During the past 30 years, there has been spectacular growth in the use of risk analysis and risk management tools developed by engineers in the financial and insurance sectors. The insurance, the reinsurance, and the investment banking sectors have enthusiastically adopted loss estimation tools developed by engineers in developing their business strategies and for managing their financial risks. As a result, insurance/reinsurance strategy has evolved as a major risk mitigation tool in managing catastrophe risk at the individual, corporate, and government level. This is particularly true in developed countries such as US, Western Europe, and Japan. Unfortunately, it has not received the needed attention in developing countries, where such a strategy for risk management is most needed. Fortunately, in the last five years, there has been excellent focus in developing ＂Insur Tech＂ tools to address the much needed ＂Insurance for the Masses＂, especially for the Asian Markets. In the earlier years of catastrophe model development, risk analysts were mainly concerned with risk reduction options through engineering strategies, and relatively little attention was given to financial and economic strategies. Such state-of-affairs still exists in many developing countries. The new developments in the science and technologies of loss estimation due to natural catastrophes have made it possible for financial sectors to model their business strategies such as peril and geographic diversification, premium calculations, reserve strategies, reinsurance contracts, and other underwriting tools. Surgically assisted mandibular arch expansion has proven to be an effective treatment modality in alleviating constriction and crowding issues. However, only mandibular symphyseal osteotomy has been in favor for the purpose of mandibular arch expansion. Besides, there is no relevant study to compare biomechanical response of different corticotomy designs on mandibular expansion. Therefore, this study is aimed to evaluate the effect of different corticotomy approaches and modes of loading on the expansion of adult mandible using *** address the research purpose, a total of 9 Finite Element models including two novel corticotomy designs were simulated. Stress, strain and displacement of crown, root and bone were calculated and compared under different corticotomy approaches and loading *** biomechanical response envisaged by the FE models in terms of displacement on X axis, was consistent from anterior to posterior teeth with parasymphyseal step corticotomy and tooth borne mode of force application. In addition the amount of displacement predicted by parasymphyseal step corticotomy with tooth borne mode was more, compared to other *** conclusion, the results of our study suggest, parasymphyseal step corticotomywith tooth borne mode to be a viable treatment option for true bony expansionin adult mandible.

Background and Aims: Distinguishing alcoholic steato-hepatitis (ASH) and nonalcoholic steatohepatitis (NASH) with biopsy alone is often difficult without a reliable clinical con-text. A novel finding on liver imaging, perivascular branching heterogeneity, has shown promise in distinguishing between these chronic liver diseases. Our study investigated the role of this finding on imaging to differentiate between ASH and NASH. The aim of this study was to determine the utility and reproducibility of this novel radiographic marker to help dis-tinguish ASH from NASH. Methods: This was a retrospective cohort study conducted between 2016 and 2020 in patients with both liver biopsy-confirmed steatohepatitis/chronic hep-atitis and abdominal magnetic resonance imaging within 13 months of each other. Two radiologists, blinded to patient clinical history and diagnosis, categorized the appearance of the liver as: 1- homogeneity, 2- mild heterogeneity, 3- mod-erate heterogeneity, 4- possible perivascular branching, 5- definite perivascular branching. Results: Of the 90 patients in the study, 60 were identified as NASH and 30 as ASH. The area under the curve (AUC) for both reader 1 and 2 when us-ing the 5-point scale was 0.69 (CI: 0.56-0.82, p=0.006) and 0.72 (CI: 0.60-0.85, p=0.001), respectively. The positive predictive value (PPV) for identification of ASH when scoring 5 was 64.7% and 66.7% for reader 1 and 2, respectively. In-terclass correlation coefficient was 0.74 in patients with ASH, indicating moderate reliability among both readers. Conclu-sions: Identification of this perivascular branching pattern on imaging is a promising novel diagnostic marker that can be used with other methods to help distinguish between ASH and NASH.

Despite several national programs to reduce infant mortality, India had repeatedly failed to achieve its set targets for infant mortality. There are approximately one million neonatal deaths in India each year which accounts for nearly two-thirds of the infant deaths in India. India’s current trajectories of neonatal and infant mortality rates make it unlikely that it will achieve its targets for infant mortality rate for 2015 set under the Millennium Development Goals. Since two-thirds of infant deaths in India are neonatal deaths, implementation of effective neonatal care strategies would be essential to reduce infant mortality considerably. The history of child health services in India suggests an inattention to qualitative parameters, hindering a reversal of its failures. We discuss a format of mixed-methods participatory research, integrated with routine district level household surveys (DLHS), as a model of health services research which would better delineate the problems encountered in delivering effective newborn care at the primary care level.

Primary pediatric mid-brain lymphoma:Report of a rare pediatric tumor in a rare location

Primary central nervous system lymphoma(PCNSL) is a rare disease in pediatric age group. A thirteen-yearold male child presented with complaints of headache for six months, vomiting and diplopia for three days. Magnetic resonance imaging of the brain showed a single lesion of 1.7 cm × 1.6 cm × 1.6 cm in the mid brain and tectum. He underwent a gross total resection of the tumor. The histopathological evaluation revealed B cell high grade non Hodgkin lymphoma. The patient was treated with High dose methotrexate and cranio spinal radiation. The patient was alive without disease 12 mo after completion of treatment. This case highlights importance of keeping PCNSL as differential in brain stem lesions of pediatric patients also. Radiation and chemotherapy remains the most important treatment for such patients.
