Esophageal perforation, a rare condition, can arise from iatrogenic, traumatic, or spontaneous origins. Even when therapy is initiated within the first 24 hours, it is associated with a mortality rate of up to 25%. Due to the varied initial presentation, treatment may be delayed, leading to poorer outcomes. Here, we present a unique case of a 27-year-old schizophrenic patient who initially presented with acute respiratory failure and septic shock and was ultimately diagnosed with cavitary pneumonia secondary to esophageal perforation.

Objective: To develop a qNMR method for quantitative analysis of triacylglycerols in fruit oil of Hippophae rhamnoides(seabuckthorn, SBT) and analyze commercial samples of SBT oils using GC–MS and ***: SBT fruit oil(IPHRFH) was extracted with hexane and the triglyceride(TAG) was isolated by vacuum liquid chromatography. Six different branded SBT oils purchased from e-commerce suppliers(Amazon) and in-house prepared SBT oil was analyzed by qNMR and fatty acyl composition of TAGs determined by using NMR. In-house oil was also analysed by GC–MS and FTIR ***: The qNMR results showed that the oil contained 80.3% of triacylglycerol(TAG). The SBT oil TAGs comprised of linolenate 6.6%, palmitoleate/oleate 65.4%, and total saturated fatty acyl chain including palmitate 28% as determined by qNMR. GC–MS analysis revealed that the major acyl functionalities present in the TAG were palmitoleic acid 36.5%, oleic acid 12.9%, palmitic acid 21.2%, and linoleic acid 18%. Of the six commercial samples analyzed, samples from only one supplier(SW) were fruit oil;All others were the seed oils or mix of fruit oil and seed oil. The labels for samples except for the SW did not indicate whether it was fruit oil or seed ***: The results suggest that SBT oil should be analyzed by combination of GC–MS, FTIR and qNMR for factual content of free fatty acid or TAGs, which are chemically different in nature and affect the quality of oil. GC–MS showed the content of omega free fatty acids after hydrolysis, while qNMR and FTIR showed the content of TAGs. The major acyl functionalities found in SBT fruit oil TAGs are palmitoleate/palmitate/oleate, while linoleate and linonelate make up a minor fraction. Furthermore,analysis of commercial samples showed discrepancies between label claims and actual content.

Robots in the medical industry are becoming more common in daily life because of various advantages such as quick response,less human interference,high dependability,improved hygiene,and reduced aging *** is why,in recent years,robotic aid has emerged as a blossoming solution to many challenges in the medical *** this manuscript,meta-heuristics(MH)algorithms,specifically the Firefly Algorithm(FF)and Genetic Algorithm(GA),are applied to tune PID controller constraints such as Proportional gain Kp Integral gain Ki and Derivative gain *** controller is used to control Mobile Robot System(MRS)at the required set *** FF arrangements are made based on various pre-analysis.A detailed simulation study indicates that the proposed PID controller tuned with Firefly Algorithm(FF-PID)for MRSis beneficial and suitable to achieve desired closed-loop system *** FF is touted as providing an easy,reliable,and efficient tuning technique for PID *** most suitable ideal performance is accomplished with FF-PID,according to the display in the time ***,the observed response is compared to those received by applying GA and conventional off-line tuning *** comparison of all tuning methods exhibits supremacy of FF-PID tuning of the given nonlinear Mobile Robot System than GA-PID tuning and conventional controller.

Differential Sensitivity of Macrocarpa and Microcarpa Types of Chickpea （Cicer arietinum L.） to Water Stress： Association of Contrasting Stress Response with Oxidative Injury

