We review evidence concerning exercise for stroke *** biological reasons suggest that exercise would be important in preventing *** definitive randomised controlled trials evaluating the impact of physical activity(PA)and exercise on preventing stroke and mortality are lacking,observational studies,small randomised controlled trials and meta-analyses have provided evidence that PA and exercise favourably modify stroke risk factors,including hypertension,dyslipidaemia,diabetes,sedentary lifestyle,obesity,excessive alcohol consumption and tobacco *** is,therefore,important to understand the factors associated with poststroke PA/exercise and cardiorespiratory *** associated factors include self-efficacy,social support and quality of patients’relationships with health *** associated factors include logistical barriers,medical comorbidities,stroke-related deficits,negative exercise beliefs,fear of falling,poststroke fatigue,arthropathy/pain and *** research is needed to specify efficacious behavioural approaches to increase poststroke *** techniques probably include physician endorsement of exercise programmesto patients,enhancement of patient-professional relationships,providing patients an exercise rationale,motivational interviewing,collaborative goal-setting with patients,addressing logistical concerns,social support in programsmes,structured exercise programming,individualised behavioural instruction,behavioural diary recording,reviewing behavioural consequences of exercise efforts,reinforcing successful exercise *** programming without counselling may increase short-term activity;simple advice or information-giving is probably *** patients or those with cognitive impairment may need increased structure,with emphasis on behaviour per se,versus self-regulation *** support the latest American Heart Association/American Stroke Associati

Background:Beginning with the concepts of stress developed by Selye,an approach to stress and pain management,known as neuro-emotional technique(NET),has been *** is a treatment approach based on the principle that the stressor effects of dormant and/or current unresolved issues or trauma are what determine one’s bodily *** responses are relatively personalized to the conditioned,experiential and emotional reality of the ***:To determine the effect of NET on patients with chronic low back pain(ClBP)over ***,setting,participants,and interventions:In a randomized,double-blinded,placebo-controlled study conducted in a single clinic,NET or control treatments were given twice weekly for 4 weeks in a population of 112 *** outcome measures:Outcome measures,including Oswestry Disability Index,Quadruple Visual Analogue Scale,the psychoneuroimmunology markers of blood serum levels of C-reactive protein,tumour necrosis factor-a,interleukin-1(Il-1),Il-6,and Il-10,and 10 dimensions of the Short Form Health Survey scale,were assessed at baseline and at 1,3 and 6 months following the intervention ***:Compared to placebo,NET produced clinical and statistical significance(Plines of virtually all physiological,pain and disability markers,accompanied by gains in quality-of-life indicators at 0(baseline),1,3 and 6 *** of the percentages of patients whose 5 biomarkers lay outside the normative range were achieved at 1,3 and 6 months by NET but not control ***:A randomized,controlled trial of ClBP patients indicated that 8 NET interventions,compared to placebo,produced clinically and statistically significant reductions in pain,disability and inflammatory biomarkers,and improvements in quality-of-life measures.

Immune dysfunction in acute alcoholic hepatitis

Acute alcoholic hepatitis(AAH) is a serious complication of alcohol misuse and has high short term mortality. It is a clinical syndrome characterised by jaundice and coagulopathy in a patient with a history of recent heavy alcohol use and is associated with profound immune dysfunction with a primed but ineffective immune response against pathogens. Here, we review the current knowledge of the pathogenesis and immune defects of AAH and identify areas requiring further study. Alcohol activates the immune system primarily through the disruption of gut tight junction integrity allowing the escape of pathogenassociated molecular particles(PAMPs) into the portal venous system. PAMPs stimulate cells expressing toll-like receptors(mainly myeloid derived cells) and initiate a network of intercellular signalling by secretion of many soluble mediators including cytokines and chemokines. The latter coordinates the infiltration of neutrophils, monocytes and T cells and results in hepatic stellate cell activation, cellular damage and hepatocyte death by necrosis or apoptosis. On the converse of this immune activation is the growing evidence of impaired microbial defence. Neutrophils have reduced phagocytic capacity and oxidative burst and there is recent evidence that T cell exhaustion plays a role in this.

Innate and adaptive immune dysfunction,also referred to as cirrhosis-associated immune dysfunction syndrome,is a major component of cirrhosis,and plays a pivotal role in the pathogenesis of both the acute and chronic worsening of liver *** the evolution of the disease,acute decompensation events associated with organ failure(s),so-called acute-on chronic liver failure,and chronic decompensation with progression of liver fibrosis and also development of disease specific complications,comprise distinct clinical entities with different immunopathology *** bacterial translocation associated with systemic endotoxemia and increased occurrence of systemic bacterial infections have substantial impacts on both clinical *** and chronic exposure to bacteria and/or their products,however,can result in variable clinical *** immune status of patients is not constant during the illness;consequently,alterations of the balance between pro-and anti-in-flammatory processes result in very different dynamic *** this review we give a detailed overview of acquired immune dysfunction and its consequences for *** demonstrate the substantial influence of inherited innate immune dysfunction on acute and chronic inflammatory processes in cirrhosis caused by the pre-existing acquired immune dysfunction with limited compensatory ***,we highlight the current facts and future perspectives of how the assessment of immune dysfunction can assist clinicians in everyday practical decision-making when establishing treatment and care strategies for the patients with end-stage liver *** and efficient recognition of inappropriate performance of the immune system is essential for overcoming complications,delaying progression and reducing mortality.

