Clinical outcomes and risk factors for perforation in gastric endoscopic submucosal dissection: A prospective pilot study

AIM: To evaluate clinical outcomes and risk factors for endoscopic perforation during endoscopic submucosal dissection (ESD) in a prospective study. METHODS: We investigated the clinical outcomes and risk factors for the development of perforation in 98 consecutive gastric neoplasms undergoing ESD regarding. Demographic and clinical parameters including patient-, tumor-, and treatment-related factors, clinical parameters, and duration of hospital stay were analyzed for risk factors for perforation. In subgroup analysis, we also compared the clinical outcomes between perforation and "silent" free air without endoscopically visible perforation detected only by computed ***: Perforation was identified in 8.2% of patients. All patients were managed conservatively by the administration of antibiotics. The mean procedure time was significantly longer in patients with endoscopic perforation than in those without. According to the receiver-operating characteristic analysis, the resulting cutoff value of the procedure time for perforation was 115 min (87.5% sensitivity, 56.7% specificity). Prolonged procedure time (≥ 115 min) was associated with an increased risk of perforation (odds ratio 9.15; 95%CI: 1.08-77.54; P = 0.04). Following ESD, body temperature and C-reactive protein level were significantly higher in patients with perforation than in those without (P = 0.02), whereas there was no difference between these patient groups on the starting day of oral intake or of hospitalization. In subgroup analysis, the post-ESD clinical course was not different between endoscopic perforation and silent free air. CONCLUSION: Only prolonged procedure time (≥ 115 min) was significantly associated with perforation. The clinical outcomes of perforation are favorable and are comparable to those of patients with or without silent free air.

In this issue of World Journal of Gastroenterology,Huang et al reported that Calculus bovis(CB),a traditional Chinese herbal medicine,impedes the growth of liver cancers in *** further in vitro studies,they showed that CB suppressed the M2 polarization of tumor-associated macrophages by suppressing the Wnt signaling pathway,which consequently inhibited the growth of liver *** the effects of traditional Chinese herbal medicine are often not scientifically proven,Huang et al successfully identified the molecular mechanism involved in the anticancer effect of CB,and it is anticipated that the molecular mechanisms involved in the effects of other traditional Chinese herbal medicines will be scientifically elucidated,as demonstrated in this article.

Adjuvant chemotherapy for early-stage cervical cancer

The aim of this review is to address the current status of adjuvant chemotherapy alone in early-stage cervical cancer treatments in the literature. At present, the therapeutic effect of adjuvant chemotherapy alone after radical surgery （RS） has not yet been established, and radiation therapy （RT） or concurrent chemoradiotherapy （CCRT） is recommended as the standard adjuvant therapy after RS for early-stage cervical cancer in various guidelines. The main purpose of adjuvant therapy after RS, however, should be to reduce extrapelvic recurrence rather than local recurrence, although adjuvant RT or CCRT has survival benefits for patients with intermediate- or high-risk factors for recurrence. Moreover, several studies reported that adjuvant therapies including RT were associated with a higher incidence of complications, such as lymphedema, bowel obstruction and urinary disturbance, and a lower grade of long-term quality of life （QOL） or sexual functioning than adjuvant chemotherapy alone. The effect of adjuvant chemotherapy alone for early-stage cervical cancer with intermediate- or high-risk factors for recurrence were not fully investigated in prospective studies, but several retrospective studies suggest that the adjuvant effects of chemotherapy alone are at least similar to that of RT or CCRT in terms of recurrence rate, disease-free survival, or overall survival （OS） with lower incidence of complications. Whereas cisplatin based combination regimens were used in these studies, paclitaxel/cisplatin （TP） regimen, which is currently recognized as a standard chemotherapy regimen for patients with metastatic, recurrent or persistent cervical cancer by Gynecologic Oncology Group （GOG）, had also survival benefit as an adjuvant therapy. Therefore, it may be worth considering a prospective randomized controlled trial （RCT） of adjuvant chemotherapy alone using TP regimen versus adjuvant RT as an alternative adjuvant therapy. This study aims to draw basic and global concepts for the physical improvement of slum riverside settlements as a fundamental step toward formalizing and securing ***,this work specifies attempts to arrange an upgrading plan derived from the basic strategy for our target location in the traditional settlement of Kuin Utara without failing to recognize its uniqueness and cultural *** paper discloses that the basic concept for physical upgrading in a slum settlement consists of three elements:(1)arranging street networks as fundamental systems that structure the settlement,(2)constructing public utility systems that are essential for livelihoods,and(3)providing common space and amenities as"external organs”that form identities and reveal the physical beauty of the *** proposed infrastructures shall be directed to protect the local cultures and sociocultural activities of the *** plan should also be adjusted with respect to the economic constraints in developing ***,the improvement strategy for poor housings shall not orientate to modernization merely for beautification without considering the basic aspect:maintaining the dweller’s livelihood.

Influence of inhabitant background on the physical changes of Banjarese houses： A case study in Kuin Utara settlement, Banjarmasin, Indonesia

The disappearance of vernacutar buitdings is a common phenomenon worldwide and in Indonesia. The Banjarese house, a type of Indonesian vernacutar architecture, is a typicat riverside dwetling that shows the strong relationship between the river and the inhabitants. In view of the number of Banjarese houses facing degradation as a riverside type of vernacutar architecture, a study is necessary. The characteristics of the inhabitants must be considered when observing the condition of the remaining houses. Therefore, this study aims to investigate the influence of inhabitant background on the physicat changes of Banjarese houses for future revitatization. A research was conducted in the embryo of Banjarmasin atong the Kuin riverside setttement in Kuin Utara sub-district through a door-to-door survey with a questionnaire. The exploration indicates the following：（1） the original form of art remaining houses has been attered, （2） most of the inhabitants are ctassified as a tow-income society, （3） the different characteristics of the present inhabitants change the physicat condition of houses, （4） the retationship of the current house condition to the socio-cutture and economy of the inhabitants prays a prominent rote in revitatizing Banjarese houses as a vatuabte asset.

