Risk factors and prognosis of pediatric rhegmatogenous retinal detachment in Egypt:a university hospital based study

AIM:To study and compare the predisposing factors and clinical features of pediatric,adult,and elderly rhegmatogenous retinal detachment(RRD).METHODS:This is an observational analytic crosssectional study in which patients with RRD admitted for surgery during 6mo period were divided into 3 age groups:pediatric(60y).Patients’demographic data,clinical features,RRD predisposing factors/features including myopia(axial length≥26.5 mm),aphakia/pseudophakia,blunt trauma,peripheral retinal degenerations,history of RRD in the fellow eye,and surgical interventions/findings were recorded and ***:Totally 142 patients(142 eyes)were studied:26(18.31%)pediatrics,86(60.56%)adults,and 30(21.13%)*** patients had a significantly higher intraocular pressures and cataracts compared to the other 2 groups(P=0.04).The RRD extent was larger in pediatric group(mostly 4 quadrants)compared to adults and elderly(mostly 2 quadrants),but it was not statistically insignificant(P=0.242).There were not statistically significantly differences in proliferative vitreoretinopathy(PVR)rate,posterior vitreous detachment(PVD)rate,number,site,shape,and size of breaks in three *** three groups had macular detachment in all *** and peripheral retinal degenerations were found to be more significant in adults(P=0.049,P=0.035,respectively),while blunt trauma was higher but insignificant in pediatric eyes(P=0.052).Pars plana vitrectomy(PPV)with silicone oil as a tamponade was the most used surgery in all ***:There are no significant difference in PVR rate in pediatric eyes but a significant higher rate of total *** trauma is more frequent in pediatrics eyes while myopia and/or peripheral retinal degenerations are more frequent in older *** rate of PPV as a choice for surgery is similar among all age groups.

Does local vaginal estrogen after tension-free transobturator vaginal tape reduce overactive bladder symptoms in postmenopausal women? A prospective randomized, controlled study

目的:比较不同冲洗方法对微创预备后根管内氢氧化钙封药的清除效果,为临床治疗提供参考。方法:使用Tru Natomy镍钛系统对32个前磨牙进行微创根管预备后,向根管内封入氢氧化钙。根据不同冲洗方法将样本随机分为4组(n=8):被动超声荡洗(PUI)组、声波荡洗(EDDY)组、XP-endo finisher(XPF)组、手用K锉(K File)组。冲洗前后拍摄CBCT,并对残余氢氧化钙进行定量计算,比较不同冲洗方法对氢氧化钙的清除效果。结果:所有冲洗技术应用后根管内均存在氢氧化钙残留,使用PUI、XPF、EDDY和K File之间存在显著差异(P<0.05),PUI和XPF组的氢氧化钙残留量相对较少。结论:在微创预备后,使用PUI和XPF对根管内的氢氧化钙去除效果更好,但PUI的效率与XPF间无显著差异。

Cardiac catheterisation in nonagenarians： Single center experience

Objective To explore the treatment, procedure related risks, and outcomes of patients older than 90 years of age undergoing cardiac catheterization. Methods We retrospectively studied 32 patients ≥ 90 years （93.0 ± 1.2 years） who underwent cardiac catheterisation in a tertiary specialist hospital （0.2% of 14,892 procedures during three years）. The results were compared to a patient cohort younger than 90 years of age. Results Baseline characteristics revealed a higher prevalence of diabetes （P 〈 0.001）, chronic obstructive pulmonary disease （P 〈 0.04）, previous myocardial infarction （P 〈 0.02）, and complex coronary anatomy （SYNTAX score 33 vs. 19） in nonagenarians. Patients 〈 90 years of age showed more hyperlipidemia （P 〈 0.01） and previous percutaneous coronary interventions （P 〈 0.015）. Nonagenarians underwent coronary angiography more often for acute coronary syndrome （ACS）（P 〈 0.003）, were presented more often in cardiogenic shock （P 〈 0.003）, and were transferred faster to coronary angiography in cases ofACS （P 〈 0.0001）. The observed in-hospital mortality rate （13% study group vs. 1% control group; P 〈 0.003） in nonagenarians was lower than the calculated rate of thrombolysis in myocardial infarction （TIMI） and global registry of acute cardiac events （GRACE） mortality and strongly influenced by the severity of clinical presentation and the presence of co-morbidities. Conclusion Despite the common scepticism that cardiac catheterisation exposes patients 〉 90 years to an unwarranted risk, our data demonstrate an acceptable incidence of complications and mortality in this group of patients.

