Repair and regeneration after spinal cord injury(SCI)is an unmet need and research into finding new avenues to achieve this are ongoing *** cells with their immense potential to proliferate and differentiate have been identified as prime candidates for spinal cord ***-clinical data on their efficacy has been very encouraging,especially in case of adult stem *** clinical translation of these findings has not been so successful,due to the ease of obtaining autologous adult bone marrow stem cells and their perceived therapeutic potential,these cells have been offered as a“therapy”in many countries without any scientific evidence generated though a valid clinical *** this review,we attempt to highlight the need for conducting clinical trials and the challenges faced in case of emerging *** current scenario of regulations governing the use of stem cells in emerging countries is also discussed.

Mortality following combined fractures of the hip and proximal humerus

BACKGROUND Hip fractures and proximal humerus fractures are known to be associated with increased mortality,but the impact on mortality of combining these two common injuries is not well *** To compare mortality,inpatient stay and discharge destination for patients with combined hip and proximal humerus fractures with those sustaining isolated hip *** Using the United Kingdom national hip fracture database,we identified all hip fracture patients over the age of 60 admitted to a single trauma unit from *** sustaining a proximal humerus fracture in addition to their hip fracture were identified using hospital coding *** calculated the 30-d and one-year mortality for both the hip fracture cohort and the combined hip and proximal humerus fracture *** variables recorded included age,gender and whether the proximal humerus was treated with or without an *** We identified 4131 patients with hip fractures within the study period and out of those 40 had sustained both a hip and a proximal humerus *** age in the hip fracture cohort was 80.9 years and in the combined fracture group 80.3 *** of the 40 patients in the combined group four were treated *** 30-d mortality for our hip fracture cohort was 7.2%compared to the mortality of our combined cohort of 12.5%(P=0.163).The one-year mortality for our hip fracture cohort was 26.4%compared to 40%for the combined fracture cohort(P=0.038).We also found patients with combined injuries were less likely to return to their own *** The 30-d and one-year mortality is higher for those patients who have sustained a combined hip and proximal humerus fracture when compared to those with a hip fracture alone.

Effect of vaginal speculum lubrication on cervical cytology and discomfort during smear examination

AIM: To evaluate the role of lubricant gel in the cytology of a Pap smear and whether it results in an improvement in the discomfort experienced by women while undergoing Pap smear ***: A total of 151 women were analyzed in the study. After screening for inclusion criteria, a Pap smear was taken with no lubricant in all the women and the discomfort experienced was rated on a visual analogue scale. The women underwent a second Pap smear on the next visit using a lubricant gel and were again rated on a visual analogue scale for the discomfort felt. The pathologist was blinded to the fact of whether the lubricating gel was ***: The number of unsatisfactory smears in the no gel group was 3 vs 5 in the gel group, P < 0.05. However, a significant difference(P = 0.00) was observed in the visual analogue pain score in both groups, suggesting that application of lubricant gel over the speculum improves the pain experienced by women. CONCLUSION: Using a small amount of lubricant overthe speculum does not impair cervical cytology but significantly improves the discomfort experienced by women while undergoing a Pap smear.

Current trends in laparoscopic groin hernia repair: A review

Hernia is a common problem of the modern world with its incidence more in developing countries. Inguinal hernia is the most common groin hernia repaired worldwide. With advancement in technology operative techniques of repair have also evolved. A Pub Med and COCHRANE database search was accomplished in this regard to establish the current status of laparoscopic inguinal hernia repair in view of recent published literature. Published literature support that laparoscopic hernia repair is best suited for recurrent and bilateral inguinal hernia although it may be offered for primary inguinal hernia if expertise is available.