A comparative study on crash-influencing factors by facility types on urban expressway

This study aims at identifying crash-influencing factors by facility type of Nagoya Urban Expressway, considering the interaction of geometry, traffic flow, and ambient conditions. Crash rate （CR） model is firstly developed separately at four facility types： basic, merge, and diverge segments and sharp curve. Traffic flows are thereby categorized, and based on the traffic categories, the significances of factors affecting crashes are analyzed by principal component analysis. The results reveal that, the CR at merge segment is significantly higher than those at basic and diverge segments in uncongested flow, while the value is not significantly different at the three facility types in congested flow. In both un- and congested flows, sharp curve has the worst safety performance in view of its highest CR. Regarding influencing factors, geometric design and traffic flow are most significant in un- and congested flows, respectively. As mainline flow increases, the effect of merging ratio affecting crash is on the rise at basic and merge segments as opposed to the decreasing significance of diverging ratio at diverge segment. Mean- while, longer acceleration and deceleration lanes are adverse to safety in uncongested flow, while shorter acceleration and deceleration lanes are adverse in congested flow. Due to its special geometric design, crashes at sharp curve are highly associated with the large centrifugal force and heavy restricted visibility.

Nucleotide-binding oligomerization domain 1 and Helicobacter pylori infection: A review

Nucleotide-binding oligomerization domain 1(NOD1) is an intracellular innate immune sensor for small molecules derived from bacterial cell components. NOD1 activation by its ligands leads to robust production of pro-inflammatory cytokines and chemokines by innate immune cells, thereby mediating mucosal host defense systems against microbes. Chronic gastric infection due to Helicobacter pylori(H. pylori) causes various upper gastrointestinal diseases, including atrophic gastritis, peptic ulcers, and gastric cancer. It is now generally accepted that detection of H. pylori by NOD1 expressed in gastric epithelial cells plays an indispensable role in mucosal host defense systems against this organism. Recent studies have revealed the molecular mechanism by which NOD1 activation caused by H. pylori infection is involved in the development of chronic gastritis and gastric cancer. In this review, we have discussed and summarized how sensing of H. pylori by NOD1 mediates the prevention of chronic gastritis and gastric cancer.

Intragingival injection of Porphyromonas gingivalis-derived lipopolysaccharide induces a transient increase in gingival tumour necrosis factor-a, but not interleukin-6,in anaesthetised rats

This study used in vivo microdialysis to examine the effects of intragingival application of lipopolysaccharide（LPS） derived from Porphyromonas gingivalis（Pg-LPS） on gingival tumour necrosis factor（TNF）-a and interleukin（IL）-6 levels in rats. A microdialysis probe with an injection needle attached to the surface of the dialysis membrane was implanted into the gingiva of the upper incisor. For comparison, the effects of LPS derived from Escherichia coli（Ec-LPS） on IL-6 and TNF-a levels were also analysed. Pg-LPS（1 mg/1 m L） or Ec-LPS（1 or 6 mg/1 m L） was applied by microsyringe, with gingival dialysates collected every hour. Enzyme-linked immunosorbent assay（ELISA） revealed that gingival dialysates contained approximately 389 pg？m L21 of IL-6 basally; basal TNF-a levels were lower than the detection limit of the ELISA. Pg-LPS failed to alter IL-6 levels but markedly increased TNF-a levels, which remained elevated for 2 h after treatment. Neither IL-6 nor TNF-a were affected by Ec-LPS. Reverse transcriptase-polymerase chain reaction（RT-PCR） analysis revealed that the gingiva expresses Toll-like receptor（TLR） 2 and TLR4 m RNA. Immunohistochemical examination showed that TLR2 and TLR4 are expressed by gingival epithelial cells. The present study provides in vivo evidence that locally applied Pg-LPS, but not Ec-LPS, into the gingiva transiently increases gingival TNF-a without affecting IL-6. The present results suggest that TLR2 but not TLR4 expressed on gingival epithelial cells may mediate the Pg-LPS-induced increase in gingival TNF-a in rats.

Assessing mixing uniformity in microreactors via in-line spectroscopy

Mixing behavior is critical for enhancing the selectivity of fast chemical reactions in microreactors.A high Reynolds number(Re)improves the mixing rate and selectivity of the reactions,but some exceptions of increasing side product yield with the higher Re have been *** study investigated the mixing uniformity in microreactors with in-line UV-vis spectroscopy to clarify the relationship between reaction selectivity and chaotic mixing with the higher Re.A colorization experiment of thymolphthalein in an acidic solution was conducted with an excess acid amount to the base to indicate a non-uniformly mixed ***-uniformity significantly increased with *** the same time,the degree of mixing,which was measured by a usual decolorization experiment,showed that the mixing rate increased with *** in-line analysis of the Villermaux-Dushman reaction during the mixing clarified that side product yield significantly increased with Re at around 300 and then decreased at around *** results suggest the compensation effect between the mixing uniformity and mixing rate on the selectivity of the mixing-sensitive *** mixing,characterized by a larger Re,can disturb mixing uniformity and,in some cases,decrease reaction selectivity.
