背景与目的我们开展了一项帕博利珠单抗用于伴未治疗脑转移的非小细胞肺癌(non-small cell lung cancer,NSCLC)或黑色素瘤患者的疗效和安全性的II期试验,旨在评估程序性死亡受体1(programmed cell death 1,PD-1)抑制剂在中枢神经系统(central nervous system,CNS)中的疗效。中期结果已发表,现报道对NSCLC队列的更新分析结果。方法这是一项开放性、单中心、II期试验。纳入标准:年龄≥18岁,诊断为晚期NSCLC并伴有≥1个5 mm-20 mm脑转移病灶,既往从未治疗或之前放疗后进展,无神经系统症状,不需要激素治疗且美国东部肿瘤协作组(Eastern Cooperative Oncology Group,ECOG)mg/kg)治疗。队列1为程序性死亡配体1(programmed cell death ligand 1,PD-L1)≥1%的患者,队列2为PD-L1<1%或未评估的患者。主要终点是脑转移患者缓解比例。所有经治患者均纳入疗效与安全性终点的分析。该研究已结束入组,并于***登记注册,注册号为NCT02085070。结果2014年3月31日-2018年5月21日,共42例患者接受治疗。中位随访时间为8.3个月(IQR:4.5个月-26.2个月)。队列1的37例患者中11例有脑转移缓解[29.7%(95%CI:15.9%-47.0%)]。队列2未观察到缓解。治疗相关的3级-4级不良事件(adverse events,AEs)包括2例肺炎、1例全身症状、1例结肠炎、1例肾上腺皮质功能不全、1例高血糖症和1例低钾血症。6例(14%)患者发生了治疗相关的严重不良事件,包括肺炎、急性肾损伤、低钾血症和肾上腺皮质功能不全。没有观察到治疗相关死亡病例。结论帕博利珠单抗治疗PD-L1≥1%的NSCLC伴脑转移患者有效,且对所有纳入的未经治疗的脑转移患者安全。需要进一步探索免疫治疗用于NSCLC合并CNS转移。

Pancreatic surgery has been one of the last areas for the application of minimally invasive surgery(mIS)because there are many factors that make laparoscopic pancreas resections *** concept of service centralization has also limited expertise to a small cadre of high-volume centres in resource rich ***,this is not the environment that many surgeons in developing countries work *** patients often do not have the opportunity to travel to high volume centres for ***,we sought to review the existing data on mIS for the pancreas and to *** this paper,we review the evolution of mIS on the pancreas and discuss the incorporation of this service into low-volume and resource-poor countries,such as those in the *** paper has two ***,we performed a literature review evaluating all studies published on laparoscopic and robotic surgery of the *** data in the Caribbean is examined and we discuss tips for incorporating this operation into resource poor hospital *** pancreatic case volume in the Caribbean,and financial barriers to mIS in general,laparoscopic distal pancreatectomy,enucleation and cystogastrostomy are feasible operations to integrate in to a resource-limited healthcare *** is because they can be performed with minimal to no consumables and require an intermediate mIS skillset to complement an open pancreatic surgeon’s peri-operative experience.

背景：随机对照试验证实，他汀类降胆固醇药物能够预防心脏事件的发生。近来的回顾性分析提示，他汀类药物还可预防癌症。目的：观测他汀类药物治疗对癌症发生和癌症死亡的影响，分析他汀类药物对不同类型癌症的作用，探讨他汀亲脂性或其来源的作用。数据来源：使用特定的检索词对2005年7月以前的mEDLINE、EmBASE、CLINAHL、Web of Science、CANCERLIT和Cochrane系统综述数据库进行系统文献检索。审查心脏病和癌症摘要，对论文参考文献亦进行人工审查。研究选择：在筛查的8943篇（n=86936）文献中，27篇符合入选标准。27篇论文共报道了26项他汀类药物随机对照试验。这些研究至少平均随访1年，每项研究至少入选100例患者。研究提供了癌症发生（n=20项研究）或癌症死亡（n=22项研究）数据。资料提取：3位研究者使用标准的资料提取工具独立地提取所有资料。应用随机效应模型（DerSimonian和Laird法），加权均值报告为优势比（odds ratios，ORs）和95％可信区间（confidence intervals，CIs）。应用Q统计评估统计学异质性。资料综合：汇总分析共包括6662例癌症和2407例癌症死亡。结果显示，他汀类药物并未降低癌症的发生（OR，1．02；95％CI，0．97～1．07）或癌症死亡（OR，1．01；95％CI，0．93～1．09），也未见任何类型癌症的减少。无论是亲水性他汀或亲脂性他汀、天然他汀或人工合成他汀都没有影响癌症的发生。结论：在随机对照试验中，他汀类药物对癌症发生和癌症死亡的危险呈中性效应。我们发现，癌症类型不受他汀类药物使用与否的影响，不同他汀类药物亚型（亲水或亲脂性、天然或人工合成）对癌症的危险亦无影响。
