Obesity pandemic in China:epidemiology,burden,challenges,and opportunities

China faces a substantial burdensome pandemic of obesity.Recent data from the Chinese Centre for Disease Control and Prevention suggested that over an estimated 8.1%of Chinese adults(85 million Chinese adults)had obesity in 2018,which was three times the level in 2004.[1]To understand the trends of overweight and obesity in China,we accessed the country/territory-level year-specific data of age-standardized summary exposure value(SEV,standing for the pooled proportion among the population),deaths,and disability-adjusted life of years(DALYs)for high body mass index(BMI,high BMI was defined as the BMI level higher than theoretical minimum risk exposure level[25 kg/m2])and its associated diseases for 204 countries/territories between 1990 and 2019 using data from the Global Burden of Disease Collaborative Network.[2]Previous literature described detailed methods for the data synthesis.[3,4]Using the country/territory-level data by year,we calculated the estimated annual percentage changes(EAPCs)with 95%uncertainty intervals(UI)to estimate the annual changing patterns of age-standardized SEV,death,and DALY rates among 204 countries/territories.To compare statistics from China and other regions,we calculated the corresponding estimates from regions including global,North Africa and Middle East,Sub-Saharan Africa,Western Pacific Region,European Union,Latin America and Caribbean,and South Asia.

目的探讨原发性醛固酮增多症(原醛症)患者经肾上腺切除手术(ADX)或螺内酯治疗后的代谢转归及其影响因素。方法回顾性分析2018年3月–2020年10月经四川大学华西医院内分泌科确诊的70例醛固酮瘤(APA)和86例特发性醛固酮增多症(IHA)患者... 详细信息

目的探讨原发性醛固酮增多症(原醛症)患者经肾上腺切除手术(ADX)或螺内酯治疗后的代谢转归及其影响因素。方法回顾性分析2018年3月–2020年10月经四川大学华西医院内分泌科确诊的70例醛固酮瘤(APA)和86例特发性醛固酮增多症(IHA)患者的临床资料,APA组患者均进行ADX治疗,IHA患者均服用盐皮质激素受体拮抗剂(螺内酯)治疗,分析患者治疗后代谢指标的转归情况及组间差异。结果 APA组与IHA组患者治疗前年龄、性别、高血压病程、最高血压、最低血压、体质量指数(BMI)、血脂、空腹血糖、肾功能差异无统计学意义,IHA组患者的腰围、血钾水平和血浆肾素活性高于APA组(P均<0.05)。治疗后所有患者的血压、血钾及血浆醛固酮浓度显著改善,出现三酰甘油升高伴肾功能恶化(P≤0.001)。多因素回归显示三酰甘油水平与IHA组患者经螺内酯治疗、治疗后BMI及肌酐水平有关。APA组患者经肾上腺切除后空腹血糖改善(P=0.041),但IHA组患者经螺内酯治疗后空腹血糖升高(P=0.037)。结论原醛症患者治疗后可能仍然存在糖脂代谢异常及肾功能恶化,螺内酯治疗可能有更差的代谢结局。

围手术期血糖异常主要包括高血糖和低血糖。围手术期血糖异常可增加手术患者的病死率，增加感染、伤口不愈合以及心脑血管事件等并发症的发病率，延长术后住院时间，影响远期预后。与非糖尿病患者相比，糖尿病患者是围手术期血糖异常的高危人群。随着我国糖尿病患病率的激增以及平均住院日的缩短，良好的围手术期血糖管理越发有赖于当事科室及医生的参与和决策。以下将结合国内外临床指南和证据，为非内分泌专科医生进行围手术期血糖管理提供支持。

总结3例原发性色素沉着性结节性肾上腺皮质病(PPNAD)患者的临床特征及诊疗方案。PPNAD除符合促肾上腺皮质激素(ACTH)非依赖性库欣综合征特征外,有以下特点:(1)Liddle试验后尿游离皮质醇反常升高;(2)伴生长停滞或Carney综合征表现;(3)肾上腺为小结节改变;(4)基因检测分别为PRKACA、PRKAR1A突变;(5)病理均为肾上腺色素沉积的结节改变;(6)治疗最终需要行双侧肾上腺切除,并予终身激素替代。PPNAD是罕见的库欣综合征类型,临床不易识别,认识其特征并行基因检测对明确诊断和治疗具有重要意义。

了解槲皮素和依那普利对糖尿病大鼠尿蛋白排泄及肾组织的血小板衍生生长因子(PDGF-B)和血管内皮生长因子(VEGF-1)含量的影响。将29只糖尿病大鼠分成3组糖尿病对照组(D组,n=12);依那普利治疗组(E组,n=10);槲皮素治疗组(Q组,n=7)。另有一组正常对照组(N组,n=5)。分别于4、8、12周收集大鼠24h尿测定尿蛋白的量,并于12周处死大鼠并取出肾脏测定PDGF-B和VEGF-1的含量。糖尿病大鼠在8、12周尿蛋白的排泄明显高于正常对照组,E、Q组的尿蛋白量明显低于D组,E组和Q组无明显差异。糖尿病大鼠肾组织中PDGF-B和VEGF-1的含量明显高于正常对照组,E、Q组的PDGF-B和VEGF-1的含量低于D组,E组和Q组无明显差异。槲皮素和依那普利能降低糖尿病大鼠尿蛋白的排泄和肾组织中的PDGF-B和VEGF-1含量。提示槲皮素和依那普利对糖尿病大鼠具有保护作用,这种作用可能与抑制PDGF-B和VEGF-1的产生有关。
