Monteggia fracture dislocation equivalents analysis of eighteen cases treated by open reduction and internal fixation

Abstract：Objective: Monteggia fracture dislocation equivalent, though already described by Bado, is still an unclassified entity. We aimed to retrospectively analyze 18 cases of Monteggia variants and discuss the injury mechanisms, management, and outcome along with a re view of the literature. Methods: A retrospective record of Monteggia fracture dislocation (2003-2008) was reviewed from medical record department of our institute. Classic Monteggia fracture dislocation, children below 12 years or adults over 50 years, as well as open grade Ⅱ & Ⅲ cases were excluded from this study. Monteggia variant inclusion criteria included fracture of the proximal ulna together with a fracture of the radial head or neck and skeletal maturity. Totally 26 patients were identified with Monteggia variants and 18 were available for follow-up, including 11 males and 7 females with the mean age of 35 years. The ulna fracture was treated by compression plating along with tension band wiring. Radial head/neck was reconstructed in 12 patients while excised in 6 patients. Results: Follow-up ranged from 1 -4 years, mean 2.6 years. Patients were assessed clinicoradiologically. Mayo Elbow Performance Score was employed to assess the outcomes. At final follow-up, the results were excellent in 10 patients, good in 4, fair in 2 and poor in 2. Mean range of motion of the elbow was 20°, 116°, 50° and 55° for extension, flexion, pronation and supination, respectively. Two patients had complications in the form of heterotopic ossification and stiffness of the elbow. One nonunion ulna, primarily treated by tension band wiring, was managed by refixation with locking reconstruction plate and bone grafting. Bone grafting was only required in this patient for nonunion. Another patient had implants removed on his request. Surface Wave Propagation in a Generalized Thermoelastic Material with Voids

In the present paper, the propagation of surface wave in a generalized thermoelastic solid with voids is considered. The governing equations are solved to obtain the general solution in x-z plane. The appropriate boundary conditions at an interface between two dissimilar half-spaces are satisfied by appropriate particular solutions to obtain the frequency equation of the surface wave in the medium. Some special cases are also discussed.

Reduced incidence and mortality from colorectal cancer with flexible-sigmoidoscopy screening: A meta-analysis

AIM:To conduct a systematic review and meta-analysis of published population-based randomized controlled trials(RCTs).METHODS:RCTs evaluating the difference in mortality and incidence of colorectal cancer(CRC)between a screening flexible sigmoidoscopy(FS)group and control group(not assigned to screening FS)with a minimum 5 years median follow-up were identified by a search of MEDLINE and EMBASE databases and the Cochrane Central Register for Controlled Trials through August *** effects model was used for ***:Four RCTs with a total of 165659 patientsin the FS group and 249707 patients in the control group were included in ***-totreat analysis showed that there was a 22%risk reduction in total incidence of CRC(RR=0.78,95%CI:0.74-0.83),31%in distal CRC incidence(RR=0.69,95%CI:0.63-0.75),and 9%in proximal CRC incidence(RR=0.91,95%CI:0.83-0.99).Those who underwent screening FS were 18%less likely to be diagnosed with advanced CRC(OR=0.82,95%CI:0.71-0.94).There was a 28%risk reduction in overall CRC mortality(RR=0.72,95%CI:0.65-0.80)and 43%in distal CRC mortality(RR=0.57,95%CI:0.45-0.72).CONCLUSION:This meta-analysis suggests that screening FS can reduce the incidence of proximal and distal CRC and mortality from distal CRC along with reduction in diagnosis of advanced CRC.

Searching systematics for nonfactorizable contributions to B^(-)and B^(0) hadronic decays

The two-body weak decays B→πD/B→ρD and B→πD are examined using isospin analysis to study nonfactorizable *** determining strong interaction phases and obtaining factorizable contributions from spectator-quark diagrams for Nc=3,we determine nonfactorizable isospin amplitudes from the experimental data for these *** results support the universality of the ratio of nonfactorizable isospin reduced amplitudes for these decays within experimental *** demonstrate that these systematics are not coincidental,we also plot our results *** ratio.

Comparative study of the results of compound tibial shaft fractures treated by Ilizarov ring fixators and limb reconstruction system fixators

Purpose： llizarov ring fixator and limb reconstruction system （LRS） fixators have been used in the management of complex tibial fractures with severe soft tissue injuries, compound tibial fractures, and infected tibial nonunion for which conventional internal fixation cannot be contemplated. Fracture union and distraction osteogenesis can be done simultaneously with these external fixators, allowing early weight bearing. Several previous studies have shown almost equal results of rail and ring fixators for the compound tibial shaft fractures. Thus we performed a prospective study to evaluate the union rate, functional outcome and amount of limb lengthening after the treatment of compound tibial shaft fractures with or without infected nonunion by ring or LRS fixators. Methods： This prospective study was done at Sarojini Naidu Medical College and Hospital, Agra, India and included 32 patients of compound tibial shaft fractures with or without infected nonunion. There were 26 males and 6 females and the average age was 40 years. Patients were randomly divided into two groups （n - 16 for each）： one underwent llizarov fixation and the other received LRS fixation. Cases were followed up for 3-24 months, 6 months on average from September 2012 to October 2014. Functional and radiological outcomes were assessed using the Association for the Study and Application of Methods of llizarov （ASAMI） criteria for both rail and ring fixators. Results： Union was achieved in all cases. Radiological outcome was found excellent in 68.75%, good in 18.75% and fair in 12.50% of cases treated by rail fixators whereas it was excellent in 56.25%, good in 18.75%, fair in 12.50% and poor in 12.50% of cases treated by ring fixators. Functional result was satis- factory in 75.00% of cases treated by rail fixator and 68.75% of cases treated by ring fixators whereas the corresponding rate of unsatisfactory was 25.00% vs. 31.25%. Conclusion： In our short-term assessment, LRS fixators show a better

