Laparoscopic vs open distal pancreatectomy for solid pseudopapillary tumor of the pancreas

AIM:To compare short-and long-term outcomes of laparoscopic vs open distal pancreatectomy for solid pseudopapillary tumor(SPT)of the ***:This retrospective study included 28 patients who underwent distal pancreatectomy for SPT of the pancreas between 1998 and *** patients were divided into two groups based on the surgical approach:the laparoscopic surgery group and the open surgery *** patients’demographic data,operative results,pathological reports,hospital courses,morbidity and mortality,and follow-up data were compared between the two ***:Fifteen patients with SPT of the pancreas underwent laparoscopic distal pancreatectomy(LDP),and 13 underwent open distal pancreatectomy(ODP).Baseline characteristics were similar between the two groups except for a female predominance in the LDP group(100.0%vs 69.2%,P=0.035).Mortality,morbidity(33.3%vs 38.5%,P=1.000),pancreatic fistula rates(26.7%vs 30.8%,P=0.728),and reoperation rates(0.0%vs 7.7%,P=0.464)were similar in the two *** were no significant differences in the operating time(171 min vs 178 min,P=0.755)between the two *** intraoperative blood loss(149 mL vs 580 mL,P=0.002),transfusion requirement(6.7%vs 46.2%,P=0.029),first flatus time(1.9d vs 3.5 d,P=0.000),diet start time(2.3 d vs 4.9 d,P=0.000),and postoperative hospital stay(8.1 d vs 12.8d,P=0.029)were significantly less in the LDP group than in the ODP *** patients had negative surgical margins at final *** were no significant differences in number of lymph nodes harvested(4.6 vs6.4,P=0.549)between the two *** median follow-up was 33(3-100)mo for the LDP group and 45(17-127)mo for the ODP *** patients were alive with one ***:LDP for SPT has short-term benefits compared with ***-term outcomes of LDP are similar to those of ODP.

Gastric adenosquamous carcinoma(ASC)is a rare type of gastric *** is a mixed neoplasm,consisting of glandular cells and squamous *** is often diagnosed at an advanced stage,thus carrying a poor *** describe a case of a 73-year-old male,who presented with refractory fever and an intra-abdominal mass on *** underwent a laparoscopic exploration followed by a successful totally laparoscopic total gastrectomy with D2 lymphadenectomy for gastric *** pathology revealed primary gastric ASC(T4aN0M0).The patient received adjuvant radiotherapy and chemotherapy with S1 and is alive 20mo after surgery without *** is the first case of advanced gastric ASC with fever as the initial presentation treated with totally laparoscopic total gastrectomy reported in the English literature.

目的：筛选胰腺导管内乳头状粘液瘤发生恶变的相关预测因素。方法：回顾性分析在邵逸夫医院自2002年到2014年所有诊断为胰腺导管内乳头状粘液瘤患者的资料。根据文献及临床经验,设立26个相关因素,通过单因素分析、Logist回归分析及ROC分析,筛选术前可以预测胰腺导管内乳头状粘液瘤恶变的相关因素。结果：术前AST (P<0.004)和ALT (P<0.002)水平,主胰管直径≥5mm(P<0.002),囊肿≥3cm(P<0.033),CT增强的固体成分(P<0.026)和主胰管直径突然变化与远端胰腺萎缩(P<0.015)与胰腺导管内乳头状粘液瘤恶变有相关性。饮酒量(P<0.046),术前AST (P<0.031)和ALT (P<0.043)水平,主胰管直径≥5mm(P<0.015),囊肿大小≥3cm(P<0.019),增强的固体成分(P<0.033),主胰管直径突然变化与远端胰腺萎缩(P<0.046)和胰腺导管内乳头状粘液瘤具有侵袭性有相关性。主胰管直径可能是预测胰腺导管内乳头状粘液瘤恶变和侵袭的关键因素[0.791(P<0.002,95%CI:0.668-0.913) and0.772(P<0.010,95%CI:0.635-0.909).结论：囊肿≥3cm,主胰管≥5mm,增强的固体成分,主胰管突然变化与远端胰腺萎缩以及术前ALT和AST水平与胰腺导管内乳头状粘液瘤恶变相关,其中主胰管直径是最重要的因素。饮酒史与胰腺导管内乳头状粘液瘤侵袭性相关。

Resection of a cholangiocarcinoma via laparoscopic hepatopancreato- duodenectomy:A case report

