noise Ratio(SNR)between the source and destination is larger than ***,we activate a relay using *** nodes transmit only if their SNR is larger than thresholdβ.If the SNRs of more than two relays is larger thanβ,there is a collision and the destination uses only the received signal from the *** all relays have SNR less thanβ,no relay is ***βis optimized to yield the lowest SEP at the *** results are compared to centralized relay selection using opportunistic Amplify and Forward(OAF),Partial and Reactive Relay Selection(PRS and RRS).We compare our results to computer simulations for Rayleigh fading channels.

In this article,we optimize the powers associated to Non Orthogonal Multiple Access(NOMA)users,sensing and harvesting duration for Cognitive Radio Networks(CRN).The secondary source harvests energy from node A ***,it senses the channel to detect primary ***,the secondary source transmits a signal that is reflected by Intelligent Reflecting Surfaces(IRS)so that all reflections have a zero phase at any user.A set Ii of reflectors are associated to user *** use ofM=Mi=512,256,128,64,32,16,8 reflectors per user offers 45,42,39,36,33,30,27 dB gain *** absence of *** also suggest the use of IRS in energy *** use P=8 reflectors for energy harvesting and M=Mi=8 reflectors per user for data communications offers 7 and 38 dB gain *** IRSM=Mi=8 and the absence of *** use of P=16 reflectors for energy harvesting and M=Mi=8 reflectors per user for data communications offers 9 and 42 dB gain *** IRS M=Mi=8 and the absence of IRS.

A reliability study of springback on the sheet metal forming process under probabilistic variation of prestrain and blank holder force

This work deals with a reliability assessment of springback problem during the sheet metal forming process. The effects of operative parameters and material properties, blank holder force and plastic prestrain, on springback are in- vestigated. A generic reliability approach was developed to control springback. Subsequently, the Monte Carlo simula- tion technique in conjunction with the Latin hypercube sam- pling method was adopted to study the probabilistic spring- back. Finite element method based on implicit/explicit al- gorithms was used to model the springback problem. The proposed constitutive law for sheet metal takes into account the adaptation of plastic parameters of the hardening law for each prestrain level considered. Rackwitz-Fiessler al- gorithm is used to find reliability properties from response surfaces of chosen springback geometrical parameters. The obtained results were analyzed using a multi-state limit reli- ability functions based on geometry compensations.

Solar Energy Harvesting Using a Timer-Based Relay Selection

In this paper,the throughput and delay of cooperative communications are derived when solar energy is used and relay node is selected using a *** source and relays harvest energy from sun using a photo voltaic *** harvested power is used by the source to transmit data to the ***,a selected relay amplifies the signal to the ***,partial and reactive relay selection are *** relay transmits when its timer *** timer is set to a value proportional to the inverse of its Signal to Noise Ratio(SNR).Therefore,the relay with largest SNR will transmit first and its signal will be detected by the other relays that will remain idle to avoid *** duration is optimized to maximize the ***’s waiting time and total delay are also *** also derive the statistics of SNR when solar energy is *** harvested power from sun is proportional to the sum of a deterministic radiation intensity and a random attenuation due to weather effects and clouds *** fixed radiation intensity depends on season,month and time t in *** throughput of cooperative communications with energy harvesting from sun was not yet studied.

In this paper,we propose a sensing scheme based on energy detection,matched filter and cyclic *** Equal Gain Combining(EGC)and optimal combination of the aforementioned detectors are investigated in cooperative and non-cooperative spectrum sensing *** packet transmission systems such as OFDM(Orthogonal Frequency Division Multiple access)systems,the proposed scheme takes advantage of utilizing more samples than individual detectors,i.e.,cyclic prefix,training or pilot samples,and data payload *** proposed combine-sensing scheme offers higher detection probability and lower false alarm probability,as compared with the performance of individual detectors over the same frame *** results are congruent with the theoretical curves and confirm the validity of our derivations.

An Eco-Friendly Approach for Reducing Carbon Emissions in Cloud Data Centers

Based on the Saudi Green initiative,which aims to improve the Kingdom’s environmental status and reduce the carbon emission of more than 278 million tons by 2030 along with a promising plan to achieve netzero carbon by 2060,NEOM city has been proposed to be the“Saudi hub”for green energy,since NEOM is estimated to generate up to 120 Gigawatts(GW)of renewable energy by ***,the Information and Communication Technology(ICT)sector is considered a key contributor to global energy consumption and carbon *** data centers are estimated to consume about 13%of the overall global electricity demand by ***,reducing the total carbon emissions of the ICT sector plays a vital factor in achieving the Saudi plan to minimize global carbon ***,this paper aims to propose an eco-friendly approach using a Mixed-Integer Linear Programming(MILP)model to reduce the carbon emissions associated with ICT infrastructure in Saudi *** approach considers the Saudi National Fiber Network(SNFN)as the backbone of Saudi Internet ***,we compare two different scenarios of data center *** first scenario considers a traditional cloud data center located in Jeddah and Riyadh,whereas the second scenario considers NEOM as a potential cloud data center new location to take advantage of its green energy ***,we calculate the energy consumption and carbon emissions of cloud data centers and their associated energy *** that,we optimize the energy efficiency of different cloud data centers’locations(in the SNFN)to reduce the associated carbon emissions and energy *** results show that the proposed approach can save up to 94%of the carbon emissions and 62%of the energy cost compared to the current cloud physical *** savings are achieved due to the shifting of cloud data centers from cities that have conventional energy sources to a city that has rich in renewable energy ***,we
