The management of cervical lymph node metastases of squamous cell carcinoma from an unknown primary site is still a therapeutic *** report here our experience in treating these patients with chemoradiotherapy as a curative *** from 40 patients were *** total,20(50%) patients underwent excisional *** patients underwent radiotherapy,which was delivered to both sides of the neck and pharyngeal mucosa(extensive field),and concurrent chemotherapy consisting of weekly cisplatin at a dose of 40 mg/*** clinical stage of the cervical nodes at presentation was N1 in 25%,N2 in 60%,and N3 in 15%.Most patients(75%) developed at least grade 3 *** patients(20%) had grade 3 xerostomia and 18 patients(45%) required esophageal dilation for *** 5-year overall survival(OS) rate of all patients was 67.5%.The 5-year OS rates of patients with N1,N2,and N3 lesions were 100%,67%,and 41%,respectively(P = 0.046).The 5-year progression-free survival rate was 62.5%.In multivariate analysis,only N stage significantly affected OS(P = 0.022).Emergence of the occult primary was very limited(1 patient only).Our results suggest that extensive irradiation of both sides of the neck and pharyngeal mucosa with concurrent chemotherapy results in a lower emergence of primary *** the survival of patients with unknown primary is comparable to that of patients with known primary,an attempt at cure should always be made.

The Effect of the Surgical Margins on the Outcome of Patients with Head and Neck Squamous Cell Carcinoma:Single Institution Experience

Objective To assess the impact of close or positive surgical margins on the outcome, and to determine whether margin status influence the recurrence rate and the overall survival for patients with head and neck cancers. Methods Records from 1996 to 2001 of 413 patients with primary head and neck squamous cell carcinoma （SCC） treated with surgery as the first line treatment were analysed. Of these patients, 82 were eligible for the study. Patients were followed up for 5 years. Results Patients with margins between 5-10 mm had 50% recurrence rate （RR）, those with surgical margins between 1-5 mm had RR of 59% and those with positive surgical margins had RR of 90% （P=O.O04）. The 5-year survival rates were 54%, 39% and 10%, respectively （P=0.002）. Conclusions Unsatisfactory surgical margin is an independent risk factor for recurrence free survival as well as overall survival regardless of the other tumor and patient characteristics.

Objective:This is a retrospective study to assess the effectiveness of consolidation radiotherapy (CRT) following palliative chemotherapy in patients with metastatic or locally advanced non-small cell lung cancer (NSCLC) who are not suitable for radical ***:This study involved retrospective analysis of a prospective database of Northampton Oncology Centre from January 2005 to December 2010,63 patients with advanced/metastatic NSCLC treated at the oncology centre were *** were either treated with high dose (39/36 Gy /13-12 fractions,group 1) or low dose (20 Gy /5 fractions,group 2) CRT or those were not offered any CRT (group 3).Results:There was no significant difference between the three groups as regard age,sex,performance status,comorbidities or chemotherapy *** there was a statistically significant difference as regard the stage P=0.009 with more stage IV patients at group II and III compared to group *** mean survival for the three groups was 27 months,14 months &15 months,*** was a statistically significant improvement of survival in patients treated with high dose palliative CRT compared to the other two groups (P=0.006).In multivariate analysis only the radiotherapy dose remains as the only statistical significant factor affecting the survival with hazard ratio 0.372 and confidence interval (0.147-0.726).Conclusion:Despite the limitation of our retrospective study,it is worth considering CRT approach for patients with advanced and metastatic NSCLC-not suitable for radical treatment-who have not progressed on chemotherapy.

In this work, we analyze the existence of discontinuous bifurcation and stability issues in discontinuous flow of bioreactor and membrane reactor models with or without recycle. The reaction is assumed to be governed by certain types of discontinuities in Monod growth kinetics curve leading to discontinuous dynamical system. The criteria for the existence and stability of steady-states of these models are established. More generally, our analysis highlights the presence of several types of bifurcation depending upon the effect of the dilution factor (residence time), biomass concentration and solid-liquid-gas separator efficiency. As well, we present bifurcation conditions defining the dynamics near steady-state branches on the border, providing a possible framework for existing of saddle-node, nonsmooth fold, persistence and grazing-sliding scenarios. It is shown that the critical values of residence time dependence upon recycle ratio, decay rate and existence of discontinuity surface. Further, the performance of the reactor at largest residence times will be discussed. In addition, numerical simulations to illustrate and confirm the results will be carried out.

Splanchnic circulation is the primary mechanism thatregulates volumes of circulating blood and systemic blood pressure in patients with cirrhosis accompanied by portal hypertension. Recently, interest has been expressed in modulating splanchnic circulation in patients with liver cirrhosis, because this capability might produce beneficial effects in cirrhotic patients undergoing a liver transplant. Pharmacologic modulation of splanchnic circulation by use of vasoconstrictors might minimize venous congestion, replenish central blood flow, and thus optimize management of blood volume during a liver transplant operation. Moreover, splanchnic modulation minimizes any high portal blood flow that may occur following liver resection and the subsequent liver transplant. This effect is significant, because high portal flow impairs liver regeneration, and thus adversely affects the postoperative recovery of a transplant patient. An increase in portal blood flow can be minimized by either surgical methods(e.g., splenic artery ligation, splenectomy or portocaval shunting) or administration of splanchnic vasoconstrictor drugs such as Vasopressin or terlipressin. Finally, modulation of splanchnic circulation can help maintain perioperative renal function. Splanchnic vasoconstrictors such as terlipressin may help protect against acute kidney injury in patients undergoing liver transplantation by reducing portal pressure and the severity of a hyperdynamic state. These effects are especially important in patients who receive a too small for size graft. Terlipressin selectively stimulates V1 receptors, and thus causes arteriolar vasoconstriction in the splanchnic region, with a consequent shift of blood from splanchnic to systemic circulation. As a result, terlipressin enhances renal perfusion by increasing both effective blood volume and mean arterial pressure.

The multi-robot systems(MRS)exploration and fire searching problem is an important application of mobile robots which require massive computation capability that exceeds the ability of traditional MRS′*** paper propose a cloud-based hybrid decentralized partially observable semi-Markov decision process(HDec-POSMDPs)*** proposed model is implemented for MRS exploration and fire searching application based on the Internet of things(IoT)cloud robotics *** this implementation the heavy and expensive computational tasks are offloaded to the cloud *** proposed model achieves a significant improvement in the computation burden of the whole task relative to a traditional *** proposed model is applied to explore and search for fire objects in an unknown environment;using different sets of robots *** preliminary evaluation of this implementation demonstrates that as the parallelism of computational instances increase the delay of new actuation commands which will be decreased,the mean time of task completion is decreased,the number of turns in the path from the start pose cells to the target cells is minimized and the energy consumption for each robot is reduced.