Some laterally advanced cholangiocarcinomas behave as ductal spread or local invasion, and hepatopancreatoduodenectomy (HPD) may be performed for R0 resection. To date, there have been no reports of laparoscopic HPD (LHPD) in the English literature. We report the first case of LHPD for the resection of a Bismuth IIIa cholangiocarcinoma invading the duodenum. The patient underwent laparoscopic pancreaticoduodenectomy and right hemihepatectomy. Child’s approach was used for the reconstruction. The patient recovered well with bile leakage from the 2nd postoperative day and was discharged on the 16th postoperative day with a drainage tube in place which was removed 2 wk after discharge. Postoperative pathology revealed a well-differentiated cholangiocarcinoma and the margin of liver parenchyma, pancreas and stomach was negative for metastases. The results suggest that LHPD is a feasible and safe procedure when performed in highly specialized centers and in suitable patients with cholangiocarcinoma.

With the rapid integration of distributed energy resources(DERs),distribution utilities are faced with new and unprecedented *** challenges introduced by high penetra-tion of DERs range from poor observability to overload and reverse power flow problems,under-/over-voltages,maloperation of legacy protection systems,and requirements for new planning *** utility personnel are not adequately trained,and legacy control centers are not properly equipped to cope with these ***,distribution energy resource management systems(DERMSs)are emerging software technologies aimed to provide distribution system operators(DSOs)with a specialized set of tools to enable them to overcome the issues caused by DERs and to maximize the benefits of the presence of high penetration of these novel ***,as DERMS technology is still emerging,its definition is vague and can refer to very different levels of software hierarchies,spanning from decentralized virtual power plants to DER aggregators and fully centralized enterprise systems(called utility DERMS).Although they are all frequently simply called DERIMS,these software technologies have different sets of tools and aim to provide different services to different *** paper explores how these different software technologies can complement each other,and how they can provide significant benefits to DSOs in enabling them to successfully manage evolving distribution networks with high penetration of DERs when they are integrated together into the control centers of distribution utilities.

Film forming systems for topical and transdermal drug delivery

Skin is considered as an important route of administration of drugs for both local and systemic effects. The effectiveness of topical therapy depends on the physicochemical properties of the drug and adherence of the patient to the treatment regimen as well as the system’s ability to adhere to skin during the therapy so as to promote drug penetration through the skin *** formulations for topical and dermatological administration of drugs have certain limitations like poor adherence to skin, poor permeability and compromised patient *** the treatment of diseases of body tissues and wounds, the drug has to be maintained at the site of treatment for an effective period of time. Topical film forming systems are such developing drug delivery systems meant for topical application to the skin, which adhere to the body, forming a thin transparent film and provide delivery of the active ingredients to the body tissue. These are intended for skin application as emollient or protective and for local action or transdermal penetration of medicament for systemic action. The transparency is an appreciable feature of this polymeric system which greatly influences the patient *** the current discussion, the film forming systems are described as a promising choice for topical and transdermal drug delivery. Further the various types of film forming systems (sprays/solutions, gels and emulsions) along with their evaluation parameters have also been reviewed.

Symmetry analysis,closed-form invariant solutions and dynamical wave structures of the generalized (3+1)-dimensional breaking soliton equation using optimal system of Lie subalgebra

Nonlinear evolution equations(NLEEs)are primarily relevant to nonlinear complex physical systems in a wide range of fields,including ocean physics,plasma physics,chemical physics,optical fibers,fluid dy-namics,biology physics,solid-state physics,and marine *** paper investigates the Lie sym-metry analysis of a generalized(3+1)-dimensional breaking soliton equation depending on five nonzero real *** derive the Lie infinitesimal generators,one-dimensional optimal system,and geo-metric vector fields via the Lie symmetry ***,using the three stages of symmetry reductions,we converted the generalized breaking soliton(GBS)equation into various nonlinear ordinary differential equations(NLODEs),which have the advantage of yielding a large number of exact closed-form *** established closed-form wave solutions include special functional parameter solutions,as well as hyperbolic trigonometric function solutions,trigonometric function solutions,dark-bright solitons,bell-shaped profiles,periodic oscillating wave profiles,combo solitons,singular solitons,wave-wave interac-tion profiles,and various dynamical wave structures,which we present for the first time in this ***,the dynamical analysis of some established solutions is revealed through three-dimensional sketches via numerical *** of the new solutions are often useful and helpful for study-ing the nonlinear wave propagation and wave-wave interactions of shallow water waves in many new high-dimensional nonlinear evolution equations.
