A comprehensive European overview of recent and ongoing research in Europe has been carried out within the framework of COST Action E12 'Urban Forests and Trees', a pan-European research network. Rapid diagnosis and choice of appropriate antibiotic treatment might be life-saving in serious infectious diseases. Still the available markers that can evaluate and monitor the diagnosis and treatment are few. Hepatocyte growth factor (HGF) has been studied as a potent regenerative factor produced and released during injuries such as infectious diseases. Monitoring of HGF levels might predict therapy results better than C-reactive protein (CRP) within the first day of treatment in pneumonia. For further investigation of previous observations we aimed to study HGF as a first-day marker in over-representing infectious diseases in comparison to procalcitonin (PCT), CRP and body temperature. Fifty-one patients with community acquired infectious diseases were included consequently at admittance and the serum samples were collected before and within 18 - 24 hours of treatment. HGF levels decreased significantly in case of efficient antibiotic therapy and HGF was shown to be better than PCT, CRP and body temperature to evaluate treatment. In patients with pneumonia, monitoring of HGF was most reasonable. HGF might be used as a therapeutic marker within the first day of empiric antibiotic treatment during infection.

The aim of this study was to evaluate the effects of pumping height and repeated pumping on the generalized stress response and gross injuries in harvest sized Atlantic salmon. Fish pumped from a net pen at either high (5.2 m) or low (3.6 m) pumping heights showed an elevated, but not severe physiological stress response (pH, pCO2, lactate, potassium, haematocrit, and sodium) compared to fish netted (not pumped), while effects of different pumping heights were overall not demonstrated. Repeated pumping (either 3 or 6 times) also caused an increase in stress response (pH, pCO2, pO2, lactate, potassium and sodium) compared to control fish, and a positive dose-response relationship was found for lactate. No fish died as a result of pumping, nor were injuries observed that could exclusively be attributed to pumping. In conclusion, although elevated from the control groups, the stress response following increasing pumping height and repeated pumping as conducted in these experiments were not indicative of causing severe stress or injuries.

Background: The aim was to evaluate safety aspects of patient-controlled sedation and analgesia (PCS) for extracorporeal shockwave lithotripsy (ESWL) and PCS to be handled by non-anaesthesiology doctors. Methods: Thirty-four ASA I-III patients used PCS with propofol and alfentanil for ESWL in this interventional study. Strict safety limits were defined regarding respiratory rate (RR), heart rate (HR), mean arterial blood pressure (MAP), oxygen saturation from pulse oximetry (SpO2), and transcutaneous partial pressures of oxygen (PtcO2) and carbon dioxide (PtcCO2). The patients' levels of consciousness was graded on a five-point scale and monitored with Bispectral Index (BIS). A nurse anaesthetist was supervising the procedure but was instructed to intervene only if safety limits were breached. No supplementary oxygen was given. Results: All patients responded to verbal stimuli during treatment. Cardiovascular stability was maintained, but respiratory variables were affected. Two patients with SpO2 2 ≥ 6.5 kPa). In 18 patients hypoxaemia was indicated as PtcO2 ≤ 8.0 kPa. All these 18 patients were given supplementary oxygen. There was no correlation between dose of drugs, age, weight or any vital variable. The 34 patients would use PCS again in the case of future treatment. Conclusions: During ESWL treatment PCS can be used with good patients’ satisfaction, and maintained cardiovascular stability, but PCS had an indisputable effect on pulmonary function with hypoxemia (resulting in need for supplementary oxygen) or hypercarbia. The person in charge of PCS must therefore be trained to perform according to the guidelines for sedation and/or analgesia by non-anaesthesiology doctors.

目的　探讨脉冲电磁场对结肠造瘘后旷置肠段粘膜的刺激作用 ,及降钙素基因相关肽在粘膜萎缩时的变化。方法　将 2 8只SD大鼠随机分为 3组 ,1组为对照组 (n =9) ,另外两组接受结肠造瘘、远端结肠旷置术 ,于术后 8d起连续 5d分别接受每日半小时的电磁场刺激 (n =10 )和不接受刺激 (n =9)。结果　病理组织学检查发现 ,电磁场刺激有效地防止了旷置部分结肠粘膜的萎缩。接受刺激的大鼠结肠粘膜的DNA和RNA含量均比未受刺激的高。结肠旷置后大鼠血浆中的降钙素基因相关肽有了明显的下降 ,而电磁场刺激未使之升高。结论　脉冲电磁场能在缺乏肠内营养的情况下刺激肠道粘膜的增生 ,但未改变降钙素基因相关肽的水平 ,提示了肠粘膜萎缩和阻断机理的复杂性。

3-Arylisothiazoloquinols as potent ligands for the benzodiazepine site of GABA_(A) receptors

3-Arylisothiazolo[5,4-b]quinolin-4(9H)-ones and 3-arylisoxazolo[5,4-b]quinolin-4(9H)-ones were synthesized and assayed for affinity for the benzodiazepine binding site of the GABAA receptors. While the 3-arylisothiazoloquinolin-4-ones were found to be potent ligands, with affinities (expressed as the affinity Ki value) down to 1 nM, the 3-arylisoxazoloquinolin-4-ones are less potent. This is suggested to depend on sterical repulsive interaction of the 3-arylisoxazoloquinolin-4-ones with the receptor essential volume of the binding site, and a higher electron density at the nitrogen in the azole ring (N-2) as well as the carbonyl oxygen in the isothiazoloquinolin-4-ones enabling them to interact stronger with hydrogen bond donor sites at the binding site.