On the origin of X-shaped radio galaxies

After a brief, critical review of the leading explanations proposed for the small but important subset of radio galaxies showing an X-shaped morphology （XRGs） we propose a generalized model, based on the jet-shell interaction and spin- flip hypotheses. The most popular scenarios for this intriguing phenomenon invoke either hydrodynamical backflows and over-pressured cocoons or rapid jet reorienta- tions, presumably from the spin-flips of central engines following the mergers of pairs of galaxies, each of which contains a supermassive black hole. We confront these models with a number of key observations, and thus argue that none of the models is capable of explaining the entire range of the salient observational properties of XRGs, although some of the arguments raised in the literature against the spin-flip scenario are probably not tenable. We then propose a new scenario which also involves galac- tic mergers but would allow the spin of the central engine to maintain its direction. Motivated by detailed multi-band observations of the nearest radio galaxy, Centaurus A, this new model emphasizes the role of the interactions between the jets and the shells of stars and gas that form and rotate around the merged galaxy and can cause temporary deflections of the jets, occasionally giving rise to an X-shaped radio struc- ture. Although each model is likely to be relevant to a subset of XRGs, the bulk of the evidence indicates that most of them are best explained by the jet-shell interaction or spin-flip hypotheses.

Estimating evapotranspiration from terrestrial groundwater-dependent ecosystems using landsat images

Understanding and mitigating against the impact of groundwater extraction on groundwater-dependent ecosystems(GDE)requires information of evapotranspiration(ET)of these *** this pilot study,we tested two remotesensing methods,Surface Energy Balance Algorithms for land(SEBAl)and Vegetation Index/Temperature Trapezoid(VITT),for ET estimation from terrestrial ***-temporal landsat Thematic Mapper(TM)and Enhanced Thematic Mapper(ETM)images were used to derive vegetation indices and land surface temperatures for ET *** transfer model was used for atmospheric correction of the landsat *** measurements were used to validate the remote sensing estimation of VI and surface *** methods have been implemented in a geographic information system(GIS)using automated scripts and ancillary GIS data for quality control *** of predicted ET by SEBAl to VITT model indicates relatively good agreement(R20.90)and promise for use in groundwater *** average ET from woodland GDEs within the zone of influence of the pumping stations is in general lower than similar woodlands outside of the pumping area,particularly in summer seasons which demonstrates that the pumping regime has an impact on those *** study also demonstrates that even a simple physical ET model can provide useful information for groundwater management,and more broadly other applications in hydrologic modelling and digital earth studies.

Perturbation finite element solution for chemo-elastic boundary value problems under chemical equilibrium

Modeling the elastic behavior of solids in energy conversion and storage devices such as fuel cells and lithium-ion batteries is usually difficult because of the nonlinear characteristics and the coupled chemo-mechanical behavior of these *** this work,a perturbation finite element(FE)formulation is developed to analyze chemo-elastic boundary value problems(BVPs)under chemical *** perturbation method is applied to the FE equations because of the nonlinearity in the chemical potential expression as a function of solute *** compositional expansion coefficient is used as the perturbation *** the perturbation expansion,a system of partial differential equations for the displacement and dimensionless solute concentration functions is obtained and solved in consecutive *** presence of a numerical solution enables modeling 3D chemo-elastic solids,such as battery electrodes or ionic gels,of any geometric shape with defects of different *** proposed method is tested in several numerical examples such as plates with circular or elliptical holes,and *** numerical examples showed how the shape of the defect can change the distribution of solute concentration around the *** in chemo-elastic solids create sharp peaks in solute concentration around crack tips,and the intensity of these peaks increases as the far field solute concentration decreases.

Derived, still living cockroach genus Cariblattoides （Blattida： Blattellidae） from the Eocene sediments of Green River in Colorado, USA

Cariblattoides labandeirai sp.n. from the Eocene sediments of Green River in Colorado, USA bear only two plesiomorphies, but also several significant autapomorphies within the advanced and highly derived living cockroach genus. Thus, Cariblattoides with extant occurrence in the Caribbean and South America was historically common in the Nearctic, and represents important evidence for the occurrence of derived living genera of cockroaches -50 Ma ago. Generally, the vast majority of living genera were absent during the Palaeocene, thus the diversification of most living cockroach lineages near the Palaeocene/Eocene boundary must have been extremely rapid. Females of living C. suave, the type species, have identical （sophisticated） coloration ofpronotum, but the most related living taxa are C. piraiensis and *** from Brazil （supported by phylogenetical analysis）.

Disagreement between symptom-reflux association analysis parameters in pediatric gastroesophageal reflux disease investigation

AIM: To assess the agreement within 3 commonly used symptom-reflux association analysis (SAA) parameters investigating gastroesophageal reflux disease (GERD) in infants. METHODS: Twenty three infants with suspected GERD were included in this study. Symptom index (SI), Symptom sensitivity index (SSI) and symptom association probability (SAP) related to cough and irritability were calculated after 24 h combined pH/multiple intraluminal impedance (MII) monitoring. Through defined cutoff values, SI, SSI and SAP values are differentiated in normal and abnormal, whereas abnormal values point towards gastroesophageal reflux (GER) as the origin of symptoms. We analyzed the correlation and the concordance of the diagnostic classification of these 3 SAA ***: Evaluating the GER-irritability association, SI, SSI and SAP showed non-identical classification of normal and abnormal cases in 39.2% of the infants. When irritability was taken as a symptom, there was only a poor inter-parameter association between SI and SSI, and between SI and SAP (Kendall’s tau b = 0.37, P ll’s tau b = 0.36, P ly). Evaluating the GER-cough association, SI, SSI and SAP showed non-identical classification of normal and abnormal cases in 52.2% of the patients. When cough was taken as a symptom, only SI and SSI showed a poor inter-parameter association (Kendall’s tau b = 0.33, P lUSION: In infants investigated for suspected GERD with pH/MII-monitoring, SI, SSI and SAP showed a poor inter-parameter association and important dis-agreements in diagnostic classification. These limitations must be taken into consideration when interpreting the results of SAA in infants.