Microstructural Investigation of PM-355 Nuclear Track Detector Subjected to Low-Dose Gamma Irradiation: A Positron Annihilation Lifetime Study

Samples of the PM-355 polymeric solid state nuclear track detector were exposed to low gamma absorbed doses from 1 kGy (0.1 Mrad) up to 9 kGy (0.9 Mrad). Positron annihilation lifetime (PAL) in conjunction with transmission electron microscopy (TEM) and Thermo-gravimetric analysis (TGA) were performed on irradiated and pristine samples at room temperature. The observed lifetime spectra were resolved into three components where the ortho-positronium (o-Ps) lifetime component was associated with the pick-off annihilation of positronium trapped by the free volume. PAL studies of irradiated PM-355 samples showed that ortho-positronium (o-Ps) lifetime increases with an increase in dose up to 4 kGy and decreases at higher doses. In contrast, the intensity of the o-Ps component, I3, decreases with the dose up to about 2 kGy, followed by a much smooth decrease up to 7 kGy, and then it levels off. TGA analysis indicated that the PM-355 detector decomposed in one main breakdown stage. These results are discussed on the basis of chemical and physical changes occurring at the microscopic level in the PM-355 due to irradiation. Crosslinking dominates for doses between 1 and 4 kGy, while the degradation mechanism (Chain scission) prevails for doses up to 9 kGy.

Burden of pediatric hepatitis C

Effect of Annealing Temperature on Prepared Nanoparticles Li-Ferrite Using Positron Annihilation Lifetime Technique

Lithium ferrite nanoparticles were synthesized by a sol-gel auto-combustion method. For prepared samples, the nanograins were increased with increasing the annealing temperature. Positron annihilation lifetime spectroscopy (PALS) was used to study defects at different sites for nanograins Li-ferrites. The analysis of the PAL spectrum indicated two lifetime components τ1 and τ2 for the annihilation of the positrons, and their corresponding relative intensities I1% and I2%. For nanoparticles Li-ferrite there are correlations between: 1) I2, τ2, annealing temperature and the total porosity (Pt) with the grain size;2) I1, μi, Ms and the homogeneity with grain size.

Role of Fungal Biomass in N-Hexane Biofiltration

The biofiltration of n-hexane is studied to optimize determinants factors of hydrophobic VOC filtration efficiency. Four trickle-bed air biofilters (TBABs) were employed;two of which were supplied with nutrients buffered at a neutral pH, while another two at an acidic pH of 4 to induce and enhance fungal growth. The loading rate of n-hexane was kept constant in all TBABs at 13 g/m3/h. At each pH levels studied, the biomass of the TBABs was pre-acclimated using different ratios of n-hexane and methanol. The fungal biomass responsible for the degradation of n-hexane was then examined and quantified. Dichloran Rose Bengal Chloramphenicol agar was used for fungi quantification, and optical microscopy for classification. Effluent biomass was validated by measuring volatile suspended solids. Fungal counts resulting from n-hexane biodegradation were related to nitrate and carbon consumption. It was found that n-hexane elimination capacity closely followed biomass growth, and reached a steady-state at an optimum biomass density of roughly 3000 cfu/ml. Major shifts in fungal species were observed in all TBABs. Dominant fungal species grew slowly to become the most numerous, and were found to provide maximum elimination capacity, although TBABs pre-acclimated to higher methanol concentrations took less time to reach this steady-state. It was concluded, therefore, that steady and monitored growth of TBAB biomass is an essential factor in maximizing fungi’s ability to metabolize VOCs and that a new ecological biofiltration model may be the most effective at VOC purification.

Amoxicillin–clavulanic acid induced sperm abnormalities and histopathological changes in mice

Objective: To explore the genotoxic potential and histopathological changes induced in liver, kidney, testis, brain and heart after using the antibiotic drug amoxicillin/clavulanic acid(4:1).Methods: The study included chromosomal aberration analysis in bone-marrow and mouse spermatocytes, induction of sperm morphological abnormalities and histopathological changes in different body organs. The drug was administrated orally at a dose of81 mg/kg body weight twice daily(Total = 162 mg/kg/day) for various periods of time equivalent to 625 mg/men(twice daily).Results: The results revealed non-significant chromosomal aberrations induced after treatment with amoxicillin/clavulanic acid(AC) in both bone marrow and mouse spermatocytes after 7 and 10 days treatment. On the other hand, statistically significant percentages of sperm morphological abnormalities were recorded. Such percentage reached 8.10 ± 0.55, 9.86 ± 0.63 and 12.12 ± 0.58 at the three time intervals tested(7, 14 and 35 days after the 1 st treatment respectively)(treatment performed for 5 successive days) compared with 2.78 ± 0.48 for the control. The results also revealed histopathological changes in different body organs after AC treatment which increased with the prolongation of the period of therapy. Congestion of central vain, liver hemorrhage and hydropic changes in hepatocytes were noticed in the liver. Degenerative changes were found in kidney glomerulus and tubules while testis showed atrophy of seminiferous tubules, and reduction of spermatogenesis. AC also induced neurotoxicity and altered brain neurotransmitter levels. Hemorrhage in the myocardium, disruption of cardiac muscle fibers and pyknotic nuclei in cardiomyocytes were recorded as side effects of AC in heart ***: The results concluded that AC treatment induced sperm morphological abnormalities and histopathological changes in different body organs. Clinicians must be aware of such results while describing the drug.