A Study of Determinants of Sensorineural Hearing Loss in Chronic Suppurative Otitis Media with or without Cholesteatoma

Aim: This study was carried out to assess association between Sensorineural hearing loss (SNHL) and Chronic Suppurative Otitis Media (CSOM), to identify possible risk factors and determine correlation of duration of CSOM and incidence of SNHL. Material and Method: The study consisted of 100 patients of either sex between 15 and 40 years of age, suffering from unilateral CSOM for the past three months or more, the normal ear served as control. Results: This study showed duration of symptoms is 18% patients for 5 - 10 years and below 5 years in 73% patients. It was found that mean BC thresholds were significantly higher in the diseased ears as compared to the control ears. Though the higher frequencies are more affected than the lower frequencies, in subtotal and attic perforations the speech frequencies seemed to be maximally affected. Conclusion: CSOM should be diagnosed early and should be managed effectively so as to prevent the chances of developing SNHL.

Farming methods impact on soil and water conservation efficiency under tea[Camellia sinensis(L.)]plantation in Nilgiris of South India

Growing of tea on sloping land without any soil and water conservation measures causes enormous soil loss especially in the initial *** sound soil and water conservation planning,there is a need to evaluate the various conservation measures as related to the amount of expected runoff and soil *** this context,a field study was conducted in the farmer's field in Nigiris of South India for evaluating the impact of farming methods on soil and water conservation eficiency under new tea plantation One year old B-6 tea clones were planted at double hedge spacing(135 cm×75 cm×75 cm)in two slopes(8-12%and 30-35%)with treatments viz,contour staggered trenches(CST),vegetative barrier(VB),CST alternate with VB,CST with cover crop of beans and farmers'practice of *** runoff(14.6%)was observed from CST with cover crop of beans followed by CST(15.4%)under 8-12%slope range with exactly similar trend in runoff from the plots under 30-35%*** to runof,minimum soil loss was observed from CST(4.9 and 6.9t ha^(-1)yr^(-1))fllowed by CST with cover crop of beans(5.3 and 73 t ha^(-1)yr^(-1))under 8-12%and 30-35%*** CST and CST with cover crop of beans are resulted in better soil moisture under both the slope ranges in comparison to remaining measures as well as farmers'practice of ***,either CST alone or in combination with cover crop of beans are recommended for soil and water conservation under new tea plantation in the hill slopes.

Malaria： Autophagy as a Potential Therapeutic Target

Malaria, where the causative agent Plasmodium is rapidly gaining resistance to practically all the antimalarial drugs in clinical use, requires the identification of new drug targets to enable the discovery of novel, more effective and safer drugs to treat the disease. Advancements in molecular techniques have provided the proof of existence of autophagy in Plasmodium; however, its role(s) in malaria is only becoming to be understood. Nevertheless, some of the recently explored dimensions of autophagy in Plasmodium have indicated its fairly larger role in parasite survival and growth. But then, autophagy is also essential for host cell survival and defence. There is thus need to explore chemotherapeutic strategies to specifically target autophagy in both the parasite and host. This review focuses on autophagy pathways in Plasmodium and in host with a view to identify autophagy-related new drug targets for the discovery of novel antimalarial drugs.

A New Model for the Etching Characteristics of Corners Formed by Si<sub>{111}</sub>Planes on Si<sub>{110}</sub>Wafer Surface

The etching characteristics of concave and convex corners formed in a microstructure by the intersection of {111} planes in wet anisotropic etchant are exactly opposite to each other. The convex corners are severely attacked by anisotropic Fetchant, while the concave corners remain unaffected. In this paper, we present a new model which explains the root cause of the initiation and advancement of undercutting phenomenon at convex corners and its absence at concave corners on {110} silicon wafers. This contrary etching characteristics of convex and concave corners is explained by utilizing the role of dangling bond in etching process and the etching behavior of the tangent plane at the convex corner. The silicon atoms at the convex edge/ridge belong to a high etch rate tangent plane as compared to {111} sidewalls, which leads to the initiation of undercutting at the convex corner. On the other hand, all the bonds of silicon atoms pertaining to concave edges/ridge are engaged with neighboring atoms and consequently contain no dangling bond, thus resulting in no-undercutting at concave edges/corners.

Anti-viral role of toll like receptor 4 in hepatitis B virus infection: An in vitro study

AIM Toll like receptors plays a significant anti-viral role in different infections. The aim of this study was to look into the role of toll like receptor 4(TLR4) in hepatitis B virus(HBV) *** Real time PCR was used to analyze the transcription of TLR4 signaling molecules, cell cycle regulators and HBV DNA viral load after triggering the Hep G2.2.15 cells with TLR4 specific ligand. Nuclear factor(NF)-κB translocation on TLR4 activation was analyzed using microscopic techniques. Protein and cell cycle analysis was done using Western Blot and FACS *** The present study shows that TLR4 activation represses HBV infection. As a result of HBV suppression, there are several changes in host factors which include partial release in G1/S cell cycle arrest and changes in host epigenetic marks. Finally, it was observed that anti-viral action of TLR4 takes place through the NF-κB *** The study shows that TLR4 activation in HBV infection brings about changes in hepatocyte microenvironment and can be used for developing a promising therapeutic target in future.