Chickpea （Cicer arietinum L.） Is particularly sensitive to water stress at Its reproductive phase and, under conditions of water stress, will abort flowers and pods, thus reducing yield potential. There are two types of chickpea：（i） Macrocarpa （＂Kabull＂）, which has large, rams head-shaped, light brown seeds; and （11） Microcarpa （＂Desl＂）, which has small, angular and dark-brown seeds. Relatively speaking, ＂Kabull＂ has been reported to be more sensitive to water stress than ＂Desl＂. The underlying mechanisms associated with contrasting sensitivity to water stress at the metabolic level are not well understood. We hypothesized that one of the reasons for contrasting water stress sensitivity In the two types of chickpea may be a variation in oxidative injury. In the present study, plants of both types were water stressed at the reproductive stage for 14 d. As a result of the stress, the ＂Kabull＂ type exhibited an 80% reduction in seed yield over control compared with a 64% reduction observed for the ＂Desi＂ type. The decrease in leaf water potential （ψw） was faster In the ＂Kabull＂ compared with the ＂Desl＂ type. At the end of the water stress period, ψw was reduced to -2.9 and -3.1 MPa In the ＂Desi＂ and ＂Kabuli＂ types, respectively, without any significant difference between them. On the last day of stress, ＂Kabuli＂ experienced 20% more membrane injury than ＂Desi＂. The chlorophyll content and photosynthetic rate were significantly greater in ＂Desi＂ compared with ＂Kabuli＂. The malondialdehyde and H202 content were markedly higher at the end of the water stress In ＂Kabull＂ compared with ＂Desl＂, Indicating greater oxidative stress In the former. Levels of anti-oxidants, such as ascorblc acid and glutathlone, were significantly higher In ＂Desl＂ than ＂Kabull＂. Appropriate oral hygiene significantly reduces the possibility of oral infections. However， dental caries and periodontal diseases are major oral health issues causing chronic diseases due to poor oral health. Recently， herbal compounds have gained interest in maintaining oral health. Extracts of burdock root (Arctium)， noni fruit (Morinda citrifolia)， and neem leaf (Azadirachta indica) are now used as intracanal medicaments in endodontics and periodontics. Plectranthus amboinicus species and other plants produces essential oil like β-caryophyllene， p-cymene， and γ-terpinenecan exhibiting antibacterial activity， highlighting phytoconstituents plays a vital role in oral health. The COVID-19 pandemic highlighted the importance of hygiene and sanitization， to curb SARS-CoV-2. Oral cavity is among the gateways for virus entry into saliva. Saliva is a potential reservoir of SARS-CoV-2， and there is an increased risk of infection if there is any fissure in the mouth. This enables entry of virus into the vascular system through gingival or periodontal pocket， possibly reaching lung periphery then to lung vessels by interacting with endothelial surface receptors triggering pulmonary vasoconstriction and lung damage due to endothelial dysfunction. This review aims to draw attention to the possible route of SARS-CoV-2 infection via the oral cavity and the importance of oral hygiene against COVID-19.

AIM: To determine the causative agents of fungal keratitis and study the predisposing factors over a period of ten years in a single tertiary care hospital. ·METHODS: A retrospective analysis of fungal corneal ulcers was done from 2003-2012. Patients’ clinical data were noted from the file records. Correlation of histopathological diagnosis was done with the report on fungal culture. · RESULTS: Mycotic keratitis was established in 44 cases by a positive fungal culture. Direct microscopic examination of potassium hydroxide(KOH) mounts revealed fungal elements in 39 cases while 40 cases showed fungus on Gram stained smears. Males(54.55%) were more commonly affected than the females(45.45%). The age ranged from 18 to 82 years. Most common age group to be involved was 41-60 years. Predisposing risk factors were seen in 34(77.27%) cases. Most common findings on clinical examination were anterior chamber reaction and conjunctival injection seen in all the cases. Other common findings were stromal infiltration and hypopyon seen in 20(45.45%) and 18(40.91%) cases respectively. On histopathological examination the fungus was typed,as aspergillus in 34 cases while no definite typing was possible in 10 cases. The predominant isolate was aspergillus flavus(59.09%) followed by fusarium(15.91%). Mixed fungal and bacterial infection was seen in 3(6.82%) cases. ·CONCLUSION: Although culture is the gold standard for definitive diagnosis of fungal keratitis,direct microscopic examination of corneal scrapings or histomorphological evaluation of biopsies allow a rapidpreliminary diagnosis. Early administration of antifungal treatment helps in preventing dreadful complications.

Secure Spread-Spectrum Watermarking for Telemedicine Applications

This paper presents a secure spread-spectrum watermarking algorithm for digital images in discrete wavelet transform (DWT) domain. The algorithm is applied for embedding watermarks like patient identification /source identification or doctors signature in binary image format into host digital radiological image for potential telemedicine applications. Performance of the algorithm is analysed by varying the gain factor, subband decomposition levels, size of watermark, wavelet filters and medical image modalities. Simulation results show that the proposed method achieves higher security and robustness against various attacks.
